관절가동술과 TENS가 만성요통환자의 통증역치 및 호르몬 변화에 미치는 영향 (The Effects of Joint Mobilization and TENS on Pain Threshold and Hormonal Changes in Patients with Chronic Low Back Pain)

  • Background: The purpose of this research is to investigate the effects of joint mobilization and transcutaneous electrical nerve stimulation (TENS) on pain threshold and hormonal changes in patients with chronic LBP. Methods: 14 patients with chronic LBP were divided into the experimental group and the control group with 7 patients each, and both groups of patients were evaluated on pain duration, blood tests, and pressure pain threshold (PPT) on their first visit. On their second visit, joint mobilization was applied to the experimental group for 10 min and TENS to the control group for 20 min. Results: Pain threshold and duration of analgesia increased from after treatment in the experimental group. Serotonin and Cortisol decreased after treatment in the experimental group. Conclusion: Joint mobilization in patients with chronic LBP caused an increase in pain duration and pain thresholds and a decrease in serotonin, whereas there was little difference in cortisol.

Influence on Autonomic Function Before, During, and After Transcutaneous Electrical Nerve Stimulation in Senile Patients with Chronic Low Back Pain

  • Twenty five patients, 14 males and 11 females with an average age of 75years with chronic Low Back Pain were tested before, during, and after 10minutes transcutaneous electrical nerve stimulation. It is important to consider the effect of TENS on the autonomic function because the pain pathway and the ANS pathway are somehow correlated to each other. There is still controversy among researchers who have reported the effect of TENS on autonomic function. So the purpose of this study is to measure the effect of TENS on the ANS and to see the relation of the pain control mechanism and the change in the ANS. The results were as follows : 1) Systolic blood pressure between before and during the experiment was apt to be a little increased, but statistically there was no significant change(P>.05). 2) Systolic blood pressure between during and after 10 minutes the experiment was apt to be a little decreased, but statistically there was no significant change(P>.05). 3) Systolic blood pressure between before and after 10 minutes experiment was apt to be a little decreased, but statistically there was no significant change(P>.05). 4) Diastolic blood pressure between before and during experiment was apt to be a little increased, but statistically there was no significant change(P>.05). 5) Diastolic blood pressure between during and after 10 minutes experiment was significantly decreased(P<.05). 6) Diastolic blood pressure between before and after 10 minutes the experiment was apt to be a little decreased, but statistically there was no significant change(P>.05). 7) Heart rate between before and during the experiment was apt to be a little decreased, but statistically there was no significant change(P>.05). 8) Heart rate between during and after 10 minutes the experiment was significantly decreased(P<.05). 9) Heart rate between before and after 10 minutes the experiment was significantly decreased(P<.05). 10) Skin temperature between before and during the experiment was significantly increased(P<.05). 11) Skin temperature between during and after 10 minutes the experiment was apt to be a little increased, but statistically there was no significant change(P>.05). 12) Skin temperature between before and after 10 minutes the experiment was significantly increased(P>.05). 13) Respiratory rate between before and during the experiment was apt to be a little increased, but statistically there was no significant change(Pgt;.05). 14) Respiratory rate between during and after 10 minutes the experiment was apt to be a little decreased, but statistically there was no significant change(Pgt;.05). 15) Respiratory rate between before and after 10 minutes the experiment was apt to be a little decreased, but statistically there was no significant change(P>.05).

The effects of complex decongestive therapy on pain and functionality in individuals with breast cancer who developed adhesive capsulitis due to lymphedema: an evaluation by an isokinetic computerized system

  • Background: This study aimed to determine the impact of complex decongestive therapy applications on upper extremity function in breast cancer patients who developed adhesive capsulitis after lymphedema. Methods: Thirty patients who developed adhesive capsulitis due to lymphedema were divided into two groups as study (n = 15) and control (n = 15) groups. Both groups received 20 minutes of exercise five days a week for three weeks using a Biodex isokinetic dynamometer, as well as a hot pack and TENS (Transcutaneous Electrical Nerve Stimulation) treatment to the shoulder joint. The study group received 45 minutes of intensive decongestive therapy along with the adhesive capsulitis treatment. The visual analogue scale was used to assess pain, circumference, and volumetric measurements were used to assess edema, and the Arm, Shoulder, and Hand Problems Questionnaire (DASH: Disabilities of the Arm, Shoulder, and Hand) was used to assess upper extremity functionality. The shoulder range of motion was evaluated. Results: Both groups had improvements in pain (P < 0.001), shoulder joint range of motion (P < 0.001), and upper extremity functionality (P < 0.001) after the treatment. There was a significant decrease in circumference and volumetric measurements in the study group (P < 0.001). However, no differences were seen in measurements in the control group. Conclusions: The results showed that complex decongestive therapy was beneficial in reducing lymphedema in breast cancer patients who acquired adhesive capsulitis due to lymphedema. Consequently, the authors believe that supplementing conventional physiotherapy with complex decongestive therapy will benefit patients.

  • Chronic pelvic pain in women is a very annoying condition that is responsible for substantial suffering and medical expense. But dealing with this pain can be tough, because there are numerous possible causes for the pelvic pain such as urologic, gynecologic, gastrointestinal, neurologic, or musculoskeletal problems. Of these, musculoskeletal problem may be a primary cause of chronic pelvic pain in patients with a preceding trauma to the low back, pelvis, or lower extremities. Here, we report the case of a 54-year-old female patient with severe chronic pelvic pain after a transcutaneous electrical nerve stimulation (TENS) accident that was successfully managed with image-guided trigger point injections on several pelvic stabilizing muscles.

  • This study was to investigate diagnosis of shoulder pain on cerebrovascular attack patients and treatment effects by using physical treatment and orienatal diagnosis aparatus, as oriental diagnosis equipments, these were used thermography, maegzin and yangdorag machine. as physical treatment equipments, these were used Hot pack, TENS (Transcutaneous electrical nerve stimulation), ICT(Interferential current therapy) the results were followed 1. In Sex and Age distribution on cerebrovascular attack patients with shoulder pain, male(51.7%)had many than female(48.3%) and 60s were the top, next 70s 50s 40s. 2. In the reason of cerebrovascular attack and the type of motor paresis, cerebrovascular infarction(73.3%) had many than hemorrhage(26.7%) 3. In hospitalizied period of cerebrovascular attack patients with shoulder pain, the period from 11 to 60 days had almost occupied. 4. Oriental medical diagnositic propriety of shoulder pain by using maegzin and yangdorag machine were not recognized. 5. In measuring passive range of motion of pre and post treatment, improvement effect on Hot pack was not showed, but effects on TENS, ICT were showed. 6. In measuring temperature of painful shouler joint of pre and post treatment by using thermography, the effect of Hot pack was not recognized but the effects of TENS, ICT were recognized.

경추 신연요법을 적용한 연성 경추 추간판 탈출증 환자 치험 1례 (A Case Report on Herniated Intervertebral Cervical Disc Using Chuna Manual Theraphy)

  • Objectives: The object of this study is to report a clinical effect of chuna manual theraphy for the soft herniated intervertebral cervical disc. Methods: The patient was treated by cervical traction technique which is one of the chuna manual theraphy with other conservative treatments including acupuncture, herbal mixture, bee-venom theraphy and TENS(Transcutaneous Electrical Nerve Stimulation). Visual analogue scale(VAS) have been used for result report. Results: After the cervical traction technique, the VAS was significantly improved. Conclusions: It is thought that the chuna manual theraphy might be effective to the patient with soft herniated intervertebral cervical disc.

본태성 수전증으로 의심되는 환자에 대한 추나요법 병행치료 증례보고 (A Case Report of Suspected Essential Tremor Treated by Chuna Manual Therapy)

  • Objectives : The purpose of the this study is to report the improvement after Chuna manual therapy about patient with essential tremor. Methods : The patient of essential tremor was treated by Myofascial Release Technique(MRT) and Chuna manual therapy with acupuncture, cupping, Transcutaneous electrical nerve stimulation(TENS), herbal medicine therapy. Results : According to evaluation of Handwriting test and VAS, The patient has shown improvement aftert Chuna Manual Therapy. Conclusions : This study suggest that Chuna manual therapy with other treatment would be effective for patient of essential tremor.

상부 승모근 근막 통증 환자에게 경피신경자극이 근 활성도에 미치는 영향 (Effect of Transcutaneous Electrical Nerve Stimulation on Muscle Activity of Upper Trapezius in Subjects With Myofascial Pain Syndrome)

  • 본 연구에서는 상부 승모근의 근막통증(myofascial pain syndrome; MPS)를 대상으로 이완상태에서 근 활성도를 측정해보고, 통증을 감소시키는 데 주로 이용되는 경피신경자극 치료 후 근 활성도에 어떠한 영향을 미치는지 알아보기 위해 실시하였다. 본 연구의 대상자는 근막 통증으로 진단을 받은 총 10명을 대상으로 실시하였다. 주관적 시각 척도(visual analogue scale; VAS)와 압통 역치 측정계(pressure threshold meter)를 이용하여 경피신경자극 전, 후 통증의 정도를 평가하였고, 표면 근전도를 이용하여 이완시 근 활성도를 측정하였다. 치료 기구는 경피신경자극기(TENS: HAT-2000)를 이용하였다. 치료 전과 비교하여 VAS는 통증이 심한 쪽과 약한 쪽 모두에서 유의하게 감소하였으며(p<.05), 압통 역치는 통증이 심한 쪽과 약한 쪽 모두에서 유의한 차이가 없었고(p>.05), 근 활성도는 통증이 심한 쪽에서 유의하게 감소하였다(p<.05).

양성돌발체위현기증 환자 치험 1례 (Case Study of Peripheral Vertigo and Nausea Diagnosed as Benign Paroxysmal Positional Vertigo Improved by Traditional Korean Medicine)

  • Objective: We report a case of a 73-year-old Korean male with vertigo and nausea, both of which were brought about by head repositioning. The condition was diagnosed as benign paroxysmal positional vertigo (BPPV).Method: The patient was treated with acupuncture, electroacupuncture, cupping, transcutaneous electrical nerve stimulation (TENS), and herbal medicine (Bosimsahwacheonggan-tang [補心瀉火淸肝湯]). We conducted the vertigo symptoms scale (VSS) and the dizziness handicap inventory (DHI) to evaluate the degree of the patient’s symptoms.Results: Vertigo and nausea started to improve after the administration of Bosimsahwacheonggan-tang, but although the patient took the anti-vertigo medication and a tranquilizer after the symptoms became aggravated, no prominent improvements could be observed.Conclusion: These results suggest that traditional Korean medical treatment may be effective in treating peripheral vertigo due to BPPV.

Effects of Lumbar Mobilization on the Paravertebral Muscle Activity and Muscle Tone in Patients with Lumbar Spinal Stenosis

  • Background: Patients with lumbar spinal stenosis show abnormal changes in muscle activity due to pain and limited range of motion of the lumbar spine. Excessive increased muscle tone and decreased muscle activity patterns threaten the patients' quality of life. However, there have been a few studies showing how to improve muscle performance in patients with lumbar spinal stenosis. Among these, joint mobilization is one way of improving muscle performance through pain relief and increasing the range of motion. Objectives: To investigate the effect of lumbar mobilization by orthopedic manual physical therapy on paravertebral muscle activity and tone in patients with lumbar spinal stenosis. Design: A randomized controlled trial. Methods: In this study, 24 patients with lumbar spinal stenosis were randomized (1:1 ratio) into two groups. The experimental group underwent lumbar posteroanterior mobilization, and the control group underwent conventional physical therapy (conventional transcutaneous electrical nerve stimulation) for 15 minutes each. For outcome measures, Myoton®PRO was used to evaluate muscle tone when resting of the paravertebral muscle in the pain area. For muscle activity evaluation, the reference voluntary contraction of the paravertebral muscle was evaluated using surface electromyography. Results: Muscle tone and activity were significantly improved after intervention in both the experimental and control groups. In addition, the experimental group showed more significant decrease in muscle tone and activity than the control group. Conclusion: These results suggest that lumbar mobilization improving muscle performance in patients with lumbar spinal stenosis.
