Review on Molecular and Chemopreventive Potential of Nimbolide in Cancer

  • Cancer is the most dreaded disease in human and also major health problem worldwide. Despite its high occurrence, the exact molecular mechanisms of the development and progression are not fully understood. The existing cancer therapy based on allopathic medicine is expensive, exhibits side effects; and may also alter the normal functioning of genes. Thus, a non-toxic and effective mode of treatment is needed to control cancer development and progression. Some medicinal plants offer a safe, effective and affordable remedy to control the cancer progression. Nimbolide, a limnoid derived from the neem (Azadirachta indica) leaves and flowers of neem, is widely used in traditional medical practices for treating various human diseases. Nimbolide exhibits several pharmacological effects among which its anticancer activity is the most promising. The previous studies carried out over the decades have shown that nimbolide inhibits cell proliferation and metastasis of cancer cells. This review highlights the current knowledge on the molecular targets that contribute to the observed anticancer activity of nimbolide related to induction of apoptosis and cell cycle arrest; and inhibition of signaling pathways related to cancer progression.

"어의촬요(御醫撮要)"-실전의서의 복원 I (${\ulcorner}$Aheuichaulyo御醫撮要${\lrcorner}$-Restoration of the lost medical book in ancient Korea I)

  • $\ulcorner$Aheuichaulyo御醫撮要$\lrcorner$ was known to be published by Choi Jong-jun崔宗峻 in Korea高麗 Dynasty(A.D.1226), however its original copy is not available at the present. It was thought that Korean folk medicine and the remedies were recorded in this book, and these were recorded in $\ulcorner$Euibangyoochui醫方類聚$\lrcorner$ published in Chosun 朝鮮 dynasty(1445). $\ulcorner$Euibangyoochui$\lrcorner$ is called a treasure-house of the knowledge of traditional oriental medicine which contains over 50,000 prescriptions and enormous amount of medical information. The restoration of $\ulcorner$Aheuichaulyo$\lrcorner$was possible during the process of establishment of data base work of $\ulcorner$Euibangyoochui$\lrcorner$. $\ulcorner$Aheuichaulyo$\lrcorner$ was restored as 131 chapters mainly from the related contents of $\ulcorner$Euibangyoochui$\lrcorner$ and partially from the contents of $\ulcorner$Hyangyakjipsungbang鄕藥集成方$\lrcorner$ and $\ulcorner$Boonmoononyukyihaebang分門瘟易解方$\lrcorner$ for the supplement.

  • ${\ll}$Bangyakhappyeon(方藥合編)${\gg}$ was written by Hwang Do-Yeon (黃度淵) and his son Hwang Pil-Soo (黃泌秀) in the late days of Chosun Dynasty, when the country was in a state of social confusion by the first introduced western civilization and invasion of the foreign powers. In response to this situation where realistic knowledge and solution was of importance than theological review, Hwang and his son wrote the book to satisfy the needs. The book, ${\ll}$Bangyakhappyeon(方藥合編)${\gg}$ furthered new publishment of abstract asian medical science in a more compact form, emphasizing more realistic means of it. This new publishment were a significant counter-measure to cope with the inflow of practical western medicine, which the move has moreover enhanced popularization of Han Medicine(韓醫學). After reviewing ${\ll}$Bangyakhappyeon(方藥合編)${\gg}$ in xylographical approach, I have acquired conclusions as below.

Cultural Barriers Influencing Midwives' Sexual Conversation with Menopausal Women

  • Objectives: The present study aimed to determine the barriers influencing the sexual conversation. Methods: In this descriptive cross-sectional study, 200 midwives were selected through convenience sampling method from private and public clinics in Mashhad, North East of Iran. A self-structured questionnaire was used to collect the study data. Results: The mean age of subjects was $39.58{\pm}8.12years$ with $13.49{\pm}7.59years$ of work experience. A number of cultural conditions act as an inhibitory force for the midwives to address sexual issues with menopausal women. Menopausal women visit a doctor at the acute stage when emotional and physical problems make sexual discussion difficult for the midwives (86.5%). Other related causes for not having proper sexual conversation were insufficient knowledge (51.4%), inadequate education provided via public media through health providers (83.5%), midwives or their patient's shame (51.5%), and attempt to get help from traditional healers, friends, relatives and supplicants instead of midwifery staff (78.5%). Also, we found that sexual workshops, communication workshops, and work experiences had a significant influence in changing the views of midwives. Conclusions: Cultural barriers prevent the patients and providers from communicating effectively with each other, thus highlighting the need for sexual and communication workshops for the health care providers.

역대의학성씨(歷代醫學姓氏)의 침(針)과 침(鍼)에 대하여 (Wording on Acupuncture "鍼" & "針" Used by Historic Doctors)

  • From the part "歷代醫學姓氏(Historic Doctors)" in "醫林撮要(Uirimchualyo)", the following has been noticed and concluded. 1. Because acupuncture was originated from stony needle, the word "石(sok)" contains the meaning of needle, and from this point on, words like 石(sok), 砭石(pyumsok), 箴石(Jamsok), 鑱石(Chamsok) had been derived. 2. The word 砭石(pyumsok) used in "Hwangjenaekyong(Yellow Emperor's Canon of Medicine or Hwangdineijing)" should be interpreted as acupuncture in a verb form, not a noun form. 3. 鑱石(Chamsok) or 鑱鍼(Chamchim) was used for surgical treatment for tumor, by cutting open tumors and pressing the pus out. Therefore, 砭石(pyumsok), 鑱石(Chamsok) are the same kind of needles, and 鑱鍼(Chamchim) is the tool improved from 鑱石(Chamsok) used in the Bronze Age. 4. Kwakpak put a note on 鑱石(Chamsok) in "山海經(Sanhaekyong)" that reads "it is defined as 砥鍼(Jichim) and treats tumor." This let us know the shape of 石(sok), 砭石(pyumsok), 鑱(Chamsok), 鑱鍼(Chamchim), and the stone that can be used as a surgical tool with slim & sharp shape is obsidian. 5. Because obsidian is only found around Mt. Baekdu and limited area in South Korea & Japan in Asia, it is closely related with the life & medical environment of the tribe "Mt. Baekdu". 6. The development of 鑱鍼(Chamchim) was influenced by surgical treatment used in early stage of civilization, and its origin is traced upto Gochosun dynasty. Korea's own traditional medical knowledge is derived from this surgical treatment skill. 7. Because the acupuncture is originated from Gochosun dynasty, 鍼(chim) was derived from 箴(Jam) of 箴石(Jamsok), 䥠(Chim) & 䥠(Chim) both were used for a time being, and finally settled into 鍼(Chim). 8. The word 針(Chim) showed up at Myung dynasty, and started to be used in Korea from early Chosun dynasty. 9. In the early Chosun dynasty, 鍼(Chim) was used for medical term, and 針(Chim) for non-medical term. In the mid Chosun dynasty, 針(Chim) was used as a term for tool, and 鍼(Chim) as a term for acupunctural medical treatment. 10. Under the order of King Sunjo, Dr. Yesoo Yang published "醫林撮要(Uirimchualyo)", added "醫林撮要續集(Sequel to Uirimchualyo)", and added "歷代醫學姓氏(Historic Doctors)" again which eventually made totally 13 books of "醫林撮要(Uirimchualyo)". In addition, many parts of "醫林撮要續集(Sequel to Uirimchualyo)" were quoted in "東醫寶鑑(Donguibogam)", and influenced much in publishing "Donguibogam". 11. In "歷代醫學姓氏(Historic Doctors)" of "醫林撮要(Uirimchualyo)", the same way in "Donguibogam", referred to 針(Chim) as a term for a needle, and 鍼(Chim) as a term for Acupuncture. 12. From the usage of 針(Chim) & 鍼(Chim), shown in "鄕藥集成方(Hyangyakjipsungbang)", "醫林撮要(Uirimchualyo)" and "東醫寶鑑(Donguibogam)", we can notice the spirit of doctors who tried to take over the legitimacy of Korean tradition, and their elaboration & historical view that expresses confidence on our own medical technology, through the wording 鍼(Chim).

시뮬레이션을 활용한 외상환자 실습교육이 응급구조학과 학생들의 지식과 만족도 및 문제해결에 미치는 효과 (The Effects of Trauma-patient Training Education Using Simulation on Knowledge, Satisfaction and Problem-solving in Emergency medical Students)

  • 본 연구는 시뮬레이션을 활용한 외상환자 실습교육이 응급구조학과 학생들의 지식과 만족도 및 문제해결에 미치는 효과를 평가하고자 조사하였다. 이를 위해 2015년 3월 3일부터 3월 31일까지 진행하였다. 수집된 자료를 분석하기 위하여 SPSS Win 20.0을 사용하였으며, 대상자의 일반적 특성은 빈도분석 및 동질성 검사, 시뮬레이션을 활용한 외상환자 실습교육 후 지식 및 문제해결은 t-test와 수업만족도의 하위요인인 피어슨상관관계를 이용하였다. 일개 대학의 응급구조학과 학생들의 교육의 적절성을 규명하기 위하여 남녀 학생 28명을 대상으로 실험군 14명과 대조군 14명으로 두 집단으로 나누어 비교하였다. 실험군은 시뮬레이터를 적용한 외상환자 시뮬레이션을 실습교육을 대조군은 전통적인 강의식 수업을 통해 이론 및 실기교육을 실시하였다. 연구결과 4주간의 각각의 교육방식을 진행 후 사후 지식조사와 만족도에 있어서 대조군에 비해 실험군이 유의하게 더 높은 것을 알 수 있다(t=2.09, p<.05). 또한, 만족도 하위요인의 변수간의 상관관계 결과 흥미와 임상에 도움, 또는 흥미와 문제해결능력간의 상관관계를 보였다(p<.05, p<.01). 본 연구 결과를 기초로 응급구조학과의 시뮬레이션 실습교육이 필요하고 적절한 시나리오 개발을 통해 지속적으로 실습교육을 적용해야 할 것으로 사료된다.

한의약건강증진사업 발전을 위한 공중보건한의사 역할 탐색 (Exploring the roles of Korean public health doctors in the development of Korean medicine health promotion project)

  • Objectives : In an effort to suggest a developmental plan of Traditional Korean Medicine health promotion project(TKM-HPP), this study was conducted to identify the problems experienced by public health Traditional Korean Medicine(PH-TKM) doctors in duty and to analyze successful cases of the health promotion project. Methods : Two PH-TKM doctors in duty at the Public Health Centre branch and Public Health Centre respectively, and one TKM doctor in charge of Public Health Centre participated in the in-depth interview. The research procedure was as follows; 1) inform study objectives and interview questions to participants, 2) in-depth interview with semi-structured questions, and 3) post-confirmation and verification. The interview was recorded and transcripted. In-depth interview was composed of 6 subsections: 1) Introducing participants and share purpose of interview, 2) Confirmation of the job status and difficulty of public health centre, 3) Reflecting and sharing experiences of public health clinic job performance cases, 4) Current status and improvement needs of TKM-HPP, 5) Discussions on improvement of TKM-HPP in public health center, 6) Sharing and reconfirmation of interview results. Thematic analysis was conducted from the narratives. Results : The study showed that; 1) the support from the local government was critical for the successful TKM-HPP and participation of the PH-TKMs; 2) the major problems experienced by PH-TKMs were lack of training for TKM-HPP in the field and lack of knowledge at local government level; 3) the suggestions to provide PH-TKMs with manual to educate residents effectively and redefine roles of PH-TKMs in the project. It also suggests that TKM-HPP model should be developed by the district in a way of reflecting regional characteristics. Conclusions : Firstly, more efforts should be made on the projects rather than medical treatment in public health centers, except for areas where medical institutions are scarce. Secondly, in order to facilitate participation of PH-TKM doctors, each local office should reduce medical care and increase efforts related with projects. Thirdly, the institution has to develop feasible TKM-HPP plans that can be carried out by PH-TKMs by the manual. Lastly, pre-training should be provided to PH-TKMs to understand contents of TKM-HPP related to public health centre work before the placement.

소아 식욕부진의 병인, 변증, 치료에 대한 고찰 -중의학 논문을 중심으로- (A Review of Etiology, Pattern Identification, Treatment of Traditional Chinese Medicine for Childhood Anorexia)

  • Objectives This study aimed to provide a basis for applying Korean medical treatment for childhood anorexia in clinical practice by examining Korean medical etiology, pattern differentiation, and treatment, and focusing on research articles on Chinese medicine. Methods Articles on Chinese medicine related to childhood anorexia published before November 4, 2021, in the China National Knowledge Infrastructure (CNKI), were analyzed. The etiology, pattern differentiation, and Chinese medical treatment were summarized. Results Of a total of 73 studies, 13 were randomized controlled trials (RCT), 32 were case studies, and 28 were review papers. The most common Chinese medical etiology of childhood anorexia was emotional instability, and the western medical etiology was problems with diet and lifestyle. The most frequently reported pattern differentiations were spleen-stomach-qi deficiency (脾胃氣虛), stomach-yin deficiency (胃陰不足), and spleen failing in transportation syndrome (脾失健運). The most frequent prescriptions were modified Yangwijeungaektang (养胃增液湯加減), Samryongbakchulsan (蔘苓白术散加减), and Ekongsan (異功散加減). As frequntly used tuina acupoints, Naepalgwae (内八卦), Joksamli (足三里), and Bigyeong (脾經) were mentioned. Conclusions This study analyzed the etiology, pattern differentiation, and Korean medical treatment of anorexia in children. Based on this study, standardization and well-designed clinical studies on Korean medical treatments for childhood anorexia can be expected in the future.

중년기 여성의 폐경경험 (Korean Urban Woman's Experience of Menopause : Newlife)

  • What is the meaning of menopause experienced by urban Korean women? Nurses need an under standing of menopause as it is experienced by women themselves. Nursing needs to build knowledge of womens' health experiences. This phenomenological study examined what menopause means to modern Korean woman to build a structure of knowledge useful for practice to enhance the quality of life of women throughout this experience. Traditional definition of menopause according to physiological changes, as illness and more recently as psychosociocultural phenomena were examined along with the folk lore information generally available in the society A review of the research and scientific literature was done from the perspectives of four models including the medical model of menopause as disease, the psychosocial model as positive and negative behavioral responses to menopause, a feminist model of menopause as a time of rebirth and a nursing model of the changing patterns of meaning, rythms and transformation women experience through menopause. Van Kaam's method was used to analyse data audio-recorded during interviews by the investigator with 65 women, 40 to 60 years of agey whose confidentility was assured. Interpretation of the data was enhanced luther by consultation with professional colleugues and with informants. Four rhythmical patterns of process emerged : from suffering to comfort, from oppression to freedom from being a good wife and wise mother to becoming a woman and from a hard life to an abundant life. The detailed common elements making up each of the four patterns and definitions of each pattern were presented. Each pattern was discussed critically from the point of view of medical, psychosociocultural, womens' and nursing models. The structural definition of the synthesis of the four process patterns was stated as : in spite of suffering the middle-aged urban Korean woman find she is able to help herself to feel comfortable and to realize release as she moves from oppression to liberation and freedom from being a good wife and wise mother she experiences rebirth as a woman : she begins to live a profitable and valuable life : her life becomes one of transformed abundant living. The definition transcends the medical and phychosociocultural model to embody a nursing model. The analysis was critiqued by using Parse' Human Becomming theory of nursing because the emerging themes were process patterns. Parse' theory provides and explanation of the experience of menopause consistant with the data which enhances nursing understanding of womens' experience of menopause. Parse' practice methodology provide guidance for promoting womens' quality of life throughout the experience of menopause. Feminist analysis contributes valuable critique to nursing research, richly expanding the perspective from traditional approaches to promote understanding of the meaning of womens' health experiences.

초음파영상검사와 한의변증분류와의 관계와 관련된 중의학 임상연구에 대한 문헌고찰 (Literature Review of Clinical Studies for the Relationship between Ultrasonographic Examination and Syndrome Differentiation Classification in Chinese Medicine)

  • This study was to investigate the relationship between ultrasonographic examination and pattern identification classification on cinical studies in chinese medicine. We searched clinical studies related correlation between ultrasonographic examination and pattern identification classification in chinese medicine, that published from 2013 to 2016 in China National Knowledge Infrastructure (CNKI) databases by keywords, 'ultrasound(超?)', 'chinese medicine(中?)', 'syndrome differentiation (辨?)'. Seventeen studies were found. There were 7 studies of gynecological diseases including polycystic ovary syndrome and uterine myoma, 5 studies of fatty liver, 3 studies of arthritis, and 1 studie of thyroid nodule and lymphadenopathy respectively. As a result, ii is thought that there was a certain degree of correlation between the change of the ultrasonographic image and the pathological types according to traditional chinese medicine (TCM) syndrome differentiation and ultrasonographic examination could be used as secondary means for the TCM syndrome differentiation classification. In conclusion, by using ultrasonograph device in a medicinal way of TCM and traditional korean medicine (TKM), it is thought that more detailed and accurate diagnosis and treatment are possible and the evidence for reasonableness of syndrome differentiation in TCM and TKM its validity can be secured.
