거골 경부 골절의 단기간 추시 결과 (Short-term Results of Talar Neck Fractures)

  • Study design: Clinical results were retrospectively analyzed in 11 patients with fractures of talar neck who were treated in our department from Jan. 1994 to Dec. 1999. Objective: The purpose of this study is to evaluate the short-term results and to assess the prognostic factors of talar neck fractures. Material and Method: 11 cases with fractures of talar neck were reviewed retrospectively with minimum 1 year follow-up. There were 8 men & 3 women, and the average age was 25. The most common cause was traffic accident. According to the modified Hawkins classification, type I was in 4 cases, type II in 5, type III in 2, and type IV was none. All type I fractures were treated conservatively, and others were treated operatively. Results: According to Hawkins criteria, there was excellent result in 7 cases(64%), good in 2(18%), and fair in 2(18%). Post-traumatic arthritis occurred in 2 cases, but there was no avascular necrosis. Conclusion: Careful selection of method of treatment and urgent management are important prognostic factors in talar neck fractures. The longer follow-up in more cases is necessary to evaluate the long-term clinical results and complications more accurately.

  • To investigate epidermiologic trend in maxillofacial fractures. We retrospectively studied 934 patients with maxillofacial fractures between $1981{\sim}1987$ and $1995{\sim}1999$. The results were compared in the previous group (Group A, patients treated between $1981{\sim}1987$) with those in the recent group (Group B, $1995{\sim}1999$). Also, we studied 516 patient between $1995{\sim}1999$ for occupation, associated injuries, treatment and complications. The sex ratio of men to women decreased (5.6 : 1 in Group A vs. 3.5 : 1 in Group B), and the largest age group was 20 to 29 years. There was the highest incidence in September and Fall. The most frequent cause of maxillofacial fracture in both Group A and B was traffic accidents. The distribution of fracture site did not change appreciably, but the frequency of midfacial fracture increased. In the recent group, the largest occupational group was salaried men, and the largest associated injuries of maxillofacial fracture was head and neck injury. Open reduction was used in 91% of the cases, and post-operation complications were infection, neurologic problem, malocclusion, and mouth opening limitation etc. Our results suggest that the clinical features of patients with maxillofacial fractures have changed during the past decade.

Artificial and Biological Particles in the Springtime Atmosphere

  • This study focused on a comprehensive and detailed interpretation for the springtime air quality influenced by both artificial (particulate matter (PM) and asbestos) and biological (pollen) sources in Fukuoka Prefecture, Japan. An intensive measurement of PM was conducted at four characteristic sites (i.e., a heavy traffic area, a residential area, an industrial area, and a desolate area) in the Fukuoka Prefecture during spring of 2007. Analysis of major ionic species in $PM_{2.5}$ was performed by an Ion Chromatography, and asbestos and pollen were identified by Scanning Electron Microscopy with an energy dispersive X-ray spectrometer (EDX). $PM_{2.5}$ concentration ($65.3{\mu}gm^{-3}$) measured in an industrial area (site C) was extraordinarily high compared to those monitored in other areas; it greatly exceeded the Japan's $PM_{2.5}$ criteria (a daily average of $35{\mu}gm^{-3}$). NOAA's HYSPLIT dispersion model suggests that this high level of $PM_{2.5}$ monitored at site C is unlikely to affect the Asian continent. The ambient concentrations of $PM_{2.5}$-related anions ($NH_4{^+}$, $NO_3{^-}$, and $SO_4{^{2-}}$) and their relative contributions to $PM_{2.5}$ were also investigated in four study areas. The concentrations of these major water-soluble ions exhibit not only strong spatial dependence but also different ratios to each other. Asbestos fiber (crocidolite and amosite) concentration values changed in the range of 2.5 to 14.4 f per liter of air. The number of pollen grains showed that Cedar ranked higher in concentration than other types of pollen, with the maximum concentration at site A.

Epidemiology and patterns of nasal bone fracture in elderly patients in comparison to other age groups: an 8-year single-center retrospective analysis

  • Background: Nasal bone fractures are the most common type of facial bone fracture, but are under-studied in adults above 65 years of age. Therefore, we investigated the epidemiology and patterns of nasal bone fractures among older adults in comparison to different age groups. Methods: This retrospective study included 2,321 nasal bone fracture patients who underwent surgery at our hospital from January 2010 to December 2017. The patients were classified by age as preschoolers, school-age children, young and middle-aged adults, and the elderly. We performed pairwise comparisons between elderly patients and each other age group in terms of sex, cause of injury, and fracture type. Results: The 2,321 nasal bone fracture patients included 76 elderly patients (50 men [65.8%] and 26 women [34.2%]). In these patients, the two most common injury causes were falling or slipping down (n= 39; 51.3%) and road traffic accidents (n= 19; 25.0%). According to the Stranc and Robertson classification, the most common force vector was lateral, and plane 2 fractures with lateral forces predominated. Conclusion: The elderly showed similar patterns of nasal bone fractures to those observed in young and middle-aged adults, but significant differences from preschoolers (in the injury vector and plane of fracture) and from school-age children (in the sex ratio and plane of fracture). However, elderly patients presented significantly different epidemiological characteristics compared to the other three groups. Therefore, it is necessary to improve the quality of life of the elderly and prepare for the upcoming super-aged society by taking steps to reduce the incidence and severity of fractures. Possible options for doing so include strengthening individual-level safety factors and expanding the social safety net for the elderly.

위험운전행동에 대한 운전자 성별 간 상호이해도 분석 (Analysis of Mutual Understanding about Dangerous Driving Behaviors between Male and Female Drivers by Co-orientation Model)

  • 이 연구는 위험운전 행동에 대한 남녀운전자 상호간의 인식 차이를 다각적으로 비교, 분석하고, 인식 차이를 유발시키는 요인에 대한 파악을 목적으로 하였다. 위험운전 행동에 대한 상호인식 차이를 파악하기 위해 운전행동 분석도구인 DBQ(Driving Behavior Questionnaire)를 평가척도로 적용하였고, 그 결과를 상호지향성 모델(Co-orientation Model)에 적용시켜 남성운전자와 여성운전자의 상호 간 인식 차이를 비교, 분석하였다. 또한 요인분석을 통해 남녀 상호간 인식 차이를 발생시키는 요인을 도출하였다. 분석결과 객관적 일치도의 경우, 남녀 상호간 상대방의 운전행동이 더 위험하다는 인식차이를 확인하였고 주관적 일치도 또한 남성과 여성 모두 상대방이 자신의 실제 운전행동의 위험도보다 더 큰 위험도로 판단할 것으로 추측하여 차이가 나타났다. 정확도에서는 남성은 남성의 위험운전행동에 대해 본인의 추측보다 여성이 더 위험하게 인식하고 있는 것으로 나타나 판단이 어려우나, 여성운전행동에 대한 여성의 추측은 모든 요인에서 남성의 인식과 일치하여 남성을 정확하게 이해하고 있는 것으로 분석되었다. 또한 예측 모형의 결합을 통해 남성운전자의 관점과 여성운전자의 관점에서 비교분석하였으며, 그 결과 남녀 상호간에 상대방이 더 위험한 운전행동을 하는 인식을 갖고 있는 것으로 파악되었다.

서울시 대중교통 이용자의 통행패턴 분석 (Travel Patterns of Transit Users in the Metropolitan Seoul)

  • 본 연구의 목적은 도시성장과 함께 통행거리와 교통량이 크게 늘어나 심각한 교통체증현상을 보이고 있는 서울을 대상으로 교통흐름의 공간적 특징을 파악하고, 이를 수도권 지역의 토지이용 및 시설 분포와 연결시켜 수도권 지역의 기능적 연계의 공간구조를 밝히는 것이다. 이를 위해 본 연구에서는 교통카드를 이용하여 이동하는 대중교통 이용자들이 만들어 내는 통행 거래 자료를 바탕으로 통행행태와 통행흐름의 공간적 특징을 분석하였다. 교통카드 데이터는 하루 천만 건이 넘으며, 각 통행거래자료 마다 승 하차 및 환승의 위치와 시간, 이용 교통수단 등 방대한 정보를 담고 있어 이의 효율적 분석을 위하여 데이터마이닝 기법을 도입하였다. 특히 대중교통이용자의 통행행태를 파악하고 각 지점의 통행 수요를 파악하기 위한 새로운 알고리즘 개발하여 적용하였다. 분석결과의 공간적 특성을 파악하기위하여 지리정보체계를 이용하여 시각화시키고, 그에 입각하여 공간적 특성을 분석하였다. 또한 출발 교통량, 도착교통량, 총 지지도들 간의 관계성을 파악하기 위하여 상관관계분석을 시도하였다. 통행수요에 있어 강남의 2호선 지하철역을 따라 가장 많은 통행 수요가 나타나고 있으며, 그 다음으로 강북의 구도심지역이 또 하나의 중심축을 이룬다. 그밖에도 대단위 고층 아파트가 밀집되어 있는 주거지역들이 부수적인 중심축을 형성한다. 기 종점 수요와 함께 통과 수송량 까지 나타내는 총지지도는 강남의 구로-신도림 역이 가장 놓게 나타나며 강남지역에 위치한 지하철 2호선 역들과 환승역들에서 높게 나타나고 있다. 이러한 통행패턴 분석은 일차적으로는 교통망 상의 교통흐름과 각 지점의 통행수요를 나타나며 지역 내에서 지점 간의 기능적 연계를 반영하고 있기 때문에 도시의 교통계획은 물론 지역의 토지이용 및 시설 입지 계획 수립에 필수적이다.

Trade Exhibition for Small & Medium Enterprises by Using of Special Conditions in Foreign Marketing Insurance

  • Korea's trading volume is expected to surpass USD 1 trillion in 2011. Korean economy achieves this largely due to its dependence on export and enhanced technological capacity and product quality. Improved recognition of Korean enterprises in the global arena also helped. However, the largest reason behind theses could be found in exporters' ceaseless marketing endeavors and continued government supports. Today, more and more people become to rely on trade exhibitions to boost export effectively. Trade exhibitions are employed as a useful tool to attract buyers and enter a market. This is because such exhibitions' marketing effect and professional aspects. South Korean export relies on large conglomerates for most part. However, 95% of the country's entire industry are small and medium-sized companies. This means that SMEs' export has a huge impact on the national economy. Therefore, as a way to improve SME export and minimize their losses, we need to use trade exhibitions more actively. The overseas policy insures only promotional activities regarding foreign exhibition and fair (including international events held in Korea), foreign distributor, shopping mall, home shopping, etc. and does not include air fare, traffic cost in the local place, accommodation cost and dining expenditures as costs to be excluded mentioned above. It is not easy for them to take part in events abroad if they have to pay for such expenses. If full financial support is difficult, the Korea trade insurance corporation still may give a certain level of aid for successful exporters to engage in marketing activities abroad more actively and further stimulate SMEs' export.

Single-Stage Reconstruction with Titanium Mesh for Compound Comminuted Depressed Skull Fracture

  • Objective : Traditionally, staged surgery has been preferred in the treatment of compound comminuted depressed fracture (FCCD) after traumatic brain injury (TBI) and involves the removal of primarily damaged bone and subsequent cranioplasty. The main reason for delayed cranioplasty was to reduce the risk of infection-related complications. Here, the author performed immediate reconstruction using a titanium mesh in consecutive patients with FCCD after TBI, reported the surgical results, and reviewed previous studies. Methods : Nineteen consecutive patients who underwent single-stage reconstruction with titanium mesh for FCCD of the skull from April 2014 to June 2018 were retrospectively analyzed. The demographic and radiological characteristics of the patients with FCCD were investigated. The characteristics associated with surgery and outcome were also evaluated. Results : The frequency of TBI in men (94.7%) was significantly higher than that in women. Most FCCDs (73.7%) occurred during work, the rest were caused by traffic accidents. The mean interval between TBI and surgery was 7.0±3.9 hours. The median Glasgow coma scale score was 15 (range, 8-15) at admission and 15 (range, 10-15) at discharge. FCCD was frequently located in the frontal (57.9%) and parietal (31.6%) bones than in other regions. Of the patients with FCCDs in the frontal bone, 62.5% had paranasal sinus injury. There were five patients with fractures of orbital bone, and they were easily reconstructed using titanium mesh. These patients were cosmetically satisfied. Postoperatively, antibiotics were used for an average of 12.6 days. The mean hospital stay was 17.6±7.5 days (range, 8-33). There was no postoperative seizure or complications, such as infection. Conclusion : Immediate bony fragments replacement and reconstruction with reconstruction titanium mesh for FCCD did not increase infectious sequelae, even though FCCD involved sinus. This suggests that immediate single-stage reconstruction with titanium mesh for FCCD is a suitable surgical option with potential benefits in terms of cost-effectiveness, safety, and cosmetic and psychological outcomes.

관골 골절의 양상 및 정도에 관한 임상적 연구 (A CLINICAL STUDY ABOUT NATURE AND SEVERITY OF THE ZYGOMATIC BONE FRACTURES)

  • This is a retrospective study on the nature and severity of zygomatic bone fractures. This study was based on a series of 358 patients with zygomatic bone fractures who treated as in-patient at Soonchunhyang Chonan Hospital during the period of Jan. 1993 through Dec. 1996. The results obtained are as follows : 1. The ratio of men to women was 4.41:1. The age frequency was highest in the second, third decade. The yearly distribution was the highest in 1993. The monthly distribution was highest in Feb. and Jun. 2. The most frequent cause was the traffic accident.(66.2%) 3. The most frequent type of fractures was the class I(undisplaced fx.).(25%) 4. The most frequent associated oral and maxillofacial injuries was soft tissue.(46.9%) 5. The most frequent associated systemic injuries was upper, lower extremity.(38%) 6. In A.I.S of oral and maxillofacial area, the mean score of A.I.S was $2.5{\sim}3$. In I.S.S, the highest mean score was in the ped TA.(19) 7. In respect of treatment, the most common method was open reduction with rigid fixation on fronto-zygomatic suture area.(47.8%) 8. The incidence of complication was 12% and the most common complication was malunion.(6.1%)

구강악안면 외상환자의 조직손상의 양상 및 정도에 관한 임상적 연구 (A CLINICAL STUDY OF THE APPEARANCE AND DEGREE OF THE FACIAL INJURIES)

  • This is a clinical and retrospective study on the patients with oral and maxillofacial trauma. This study was based on a series of 917 patients were treated as in-patients, at Chon-buk National University Hospital, during the period of Jan., 1989 through Dec., 1993. The results obtained were as follows : 1. The ratio of men to women were 3.59 : 1 in oral and maxillofacial injuries, 3.92 : 1 in facial bone fractures, and 3.18 : 1 in soft tissue injuries. 2. The oral and maxillofacial injuries occurred most frequently in the third decade(32.2%), and fourth, second, fifth decade in orders. 3. The major etiologic factors were traffic accident(57.4%) and fall-down(17.1%), interpersonal accident(16.6%), and industrial accident were next in order of frequency. 4. The incidence of facial bone fracture was 72.1%, soft tissue injury 58.8%, and dental injury 40.5%. 5. The most common site of fracture were mandible(62.9%) and maxilla(19.9%), zygoma and zygomatic arch(18.7%), and nasal bone(4.7%) were next in order of frequency. 6. The most common type of soft tissue injury was laceration(51.0%). The lesion of soft tissue injuries were mostly 1 or 2 lesions and deep.
