Transtheoretical Model을 적용한 국내 간호연구 분석 (Analysis of Korean Nursing Studies Applied for the Transtheoretical Model)

  • Purpose: The purpose of this study was to explore the utilization of the Transtheoretical Model(TTM) for Korean nursing studies on health behaviors and identify the strategies for improvement of further studies. Methods: From a main and seven branch journals on the Korean Society of Nursing Science, a total of 26 studies, which had been conducted based upon the TTM, were selected. The selected studies were analyzed from the September of 2006 to the February of 2007 by the criteria; name of journal, year of publication, health behavior, population, sampling, sample size, research design, instrument (translation process, reliability and validity), and statistical analysis. Results: TTM on the Korean nursing studies was mainly applied for smoking cessation or exercise with elderly or middle aged women. The research findings could not be generalized with the limitation of the use of non representative samples. The most frequently used research design was a cross-sectional survey. Some studies had a lack of reliable and valid instruments, so that the internal validity of the study findings may be threatened. Statistically related problems increased type I error and decreased power. Conclusion: Korean nursing studies based on TTM can be the beginning stage in quality and quantity.

  • Purpose: The purpose of this study was to develop an exercise intervention program based on stage of exercise using the Transtheoretical Model (TTM) for patients with type 2 diabetes mellitus (DM). Method : A methodological research design was used to develop the exercise intervention program based on stage of exercise using TTM. Result: The exercise intervention program consisted of theoretical background and goals of program, assessment tool for stage of change, and an exercise intervention program based on stage of exercise. Details for the exercise and a glossary are included, Conclusion : The exercise intervention based stage of exercise can apply for DM patients who are in any stages properly.

범이론적 모형에 근거한 성인의 운동실천행위 분석 (Analysis of the Exercise Behavior of Early Adulthood : an Application of Transtheoretical Model(TTM))

  • This study examined the applicability of the transtheoretical model(TTM) to understanding exercise behavior of early adulthood. A cross-sectional study was conducted and data wee collected from 1618 adults aged from 20 to 39 through internet survey. Major conclusions wee as follows: 1. Stage distribution for exercise behavior was 15.6%, 26%, 25%, 19%, 14.4% for precentemplation, contemplation, preparation, action, maintenance stages, respectively. 2. Behavioral processes were more frequently utilized than cognitive processes for action and maintenance stage. 3. Self-reevaluation, social liberation and reinforcement were frequently used in precontemplation, contemplation and preparation stages. Self-reevaluation, reinforcement, self-liberation were frequently used in action and maintenance stages. 4. Pros of exercise behavior began to be higher than cons of exercise behavior from the actions stage and continued to increase to maintenance stage. 5. Self-efficacy was higher in action stage than precontemplation, contemplation and preparation stages and the highest in maintenance stage.

유방암 생존자의 운동실천을 위한 앱 기반 자가관리 프로그램 개발 및 효과: 비무작위 대조군 실험연구 (Development and Evaluation of an App-Based Self-Management Program for Exercise Practice of Breast Cancer Survivors: A Non-Randomized Controlled Trial)

  • Purpose: This study aimed to develop an app-based self-management program based on the transtheoretical model (TTM) for breast cancer survivors' exercise practice, as well as to investigate the program's effects on the stage of change for exercise, exercise self-efficacy, exercise decisional balance, exercise amount, and body composition. Methods: This non-randomized controlled study included 52 participants (26 in each of the experimental and control groups, respectively). An app-based self-management program based on the TTM was conducted with the experimental group for a 12-week period. The program comprised three components: individual coaching for each stage of change for exercise based on TTM, amount of exercise and body composition monitoring, and online self-help meetings. Results: Compared with the control group, the experimental group had significantly higher stages of change for exercise (p < .001), exercise self-efficacy (p < .001), exercise decisional balance (p = .002), exercise amount (p < .001), and body composition (body weight [p = .006], body mass index [p = .005], and body fat percentage [p = .010]) immediately and four weeks after the intervention. Conclusion: An app-based self-management program based on the TTM improves exercise behaviors in breast cancer survivors and provides physical benefits.

Testing the Transtheoretical Model in Predicting Smoking Relapse among Malaysian Adult Smokers Receiving Assistance in Quitting

  • The role of The Transtheoretical Model (TTM) in predicting relapse is limited. We aimed to assess whether this model can be utilised to predict relapse during the action stage. The participants included 120 smokers who had abstained from smoking for at least 24 hours following two Malaysian universities' smoking cessation programme. The smokers who relapsed perceived significantly greater advantages related to smoking and increasing doubt in their ability to quit. In contrast, former smokers with greater self-liberation and determination to abstain were less likely to relapse. The findings suggest that TTM can be used to predict relapse among quitting smokers.

범이론모형(Transtheoretical Model) 기반의 운동행위강화 프로그램이 고혈압 노인의 혈압 및 신체활동량에 미치는 효과 (Effects of a Transtheoretical Model Based Exercise Behavior Improving Program on Blood Pressure and Physical Activity for Older Adults with Hypertension)

  • 본 연구는 범이론모형에 기반하여 운동행위강화 프로그램을 개발하고 이를 고혈압 노인에게 적용하여 혈압 및 신체활동량에 미치는 효과를 규명하고자 하였다. 연구대상자는 서울시 소재 노인복지관에 내원하며 연구에 참여를 허락한 65세 이상 고혈압 노인 41명으로 실험군 20명, 대조군 21명이었다. 실험군에게는 범이론모형에 기반한 운동행위강화 프로그램을 8주 동안 적용하였다. 연구결과 실험군은 대조군에 비하여 운동행위 변화단계, 전체 변화과정 점수, 행위적 변화과정 점수, 운동 자기효능감, 신체활동량이 유의하게 증가하였으나, 경험적 변화과정 점수, 의사결정균형, 혈압 등은 유의한 차이가 없었다. 이러한 결과를 바탕으로 운동행위강화 중재는 비활동 고혈압 노인의 운동행위로의 이행과 신체활동량을 증진시키는데 효과가 있음을 확인하였다. 본 프로그램이 지역사회에 거주하는 고혈압 노인에게 적용된다면 운동습관을 형성하게 하여 고혈압 합병증을 예방하는데 기여할 것으로 기대된다.

근감소증 노인을 위한 범이론적 모델 적용 복합운동프로그램 개발 및 효과 검증 (Development and Effects of Combined Exercise Program for Older Adults with Sarcopenia Based on Transtheoretical Model)

  • Purpose: The purpose of this study was to develop and examine the effects of combined exercise program for older adults with sarcopenia based on transtheoretical model (TTM). Methods: A non-equivalent control group with a pretest-posttest design was used. The subjects consisted of 43 older adults with sarcopenia in precontemplation stage, contemplation stage and preparation stage of TTM (experimental group: 22, control group: 21). The developed program consisted of 36 sessions for 12 weeks including combined exercise (60 minutes) and TTM based strategies for enhancing exercise behavior (10 minutes) per session. Data were collected before, immediately after the program between July 31 to October 27, 2017. The data were analyzed using independent t-test, Mann-Whitney U test with SPSS/WIN 18.0. Results: Compared with their counterparts in the control groups, older adults with sarcopenia in the experimental group showed a significantly greater improvement in process of exercise behavior change, pros and cons of decisional balance for exercise behavior, exercise self-efficacy, parameters of muscle, and the level of physical performance. Conclusion: The study findings indicate that this combined exercise program for older adults with sarcopenia based on TTM model was effective and can be recommended as a nursing intervention for older adults with sarcopenia.

금연 변화 단계에 따른 변화 과정, 의사결정 균형, 흡연 유혹의 차이 (Differences in Processes of Change, Decisional Balance, and Temptation Across the Stages of Change for Smoking Cessation)

  • Purpose: The purpose of this study was to investigate smoking cessation behavior of male taxi drivers in Korea on the basis of the Transtheoretical model(TTM), and to validate the usefulness of TTM. Method: Data were collected using a self-reported questionnaire including smoking history and major factors of TTM from 208 subjects who were current smokers or ex-smokers. Data were analyzed by descriptive statistics and ANOVA. Result: Most subjects ($85.1\%$) were current smokers. Stages of change were precontemplation ($44.7\%$) and contemplation ($27.4\%$). Subjects in precontemplation stages had the lowest mean score in processes of change and the highest mean scores in decisional balance(pros) and temptation(positive affective, habitual/craving). According to stages of change, there were statistically significant differences in processes of change, decisional balance, and temptation. Conclusion: This study supported the generalization of TTM. As this study showed that the subjects didn't have motivation in smoking cessation, applying tailored smoking cessation programs for taxi drivers is needed.

Empowering Rural Housewives in Iran: Utilizing the Transtheoretical Model to Increase Physical Activity

  • Objectives: Rural housewives are integral to household management and family care, yet their sedentary lifestyles present significant health risks. This study used the transtheoretical model (TTM) to investigate strategies that encourage and maintain regular exercise habits among rural housewives. Methods: A semi-experimental study was conducted in 2021 with 114 housewives aged 30 to 59 who attended rural health centers in Gorgan, Iran. Participants were randomly assigned to 1 of 2 groups. Data collection involved a validated questionnaire that gathered demographic information and constructs of the TTM. The intervention group participated in a comprehensive educational program, which included four 60-minute sessions. Data were collected again 6 months post-intervention and analyzed using descriptive and inferential statistics in SPSS version 21. Results: The study encompassed women with an average age of 39.75±6.05 years, the majority of whom had educational levels below a diploma, and over 90% were married. We observed strong correlations between the processes of change, self-efficacy, and decisional balance. At the outset, there were no significant differences in demographics or model structures between the 2 groups. However, 6 months post-intervention, the intervention group exhibited statistically significant differences in the mean scores of model structures, stages of change, and body mass index (p<0.05). Conclusions: This study highlights the importance of physical activity training for rural housewives. The findings suggest that the educational intervention, which utilized the TTM, significantly impacted the participants' model structures and their stages of change.

걷기행사 참가의 결정요인 (Factors Associated with Perticipation in a Walking Campaign)

  • Objective: The purposes of this study are to understand the characteristics of the participants in a community walking campaign and to analyze the factors related to their participation based on the Transtheoretical Model (TTM). Methods: The study composed of the description of participant characteristics and comparison of them with non-participant characteristics in a walking campaign in K province. The data were collected through a survey of 2,590 participants and 258 non-participants from the same community. The survey instrument included questions about stages of walking and exercise, knowledge and attitude toward walking, and environmental condition for walking. Results: A majority of the walking campaign participants were in the action(24.8%) and maintenance(43.6%) stages of walking and exercise behavior. The non-participant group was split between maintenance(51.6%) and precontemplation (30.6%) stages. Among the participants, effective campaign promotion channels differed by age group while motivation for participation and participation patterns were associated with both age and gender. Favorable physical environment was a significant factor of participating in walking campaign(OR=1.396, CI=1.149-1.696). Although the campaign participants scored higher than the non-participants in most attitude toward walking questions, differences in knowledge scores between two groups were less significant. Conclusion: In conclusion, further social marketing to increase the awareness and to increase the concern of population in the community is needed based on the survey result. Transtheoretical model seems appropriate to apply to the evaluation and the planning the program of the behavior change in the community. Also, more organized and sustainable support in need to maintain the good habit of walking for the participants in walking campaign.
