일반 성인의 사상체질에 따른 자기자비와 불안, 우울 및 색선호도에 대한 연구 (A Study on Adults' Self-Compassion, Anxiety, Depression, and Color Preference Based on Sasang Constitution)

  • 본 연구는 사상체질유형에 따라, 정신건강 지표 중 자기자비, 특질불안, 우울 수준 및 색선호도에서 차이가 있는지 알아보는 데 목적이 있다. 아울러 자기자비와 불안 및 우울 간에 어떠한 관계가 있는지 탐색하였다. 성인 남녀 총 205명의 연구참여자들을 대상으로 사상체질 진단을 위한 2단계 설문지(TS-QSCD)를 이용하여 태양인, 태음인, 소양인, 소음인 등 4체질로 진단하고, 자기자비척도(Self-Compassion Scale), 특질불안척도(SATI-X-2), 우울척도(CES-D), 그리고 색선호도 설문지(CRR)를 사용하였다. 그 결과, 태양인은 소음인보다 자기자비 수준이 더 높았고 특질불안 수준이 더 낮았다. 또한 자기자비와 특질 불안 및 우울 간 관계에서는 자기자비가 높을수록 특질불안과 우울 성향이 낮았고, 특질불안이 높을수록 우울 수준도 높았다. 사상체질에 따른 색 선호도와 우울 수준에서는 집단 간 유의미한 차이가 나타나지 않았다. 마지막으로 정신건강증진을 위한 본 연구의 함의와 한계점을 논의하였다.

암화학요법을 받는 유방암 환자의 불안, 절망감 및 가족지지와의 관계 (The Relationship of Anxiety, Hopelessness, and Family Suppoort of Breast Cancer Patients Undergoing Chemotherapy)

  • This study was attempted to provide the basic data for nursing intervention to improve the psychosociological adaptation of patients receiving chemotherapy for breast cancer by examining relationship between anxiety and hopelessness that they are experiencing and family support, in order to help them successfully cope with various psychological problems. This study was carried out with 93 breast cancer patients who are receiving chemotherapy in the injection treatment room of K University Hospital located in the downtown of Taegu after having underwent mastectomy in the hospital between December 1995 and August 1996. This study used the systematized questionnaires which contain 7 questions about general characteristics, Spielberger's trait anxiety & state anxiety scale, the tool that WON(1987) modified the hopelessness scale which was developed by Beck et al.(1967) and the family support tool made by TAE(1985). By using the SPSS/PC program, this study obtained the real number and percentage for the general characteristics of the subjects, and mean and standard variation for the degrees of trait anxiety, state anxiety, hopelessness and family support. The correlation between each variables was identified on the basis of the Pearson Correlation, and the degrees of trait anxiety, state anxiety, hopelessness and family support in the general characteristics of the subjects were analyzed by using the t-test, ANOVA, and Duncan test. The results of this study were summarized as follows. In the general characteristics of the subjects, most of each group were 51 years old or more and the middle class in income, had educational background under elementary school, no job, Buddhism in religion and spouse, and were receiving chemotherapy using MTX and 5FU. It was shown that the degree of the subjects' trait anxiety is, on an average, 50. 29, state anxiety 49. 68, hopelessness 51. 46 and family support 34. 28. Both trait anxiety and hopelessness showed normal correlation ; the higher the degree of trait anxiety is, the higher the degree of hopelessness is, while trait anxiety and family support showed reverse correlation ; the higher the degree of trait anxiety, the lower the degree of family support that the subjects perceive is. State anxiety and hopelessness also showed normal correlation ; the higher the degree of state anxiety is, the higher the degree of hopelessness is. Family support and hopelessness showed reverse correlation ; the higher the degree of family support is, the lower the degree of hopelessness that the subjects perceive is. And family support and state anxiety showed reverse correlation but there was a statistically significant difference. The degree of trait anxiety in the general characteristics of the subjects showed a significant difference by age, job and religion, the degree of state anxiety a signigicant difference by job and religion, the degree of hopelessness a signigicant difference by age, educational background and existence or not of spouse. In conclusion, the breast cancer patients receiving chemotherapy perceive anxiety and hopelessness due to several causes such as diagnosis itself or side effects of chemotherapy, so that it is required not only to develop specific nursing interventions including family support to alleviate anxiety and hopelessness but also to apply such interventions to clinical practice.

Trait Anxiety as a Mediator of the Association between Attention Deficit Hyperactivity Disorder Symptom Severity and Functional Impairment

  • Objective: Anxiety has been shown to influence functional impairment in patients with attention deficit hyperactivity disorder (ADHD). This study aimed to compare functional impairment in subjects with and without adult ADHD and to investigate the associations among trait anxiety, functional impairment, and ADHD symptom severity. Moreover, the effects of ADHD symptom subtypes on trait anxiety and functional impairment were also examined. Methods: The sample included 209 adults between the ages of 20 and 31 years. Fifty-one adults received a diagnosis of ADHD, and an additional age, sex-matched group of 51 adults comprised the adult control. Participants were assessed with Conners' Adult ADHD Rating Scales (CAARS), the Beck Depression Inventory (BDI), the Spielberg Trait Anxiety Inventory (STAI-T), and the Sheehan Disability Scale (SDS). The relationships among ADHD severity, anxiety, and functional impairment were investigated using Pearson's correlation analysis. Subtypes of ADHD symptoms that predicted anxiety and functional impairment were investigated using regression analyses. Results: Adult ADHD patients significantly differed from normal control subjects according to BDI, STAI-T, and SDS assessment. Significant positive correlations were noted between ADHD severity, anxiety, and functional impairment. Multiple linear regression analysis confirmed anxiety as a mediator between functional impairment and ADHD CAARS symptom subscales. Conclusion: Patients with adult ADHD showed higher levels of anxiety, depression, and functional impairment. Additionally, ADHD symptoms and anxiety impacted subject functional impairment. Our results suggest that anxiety may be a strong mediator between ADHD severity and functional impairment.

주요우울장애 환자의 증상 심각도에 따른 신경인지적 특성 (Neurocognitive Characteristics According to Depression Severity in Patients with Major Depressive Disorder)

  • Objectives This study investigated associations between symptom severity and neurocognitive functions in patients with major depressive disorder using comprehensive neuropsychological tests. Methods Ten patients with low depression (LD) and 22 patients with high depression (HD) participated. Symptom severity was determined by both the Beck Depression Inventory and the Hamilton Depression Rating Scale. Additionally, anxiety was measured by the Korean version of the State-Trait Anxiety Inventory (STAI-KYZ). Ten subtests of the Korean-Wechsler Adult Intelligence scale (K-WAIS), Stroop test, the word/design fluency tests were administered to assess cognition. Results The LD and HD groups did not differ in any of cognitive measures but anxiety level. Namely, the HD group obtained significantly higher scores on the state (U = 29.50, p < 0.01) and trait (U = 28.50, p < 0.001) anxiety scales than the LD group. A significant interaction effect between trait anxiety and depression severity was observed on the picture arrangement subtest of the K-WAIS [F (1, 27) = 5.09, p < 0.05]. Conclusions Cognitive deficits observed in patients with major depressive disorder may be related to other factors rather than current symptom severity. Trait anxiety possibly moderates the effect of current symptom severity on social judgment in these patients.

중학교 청소년의 부모간 갈등과 자아분화간 관계에서 특질불안이 미치는 매개 효과 (The Mediation Effects of Trait Anxiety in The Relationships between Middle School Students' Self Differentiation and Parents' Marital Conflicts)

  • 본격적인 청소년기로 진입하는 중학교 청소년의 건강한 자아분화 지원에 도움을 주고자, 중학교 청소년의 자아분화에 미치는 부모간 갈등과 특질불안의 영향을 특질불안의 매개 효과를 통해 살펴보았다. 이를 위해 대구 경북지역 중학교에 재학중인 중 3 남녀 청소년 252명을 대상으로 Skowron과 Fiedlander(1998)의 자아분화(DSI-R) 척도를 기초로 정경연(2007)이 개발한 자아분화 척도, Spielberger(1973)의 특질불안 척도(STAIC) 척도, 권영옥과 이정덕(1997)이 개발한 아동이 지각한 부모갈등 척도를 사용하여 자료수집 하였다. 수집된 자료는 SPSS 19.0 프로그램의 Pearson의 적률상관관계, 단순 및 중회귀분석을 통해 유의수준은 .05 미만에서 통계적 유의성을 검증하였다. 연구결과 첫째, 중학교 청소년의 자아분화는 부모간 갈등에 의해 부적으로 영향 받는 것으로 나타났다. 부모간 갈등이 높을수록 중학교 청소년의 자아분화는 낮게 나타났다. 둘째, 중학교 청소년의 특질불안은 부모간 갈등에 의해 정적으로 영향 받는 것으로 나타났다. 부모간 갈등이 높을수록 중학교 청소년의 특질불안은 높게 나타났다. 셋째, 중학교 청소년의 자아분화에 미치는 부모간 갈등의 부적인 영향력은 청소년 자신의 특질불안이 통제될 때 상당히 떨어지는 것으로 나타났다. 이는 중학교 청소년의 자아분화에 미치는 부모간 갈등의 부적인 영향력이 중학교 청소년 자신의 특질불안에 의해 부분매개 됨을 시사하는 것이다.

화상환자가 지각하는 통증과 불안에 관한 연구 (A Study on Pain and Anxiety of Burn Patients)

  • The purpose of this study is to examine intensity of pain and anxiety of burn patients. This study selected 99 adult patients of 18 years old or older who have been hospitalized and treated for a second-degree burn at the surgery ward or burn ward of 3 university hospitals which are located in 2 metropolitan cities, and a university hospital in Seoul Special City from July 1 to October 20, 2000. This study used STAI(State-Trait Anxiety Inventory) developed by Spielberger, and translated by Kim, Jung Taek and Shin, Dong Kyun to examine the degree of anxiety and VAS to examine the pain. This study was carried out by interview and using SPSS 9.0 statistical program. Results were as follows : 1) As for general characteristics of the subjects, they were mostly male. 30s-40s, married, and high school graduates. 2) Forty-seven patients were burned by fire and $49.6\%$ of the patients had burn 10 to $20\%$ of BSA. seventy-nine percents of patients received one dressing change a day and $74.3\%$ received medication once or twice a day. 3) As for intensity of anxiety and pain. the mean intensity of pain was 6.41. The intensity of trait anxiety and the state anxiety were 39.76. and 51.81. 4) Subject's age was significantly different in their pain intensity and other variables were insignificant. Correlations between trait-anxiety and state-anxiety(r=.286, p=.004), and state-anxiety and VAS(r=.294, p=.003) showed weak correlation. There wasn't statistically any significance in trait- anxiety and VAS(r=.183, p=.070).

암환자의 불안관리를 위한 이압요법 적용 효과 (The Effect of Auricular Acupressure Therapy on Anxiety of Cancer Pateint)

  • The purpose of this study wsa to necessitate auricular acupressure therapy as an independent nursing intervention on cancer paitents by confirming its effectiveness. Method: The experimental study was unequivalently controlled pre-post measure study. The subjects were 40 cancer patients who were hospitalized in K medical center in Seoul. The experimental group (20) and the control group (20) were randomly assigned. As measured tools, Spielberger's State-trait Anxiety (1976) measured tool by Kim's transplation (1978). The auricular acupressure therapy was applied to experimental group, and the pre-post measure was performed to both group. The data was analyzed by using SPSS computer program that included descriptive statistics, 2-test, and t-test. Result: 1) The experimental group with the auricular acupressure therapy showed lower trait anxiety scores in comparison with the control group (t= 8.036, p=.000). 2) The experimental group which applied the auricular acupressure therapy showed lower state anxiety scores in comparison with the control group (t= 19.616, p=.000). This result showed that cancer patients with the auricular acupressure therapy applied cancer patients decreased state anxiety and trait anxiety. Therefore , effectiveness of the auricular acupressure therapy was confirmed through this study. Conclusion: According to the result, anxiety of cancer pateint should be decreased and controlled by the auricular acupressure therapy as independent nursung intervension. In addition, the auricular acupressure therapy will provide effective independent nursing intervention that will decrease anxiety on patient with other disease and will improve quality of their lives.

Family of Origin Influences on Anxiety, Open Communication, and Relationship Satisfaction -A Test of Bowenian theory of Anxiety as a Mediator in the Intergenerational Transmission

  • The purpose of this study was to investigate the role of anxiety in mediating the influence of family of origin characteristics on subsequent relationship processes. Using Bowenian theory as a foundation, this study examined the causal relationships among gender, religion, family of origin-fusion, intimacy, intimidation, personal-trait-anxiety, couple open communication and relationship satisfaction. Results of the path analysis indicated that: (a) gender, religion, family of origin-fusion, and family of origin-intimidation were directly related to personal-trait-anxiety, (b) personal-trait-anxiety was directly related to couple open communication and not related to relationship satisfaction, (c) couple open communication was directly related to relationship satisfaction, and (d) family of origin factors were not directly related to couple open communication and relationship satisfaction; rather, as it was expected, indirect associations, through the mediation of anxiety, were found. The results generally support Bowen's intergenerational process theory, which asserts that anxiety is the mediator of the process. Limitations of the current study and suggestions for further research were discussed.

Anxiety and Pain in Surgically Treated Breast Cancer Patients

  • The aim of the study was to evaluate the level of anxiety and pain in women with breast cancer. Patients who had been treated with modified radical mastectomy or breast conserving surgery were included. Data were gathered using the state-trait anxiety inventory and the visual analog scale. The pain levels and analgesic consumption of the patients were evaluated after surgery. The study sample consisted of 150 women. The mean age of the participants was $50.54{\pm}10.02$. Most of the patients (58%) received breast conserving surgery. The mean state anxiety score was $44.74{\pm}11.91$, and the mean trait anxiety score was $48.78{\pm}9.48$ before surgery. The mean pain level on the first day following surgery was $3.26{\pm}1.91$ and analgesic consumption was $2.98{\pm}1.08$. There was no correlation between patient pain and anxiety levels. There was very slightly positive correlation between trait anxiety and total analgesic consumption. Assessing the levels of anxiety in breast cancer patients before surgery may contribute to the determination of postoperative pain.

성인의 수면의 질에 따른 심박변이도, 피로, 우울 및 불안과의 관계 (Relationship among Sleep Quality, Heart Rate Variability, Fatigue, Depression, and Anxiety in Adults)

  • Purpose: The purpose of this study was to investigate the relationship among sleep quality, heart rate variability (HRV), fatigue, depression, and anxiety reported by Korean adults. Methods: A cross-sectional study of 208 adults aged 20~60 years was conducted using a short-term HRV analysis and self-reported questionnaires of Pittsburgh Sleep Quality Index (PSQI), Fatigue Severity Scale, Beck Depression Inventory, and State Trait Anxiety Inventory. Results: Subjects with good sleep quality (PSQI $score{\leq}5$) had higher HRV total power (t=2.03, p=.043) and high-frequency (t=2.04, p=.043) with lower fatigue (t=-4.08, p<.001), depressive mood (t=-3.66, p<.001), and trait anxiety (t=-3.84, p<.001) than subjects with poor sleep quality. Poor sleep quality was negatively correlated with HRV total power (r=-.17, p=.016), high-frequency (r=-.14, p=.049), and positively fatigue (r=.39, p<.001), depression (r=.44, p<.001), state anxiety (r=.23, p=.001) and trait anxiety (r=.34, p<.001). Conclusion: The results indicated that sleep quality is correlated with HRV which reflects the activities of the autonomic nerve system, fatigue, depression, and anxiety in adults.
