The Comparative Study on Effect of Jungsongouhyul Phamacopuncture and Electroacupuncture in Patients with Acute Traumatic Shoulder Pain (급성 외상성 어깨 통증 환자에 대한 중성어혈약침과 전침의 효과에 관한 비교 연구)

  • Objectives : The purpose of this study is to report the effect of Jungsongouhyul phamacopuncture and electroacupuncture on patients with acute traumatic shoulder pain. Methods : The patients with acute traumatic shoulder pain were diagnosed by blood stasis and divided two groups. One group was treated byJungsongouhyul phamacopuncture. The other group was treated by electroacupuncture. Each group was treated 3 times a week for 2 weeks. The results were measured by visual analog scale(VAS), range of abduction and internal rotation. Results : Two groups had no significant differences before treatment. Jungsongouhyul phamacopuncture group had effects on VAS, range of abduction and internal rotation significantly. Electroacupuncture group also had effects on VAS, range of abduction and internal rotation significantly. But there was no significant differences between two groups. Conclusions : Jungsongouhyul phamacopuncture and electroacupuncture have effect on patients with acute traumatic shoulder pain significantly.

The Effect of Balance Training and environmental enrichment on motor performance after Traumatic Cerebellar Injury in the Rat (균형훈련과 환경강화가 외상성 소뇌 손상 흰쥐의 운동 수행력에 미치는 영향)

  • The purposes of this study were to test the effect of balance training and environmental enrichment on motor performance after traumatic cerebellar injury in the rat. Traumatic cerebellar injury was induced by weight drop model and after operation they were housed in individual standard cages for 24 hours. Twenty two Sprague-Dawley rats were randomly assigned into control and experimental groups. The control group was housed in standard cage for 7 days. The experimental group I was housed in standard cage after balance training for 7 days. The experimental group II was housed in environmental enrichment cage after balance training for 7 days. Vestibular drop test and tilting plane test was examined at preoperation, on 1st day after operation and 7th day after operation to the rats. The results of this experiment were as follows: 1. In motor behavioral test, at preoperation and on 1 day after operation, the outcomes were not different among the groups. But on 7th day after operation, the outcomes of group I were better than that in control group, and group II were better than group I. These results suggest that behavioral motor outcome by simultaneously application of balance training and environmental enrichment in traumatic cerebellar injured rats were more improve than that by just balance training.

Comparison of Computed Tomography Findings between Aneurysmal and Traumatic Subarachnoid Hemorrhage

  • Objective : The purpose of this study is to identify any differential point in computerized tomographic[CT] findings between aneurysmal subarachnoid hemorrhage[ASAH] and traumatic subarachnoid hemorrhage[TSAH], which sometimes make us not confident in differentiation. Methods : CT of 142 ASAH and 82 TSAH patients over the last 2 years were retrospectively reviewed. We evaluated the thickness of SAH, the laterality of sylvian cisternal hemorrhage, the location, the number of involved cisterns, and the associated other lesions between two types of SAH. Results : Suprasellar cisterns and sylvian cisterns were most prominently and frequently involved cisterns in ASAH but cortical sulci and sylvian cisterns were most frequently involved in TSAH. Intraventricular and intracerebral hemorrhage were frequently seen in ASAH. Thickness of SAH over 1mm, bilateral sylvian SAH, multiple cisternal SAH were in favor of ASAH. The number of involved cisterns were more frequently seen in ASAH than in TSAH. In ASAH, bilateral sylvian hemorrhages were more frequently seen than in TSAH. Skull fracture, subdural hematoma, subgaleal hematoma, and hemorrhagic contusion were frequently associated with TSAH. Conclusion : As a result of our study, the authors conclude that when IVH, hydrocephalus, thick SAH > 1mm bilateral sylvian SAH, and multiple cisternal SAH are seen in CT, immediate angiography should be performed to rule out cerebral aneurysms whether associated with other traumatic lesions or not.

Endovascular Treatment of Traumatic Arteriovenous Fistula in Young Adults with Pulsatile Tinnitus

  • Traumatic arteriovenous fistulas (AVFs) involving the external carotid artery are exceedingly rare in young adults. Since an AVF is the most common life-threatening cause for pulsatile tinnitus (PT), meticulous evaluation and treatment of patients with PT is crucial. Here, we present two traumatic AVF cases treated with coil embolization leading to no residual fistulous connections followed by an immediate and complete resolution of PT. A 20-year-old man developed left ear tinnitus three months after a traumatic brain injury involving the right temporal bone fracture. Cerebral angiography demonstrated an enlarged left middle meningeal artery (MMA) and a fistular point at the posterior branch of the MMA draining to the middle meningeal vein (MMV) and the left pterygoid plexus, suggesting an AVF. Another 18-year-old girl developed left tinnitus, left exophthalmos, and conjunctival injection 6 months after a traffic accident involving no demonstrable abnormal findings in the radiologic exam. Magnetic resonance angiography demonstrated a markedly dilated left MMA draining to the MMV, left cavernous sinus, and left superior ophthalmic vein. In both cases, coil embolization was performed with total obliteration of the fistular point.

Traumatic Myiasis Caused by an Association of Sarcophaga tibialis (Diptera: Sarcophagidae) and Lucilia sericata (Diptera: Calliphoridae) in a Domestic Cat in Italy

  • We describe here a rare case of traumatic myiasis occurred in August 2014, caused by an association of 2 Diptera species, Sarcophaga tibialis Macquart (Diptera: Sarcophagidae) and Lucilia sericata (Meigen) (Diptera: Calliphoridae), in a domestic cat in northern Italy. Species identification was based on adult male morphology. The present case is the first report of S. tibialis as an agent of myiasis in Italy, and also the first ever report of myiasis caused by an association of S. tibialis and L. sericata. The cat developed an extensive traumatic myiasis in a large wound on the rump, which was treated pharmacologically and surgically. The biology, ecology, and distribution of S. tibialis and L. sericata are also discussed. A literature review is provided on cases of myiasis caused by S. tibialis, and cases of myiasis by L. sericata involving cats worldwide and humans and animals in Italy.

The Clinical Analysis of Traumatic Diaphragmatic Iinjuries (외상성 횡경막 손상의 임상적 분석)

  • We evaluated fifty three cases of traumatic diaphragmatic injuries that we have experienced from Jan.1973 to Oct.1994. The age distribution of the pateint was ranged from 1 to 74 years. Sex ratio is 39:14 with male dominence. The traumatic diaphragmatic injuries were due to blunt trauma in 37[Left 22, Right 15 cases and penetrating trauma in 16[Left 9, Right 7 cases. In blunt trauma, Preoperative diagnosis of the diaphragmatic injuries was possible in 27 patients[72% , and in penetrating trauma, 14 patients[88% . Among 37 in blunt traumas, 22[58% cases, and among 16 in penetrating traumas, 13[88% cases were operated within 24 hours. The most common herniated abdominal organ in the thorax was stomach[14/53 . The traumatic diaphragmatic repair of 50 cases were performed by thoracic approach in 23 cases, thoracoabdominal approach in 8 cases and abdominal approach in 19 cases, and in 3 cases, not operated. Hospital mortality [including not operated patients[3 was 17%[9/53 and the causes of death were intracranial hematoma[1 , hypertensive encephalopathy[1 and asphyxia[1 , and among operated patients[6 , combined head injury[2 , multiorgan failure[2 , hypovolemic shock[1 , and pulmonary edema & renal failure[1 . All deaths had related to the severity of associated injuries.

Relation of necessity of the first aid education and attitude about traumatic dental injuries in some military men (일부 군인들의 치아외상에 대한 태도와 응급처치 교육필요성과의 관련성)

  • Objectives : The purpose of this study was to investigate the traumatic injuries by first aid education and mouth protector in some military men. Methods : Among 195 military men, a total of 164 military men filled out the self-administered questionnaire and the return rate was 84.1%. The data were evaluated statistically using chi-square analysis. Results : 81.5% of respondents agreed that first aid education is necessary and 48.2% of respondents agreed that mouth protector of is necessary. In managing tooth fracture, subluxation, and avulsion, the military men answered the necessity of the first aid education revealed 87.2%, 86.8%, and 91.5%, respectively. The questions in managing tooth fracture, subluxation showed that mouth protector is necessary 56.3%, 49.0% respectively. In managing tooth fracture with reattachment, the necessity of the first aid education showed 88.9%. Conclusions : In order to get the knowledge about emergency treatment of traumatic injuries in military men, it is necessary to develop the professional education program for the dental hygiene personnel.

A Study on Differences of the Effects of Firefighter's Traumatic Experiences on Their Burnout, Depression, Anxiety and Social Relationships (소방공무원이 지각한 외상경험이 직무소진과 우울, 불안 및 대인관계에 대한 차이 비교 연구)

  • The goal of this study is to investigate the factors affecting firefighter's traumatic experiences and to examine and compare the differences of how those factors contribute to firefighter's burnout, depression, anxiety and social relationships. This study suggests that it is imperative for firefighters to have mindfulness, which brings mental and emotional healing, for their job-satisfaction. To empirically examine the research model of this study, the questionnaire survey was carried out to firefighters in InCheon Metropolitan City for two weeks; from October 19thto November 3rd, 2017. Out of 180 questionnaires, 170were retrieved. Total number of 160 questionnaires are empirically examined this study, excluding10bad survey responses. The t-test was conducted to determine whether there exist differences by gender among traumatic experienced firefighters, and a meaningful difference was found from perceived intrusiveness, a sub factor to traumatic experiences. Male firefighters showed higher mean than that of female firefighters in perceived angle, evasion and intrusiveness. Second, the t-test was conducted to determine whether there exist differences by gender in firefighter's burnout, but no meaningful differences were found. Female firefighters showed higher mean in emotional exhaustion, impersonalization and in decrease in self-achievements. Third, the t-test was conducted to determine whether there exist differences by gender in firefighter's depression and relationships, but no meaningful differences were found.

The Effect of Korean Medical Treatment on 4 Patients with Acute Traumatic Meniscal Tear (급성 외상성 반월연골 파열에 대한 한의학적 치료 증례보고 4례)

  • Objectives : The purpose of this study is to report the effect of Korean medical treatment on 4 patients with acute traumatic meniscal tear, presenting severe knee pain and limitation of range of motion of the knee joint. Methods : 4 patients with acute traumatic meniscal tear had been treated with acupuncture, herbal treatment, moxibustion, and wet-cupping therapy. From administration to discharge, we recorded subjective pain by using a visual analogue scale and by measuring the range of motion of the knee joint once per week. Results : The visual analogue scale of knee pain of all 4 cases reduced to 0 after treatment. The range of motion of the knee joint also increased considerably after treatment. Conclusions : In this study, Korean medical treatment may have been effective in reducing knee pain and increasing the range of motion of the knee joint.

Factors Affecting Post-Traumatic Stress of General Hospital Nurses after the Epidemic of Middle East Respiratory Syndrome Infection (메르스 감염 유행 후 상급종합병원 간호사의 외상 후 스트레스 영향요인)

  • Purpose: This study examined post-traumatic stress (PTS) and the factors affecting it among general hospital nurses after the MERS(Middle East Respiratory Syndrome) epidemic. Methods: Data were collected from 170 nurses who worked at general hospitals since the first reported MERS outbreak. The IES-R-K assessed PTS. Data were analyzed using SPSS. Results: The mean PTS level was 7.80 points (range: 0~88); 7.1% of the participants were at a high risk. Nurses who had been in contact with patients suspected or diagnosed with MERS had high post-traumatic levels; those who had been quarantined during the MERS outbreak had relatively higher PTS levels. Shift-work nurses had higher PTS levels than those with fixed working hours. Above charge' nurses stress levels were higher than staff nurses' stress levels. The results showed that factors including contact with an MERS-suspected or diagnosed patient, position at work, and working status of MERS-affected nurses explained 16% of the PTS. Among the main variables, nurses' above charge position was the greatest factor affecting PTS. Discussion: It is necessary to develop intervention studies and programs considering these variables. Furthermore, development and implementation of differentiated programs should be done considering the position of above charge nurses.
