A Case of Gitelman Syndrome Presented with Epileptic Seizure (간질 발작으로 내원하여 진단된 Gitelman 증후군 1례)

  • Both Gitelman syndrome and Bartter syndrome are autosomal recessively inherited renal tubular disorders characterized by hypokalemic metabolic alkalosis, salt wasting and normal to low blood pressure. Gitelman syndrome is caused by mutations in the thiazide-sensitive Na-Cl cotransporter (NCCT) and distinguished from Bartter syndrome, which is associated with mutations of several genes, by the presence of hypomagnesemia and hypocalciuria. In most of the patients with Gitelman syndrome, the disease manifests with transient episodes of muscular weakness and tetany in the adult period, but, often, is asymptomatic. We report here an 11 years-old female with Gitelman syndrome who presented with aggravation of epileptic seizure. The diagnostic work-up showed typical clinical features of metabolic alkalosis, hypokalemia, hypomagnesemia and hypocalciuria. We also identified a heterozygote mutation($^{642}$CGC(Arg)>TGC(Cys)) and an abnormal splicing in the SLC12A3 gene encoding NCCT.

Assessment of Cerebral Collateral Circulation Using $^{99m}Tc$-Hexamethyleneamine Oxime (HMPAO) SPECT During Internal Carotid Artery Balloon Test Occlusion (내경동맥 풍선 시험 결찰술(BTO)시 $^{99m}Tc$-HMPAO 뇌 SPECT를 이용한 대뇌 측부 순환의 평가)

  • To predict preoperatively the safety of permanent occlusion of an internal carotid artery with $^{99m}Tc$-HMPAO brain single photon emission computed tomography(SPECT) from an objective point of view, Twenty-four patients underwent balloon test occlusion (BTO) of the internal carotid arteries because of neck and skull base tumors. The authors assessed the uptake of both middle cerebral artery territories before and during BTO with $^{99m}Tc$-HMPAO brain SPECT using semiquantitative analysis method and compared the results with other factors(neurologic examination, arterial stump pressure and electroenceph-alogram). Nineteen patients had not experienced neurological deteriorating or any problem during BTO. Their comparative uptakes of the middle cerebral artery territories were 95 to 101% of the pre-BTO state. The remaining five patients showed severe neurologic symptoms such as transient hemiplegia and unconsciousness. Their comparative uptake of the middle cerebral artery territories were 77 to 85% of the pre-BTO state, and were well matched with other factors. $^{99m}Tc$-HMPAO brain SPECT before and during BTO seems to be a simple and objective method for prediction of permanent neurologic deficits when the comparative uptake of middle cerebral artery territories during BTO is lower than 85% of that before BTO.

Effect of Heat-epimerized-catechin-mixture Rich in Gallocatechin-3-gallate on Skin Barrier Recovery (갈로카테킨-3-갈레이트가 풍부한 열전환 카테킨의 피부 장벽 회복에 대한 개선 효과)

  • Until now, (-)-epigallocatechin-3-gallate(EGCG) is known as the most powerful antioxidant among green tea catechins having many beneficial effects on human skin. Considering that the content of catechins is variable according to many conditions such as solvent, temperature and pressure, we prepared the heat-epimerized-EGCG-mixture (HE-EGCG-mix) containing high content of gallocatechin-3-gallate(GCG) by epimerization during autoclaving process and found out its optimal condition for maximizing conversion from EGCG to GCG. To investigate the effects of EGCG and HE-EGCG-mix on skin barrier function, we performed in vivo experiments with hairless mice. We found that HE-EGCG-mix has more potent stimulating activity than EGCG for the production of involucrin 7(INV7) and for recovery of barrier function in SKH-1 mice. Also, we found that GCG stimulates $PPAR-{\alpha}$ transactivation more effectively than EGCG in vitro by transient transfection assay for $PPAR-{\alpha}$ activation activity. These imply that HE-EGCG-mix consisting of high content of GCG should stimulate more efficiently recovery of skin barrier through PPAR-mediated-kerationocyte differentiation than EGCG. In conclusion, our study may provide a possibility that GCG, the C-2 epimer of EGCG, could be a potentially effective agent for development of new cosmetics or health foods for recovery of skin barrier.

A Study on Estimation of Initial Gas in Place for Coalbed Methane Field Using Production Data at Canada (생산자료를 이용한 캐나다 CBM 원시부존량 평가 연구)

  • This paper presents the prediction of the original gas in place(OGIP) by using the material balance method and decline curve analysis method with production history and pressure transient test data for four coalbed methane wells in the Horseshoe Canyon field. In this study, the conventional gas equation and the Jensen and Smith(J&S) equation were used to material balance analysis, and the Arps' empirical correlation and Khaled method were applied to decline curve analysis. From the results, the OGIP estimated from the conventional gas and the J&S method was small in difference as under 12%. Also, in case of decline curve analysis, it was found that the Khaled method has appropriated to calculate the OGIP, because the OGIP was estimated as unlimited value by the Arps' equation from the decline exponent of 1 - 3.5. The OGIP difference between conventional gas method and Khaled method was calculated as 8.67% ~ 31.04%, and those between J&S method and Khaled method was 13.67% ~ 26.49%.

  • This is an explicit-Implicit, finite element analysis for linear as well as nonlinear hygrothermal stress problems. Additional features, such as moisture diffusion equation, crack element and virtual crack extension(VCE ) method for evaluating J-integral are implemented in this program. The Linear Elastic Fracture Mechanics(LEFM) Theory is employed to estimate the crack driving force under the transient condition for and existing crack. Pores in materials are assumed to be saturated with moisture in the liquid form at the room temperature, which may vaporize as the temperature increases. The vaporization effects on the crack driving force are also studied. The Ideal gas equation is employed to estimate the thermodynamic pressure due to vaporization at each time step after solving basic nodal values. A set of field equations governing the time dependent response of porous media are derived from balance laws based on the mixture theory Darcy's law Is assumed for the fluid flow through the porous media. Perzyna's viscoplastic model incorporating the Von-Mises yield criterion are implemented. The Green-Naghdi stress rate is used for the invariant of stress tensor under superposed rigid body motion. Isotropic elements are used for the spatial discretization and an iterative scheme based on the full newton-Raphson method is used for solving the nonlinear governing equations.

  • The Ulchin 3&4, which are 2825 MWt PWRs, adopted Safety Depressurization System (SDS) to mitigate the beyond design basis event of Total Less of Feedwater(TLOFW). In this study the results and methodology of the analyses for the determination of SDS bleed capacity are discussed. The SDS design bleed capacity has been determined from the CEFLASH-4AS/REM simulation according to the following design criteria : 1) Each SDS flow path, in conjunction with one of two High Pressure Safety Injection (HPSI) pumps, is designed to have a sufficient capacity to prevent core uncovery if one SDS path is opened simultaneously with the opening of the Pressurizer Safety Valves (PSVs). 2) Both SDS bleed paths are designed to have sufficient total capacity with both HPSI pumps operating to prevent core uncovery if the Feed and Bleed (F&B) initiation is delayed up to thirty minutes from the time of the PSVs lift. To verify the results of CEFLASH-4AS/REM simulation a comparative analysis kas also been per-formed by more sophisticated computer code, RELAP5/MOD3. The TLOFW event without operator recovery and TLOFW event with F&B are analyzed. The predictions by the CEFLASH-4AS/REM of the transient too phase system behavior are in good qualitative and quantitative agreement with those by the RELAP5/MOD3 simulation. Both of the results of analyses by CEFLASH-4AS/REM and RELAP5/MOD3 have demonstrated that decay heat removal and core inventory make-up can be successfully accomplished by F&B operation during now event for the Ulchin 3&4.

  • Slot coating process, in continuous and patch modes, has been applied for the many precise coating products, e.g., flat panel displays and second batteries. However, manufacturing uniform coating products is not a trivial task at high-speed operations because various flow instabilities or defects such as leaking, bubbles, ribbing, and rivulets are frequently observed in this process. It is no wonder, therefore, that many efforts to understand the various aspects of dynamics and coating windows of this process have been made both in academia and industry. In this study, as the first topic, flow dynamics within the coating bead in slot coating process has been investigated using the one-dimensional viscocapillary model by lubrication approximation and two-dimensional model by Flow-3D software. Especially, operability windows in both 1D and 2D cases with various slot die lip designs have been successfully portrayed. Also, effects of process conditions like viscosity and coating gap size on slot coating window have been analyzed. Also, some experiments to find minimum coating thickness and coating windows have been conducted using slot die coater implemented with flow visualization device, corroborating the numerical results. As the second topic, flow dynamics of both Newtonian and Non-Newtonian fluids in patch or pattern slot coating process, which is employed in manufacturing IT products such as secondary batteries, has been investigated for the purpose of optimal process designs. As a matter of fact, the flow control in this system is more difficult than in continuous case because od its transient or time-dependent nature. The internal die and die lip designs for patterned uniform coating products have been obtained by controlling flow behaviors of coating liquids issuing from slot. Numerical simulations have been performed using Fluent and Flow-3D packages. Flow behavior and pressure distribution inside the slot die has been compared with various die internal shapes and geometries. In the coating bead region, efforts to reduce irregular coating defects in head and tail parts of one patterned coating unit have been tried by changing die lip shapes. It has been concluded that optimal die internal design gas been developed, guaranteeing uniform velocity distribution of both Newtonian and shear thinning fluids at the die exit. And also optimal die lip design has been established, providing the longer uniform coating layer thickness within one coating unit.

Scenario-based Vulnerability Assessment of Hydroelectric Power Plant (시나리오 기반 수력플랜트 설비의 취약성 평가)

  • Recently, the importance of eco-friendly power generation facility using renewable energy has newly appeared. Hydropower plant is a very important source of electricity generation and supply which is very important to secure safety because it is commonly connected with multi facility and operated on a large scale. In this study, a scenario-based analysis method was suggested to assess vulnerability of a penstock system caused by water hammer commonly occurred in the operation of hydropower plants. A hypothetical hydropower plant was used to demonstrate the applicability of a transient analysis model. In order to verify reliability of the model, the prediction of pressure behaviors were compared with the results of commercial model (SIMSEN) and measured data, then a real hydroelectric power plant was applied to develop all potential water hammer scenarios during the actual operation. The scenario-based simulation and vulnerability assessment for water hammer in the penstock system were performed with internal and external load conditions. The simulation results indicated that the vulnerability of a penstock system was varied with the operating conditions of hydropower facilities and significantly affected by load combination consisting of different load scenarios. The proposed numerical method could be an useful tool for the vulnerabilityty assessment of the hydropower plants due to water hammer.

Outcomes of the Warden Procedure for Anomalous Pulmonary Venous Return to the Superior Vena Cava: A 17-Year Experience

  • Background: Surgical repair of partial anomalous pulmonary venous return (PAPVR) to the superior vena cava (SVC) using the Warden procedure has favorable outcomes. However, there remain some concerns after the Warden procedure, such as sinoatrial nodal dysfunction and systemic or pulmonary venous stenosis. We investigated the outcomes of the Warden procedure for repair of PAPVR to the SVC. Methods: This retrospective study included 22 consecutive patients who underwent the Warden procedure for PAPVR between 2002 and 2018. The median age and body weight at operation were 27.5 months (interquartile range [IQR], 5.0-56.8 months) and 13.2 kg (IQR, 6.5-16.0 kg), respectively. The median follow-up duration was 6.2 years (IQR, 3.5-11.6 years). Results: There were no cases of early or late mortality. No patients had postoperative heart rhythm problems, except 1 patient who showed transient sinoatrial nodal dysfunction in the immediate postoperative period. Procedure-related complications requiring reintervention occurred in 5 patients, including 3 of 4 SVC stenosis cases and 2 pulmonary venous stenosis cases during follow-up. The rate of freedom from reintervention related to the Warden procedure was 75.9% at 10 years. Conclusion: In cases requiring extension or creation of an atrial septal defect to achieve a sufficient venous pathway, or interposition of an entire circumferential conduit between the SVC and right atrium due to the shortness of the SVC in the Warden procedure, stenotic complications of the venous pathway occurred. Careful observation of changes in the pressure gradient or anatomical stenosis is required in such patients.

Effects of Aqua-Acupuncture of Carthami Flos(GV15) on the Changes of Cerebral Hemodynamics in Rats (아문(瘂門)(GV15)의 홍화약침(紅花藥針)이 백서(白鼠)의 뇌혈류역학(腦血流力學) 변동(變動)에 미치는 영향(影響))

  • Objective : Carthami Flos has been used as a herb to promote blood circulation to remove blood stasis in oriental medicine for many centuries, and Amun(GV15) has been used as a meridian point to treat apoplexy etc. To investigate treatment of cerevral vascular disease(CVA) by promoting blood circulation and removing blood stasis(活血化瘀法), we observed the experimental effects and mechanism of auqa-acupunture of Carthami Flos(ACF) injected into GV15 on cerevral hemodynamics and cardiovascular system of rats. Method : Aqua-acupuncture of Carthami Flos(ACF) was injected into GV15, and then we investigated experimental effects and mechanism of ACF on the cerebral hemodynamics[regional cerebral blood flow(rCBF), pial arterial diameter(PAD), meal arterial blood pressure(MABP)] and cardiovascular system[cardiac muscle contractile force(CMF), heart rate(HR)I by pretreatment with methylene blue(MTB) and indomethacin(IDN). The changes in rCBF, MABP, CMF and HR were tested by Laser Doppler Flowmetry(LDF), and the changes in PAD was determinated by video microscopy methods and video analyzer. Results :The results were as follows in normal rats ; The changes of rCBF and PAD were significantly increased by ACF($120{\mu}{\ell}/kg$) in a injected time-dependent manner, but MABP was not changed by ACF. The changes of cardiovascular system were increased by ACF in a injected time-dependent manner. And pretreatment with MTB was significantly inhibited ACE induced increase of rCBF and PAD, and was decreased ACF induced increase of HR. And pretreatment with IDN was increased ACF induced MABP and CMF. And the results were as follows in cerebral ischemic rats ; The changes of rCBF was increased stabilizly by treatment with ACF($120{\mu}{\ell}/kg$) in during the period of cerebral reperfusion, but pretreatment with MTB was increased ACF induced increase of rCBF during the period of cerebral reperfusion. The results were as follows in normal rats ; The changes of rCBF and PAD were significantly increased by ACF($120{\mu}{\ell}/kg$) in a injected time-dependent manner, but MABP was not changed by ACF. The changes of cardiovascular system were increased by ACF in a injected time-dependent manner. I suggested that ACF has an anti-ischemic effect through the improvement of crebral hemodynamics in a transient cerebral ischemia.
