Sasang Constitutional Medicine (SCM) is based on the theory of Sasang (Taeyang, Soyang, Taeum, Soeum) in Traditional Korean Medicine (TKM). It is hypothesized that all of the things in the material world can be divided into the four categories of SCM. We made a new attempt to combine nutritive components, and patterns in physical activities with SCM. At first, we hypothetically divided the nutrients into 4 compartments according to SCM theory, and human body composition as well. These factors could be basically related to Sasangin Exercise Treatment (SET). Therefore, we applied sasangin theory to exercise prescription. As a research method, we hypothesized that human beings' types can be distinguished by mind and body. Mind was focused on exercise psychological aspects, and body also handled by nutrients, body compositions, body shapes, and exercise physiological aspects. For application of sasang theory to the exercise prescriptions, Taeumin as a sample sasangin was selected in this study. We concluded that in perspective of general traits, Taeumin have to reduce their carbohydrate intake volume, but greatly increase their intake of vitamins, minerals, and water in daily diet. They are also recommended exercises that can increase cardiorespiratory function and make them sweat, like endurance running, hiking, swimming, cycling, etc.
Objectives : To evaluate the evidence supporting the effectiveness of Chuna manual therapy for functional dyspepsia. Methods : We conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for functional dyspepsia. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Thirteen RCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy combined with medication treatments in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with medication plus specific electromagnetic therapy to medication treatments alone. Conclusions : Our systematic review found encouraging but limited evidence of Chuna manual therapy for functional dyspepsia. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend sham-controlled RCTs or comparative effectiveness research to test the effectiveness of Chuna manual therapy.
Objectives: Attention-Deficit/Hyperactivity Disorder (ADHD) is characterized by a persistent pattern of inattention and/or hyperactivity impulsivity that interferes with function or development in children. In traditional Korean medicine (TKM) and traditional Chinese medicine (TCM), ADHD is classified by several patterns based on symptoms and signs. However, currently, there is no objective diagnostic tool for ADHD in traditional medicine. The objective of this study was to develop the Pattern Identification Questionnaire for ADHD (parents-survey style) to be used in Korean medicine, through a literature review and consultation with groups of experts. Methods: The types of pattern identifications of ADHD mentioned in 13 pieces of Korean and Chinese literatures and their symptoms and signs were analyzed. The advisory committee (15 Neuropsychiatrist and 11 Pediatrist in Korean Medicine) assessed the appropriateness of the literature selection and the types of pattern identification selection and their symptoms and signs, and weighed the significance of the symptoms and signs. The Pattern Identification Questionnaire for ADHD was developed using the calculated weights by evaluated significance. The translation of symptoms and signs to the Korean language was achieved through consultation with expert translators. Results: 1. Four pattern identification types and their symptoms and signs were selected according to frequency of appearance in the Korean and Chinese literatures, and were reviewed by the advisory committee: Kidney yin deficiency and liver yang ascendant hyperactivity (腎虛肝亢), Dual deficiencies in the heart and spleen (心脾兩虛), Phlegm-fire harassing the heart (痰火擾心), and Spleen weakness and liver energy preponderance (脾虛肝旺). 2. The weights of all the symptoms and signs in the four patterns were calculated using the means and standard deviations of the symptoms and signs' importance that were obtained from specialists' significance weighting. 3. The Pattern Identification Questionnaire for ADHD (parents-survey style) in Korean medicine composed of 38 questions was suggested. Conclusions: Using a review of the literature and expert advice, Pattern Identification Questionnaire for ADHD (parents-survey style) in Korean medicine was developed. Further clinical study is required to develop a final version of the questionnaire through the evaluation of reliability and validity.
Objectives The purpose of this study is to evaluate the effectiveness of Chuna therapy for primary dysmenorrhea. Methods Researchers searched on 7 electronic databases (Pubmed, National Digital Science Library, China National Knowledge Infrastructure, Wangfang med online, Korean Studies Information Service System, Research Information Sharing Service and Oriental medicine Advanced Searching Integrated System). The search included Korean, English, Chinese reports and there was no limit on the search period. All of randomized controlled clinical trials (RCTs) that used Chuna manual therapy for primary dysmenorrhea were selected. Results 27 RCTs met required condition. Meta-analysis showed positive results for Chuna manual therapy for primary dysmenorrhea in terms of therapeutic effects and reduction of symptom scores compared to west medicine, herbal medicine, acupucture and other treatments. Conclusions Above results showed that performing chuna is effective in treating dysmenorrhea. However, in some studies, there was no statistic significance between the experimental group and the control group. Also, according to Cochrane Risk of Bias (RoB) evaluation method, quality of the studies were not high enough. Since most of the materials were in Chinese, more high-quality clinical trials about Chuna therapy for primary dysmenorrhea are needed in Korea.
From January 2014 to December 2016, 38 out of 153 inpatients admitted to the Korean Medicine Hospital for more than one month underwent follow-up blood tests to evaluate their liver function levels, and one drug-induced liver injury (DILI) patient was observed. 1. At the time of admission, six (15.8%) of the 38 patients had abnormal alanine aminotransferase (ALT) levels and five of them recovered after hospitalization. 2. At follow-up, two (5.3%) patients had a Council for International Organizations of Medical Science (CIOMS)-based liver injury. One was identified as a liver injury accompanied by pneumonia, and one was judged as a liver injury caused by the drug. 3. The patient had a suspected DILI and completely recovered after 13 days while discontinuing the herbal medication and by using Sanggangunbi-tang (生肝健脾湯).
Objectives: This study proposes a new form of English translation in comparison with that shown in English books related to Sasang Constitutional medicine formulae which have been published up to now. Methods: This study formulated a new form of English translation guidelines by making a comparative analysis of the English translation cases listed in "Korean-English Dictionary of Oriental Medicine", "Longevity and Life Preservation in Eastern Medicine", and "Introduction to Sasang Constitutional Medicine". Results: This study as a common rules suggested that names of preparation form should be placed at the very end of the formula name and the first letters of the major words of the formula name should be capitalized. In addition, for the sake of accuracy in English translation of the formula name, this study divided the formula names into 8 types. The 8 sorts of types include: (a) Materia Medica + Preparation Form, (b) Materia Medica + Indication + Preparation Form, (c) Materia Medica Numbers + Preparation Form, (d) Indication + Preparation Form, (e) Concept + Preparation Form, (f) Concept + Materia Medica + Preparation Form, (g) Concept + Indication + Preparation Form, (h) Etcetera. In these types, medicinal herbs should be written in scientific name, and translation of indication can follow Noun+~ing participle form. In case of the terms having a Korean-medicine-based meaning, this study aimed for source-oriented translation. Conclusions: This study provides a general English translation method and examples of the formula names for Sasang constitutional medicine.
Alimentotherapy refers to the treatment or prevention by means of food. As food is consumed in everyday life and its constituents varies enormously. the practice of alimentotherapy was often misunderstood. By the research on ancient classics we could find 3 prospective about this practice. Firstly, alimentotherapy was used during the recovery and also for prevention. This is a type of Life-nurturing method, which we suggest to be called life-nurturing alimentotherapy. Secondly, it was also used as a dietary prescription made by physicians. Sometimes they made prescriptions combining materials derived from food with herbal medication. This practice was exclusively made by physicians as they had to consider the characteristic of each food as well as the condition of the patient. Thirdly, sometime alimentotherapy was used in cuisine form. They used food components in major part and herbal medicine used as additional ingredients. It was primarily focused on the taste and in some part its functional aspects. The underlying theories in alimentotherapy is exactly the same as used for understanding herbal medicine in TKM. Therefore, except in the case of life-nurturing method, it should be used by expert physicians. Finally, we suggest to differentiate the terms of alimentotherapy in the aspect of life-nurturing, dietary prescriptions and cuisine to prevent confusions.
Insampaedok-san is a traditional medicine used for the treatment of colds. We investigated several compounds in Insampeadok-san, and tested their neuroprotective and anti-oxidative activities after fermentation with Lactobacillus. The amounts of four marker compounds (ferulic acid, hesperidin, 6-gingerol and glycyrrhizin) and unidentified compounds in Insampaedok-san (IS) and fermented Insampaedok-san (FIS) were measured and compared by an established HPLC-DAD method. Neuroprotective activity of IS and FIS extracts was evaluated and compared after glutamate-induced neurotoxicity in HT22 cells. Anti-oxidative activity of IS and FIS was also compared in DPPH free radical, hydroxyl radical and hydrogen peroxide scavenging activity assays. Contents of two compounds, ferulic acid and glycyrrhizin were decreased while 6-gingerol was increased by fermentation. FIS showed more potent neuroprotective activity than IS. DPPH, hydroxyl radical and hydrogen peroxide scavenging was slightly increased by FIS when compared to IS. In conclusion, fermentation with Lactobacillus can vary the amounts of the marker compounds in IS and improve neuroprotective and anti-oxidative activities of IS.
Objectives: The aim of this study was to demonstrate the benefit of Traditional Korean Medicine as an adjuvant therapy in management of mid-trimester oligohydramnios. Methods: It is a case report of a 31 year-old woman hospitalized for oligohydramnios at $24^{+4/7}$weeks of gestation. This patient diagnosed with special oligohydramnios had no abnormal findings such as fetal urinary abnormalities or other anomalies. Also, symptom of PPROM (preterm premature rupture of membrane) was not confirmed. The decoction, Gami-danggui-san (DG) was prescribed for the purpose of reducing unnecessary contraction of uterine muscle during pregnancy and promoting blood circulation and metabolism, thereby improving placental function and contributing to the increase of the fluid. DG decoction was administered twice a day until 19th of June, which was 10 days in total. During the treatment, level of amniotic fluid had been monitored by measuring AFI (amnioti fluid index). Results: After these conventional therapies, the amount of amniotic fluid increased steadily, and eventually reached the optimal level. AFI was found to be 3.2 on the $24^{+4/7}$ weeks, 8 on the $26^{+1/7}$ weeks, 11.5 on the $27^{+0/7}$ weeks of gestation. In the same periods, EFW (expected fetal weight) was also found to be increasing gradually: 545 g, 630, and 760 g. Conclusions: Our report implies the potential of herbal medicine as a effective therapy for oligohydramnios tratment. Further studies are needed to assess the efficacy of TKM herbal medicine and reveal the mechanisms of the decoction.
최근 의학기술의 발전과 더불어 서양의학 뿐만 아니라 한의학 분야에서도 임상 데이터에 대한 통합 및 표준화에 관한 연구가 활발히 진행 중에 있다. 유사한 임상시험 뿐만 아니라 전혀 다른 임상시험의 데이터도 하나의 표준에 맞춰 통합 구축된다면 통합된 의료데이터는 암묵적 한의의료지식 도출연구에 활용될 수 있다. 따라서 본 논문에서는 한의임상 정보를 효율적으로 저장하기 위하여 국제표준으로 널리 사용되는 CDISC 표준안을 기반으로 한의임상 데이터베이스를 구축하였고, 임상현장에서 편리한 데이터 입력을 위해 E-CRF를 구축하였다. 아울러, 실제 4개의 임상연구에 대한 데이터 저장과정을 거쳐 한의임상 데이터 통합에 대한 예를 보였다. 우리의 연구 결과는 통합된 데이터로부터 암묵적 의료지식도출을 위한 기반을 마련하였고, 데이터 통합을 통한 효율적 관리뿐만 아니라 반복적이거나 불필요한 임상시험 방지, 정제 된 데이터의 재배포를 통하여 연구의 편리성과 협업을 촉진할 수 있다.
