장애물보행훈련이 뇌졸중환자의 보행 및 균형에 미치는 효과 (The Effects of the Obstacle Walking Training on Gait and Balance in Stroke Patients)

  • 장애물보행훈련이 뇌졸중 환자의 보행 및 균형에 미치는 효과를 확인하고자 장애물보행과 평지보행훈련 후 변화를 보기 위해 보행과 균형을 평가하였다. 훈련 전후 보행과 균형에서 장애물보행훈련을 한 실험군이 유의미한 차이를 보였고 평지보행을 한 대조군은 유의미한 차이를 보이지 않았으며 두 군 간에는 유의미한 차이를 보였다.

교통수요예측시 O/D존 및 네트워크 집계수준에 따른 영향 분석 (Impacts of number of O/D zone and Network aggregation level in Transportation Demand Forecast)

  • 교통수요분석시 기종점(O/D) 존의 집계수준과 네트워크의 구축수준이 어느 정도 균형(balance)이 이루어져야 하는데, 실제로는 불균형을 이루고 있는 경우가 있어 실제교통량과 예측교통량간의 차이가 발생하는 원인중의 하나로 간주되고 있다. 본 연구에서는 소규모 네트워크를 대상으로 O/D존 수와 네트워크의 구축수준에 따른 통행수요의 변화를 살펴보고, 이를 통하여 O/D존수와 네트워크간의 균형(balance)문제를 검토해 보았다. 분석방법은 Sioux-Falls network를 대상으로 네트워크를 추가/삭제시키고, 그에 따라 O/D존의 수도 일정비율로 변화시켜 분석하였다. 분석된 자료를 검증척도인 RMSE와 F-검정을 통해 비교 분석한 결과, 네트워크 증가시에는 예상과는 달리, 네트워크 상세화에 대하여 O/D존수의 증가/감소가 민감하게 반응하지 않는 것으로 나타났으나, 네트워크 집계(삭제)시에는 O/D존의 수도 역시 함께 감소시켜야 오차를 줄일 수 있는 것으로 나타나 이들 간에는 밀접한 관계가 있음을 보여주었다. 이는 교통수요분석을 위하여 네트워크를 구축할 때, O/D존수(존의 크기)도 함께 고려해야 하며, 이들간에 균형(balance)이 이루어지도록 설계해야 함을 의미한다.

리듬청각자극을 동반한 경사 트레드밀 보행훈련이 뇌졸중 환자의 균형 및 보행에 미치는 영향: 예비연구 (Effects of Inclined Treadmill Walking Training with Rhythmic Auditory Stimulation on Balance and Gait in Stroke Patients: A pilot study)

  • Purpose: The purpose of this study was to identify whether inclined treadmill gait training with rhythmic auditory simulation (RAS) could improve on balance and gait in stroke patients. Method: Fifteen stroke patients who had agreed with the study were allocated to the group 1(n=5), group 2(n=5), or group 3(n=5). The group 1, group 2 and group 3 performed RAS with inclined treadmill gait training, inclined treadmill gait training and treadmill without incline gait training respectively for 3 weeks (30 minutes per session, 5 times in a week). The balance was assessed using Timed Up & Go (TUG) and Berg Balance Sale (BBS), and the gait was evaluated using 6 Minutes Walking Test (6MWT) and spatio-temporal walking variables as walking speed, cadence, Single Limb Support of affected side(SLS) and Symmetric Index(SI) before and after training. Result: Both the group 1 and group 2 showed significant improvement after training in all variables of balance and gait. The group 3 showed significant improvement in TUG values, 6MWT values, walking speed, cadence and SI. The changes in the group 1 were significantly greater in all dependent variables of balance and gait than those of the group 2 and group 3. The changes in the group 2 were significantly greater in TUG values, BBS scores, 6MWT values, walking speed, and cadence than those of the group 3. Conclusion: The result of this study show inclined treadmill gait training with RAS is more effective to improve balance and gait in stoke patients than inclined treadmill or general treadmill gait training without RAS.

혈류제한 운동이 만성 발목 불안정성을 가진 성인의 발목 근력 및 균형능력에 미치는 영향 (Effects of Blood Flow Restriction Exercise on Ankle Strength and Balance Ability in Adults with Chronic Ankle Instability)

  • Purpose : This study aimed to investigate the effect of blood flow restriction exercise on ankle muscle strength and balance ability to achieve maximum efficiency with the same exercise intensity and time. Methods : Twenty-six adults are randomly assigned to experimental group (n=13) and comparison group (n=13). The experimental group performed ankle joint strength exercises with blood flow restriction applied while the comparison group performed ankle joint strength exercises without blood flow restriction applied three times a week for four weeks. The digital muscle measurement, Y-balance test, and Cumberland ankle instability tool were used to evaluate the subject's muscle strength, dynamic balance, and ankle instability index before and after the intervention. Results : In within-group comparison muscle strength, all the two groups showed significant improvements post intervention (p<.05). In between-group comparison, there was significant difference in the change of dorsiflexion, eversion strength pre and post intervention (p<.05). but plantarflexion was no significant difference between pre and post intervention in the group comparison (p>.05). In within-group comparison dynamic balance, all the two groups showed significant improvements post intervention (p<.05). In between-group comparison, there was no significant difference in the change of Y-balance score pre and post intervention (p>.05). In within-group comparison ankle instability index, all the two groups showed significant improvements post intervention (p<.05). In between-group comparison, there was no significant difference in the change of CAIT score pre and post intervention (p>.05). Conclusion : The results of this study show that ankle joint strength exercise improved the strength and balance ability of those complaining of chronic ankle instability, and ankle joint strength exercise applied with blood flow restriction was more effective in dorsiflexion and eversion strength exercise than ankle joint strength exercise without blood flow restriction.

  • 본 연구는 발목운동프로그램을 중년여성에게 적용하여 근력과 균형에 미치는 영향을 알아보고자 하였다. 연구에 동의한 중년여성 8명중 고유수용성감각 훈련을 시행한 실험군 4명 근력강화 훈련을 시행한 대조군 4명에게 총 4주간 매회 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 근력은 발등굽힘과 발바닥굽힘 검사를 시행하였고 균형은 기능적뻗기와 한발서기로 검사하고 훈련 후 다시 재측정하였다. 이에 실험군과 대조군 모두 근력과 균형검사에서 유의한 차이를 보였다. 이에 불안정한 지지면과 안정된 지지면에서의 발목운동은 중년여성의 근력과 균형이 향상되는 프로그램이다.

기능적 발목 불안정성에 대한 안정지지면과 불안정지지면에서의 균형훈련의 효과 비교 (Comparison of the Effects of Balance Training on the Stable and Unstable Supporting Surfaces for the Functional Ankle Instability)

  • Purpose: The purpose of this study was to compare the effects of the balance training on the stable and unstable supporting surfaces for the subjects with functional ankle instability. Methods: Twenty-nine subjects with functional ankle instability were randomly assigned to the stable group (n=14) and the unstable group (n=15). Balance training was conducted twenty minutes a day on the stable surface for the stable group and on the airostep for the unstable group three times a week for four weeks. Balance training program was consisted of ten steps by eye opened or closed and two or one leg standing. X-speed for transverse balance and Y-speed for longitudinal balance were measured with eye opened and closed in affected leg standing position. Results: There were significant improvements of balance ability in eye opened (p<.01) and eye closed standing (p<.01) in stable group. Unstable group also showed significant improvements of balance ability in eye opened (p<.01) and eye closed standing (p<.01). There were no statistical differences in the magnitude of improvement between the groups (p<.05). Conclusions: Based on such results, it can be said that balance training on the stable surface is effective as much as training on the unstable surface for the subjects with functional ankle instability.

탄성밴드를 이용한 저항운동이 뇌졸중 환자의 근력, 근긴장도, 균형 및 보행에 미치는 영향 (The Effect of Resistance Exercise Using Elastic Band on Muscle Strength, Muscle Tone, Balance and Gait in Stroke Patients)

  • Purpose : This study aimed to identify whether resistance exercise using elastic bands for six weeks can improve muscle strength, muscle tone, balance, and gait in patients with stroke. Methods : In total, 35 patients with stroke were randomly divided into three groups: resistance exercise using elastic band training combined with less affected side training group, more affected side training group, and both sides training group. Muscle strength, muscle tone, balance, and gait were assessed using a hand-held dynamometer, the modified Ashworth scale, the Berg Balance scale (BBS), and wireless 3-axis accelerometer, before and after training. Results : All three groups showed a significant increase in muscle strength of the lower extremity after training, and there was a significant difference among the groups. There was no change in muscle tone in all three groups. BBS scores increased significantly in all three groups after training, but these scores were not significantly different. The gait speed increased significantly in all three groups after the training, but the difference was not significant. The cadence increased significantly in Group 2 after training; however, there was no significant difference between Groups 1 and 3. There were no significant differences between the groups before and after the training. Step length increased significantly in Groups 2 and 3 after the training, but it was not significantly different in Group 1. After training, Groups 2 and 3 were significantly greater than Group 1 in the change in step length. Conclusion : The results show that resistance exercise using elastic bands can improve strength, balance, and gait in patients with chronic stroke. Especially, more affected side training was more effective in improving muscle strength than less affected side training. More affected side and both sides training are thought to be more effective than less affected side training to improve step length.

감각운동 훈련을 병행한 순환식 과제 지향 운동프로그램이 뇌졸중 환자의 보행 및 균형에 미치는 영향 : 예비연구 (The Effects of Task-Related Circuit Exercise Program Combined with Sensorimotor Training on Balance and Walking in Persons with Stroke : A pilot study)

  • Purpose: The purpose of this study was to identify whether task-related circuit exercise program combined with sensorimotor training for 4 weeks could improve the balance and gait in stroke patients. Method: Fifteen stroke patients who had agreed with the study were randomly divided into 3 groups categorized as task-related circuit exercise program combined with sensorimotor training group (experimental group 1, n=5), task-related circuit exercise program group (experimental group 2, n=5), and control subjects performed conventional physical therapy (control group, n=5). The balance and gait were assessed by BT-4 force platform system, Berg Balance Scale, 10meter Walk Test and Smart Step at before training and after training. Wilcoxon signed rank test was used to analyze change before and after intervention in intra-group. Kruskal Wallis H test, Mann-Whitney U test and Bonfferoni correction were used to analyze changes of all variables in inter-groups. Result: The experimental group 1 showed significant improvements in postural sway area, BBS scores, walking velocity and plantar pressures of affected foot, whereas the experimental group 2 showed significant improvements in BBS scores, and the control group were no significantly different in all variables following training. The changes of postural sway area and BBS scores in the experimental group 1 were significantly greater than them of the control group. The changes of postural sway area in the experimental group 1 was significantly greater than that of the experimental group 2. Conclusion: The result of this study suggest the task-related circuit exercise program combined with sensorimotor training is an effective intervention to improve balance and gait in stoke patients.

Methods : A total of 11 stroke patients who had agreed to participate in the study were randomly divided into 3 groups receiving elastic band resistance training on their less affected side (experimental group 1, n=4), affected side (experimental group 2, n=3), and both the sides (experimental group 3, n=4). Muscle strength, muscle tone, balance and gait were assessed using a hand-held dynamometer, the modified ashworth scale (MAS), the berg balance scale (BBS), and G-walk respectively, before and after training. Wilcoxon signed-rank test was used to analyze intergroup changes after the intervention, whereas the Kruskal Wallis H test, and Mann-Whitney U test were used to analyze intergroup changes in all variables. Results : Strength in all the muscle, except for the flexor and extensor muscles in the affected knees, was increased in all the groups, however, no significant difference was observed. No change in the muscle tone (MAS) was found in any group. All the groups showed increased BBS scores walking speed, and walking symmetry following training, although, the differences were not significant. Changes in the affected step length of experimental group 1 were significantly greater than those of experimental group 3, whereas changes in the affected step length in experimental group 3 were significantly greater than those of experimental group 2. Conclusion : Our results suggest that resistance exercise using elastic bands are a potential intervention for improving lower extremity muscle strength, balance, and gait among stoke patients.

과제 지향적 훈련프로그램이 뇌졸중 환자의 균형, 일상생활동작 수행능력 및 자기효능감에 미치는 영향 : 예비연구 (The Effects of Task-Oriented Training Program on Balance, Activities of Daily Living Performance and Self-Efficacy in Stroke Patients : A Pilot Study)

  • Purpose : The purpose of this pilot study was to examine the effects of task-oriented training program on balance, activities of daily living(ADL) performance, and self-efficacy in stroke patients. Method : Two subjects with stroke in experimental group participated in the task-oriented training program, while two subjects with stroke in control group received traditional rehabilitation therapy for 4 weeks, 30 minutes per session, four times per week. The task-oriented training program consisted of four tasks with 4 difficulty levels. In two groups, balance was examined with using the Berg Balance Scale(BBS), ADL performance was examined with using the Modified Barthel Index(MBI), and Self Efficacy was evaluated with using the Self-Efficacy scale(SES) before and after 4-week training. Result : After 4 weeks training, all scores of measurement variables increased in both the experimental group and the control group, but the average rates of change differed between the two groups. After the training program, the scores of BBS, MBI, and SES in experimental group increased to 11.4%, 9.9%, 15.4%, respectively than pre-training. Conclusion : According to the results of this study, task-oriented training program might be proposed as a intervention to improve balance ability, ADL performance, and self-efficacy in stroke patients.
