A reflection on writing case records: Development and current demands for acupuncture practitioners

  • The written case that reflects the course of treatment for a person is central to the East Asian medical tradition. This paper examines the approaches and particularities of producing the actual written account of the clinical encounter, or a particular aspect of a case, that may be required by acupuncture practitioners and researchers. It will discuss the influences that can be brought to bear on the construction and production of these accounts. In addition, it will outline and highlight historical approaches to the case record documentation process as well as debate the value and purpose of these. This paper aims both to assist the production of helpful and authoritative case records for practitioners and researchers, and to highlight the usefulness of such case records. Moreover, it will discuss not only why the case needs to be written and for whom, but also which agencies support and control what is written. How can contemporary requirements and traditional views both be incorporated accurately, with context and with meaning? The essence of this paper is that practitioner/patient interactions need to be documented, and it will explore how this can best be supported.

An introduction to the recently excavated Chunggang Medical Records and research on their medical value

  • This study is a report on recently discovered medical records based on traditional medicine in the 1900s. First, the contents of the records and their significance are described in detail. Next, a simple example of the research follows, in order to explain the medical and historical significance the records contain and to answer the question of how this historical document can contribute to future medical and historical studies. The documents dealt with in this study, the Chunggang Medical Records, are medical records compiled by a Korean doctor of oriental medicine by the name of Younghun Kim who practiced in the center of Seoul for a period of over 60 years. The records, which eventually amounted to over 1,500 books, were made known to the academic world when the descendents recently donated them to Kyunghee University. The reason these medical records attract so much attention from academic circles, even though they are the work of one individual, is that they contain abundant information on general public medical health at the time, in addition to the fact that Kim Younghun was a well known figure among Oriental Medicine doctors in Korea. The medical records start in 1915 and continue until Kim Younhun's death in 1974, though they have some damaged or missing parts. Kim's medical records are a gold mine not only for scholars studying the medical history of the early 1900s, but also for doctors trying to emulate the techniques embedded in a great predecessor's medical practice.

"동의보감"(東醫寶鑑) 내경편(內景篇)에 수록된 의안에 대한 연구 (Study on the Medical Records in Naekyung-pyun of Dongeuibogam)

  • The Medical records (醫案) are very important materials in studying the developmental process of traditional Korean medicine. In this study, we investigated the medical records which are described in Naekyung-pyun(內景篇) of Dongeuibogam(東醫寶鑑), the most famous medical literature in Korean. There are 50 medical records in Naekyung-pyun and all of these records are cited from 12 Chinese medical literatures. The medical records were mainly cited from medical literatures written in Jin-Yuan(金元) and Ming(明) dynasty such as Yixuegangmu(醫學綱目) and Danxizanyao(丹溪纂要), although most records were written by medical scholars in Jin-Yuan(金元) dynasty, including Zhudanxi(朱丹溪) and Zhangzihe(張子和). Most of records were omitted and/or modified reflecting the Purpose of editors of Dongeuibogam. In addition, medical essay(醫論) was changed into medical record in some cases. The authors of the records used oral medication, acupuncture-moxa, psychological treatment and surgical manners. And in some cases of only having principal of treatment, the editors newly added the herbal prescription. The further study on medical records in Korean traditional medicinal literatures, would reveal the developmental progress of Korean medicine and inform more actual proof on medical condition.

청대설진의안외감병여내상병적설상특정연구 (The tongue demonstration characteristic study between diseases caused by exogenous pathogenic factor and disorder of internal organs of the glossoscopy medical records in Qing Dynasty)

  • Objective To learn if there were different characteristics of tongue demonstration between diseases caused by exogenous pathogenic factor and disorder of internal organs. Methods Choosing 3059 cases from 51 medical records of Qing Dynasty, in which 1397 cases were diagnosed as diseases caused by exogenous pathogenic factor and 1110 were disorder of internal organs. Contrasting the constituent ratio of tongue demonstration using chi-square test. Results There were significant deviation between diseases caused by exogenous pathogenic factor and disorder of internal organs in color of tongue, color of fur and quality of fur (p<0.01). Conclusions the characteristics of tongue demonstration in diseases caused by exogenous pathogenic factor were high frequency of bright red tongue, yellow fur, blank fur and dry fur; and which in disorder of internal organs were high frequency of white tongue, thin fur, greasy fur and exfoliative fur.

세종대 의원 활동 연구 - 『조선왕조실록(朝鮮王朝實錄)』을 중심으로 - (A Study on Activities of Doctors in King Sejong Period - Based on The Annals of the Joseon Dynasty -)

  • Objectives : Doctors are obviously one of the most interesting subject in medical history. Doctors are who treat patients and disease and the authors for medical records or books. Especially doctors in traditional medicine mostly tried to write medical books for new idea or their esperiences or leave their medical records for treatments, medication, prescription and so on. Therefore, many researchers have explained Korean or Chinese medical history of traditional society through those books or documents rather than doctors themselves. The Annals of the Joseon Dynasty has massive records for history, politics, society, culture, etc. Relating to medical history in traditional Korean medicine, there are ceveral researches about disease of King, disease itself, the methods of treatment and so on, through The Annals of the Joseon Dynasty. However, there are few on activities of many doctors in The Annals of the Joseon Dynasty. Methods : I tried to find out the names who had some roles of medicine in The Annals of King Sejong out of The Annals of the Joseon Dynasty. I could get 35 doctors and browsed 35 doctors in The Annals of the Joseon Dynasty again. Finally, I could have lots of articles from The Annals of the Joseon Dynasty related to 33 doctors(2 dontors had no records about medicine even they were doctors). Results : I categorized 2 ways of those articles; medical activities, non-medical activities. For medical activities, I got subcategories for medical activities; medical maltreatment, treatment for King, royal family, bureaucrat, ambassador. I also got subcategories for non-medical activities; publishing medical books, ambassador as a doctor, medical training, things related to hot spring, food therapist, veterinarian. Conclusions : Medical history of Joseon Dynasty in Korean medical history has somehow been recorded by medical books such as Hyangyakjipseongbang, Euibangyuchwi, Euilimchwalyo, Dongeuibogam, Jejungsinpyeon, Dongeuisusebowon, etc. So I have concerned that there are massive records on doctors activities in The Annals of the Joseon Dynasty and tried to focus on their various activities through this research.

몽골과 한국 전통의학의 비교 연구 (A Comparative Study of Mongolian and Korean Traditional Medicine)

  • Objectives The purpose of this study was to investigate the development process and describe the diagnosis methods, theories and treatments of Mongolian traditional medicine and Korean traditional medicine through literature records and prior studies. Methods Literature records and previous studies on traditional medicine of both countries were collected through various sites in Mongolia (Esan, Mongoliajol, Kok, Yumpu, Scribd, Science and Technology Foundation [STF]) and Korea (Koreanstudies Information Service System [KISS], Korea Institute of Science and Technology Information [KISTI], National Digital Science Library [NDSL], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS]). Also the English database was searched through PubMed. In the case of Mongolian traditional medicine, medical books published in Mongolia were mainly referenced and used for research. Results Studying the development process, basic concepts and the system of diagnosis and treatment of the two traditional medicine, several commonalities and differences were revealed. Conclusions This study showed that the scope of diagnosis methods between Mongolian and Korean traditional medicine were slightly different, and that the medical terminology for the diagnosis method had slightly different contents from each other. Although there were many similarities in treatments of Mongolian and Korean traditional medicine, the Chuna therapy is found in Korean traditional medicine only. The basic theories constituting traditional medicine were the same, but the five-element theory used by the two countries differs in the following two factors. Mongolia uses elements of air and space as the theory of five elements, while Korea uses elements of wood and iron.

전통시장의 장소성 기록화를 위한 구성요소 제언 (Suggestions for Archiving Elements for Placeness of Traditional Markets)

  • 본 연구는 전통시장이 갖는 경제적, 대중적, 행정적, 역사 문화적 기능을 중심으로 전통시장의 장소성을 드러낼 수 있는 유 무형 관련 자료의 기록화를 위한 구성요소를 제안하는데 그 목적이 있다. 연구목적 달성을 위해 문헌조사와 서울약령시 스토리 아카이브, 광주 대안예술시장 아카이브 프로젝트 D, 대구 북성로 공구 박물관의 기록화 사례를 조사하였다. 문헌조사를 통해 물리적 환경, 사회적 요소, 경험/인물요소를 전통시장의 장소성 구성요소로 정리하였으며, 이를 바탕으로 전통시장의 대중적 기능, 역사, 문화적 기능, 경제적 기능, 행정적 기능을 중심으로 전통시장의 장소성을 드러낼 수 있는 구체적 기록화 구성요소들을 제시하였다.

침구택일법(鍼灸擇日法)이 수록된 조선의서와 "승정원일기(承政院日記)" 사례 연구 (A Study on Auspicious Days for Acupuncture in Chosun Dynasty)

  • Choosing an auspicious day originated from cosmobiological concepts which were prevalent in traditional society. This method was devised to minimize the damage and maximize the effect of acupuncture and moxibustion. It is found in most of Asian traditional medicine and still regarded as important in Tibet. In Korea, historical records show that various rules were applied to make certain pills, enhance physical condition, avoid illness and to get medical care. Strict rules were also applied to avoid inauspicious days when using acupuncture and moxibustion. But this tradition lost chance of proper evaluation due to involuntary and rapid westernization. Chimgutaekilpyonjip was published in early Chosun dynasty collecting almost all existing methods from the Huangdineijing to Sung-Chin-Yuan medicine. DonguiBogam Yirimchalyo and Chimgukyunghumbang succeeded these methods in separate ways. But they are similar at particular points. In many clinical cases of Seungjeongwonilgi, most methods written in DonguiBogam were used to choose auspicious days, In this study, the characteristic of Chimgutaekilpyonjip and other medical texts of Chosun are compared by analyzing the contents regarding choosing an auspicious day. Secondly, clinical records of Seungjeongwonilgi are examined to assess their practical use. Studying and evaluating the past theories could help accelerate the procedure of setting a proper standard in our times.

진본성 확보를 위한 전자기록물 관리방안 (Management of Electronic Records to Ensure the Authenticity)

  • 전통적인 종이기록물은 그대로 보존하여야 진본이 유지된다. 그러나 전자기록물은 포맷과 메타데이터가 수정 변환되어야 하기 때문에 그 변환이 적법한지에 대한 증명과 적법하지 않은 변경은 없었다는 증명이 모두 필요하다. 이를 위해서는 전자기록물을 획득한 시점에서부터 진본임을 확인하고, 보존기간동안 유실을 방지하여야 하며 진본임을 증명할 방법이 있어야 하고 진본이 아님이 판명 났을 때의 사후 대처방안이 마련되어야 한다. 본 논문에서는 전자기록물 진본성의 취약성을 설명하고 필요기능을 파악하였으며 이를 실현할 방안들에 대하여 제안하고 그 결과 전체적인 전자기록물 관리 방안에 대하여 제시하였다.

가스상 대기오염물질에 의한 종이 기록물의 가속열화 특성 연구 (Studies on the Accelerated Aging Characteristics of Paper Records by Gaseous Air Pollutants)

  • Paper records were generally degraded by some factors from atmospheric environments, like temperature, relative humidity or air pollutants. In this study, the degradation behavior of paper records by single or mixed gases of $NO_2$, $SO_2$, HCHO and TVOC was evaluated. The mechanical, optical and chemical properties of 4 kinds of paper (acid and neutral-based printing paper, traditional Hanji, and filter paper) were directly and indirectly affected by gaseous harmful materials. The brightness and $L^*$ value in all papers were slightly increased by accelerated aging under gaseous HCHO and TVOCs, but highly decreased by conditions under gases $NO_2$ and $SO_2$. The optical properties of paper records were most vulnerable in acid-based paper and high stable in filter paper and traditional Hanji by air pollutant degradation. The aging treatments under mixed gas pollutants including $NO_2$ resulted in decrease of physical, mechanical and optical properties of paper, so it was supposed that the concentration of $NO_2$ gas would be strictly controlled for optimum indoor air quality management in domestic storage centers for paper records.
