종합병원 간호사의 다문화 가정에 대한 인식 (The Perception of Multicultural Families on Hospital Nurses)

  • Purpose: The purpose of this descriptive study was to determine the perception of nurses on multicultural families. Methods: The study population consisted of 493 nurses from eight general hospitals in B city. Data collection was done from June 1 to July 31, 2012. The Perception for Multicultural Family Inventory developed by Jun and Kim (2010) was used as the study instrument. The collected data were analyzed by t-test and ANOVA using SPSS/WIN version12.0. Results: Perception of nurses for multicultural families was 3.64 points out of 5. Most of the nurses considered that multicultural education is necessary. However, there were only a small number of nurses received training on multiculturalism. Conclusion: The results of this study indicate that education and support on multiculturalism for nurses are needed to improve health related quality of life for the multicultural families.

간호대학생의 문화적 민감성에 영향을 미치는 요인 (Factors Influencing Cultural Sensitivity among Nursing Students)

  • Purpose: This study was conducted to determine the predictors of cultural sensitivity for nursing students in Korea. Methods: The participants in this study were 332 nursing students. Data collection was conducted through the use of questionnaires which were constructed to include a Cultural Sensitivity Scale, Knowledge of Culture, Interpersonal Reaction Index, and Self-efficacy Scale. Results: Results showed that cultural sensitivity of nursing students was middle range. Knowledge level of culture was very low. There was a significant positive correlation between cultural sensitivity and knowledge of culture, empathy, and self-efficacy. The significant factors influencing cultural sensitivity of nursing students were empathy, self-efficacy, experience of an overseas trip and awareness of cultural education. These four factors explained 23.8% of the variance in cultural sensitivity of nursing students. Conclusion: These results indicate that cultural sensitivity is an important link with empathy and self-efficacy in nursing students. For nursing students to enhance cultural sensitivity, a transcultural nursing curriculum needs to develop for appropriate and effective services in cross-cultural situations of the multicultural families in Korea.

한국과 미국 여대생의 월경전후기 불편감, 월경에 대한 태도, 성역할 수용도의 횡문화적 차이에 관한 연구 (Transcultural Differences on Perimenstrual Discomforts, Menstrual Attitudes and Sex Role Acceptance between Korean and American College Students)

  • This study was performed to compare the transcultural differences on perimenstrual discomforts, menstrual attitudes and sex role acceptance between Korean and American college students. The subjects, 2557 nursing students were selected from 13 universities all around Korea, and 4 universities in the eastern, western, and middle areas of the United States. The data were collected using the MDQ (Menstrual Distress Questionnaire by Moos), the MAQ(Menstrual Attitude Questionnaire), and the Sex Role Acceptance Scale, the subjects answered the MDQ for three points (premenstrual, menstrual, intermenstrual based on their recollections) between July, 1996 and April, 1997. The findings are as follows : 1. There were significant differences in the age of menarche, duration of menstruation, and menstrual cycle between the Koreans and the Americans. 2. As for sex role acceptance, Americans more frequently denied the traditional female role than did Koreans. 3. There were significant differences between the two groups on five categories of menstrual attitude (menstruation is a phenomena that weakens women physically and psychologically, menstruation is a bothersome phenomena, menstruation is a natural phenomena, menstruation onset is a predictable phenomena, and menstruation is a phenomena that does not influence womens' behaviors non is expected to). 4. For the menstrual discomfort scores, there were significant differences between the two groups on all six categories of the MDQ(pain, attention deficit, behavioral changes, AMS response, water retention, and negative emotion) for the three points (premenstrual, menstrual, intermenstrual period). 5. The most common complaints for the Korean women were pain during the perimenstrual period and fatigue during the intermenstrual period. In contrast, for the Americans the most common complaint was negative emotions during the perimenstrual period, but less pain than the Koreans, and a similarity to Koreans during the intermenstrual period. 6. Regularity of the menstrual cycle was positively correlated with age of menarche, menstrual cycle, and duration of menstruation. 7 For the interrelationship between of Perimenstrual discomforts, menstrual attitudes, and sex role acceptance, there were significant correlations for both group as follows : First, the MA category "menstruation is a bothering phenomena" was negatively correlated with sex role acceptance, second, the MA category "menstruation is a phenomena that weakens the women physically and psychologically" was positively correlated with the MA category "menstruation onset is a predictable phenomena." For the further research, it is recommended that efficient nursing intervention programs for perimenstrual discomforts, be developed and qualitative research to demonstrate the cultural differences as the feminism view point be done.

우리나라 가족간호연구 분석 (An Analysis of Family Nursing Research in Korea)

  • The purpose of this study was to analyze the trends of family nursing research in Korea. The subjects were 62 articles, 132 master theses and 20 doctoral theses published during the period between 1970-1997.7. The results are as follows. 1. The types of research that were analyzed were factor isolating. 34.1%, factor relating, 54.2%, situation relating. 6.1% and situation producing, 3.7%. 2. Research designs were experimental studies, 6. 1%, non-experimental studies, 89.7% and qualitative research, 4.2%. Survey research was the method used in 82.7% of the papers. 3. On the research subjects, 94 papers dealt with the family member of a patient or a single person, 59 papers dealt with a patient and 7 papers dealt with whole family. 4. On the places of studies, 64.0% of the studies were done in hospital rooms, 12.9% in the communities and community facilities. 12.0% in schools, 10.7% in the home, and 0.4% an occupational setting. 5. The most frequently used family concept in the title of the articles was "family support"(59 papers) followed by family function and family burden. 6. The most frequently used family assessment tool was Family Environment Scale(FES) developed by Moos & Insel. 7, According to family nursing domain described by Murphy' and Meister'study, the subdomain, relationship of the family and disease was found 83.2%. 8. Four papers built conceptual frameworks based on various theories of researchers. Eleven papers applied family theories and five papers applied nursing theories. Based on the above findings the following suggestions are made. 1. The various domains of family nursing research should be used including in the relationship of family and health, health policy and family. transcultural family research, and theoretical approach to family. 2. Qualitative research should be used for family nursing research. 3. Family assessment tools should be developed for the Korean family.

The Effectiveness of a Cultural Competence Training Program for Public Health Nurses using Intervention Mapping

  • Purpose: This study evaluated the effects of a cultural competence training program for public health nurses (PHNs) using intervention mapping. Methods: An embedded mixed method design was used. Forty-one PHNs (experimental: 21, control: 20) and forty marriage migrant women (MMW) (20, in each group) who were provided nursing care by PHN participated in the study. The experimental group was provided with a four-week cultural competence program consisting of an eight hour offline and online course, e-mail newsletters and social networking services (BAND). Transcultural Self-efficacy (TSE) of the PHNs, client-nurse trust, and satisfaction with nursing care of MMW were measured. Ten PHNs in the experimental group were interviewed after the experimental study. Results: The experimental group showed a significantly greater improvement in TSE, client-nurse trust, and satisfaction with nursing care than did the control group. Six themes emerged from qualitative data: (a) Recognizing cultural differences, (b) Being interested in the multicultural policy, (c) Trying to communicate in MMW's own language, (d) Providing medical information using internet and smart phone, (e) Embracing culturally diverse people into society, and (f) Requiring ongoing cultural competence training. Conclusion: Cultural competence training enabled PHNs to provide culturally competent care and contribute to MMW's health outcomes.

다문화 및 북한이탈주민 가정 자녀의 정신건강 (Children's Mental Health in Multicultural Family and North Korean Defectors in South Korea)

  • South Korea is changing into a multi-cultural society, due to an increase in international marriage, foreign workers and transcultural immigration. In addition, the number of North Korean defectors entering South Korea has increased and now, there are approximately 25000. Therefore, this review have focused on a research that pertains to adaptation and mental health concerns of their children. Children who have immigrated themselves or whose parents have immigrated, might experience language barriers, difficulties in school adjustment, identity confusion and mental health problems. However, their academic performance and developmental status are known to be variable and be affected by socioeconomic status and their parents' educational level. Studies that evaluated the psychological problems of North Korean adolescent refugees' indicated the need for interests in both emotional and behavioral problems. The risk factors of North Korean adolescent refugees' mental health are suggested to be past traumatic experiences, long duration of defection and short period of adaptation. When mental health professionals provide assessment and treatment, they should consider the pre- and post-migration experiences & cultural background that affect the illness behaviors and attitudes toward mental illnesses. Lastly, the majority of children with multi-cultural background are still under an adolescent period and we should follow up with long-term perspectives.

국내 간호대학 국제보건 교과과정 분석 연구: 2015년 이후 변화를 중심으로 (Global health curricula in Korean nursing schools: Focusing on the changes since 2015)

  • Purpose: This study aimed to understand the current status of global health curricula and characteristics in nursing schools, focusing on the changes since 2015. Methods: Data were collected from the websites of 202 nursing schools nationwide in Korea. Global health curricula were analyzed using a structured framework developed by the authors. Results: Among 202 nursing schools, 173 (85.6%) schools offer global health-related courses. Of these, 72 (35.6%) schools offer a 'Multiculturalism' course, and 42 (20.8%) schools offer a 'Global Nursing' course. Fifty-nine schools (29.2%) offer both courses. Compared to the study findings in 2015, the number of global health-related courses and the percentage of global health-related courses designated as a requirement dramatically increased. An additional analysis of five syllabi of global-health related courses found several differences in the courses' aims, contents and evaluation methods. Conclusions: Due to social and political changes, nursing schools are more likely to offer global health curricula. However, there is still a lack of consensus on the core contents and approaches of such curricula, necessitating systematic discussions about the core contents and effective learning methods to increase nursing student competency in global health nursing.

여성전문병원 간호사들의 다문화 환자 돌봄 경험 (The Lived Experiences of Clinical Nurses Who working at the Women's Hospital Caring for Foreign Wives in Korea: A Narrative Study Approach)

  • 결혼이주를 통한 외국여성의 국내이주가 증가하면서 다양한 질병 및 출산으로 인한 의료기관 방문기회가 증가하고 있으며 의료관광 등 외국인 환자의 국내 의료기관 이용이 활발해 지고 있다. 따라서 본 연구는 외국인 환자의 증가에 발맞추어 여성전문병원 간호사를 대상으로 외국인 환자에 대한 인식과 돌봄 현황을 파악해 보고자 하였으며 세부적으로 임상현장에서 외국인 산모에게 돌봄을 제공하는 임상간호사의 경험을 파악해 보고자 하였다. 이를 위해 질적연구방법 중 서사연구 연구방법을 적용하여 다문화 환자 및 자녀를 돌본 경력이 5년 이상인 임상간호사 10명을 대상으로 심층 면담을 통해 자료를 수집하였다. 외국인 환자를 돌본 임상간호사의 경험은 돌봄제공자의 시각에 따라 자민족중심 시각, 수용적 시각, 그리고 문화중심시각으로 나누어 각 시각에 따라 제공되는 간호의 질을 파악해내었다. 특히 타문화에 대한 존중 및 환자 맞춤형 돌봄이 제공되고 있는 경우 돌봄제공자는 스스로를 치료적 돌봄제공자로 인식하고 있는 것으로 나타났다. 따라서 의료현장에서는 타문화에 대한 민감성과 문화중심 돌봄의 필요성에 대한 자각과 교육이 지속적으로 강화되어질 필요가 있다.

간호대학생을 위한 문화간호 역량화 교육 프로그램 (A Proposed Teaching Model to Improve Cultural Competency Care for Undergraduate Korean Nursing Students)

  • Purpose: The aim of this study was to propose a teaching model to improve the cultural competence care of undergraduate Korean nursing students. Methods: A literature review was performed from 2000 through 2014 using internet to identify studies that presented nursing curricula and teaching-learning strategies in Korea. We analyzed retrieved Korean seven articles using Campinha-Bacote's ASKED model in combination with reflective experiences of authors observing classrooms in the U.S. Results: Six syllabuses presented information about cultural diversity nursing concepts in Korea. A traditional lecture focused on presenting contents about cultural knowledge by an instructor in the classroom was identified as a commonly used teaching strategy. We found that cultural competency related to cultural desire, awareness, and encounter was lacking. Thus, we developed teaching model based on the Korean nursing education contexts and combined with Campinha-Bacote model to improve cultural competency in nursing. Conclusions: The proposed teaching model shows promise as a strategy for improving the knowledge, attitudes, and skills related to cultural competent for undergraduate nursing students in Korea. However, evidences that this model improves nursing students' clinical practices reflecting cultural competent care are still lacking. Future research focusing on determining teaching methods and contents in clinical practices will be most effective.

가정간호실무에 적용가능한 이론적틀 (Appling Nursing Theory to Clinical Practice of Home Health Care)

  • The home health care industry has grown rapidly and can be expected to continue to grow in the foreseeable future. Home health care refers to the practice of nursing applied to clients with a health condition in the clients place of residence. clients and their designated care givers are the focus at home health nursing practice. The goal of care is to initiate. manage and evaluate the resources needed to promote the clients optimal level of well-being and function. Nursing activities necessary to achieve this goal may warrant preventive maintenance and restorative emphases to prevent potential problems from developing. Many project program were suggested home health care model for Korea's health care system and policy direction for expansion and establishment of home health care .But the aim of this paper is to provide on overview for theoretical frame work in home health care. Theories and conceptual frameworks or models are important nursing because they define and guide the boundaries of professional practice and identify key nurse-patient-caregiver relationships that emerge with caring. Following is the research with an investigation of the literature review in the University of Arizona international medline database, In conclusion, are as followers: First, many nursing theorists have had a tremendous impact on nursing practice. the following highlights those nursing theorists that are particularly helpful in understanding home health care. 1. Florence Nightingale : Our earliest theoretical legacy. Nightingale's believes are reflected in basic infection control practice such as hand washing and infectious waste disposal and are key nursing interventions in home care. 2. Martha Roger's :Science of unitary human beings theory. Rorger's believed that the focus of shared. non invasive healing modelities is the human environmental field rather than direct physical care. These modelities continue to evolve as our awareness (reflecting greater diversity, faster rhythms, motions, and ways of knowing) transcends time and space, allowing individuals to get in touch with their integral nature of unbroken wholeness. On people as ever changing energy fields have special relevance in home care especially with hospice and palliative care applications. 3. Madeline Leininger's; Transcultural nursing theory. Home care nurses move through a variety of communities and often care for patients from different cultural back grounds. Therefore Leininger's work has a good that with home care because home care nursing practice is very culturally focused. 4. Dorothea Orem's : Self care deficit theory. Orem's theory views care as something to be performed by both nurses and patients. The role of the nurse is to provide education and support that help patients acquire the necessary activities to perform self-care. Orem's theory is foundational to have care because it begins to truly acknowledge the role of the patient in managing his or her own health. which is referred to as self-care. 5. Margaret Neuman's; Health as expending consciousness theory. Neuman believes that health compasses disease and reflects an underlying pattern of person-environment interaction. A key application of 'Neuman's work to home care is for nurses to understand that health and illness do not necessarily exist at opposite ends of a continuum. 6. Jean Watson's: Theory of human caring. Watson's theory of human caring in nursing proposes human caring as the moral ideal of nursing. Nurses participate human caring to protect, enhance and preserve humanity by assisting individuals to fing meaning in illness. pain and existence and to help others gain self knowledge. self control. and self healing such thinking lends richness to theory development. as well as clinical practice in home care. Second, Robin Rice : Dynamic self determination for self care. (A theoretical framework for home care) Dynamical self determination for self care can be useful to home care nurses in a variety of ways. As research tool it can be reflected in the interview process when the home visit. The home care nurse's role is that of facilitator of patient self-determination for self care through numerous strategies. including patient education and case management.
