중의학 데이터베이스인 TCMID와 TCMSP로부터 『동의수세보원 신축본』에 제시된 사상체질별 본초 선별 연구 (The study on the selection of Sasang Constitution-specific herbs in 『Dongyisusebowon Sinchuk-Bon』 from TCMID and TCMSP)

  • Objectives The network pharmacological study on herbal remedies in Traditional Chinese Medicine (TCM) is actively being carried out based on the TCM databases such as TCMID and TCMSP. It is necessary to select Sasang Constitution-specific (SC-s) herbs from these databases in order to study the prescriptions in Sasang Constitutional Medicine. Methods SC-s herbs and their processing types listed in "Dongyisusebowon Sinchukbon" were searched from TCMID and TCMSP. Results and Conclusions 1. There were total 144 herbs (Soeum 47, Soyang 37, Taeeum 44, Taeyang 16 specific herbs) listed in "Dongyisusebowon Sinchukbon". 2. There were total 130 herbs (Soeum 43, Soyang 37, Taeeum 42, Taeyang 8 specific herbs) searched from TCMID, and total 92 herbs (Soeum 31, Soyang 30, Taeeum 27, Taeyang 4 specific herbs) from TCMID. 3. In some cases, the name given in "Dongyisusebowon Sinchukbon" was different from that in TCMID and TCMSP. It was needed to be careful during the searching process. 4. Although the processing form of some herbs could be searched, there was a significant reduction in the herbal compound information compared to the raw form. It is necessary to reconsider whether or not to use the processing form. 5. Herbal scientific names given in TCMID and TCMSP or used in China and Korea were different each other in some cases. Additional screening process among such herbs would be required based on the efficacy of herbs used in clinical field.

소아 성장에 대한 한의학 치료: 국내 임상연구에 대한 문헌적 고찰 (Treatments of Korean Medicine for Pediatric Growth: A Literature Review of Clinical Studies)

  • Objectives The purpose of this study is to provide evidence of treatment of growth disorder Methods We have reviewed clinical studies of growth disorder in children and adolescent through 6 databases until February, 2020. The searching keywords were "short stature OR dwarfism OR growth disorder" AND "acupuncture OR electric acupuncture OR electroacupuncture OR moxibustion OR herb medicine OR cupping OR Korean medicine OR oriental medicine OR chuna OR pharmacopuncture OR qigong OR traditional medicine OR traditional Korean medicine OR Korean medicine". There was no limit to time and language. Results As a result of the initial search, a total of 270 papers from six domestic databases were found. Among these papers, 156 papers were selected after excluding duplicated papers, and 109 of them were further excluded after checking the title and the abstract. Additionally, 28 papers were excluded by reviewing the full text. The author, year, number of patients, treatment, evaluation tools, and results of a total of 19 papers were included in this study and were summarized. 90.5% of the studies have shown that herbal remedies have improved childhood growth. Conclusions Random control studies and large-scale observational studies are needed in future to show high-quality evidence for the treatment of Korean medicine in pediatric growth.

Anti-inflammatory Effects in LPS-treated RAW 264.7 Cells and the Influences on Drug Metabolizing Enzyme Activities by the Traditional Herbal Formulas, Yongdamsagan-Tang and Paljung-san

  • Objectives: Yongdamsagan-tang (YST) and Paljung-san (PJS) in traditional medicine and finasteride in modern medicine are used to treat benign prostatic hyperplasia (BPH). In recent, the use of combination herbal remedies with conventional drugs has been increasing. Therefore, we investigated the anti-inflammatory effects of these drugs to treat BPH and the influence of herbal formulas on finasteride metabolism. Methods: The inhibitory effects of the herbal formulas and finasteride on the production of inflammatory mediators and cytokines were determined in lipopolysaccharide (LPS)-treated RAW 264.7 cells. Additionally, the influence of herbal formulas on activities of human drug metabolizing enzymes (DMEs) was assessed using human microsomal enzymes. Results: We observed that YST, PJS and finasteride inhibited the production of nitric oxide (NO), prostaglandin E2 (PGE2) and interleukin-6 (IL-6) in RAW 264.7 cells. The half maximal inhibitory concentration (IC50) of YST on PGE2 production was calculated to be below 25 ㎍/mL. YST inhibited the activity of uridine diphosphate-glucuronosyltransterase (UGT) 1A4 with an IC50 value of 49.35 ㎍/mL. The activities of cytochrome P450 (CYP) 1A2, CYP2B6, CYP2C19, CYP3A4, and UGT1A1 were inhibited by PJS (IC50 < 100 ㎍/mL, each). Although PJS and YST inhibited the activities of CYP3A4 and UGT1A4, respectively, these formulas may not influence the metabolism of finasteride because the IC50 values of herbal formulas on DMEs are too high to affect metabolism. Conclusions: Our results suggest that the combination of finasteride and YST or PJS might not influence their drug metabolism and that the drugs may have synergistic effects against BPH.

택사 열수 및 에탄올 추출물의 항산화활성과 human LDL 산화억제 및 ACE 저해효과에 미치는 영향 (Screening of Antioxidative, Anti-atherosclerotic Effect of Alisma Rhizome Extracts)

  • Objectives : The study was conducted to evaluate antioxidative, anti-atherosclerotic and anti-hypertensive effects of natural remedies. Alisma Rhizome (AR) has been used for a long time in Asia in folk remedies for treatment of hypertension and stroke and has been used in Korean traditional medicine for the treatment of glycosuria, gonorrhea, hypercholesterolemia, hypertension, and jaundice and its diuretic effect. These pharmacological effects of AR might come from antioxidant properties of phytochemicals in these materials. Methods : In this study, the antioxidant activity of extract from AR was studied with in vitro methods by measuring the antioxidant activity by TEAC, measuring the scavenging effects on reactive oxygen species (ROS) [superoxide anion, hydroxyl radical] and on reactive nitrogen species (RNS) [nitric oxide and peroxynitrite] as well as measuring the inhibitory effect on $Cu^{2+}$ induced human LDL oxidation and on ACE. Results : The AR extracts were found to have a potent scavenging activity, as well as an inhibitory effect on LDL oxidation and on ACE against all of the reactive species tested, with the water extract showing particularly strong antioxidant activities. Conculsions : The AR extracts have antioxidative, anti-atherosclerotic and anti-hypertensive effects in an in vitro system, which can be used for developing pharmaceutical drugs against oxidative stress and atherosclerosis.

A novel herbal formulation consisting of red ginseng extract and Epimedium koreanum Nakai-attenuated dextran sulfate sodium-induced colitis in mice

  • Background: Ulcerative colitis (UC) is a commonly encountered large intestine disease in the contemporary world that terminates into colorectal cancer; therefore, the timely treatment of UC is of major concern. Panax ginseng Meyer is an extensively consumed herbal commodity in South East Asian countries, especially Korea. It exhibits a wide range of biologically beneficial qualities for almost head-to-toe ailments in the body. Epimedium koreanum Nakai (EKN) is also a widely used traditional Korean herbal medicine used for treating infertility, rheumatism, and cardiovascular diseases. Materials and methods: Separately the anti-inflammatory activities of both red ginseng extracts (RGEs) and EKNs had been demonstrated in the past in various inflammatory models; however, we sought to unravel the anti-inflammatory activities of the combination of these two extracts in dextran sulfate sodium (DSS)-induced ulcerative colitis in mice model because the allopathic remedies for UC involve more side effects than benefits. Results: Our results have shown that the combination of RGE + EKN synergistically alleviated the macroscopic lesions in DSS-induced colitic mice such as colon shortening, hematochezia, and weight loss. Moreover, it restored the histopathological lesions in mice and decreased the levels of proinflammatory mediators and cytokines through the repression of nuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) and nucleotide-binding domain (NOD)-like receptor protein 3 (NLRP-3) expression. In vitro, this combination also reduced the magnitude of nitric acid (NO), proinflammatory mediators and cytokine through NF-κB and mitogen-activated protein kinase (MAPK) pathways in RAW 264.7 mouse macrophage cells. Conclusion: In the light of these findings, we can endorse this combination extract as a functional food for the prophylactic as well as therapeutic treatment of UC in humans together with allopathic remedies.

암 환자의 건강행위 이행경험에 관한 연구 (A Study on Health Behavior Experience of the Cancer Patients)

  • The purpose of this study is to grasp the nature of health behavior to pactice in order to keep and improve the optimal health in the current status of the cancer patients. The subjects were 21 cancer patients, who knew about their disease for themselves, could communicate without mental disease history, and could understand the purpose of this study and cooperate, in a university hospital in Seoul. The data were collected by direct interview from July 15 to Oct. 17, 1994. The interview took about 1~2hours per one time for each paitent by unstructural and open questions. And they were classified into some similar contents on the basis of the phenomenological analysis and categorized. The analyzed results are as follows: 1) In the daily life before and after diagnosis as cancer patients, they were categorized into 6 areas-the state of movement, sleeping, nutrition and diet, society and economy, drinking and somking, and recognition of their health. 2) In the experience of health behavior of cancer patents, they were categorized into 7 areas-the state of movement, sleeping, nutrition and diet, society and economy, drinking and smoking, recognition of their health, and psychology etc. According to the analyzed results of daily life before diagnosis as cancer patients, it turned out that they didn't recognize the problems for their health habit and made their disease state bad by irresolute characteristics which hesitated to practice rightly, renunciation, and irresponsibility and so on, even if they had much interests in their health and were motivated. Therefore, it is necessary to recognize and have an individual-centric interests in order to change the pattern of life for optimal health state to some extent. In the health behavior of cancer patients, it turned out that they had interests in the state of nutrition and diet the most. Even though they experienced the change of serious nutritive state due to the bad gastroenteric trouble by anticancer treatment, they were trying to have a regular eating habit refraining from irritant food and use folk remedies or healthy food temperating the taste food thoroughly, they also showed the sensitive response for nutrition. In addition, they appeared to use the traditional medical treatment or the folk remedies very seriously without abuse. In consideration of it, it is desirable to use them together with the modern medical treatment intercomplementarily and necessary to look into the types for cancer patients and their benefits.

한의학과 아유르베다의학의 약재 비교 고찰(I) (A philological comparative study between the medicinal herbs of Korea Oriental medicine and Ayurvedic medicine(I))

  • Objectives & Methods : To compare the medicinal herbs between Ayurvedic medicine and traditional Korean medicine(TKM), we took reference of major publications related to Ayurvedic medicinal herbs such as Indian Herbal Remedies, Prime Ayurvedic Plant Drugs, with those of TKM. We selected most widely used 130 herbal species of Ayurvedic medicine and compared the similarities and differences with TKM. Comparative factors were the origin, habitation, synonyms, usage, medicinal parts, and precautions. Results : 1. The medicinal herbs Resinatum Lignum(沈香), Arecae Pericarpium(大腹皮), Arecae Semen(檳榔), Carthami Flos (紅花), Camphorum(樟腦), Crotonis Semen(巴豆), Curculiginis Rhizoma(仙茅) used in TKM did exactly correlate in their origins with those of Ayurvedic medicine. 2. Varieties of allied species were found in their origins. Benincasae Pericarpium(冬瓜皮), and Benincasae Semen(冬瓜子) derive from the same plant Benincasa hispida Cogn. for both Ayurvedic medicine and TKM. Interestingly, B. cerifera Savi. is also claimed for same uses in Ayurvedic medicine. This broadened use of allied species is found in various Ayurvedic herbal medicine such as Cannabis Semen(火麻仁) using Cannabis indica Lam., and Curcuma Longae Rhizoma(薑黃) using Curcuma domestica Valeton. This suggests the possibility of their usage also in TKM. 3. Myrrha(沒藥), and Curculiginis Rhizoma concorded their usage with TKM. While Arecae Pericarpium(大腹皮), Arecae Semen(檳榔), Aquilariae Resinatum Lignum(沈香), Pericarpium(冬瓜皮), Benincasae Semen(冬瓜子), Cannabis Semen(火麻仁), Carthami Flos(紅花), Camphorum(樟腦), Crotonis Semen(巴豆), Curcumae Radix(鬱金), Curcuma Longae Rhizoma(薑黃) and Zedoariae Rhizoma(莪朮) revealed varied efficacies according to their part used or usage forms. Conclusion : Both Ayurvedic medicine and TKM reflect the traditional medicine of its regions where is founded. Mutual understanding improves the capability of coping of diverse ailments of present days and also replacing some plants in the days of increasing threat to our environment. Abundant external applications of various plants found in Ayurvedic medicine were particularly useful for TKM to complement its strength in herbal intake.

우리나라 소화기암 환자들의 대체의료이용에 관한 연구 (Use of Complementary and Alternative Medicine among Cancer Patients in Korea)

  • Complementary and alternative medicine (CAM) has gained in popularity among cancer patients in recent years. The use of CAM in cancer patients is common with about one third of patients using some form of CAM in Western countries. The purpose of this study was to analyze the use of CAM and determine what factors affect to use CAM among cancer patients to provide CAM therapy information and assist therapy selection among various CAM therapies to cancer patients. The design of the study was descriptive cross-sectional, and data were collected using a 16-item questionnaire. This study was conducted in subjects with confirmed diagnosis of stomach, colon, liver, or pancreas cancer, in both out-patients clinics and inpatients setting in a tertiary hospital in Seoul Korea. As a result, among the participants, past or current CAM use was reported by 75%, which shows a statistically significant difference in income groups(P<0.05), but no difference in age and religion groups. The most common therapies use by cancer patients included traditional Korean medicine (32.1%), folk remedies (26.6%), exercise (14%), dietary supplements (11.6%), physical therapy (9.9%), diet therapy (5%), and meditation (4%). 77.8% of patients show satisfaction and 64.4% shows perceived effectiveness of CAM. Male patients with higher income, and previous treatment were more likely to use CAM. The main benefits from CAM reported by cancer patients were psychological improvement and symptom improvement. Of the cancer patients used CAM, 30.9% were dissatisfied, 25.8% did not have benefits from the use, and 7.6% experience side effects. Cancer patients who prefer CAM (more than 3 kinds) used it to cure cancer, on the contrary, the one who do not prefer CAM used to improve symptoms and psychological stability. The main sources of information about CAM were family and friends(54.4%), and media(24.5%), doctor and nurse(18.3%), and religion group(2.6%). Findings suggest that due to the relatively high use of CAM among cancer patients in Korea, this topic should be taken into account in the development of a holistic approach to cancer patients and efficient cancer patients management system and proactive and consistent management of CAM is necessary in the health care system in Korea.

Yeosin-san Increases Female Fertility through Inducing Uterine Receptivity and Ovarian Function

  • Despite the development of assisted reproduction technologies (ART) including in vitro fertilization (IVF), the poor ovarian response and endometrial receptivity remains clinically a major unmet need. Although these problems are difficulties to solve in infertility treatment, there are no good therapeutic option yet. Traditional herbal remedies and acupuncture, therefore are being proposed as alternative treatment. Our group found that traditional herbal medicines such as Paeonia lactiflora L.(PL, 芍藥), Cyperus rotundus L.(CR, 香附子), and Perilla frutescens (PF, 紫蘇葉) could improve endometrial receptivity. In this study, we found out Yeosin-san (如神散) as an optimal herbal formula via combination of the previously established herbal medicines. Yeosin-san is a traditional Korean medical formula which was established by Ziming Jin (陳自明) and recorded in Furendaiquanliangfang (婦人大全良方) at first. The formula traditionally used for treating abnormal uterine bleeding and leukorrhea. It showed a highest effect on leukemia inhibitory factor (LIF) expression and on the adhesion between trophoblastic cells and endometrial cells. In addition, it has been shown that the Yeosin-san not only increases the endometrial receptivity to improve the embryo implantation but also enhances the ovary function by expressing the angiogenesis-related genes. Here we suggest that Yeosin-san could be a novel and effective candidate for treating female infertility.

대체의학(代替醫學)과 한의학(韓醫學)의 비교연구(比較硏究) (A Comparative Study of Alternative Medicine with Oriental Medicine)

  • As there has recently been growing interest in Alternative Medicine, Oriental Medicals have been trying to incorporate Alternative Medicine into Oriental Medicine evolutionarily. The purpose of this study is to make a comparative study of the historical background and present condition, the view of life, the principle of therapeutics, and the various therapeutics in Alterative Medicine and Oriental Medicine from the viewpoint of Oriental Medicine. The conclusion of this study is; 1. Although Alternative Medicine has become influential mainly in the effort of finding a solution the limit of western medicine, the clinical effect of Alternative Medicine should be verified continuously. 2. Alternative Medicine includes all of the therapeutics such as Traditional Medicine, Naturopathy, Complementary Medicine and Selective Medicine whose characters are different from Western Medicine and Oriental Medicine can be regarded as a sort of Alternative Medicine. 3. The holistic, systematic view of life of Alternative Medicine, from which viewpoint human considered as a harmonious vital existence, is a sort of recurrence to the view of Hipporcrates. 4. From the viewpoint of Alternative Medicine, a disease is not partial but holistic disorder and is treated by utilizing natural products as a medicine based on the principle that pursues harmonious and balanced psychosomatic function. This principle of therapeutic is the same as Oriental Medicine is based on. 5. Alternative Medicine may be classified largely five category, (1) Eastern Therapy, (2) Manipulative Therapy, (3) Natural Therapy, (4) Active Therapy, (5) Therapy Involving External Powers. 6. Besides Alternative Medicine which is used in clinical treatment, Massage, Homeopathy, Bach Flower remedies, Meditation, and other Music Therapy and Color Therapy is relatively accordant with therapeutics in Oriental Medicine and should be more actively accepted in clinics and improved.
