Both Gitelman syndrome and Bartter syndrome are autosomal recessively inherited renal tubular disorders characterized by hypokalemic metabolic alkalosis, salt wasting and normal to low blood pressure. Gitelman syndrome is caused by mutations in the thiazide-sensitive Na-Cl cotransporter (NCCT) and distinguished from Bartter syndrome, which is associated with mutations of several genes, by the presence of hypomagnesemia and hypocalciuria. In most of the patients with Gitelman syndrome, the disease manifests with transient episodes of muscular weakness and tetany in the adult period, but, often, is asymptomatic. We report here an 11 years-old female with Gitelman syndrome who presented with aggravation of epileptic seizure. The diagnostic work-up showed typical clinical features of metabolic alkalosis, hypokalemia, hypomagnesemia and hypocalciuria. We also identified a heterozygote mutation($^{642}$CGC(Arg)>TGC(Cys)) and an abnormal splicing in the SLC12A3 gene encoding NCCT.
This study was conducted to investigate fasting plasma secretin and postprandial secretin concentrations after ingestion of a protein meal or a sucrose solution in 20 healthy Korean subjects. In 12 subjects, ingestion of a protein meal, hamburger resulted in a significant and sustained increase in the mean plasma secretin concentrations, from mean fasting levels of less than 10 pg/ml to $12{\sim}16\;pg/ml$, and the mean plasma secretin concentrations, $9{\sim}13\;pg/ml$, after a rice meal increased significantly but transiently compared with mean fasting levels. The magnitude of postprandial increase in the Plasma secretin concentration after the hamburger was greater than that of the rice meal. In the remaining 8 subjects, drinking of a sucrose solution resulted also in a significant but transient increase in the mean Plasma secretin concentrations, from mean fasting levels of less than 10 pg/ml to $10{\sim}14\;pg/ml$ which were significantly greater than that after a physiological saline. Significant increase in the plasma secretin concentration was not observed during the postprandial period after the physiological saline. It is inferred from the above results that the Plasma secretin levels increase significantly after ingestions of a carbohydrate meal as well as a protein meal in the Korean race.

Motion control of the system is a position control of motor. Motion control of an uncertain robot system is considered as one of the most important and fundamental research directions in the robotics. Some distinguished works using linear control, adaptive control, robust control strategies based on computed torque methodology have been reported. However, it is generally recognized within the control community that these strategies suffer from the following problems : the exact robot dynamics are needed and hard to implement, the adaptive control cannot guarantee the performance during the transient period for adaptation under the variation, the robust control algorithms such as the sliding mode control need information on the bounds of the possible uncertainty and disturbance. And it produces a large control input as well. In this dissertation, a motion control for the unmanned intelligent robot system using disturbance observer is studied. This system is affected with an impact vibration disturbance. This paper describes a stable motion control of the system with the consideration of external disturbance. To obtain the stable motion independently against the external disturbance, the disturbance rejection is strongly required. To address the above issue, this paper presents a Disturbance OBserver(DOB) control algorithm. The validity of the suggested DOB robust control scheme is confirmed by several computer simulation results. And the experiments with a motor system is performed to give the validity of applicability in the industrial field. This results make the easier implementation of the controller possible in the field.
Monitor of All-sky X-ray Image (MAXI) is a Japanese X-ray all-sky surveyer mounted on the International Space Station (ISS). It has been scanning the whole sky since 2009 during every 92-minute ISS rotation. X-ray transients are quickly found by the real-time nova-search program. As a result, MAXI has issued 133 Astronomer's Telegrams and 44 Gamma-ray burst Coordinated Networks so far. MAXI has discovered six new black holes (BH) in 4.5 years. Long-term behaviors of the MAXI BHs can be classified into two types by their outbursts; a fast-rise exponential-decay type and a fast-rise flat-top one. The slit camera is suitable for accumulating data over a long time. MAXI issued a 37-month catalog containing 500 sources above a ~0.6 mCrab detection limit at 4-10 keV in the region ${\mid}{b}{\mid}$ > $10^{\circ}$. The SSC instrument utilizing an X-ray CCD has detected diffuse soft X-rays extending over a large solid angle, such as the Cygnus super bubble. MAXI/SSC has also detcted a Ne emission line from the rapid soft X-ray nova MAXI J0158-744. The overall shapes of outbursts in Be X-ray binaries (BeXRB) are precisely observed with MAXI/GSC. BeXRB have two kinds of outbursts, a normal outburst and a giant one. The peak dates of the subsequent giant outbursts of A0535+26 repeated with a different period than the orbital one. The Be stellar disk is considered to either have a precession motion or a distorted shape. The long-term behaviors of low-mass X-ray binaries (LMXB) containing weakly magnetized neutron stars are investigated. Transient LMXBs (Aql X-1 and 4U 1608-52) repeated outbursts every 200-1000 days, which is understood by the limit-cycle of hydrogen ionization states in the outer accretion disk. A third state (very dim state) in Aql X-1 and 4U 1608-52 was interpreted as the propeller effect in the unified picture of LMXB. Cir X-1 is a peculiar source in the sense that its long-term behavior is not like typical LMXBs. The luminosity sometimes decreases suddenly at periastron. It might be explained by the stripping of the outer accretion disk by a clumpy stellar wind. MAXI observed 64 large flares from 22 active stars (RS CVns, dMe stars, Argol types, young stellar objects) over 4 years. The total energies are $10^{34}-10^{36}$ erg $s^{-1}$. Since MAXI can measure the spectrum (temperature and emission measure), we can estimate the size of the plasma and the magnetic fields. The size sometimes exceeds the size of the star. The magnetic field is in the range of 10-100 gauss, which is a typical value for solar flares.

During a model test of Hutton TLP, a "ringing" response was first observed about 20 years ago. This phenomenon is a resonant build up over the time of wave period and this burst-like motion can cause the extreme load on the TLP tether. It is often detected in the large and steep irregular waves but the generation mechanism leading to the "ringing" is not yet well understood. According to the research since then, the higher order harmonic components may account for the "ringing" on the floating offshore structures. The main purpose of the present research is, thus, to measure the higher harmonic forces exerted on a vertical truncated circular column and to compare them with available data. A vertical truncated cylinder with a diameter of 3.5inch and a draft of 10.5inch is used as a test structure, which is a scaled model of ISSC TLP column. The cylinder is installed at a distance of 45ft from the wave maker in order to avoid parasitic waves created in the wave flap. Attached to the upper part of the cylinder are two force gages to measure the horizontal (surge) and vertical (heave) forces on the cylinder. The incoming waves are Stokes waves with a slope ranging from 0.06 to 0.24. The forces and waves are measured for 60 seconds with a sampling rate of 50 Hz. Among the recorded data, the first 10 waves are excluded because of transient behavior of the waves and the next The horizontal and vertical forces are analyzed up to 5th order harmonics. The horizontal forces are then compared to the values from the theoretical model called "FNV model". In addition, force transfer functions are also investigated. Major findings in this research are below. 1) The first order forces measured are slightly larger than the theoretical values of "FNV model" 2) The "FNV model" considerably overpredicts the second order forces. 3) The larger the amplitude and more extreme the wave slope, the smaller the predictions are compared to the experimental. 4) The higher harmonic forces are significantly smaller than the first harmonic force for all wave parameters. 5) The normalized forces vs. waves slopes are almost constant in the lower harmonics but vary a lot in the higher harmonics. 6) The trend of forces is more nonlinear in the horizontal forces than in the vertical forces as the wave slope increases. 7) The part of the results above is also observed by other researchers and confirmed again through the present work.
Most of the currently available rice threshers in Korea are of head-feeding type originally developed for threshing paddy at grain moisture content of 15 to 17% on wet basis. And prevailing rice harvesting system is swathing or cutting and bundling upright rice crop at a maturity stage of grain moisture content of ca. 28 to 20%, followed by transient natural drying in the field and threshing at grain moisture content of about 17%. The system often allows rather high field loss of rice both quantitatively and qualitatively. These necessitate use of a preharvest desiccant, and diquat has been registered recently as a rice desiccant. Aim of the present study is to determine the best time and rate of diquat application and to evaluate its utility as a rice desiccant under its optimal usage conditions in Korea. Diquat was not so effective for japonica rice varieties while it was very effective for the new varieties bred from indica x japonica crosses. The best time of application was the period from 2 days before to 3 days after physiological maturity of grain (moisture content of 28% w.b.) and the optimal rate was about 330g of diquat ion/ha. Applying diquat at the right time and rate dropped grain moisture to 15 to 16% (w.b.) within a week after treatment in contrast to that of 20 to 24% in untreated rice. Desiccation of rice crop with diquat resulted in 10 to 15% increase in grain resistance to shattering and significant reduction in percent green rice. However, there was no reduction in milled rice yield and percent whole kernel and was no change in the percent chalky rice and percent tinted rice. Diquat use appears highly desirable, if there is no residue problem which has not been studied in the present study.
In general, understanding groundwater flow in fractured bedrock is critical during tunnel and underground cavern construction. In that case, borehole data may be useful to examine groundwater flow properties of the fractured bedrock from pre-excavation until completion stages, yet sufficient borehole data is not often available to acquire. This study evaluated groundwater discharge rate into Jikri tunnel in Gyeonggi province using hydraulic parameters, groundwater level data in the later stage of tunneling, national groundwater monitoring network data, and electrical resistivity survey data. Groundwater flow rate into the tunnel by means of analytical method was estimated $7.12-74.4\;m^3/day/m$ while the groundwater flow rate was determined as $64.8\;m^3/day/m$ by means of numerical modeling. The estimated values provided by the numerical modeling may be more logical than those of the analytical method because the numerical modeling could take into account spatial variation of hydraulic parameters that was not possible by using the analytical method. Transient modeling for a period of one year from the tunnel completion resulted in the recovery of pre-excavation groundwater level.
Background: Surgical repair of partial anomalous pulmonary venous return (PAPVR) to the superior vena cava (SVC) using the Warden procedure has favorable outcomes. However, there remain some concerns after the Warden procedure, such as sinoatrial nodal dysfunction and systemic or pulmonary venous stenosis. We investigated the outcomes of the Warden procedure for repair of PAPVR to the SVC. Methods: This retrospective study included 22 consecutive patients who underwent the Warden procedure for PAPVR between 2002 and 2018. The median age and body weight at operation were 27.5 months (interquartile range [IQR], 5.0-56.8 months) and 13.2 kg (IQR, 6.5-16.0 kg), respectively. The median follow-up duration was 6.2 years (IQR, 3.5-11.6 years). Results: There were no cases of early or late mortality. No patients had postoperative heart rhythm problems, except 1 patient who showed transient sinoatrial nodal dysfunction in the immediate postoperative period. Procedure-related complications requiring reintervention occurred in 5 patients, including 3 of 4 SVC stenosis cases and 2 pulmonary venous stenosis cases during follow-up. The rate of freedom from reintervention related to the Warden procedure was 75.9% at 10 years. Conclusion: In cases requiring extension or creation of an atrial septal defect to achieve a sufficient venous pathway, or interposition of an entire circumferential conduit between the SVC and right atrium due to the shortness of the SVC in the Warden procedure, stenotic complications of the venous pathway occurred. Careful observation of changes in the pressure gradient or anatomical stenosis is required in such patients.

Coronavirus disease 2019 (COVID-19) vaccination may non-specifically alter the host immune system. This study aimed to evaluate the effect of COVID-19 vaccination on hepatitis B surface Ag (HBsAg) titer and host immunity in chronic hepatitis B (CHB) patients. Consecutive 2,797 CHB patients who had serial HBsAg measurements during antiviral treatment were included in this study. Changes in the HBsAg levels after COVID-19 vaccination were analyzed. The dynamics of NK cells following COVID-19 vaccination were also examined using serial blood samples collected prospectively from 25 healthy volunteers. Vaccinated CHB patients (n=2,329) had significantly lower HBsAg levels 1-30 days post-vaccination compared to baseline (median, -21.4 IU/ml from baseline), but the levels reverted to baseline by 91-180 days (median, -3.8 IU/ml). The velocity of the HBsAg decline was transiently accelerated within 30 days after vaccination (median velocity: -0.06, -0.39, and -0.04 log10 IU/ml/year in pre-vaccination period, days 1-30, and days 31-90, respectively). In contrast, unvaccinated patients (n=468) had no change in HBsAg levels. Flow cytometric analysis showed that the frequency of NK cells expressing NKG2A, an NK inhibitory receptor, significantly decreased within 7 days after the first dose of COVID-19 vaccine (median, -13.1% from baseline; p<0.001). The decrease in the frequency of NKG2A+ NK cells was observed in the CD56dimCD16+ NK cell population regardless of type of COVID-19 vaccine. COVID-19 vaccination leads to a rapid, transient decline in HBsAg titer and a decrease in the frequency of NKG2A+ NK cells.

Purpose : Since Mendoza(1990)'s report that long term methylprednisolone pulse therapy by Mendoza protocol (MP therapy) is a good treatment option in focal segmental glomerulosclerosis(FSGS), there have been reports of the effects of this therapy in steroid-resistant nephrotic syndrome. However, no studies have been performed on the effects of MP therapy in steroid- dependent nephrotic syndrome and secondary nephrotic syndrome. In this study, we investigated the effects of long term MP therapy in primary and secondary nephrotic syndrome in which previous treatment options were not effective. Methods : We chose 10 children who were diagnosed with steroid-dependent minimal change nephrotic syndrome(SD-MCNS), who had shown frequent relapse during the immunocompromised or cytotoxic therapy Period, and 6 children with FSGS and 5 children with secondary nephrotic syndrome children, who had shown no response during the previous therapy period. We treated these patients according to Mendoza protocol involving infusions of high doses of methylprednisolone, often in combination with oral cyclophosphamide for 82 weeks. Results : In all the 10 children with SD-MCNS, complete remission was visible on average of $18{\pm}9$ days after MP therapy was started. However, all these children relapsed during or after MP therapy. In these children, the mean relapse rate prior to MP therapy was $2.1{\pm}1.0$ relpases/year, which was reduced to $1.4{\pm}0.9$ relapses/year during MP therapy(P>0.05) and rose to $2.7{\pm}1.0$ relapse/year after MP therapy. Of the 6 children with FSGS, 4 children($67\%$) showed complete remission, of whom 3 children($50\%$) remained in the remission status during the follow up period, $1.2{\pm}0.7$ years, after the end of MP therapy. 2 children($33\%$) showed no response. All of the 5 children with secondary nephrotic syndrome showed remission and remained in the remissiom status during the follow up period, $1.7{\pm}0.6$ years The only side effect of MP therapy was transient hypertension in 10 children of ail subjects during the intravenous infusion of methylprednisolone. Conclusion : We conclude that although long term MP therapy is not effective in the treatment of SD-MCNS, it is an effective therapy against intractable FSGS and secondary nephrotic syndrome.
