Treatment packages of persistent allergic rhinitis for developing PRCT protocols : An expert survey (통년성 알레르기 비염의 표준 치료 모델 개발 : 실용임상연구 프로토콜 개발을 위한 기초 연구)

  • Objective : This study was done to define the treatment protocol of Traditional Korean Medicine for persistent allergic rhinitis, which might be necessary for conducting PRCT study. Methods : Data were collected by questionnaire from Korean Medicine doctors participated in the Conference of The Korean Oriental Medical Ophthalmology & Otolaryngology & Dermatology Society in October 2012. We investigated their treatment procedures for persistent allergic rhinitis in ordinary clinical settings. Their treatment patterns including annual treatment period, frequencies and treatment times were also investigated. Finally, we combined the national insurance covered or out-of-pocket treatments as treatment packages according to the response rates of each contents, and displayed them as treatment protocols for PRCT study. Results : 50 Korean medicine doctors described the informed consent and questionnaire. Response rates of each treatment procedure were listed in Table 2 to Table 5. Treatment periods, frequencies and treatment time of each visit were listed in Table 6. Finally, treatment packages which would be defined for PRCT were listed in Table 7 as treatment package 1(response rates were over 50%), package 2(response rates were over 25%), package 3(response rates were over10%). Conclusion : These results provide the rational background and preliminary sources of defining treatment packages for developing PRCT protocols.

Factors influencing the intent to return to practice (work) of inactive RNs (유휴간호사 재취업 의향에 영향을 미치는 요인)

  • The purpose of this study is to examine factors affecting the intent of re-employment of inactive registered nurses. This study presents a secondary analysis of data collected in 'Nurse Turnover On-line Survey' by Korean Nurses Association and Korea Institute for Health and Social Affairs in 2014. The analysis shows that 70.9% of inactive RNs has an intent to return to practice, and most of them preferred 'flexible working options' (47.8%) or 'fixed day shifts' (43.3%) as a work pattern. Main reasons for resigning from their last job have been found to be 'high work intensity' (18.8%) and 'difficulties of night shifts' (16.7%). Inactive married RNs who have working histories in a general hospital or a long-term care hospital or have preferences for traditional shift works showed a stronger intent to return to practice than their reference group. Our study shows that, for inactive RNs to return to practice, it is recommendable to adopt various non-traditional working patterns, to make a staffing distribution considering the labor intensity and to develop education programs designed to increase RNs' professional satisfaction.

Report on the Korean Standard Pattern Identifications for Stroke-III (한의 중풍변증표준안-III에 대한 보고)

  • Objectives : The purpose of this study was to develop the Korean standard pattern identifications for stroke-III (KSPIS-III). KSPIS-III includes 4 major pattern identifications (PIs) and clinical indicators for each. Methods : To extract the indicators for 4 major PIs, we analyzed 1548 clinical data from 15 traditional Korean medicine hospitals. Patients got acute stroke within 30 days from onset. Two physicians independently checked 65 indicators and performed pattern diagnosis. If the PI were diagnosed the same, PI would be confirmed. First we built an assumption model that set up the relationship among pattern identifications. Second, we extracted the indicators for fire-heat pattern and qi deficiency pattern by comparison between excessive and deficiency group, heat and non-heat group. By comparing yin deficiency pattern and 3 other patterns respectively, we extracted the indicators for yin deficiency pattern. Dampness-phlegm pattern indicators were extracted by the same method. Results : After cross tabulation with 65 indicators on the basis of our assumption model, we finally extracted 19 indicators for fire-heat pattern, 11 for qi deficiency pattern, 7 for yin deficiency pattern, and 7 for dampness-phlegm pattern. Conclusions : KSPIS-III was more improved than KSPIS-II because it was based on more clinical data. Further study to establish the PI diagnostic model would be required for practical use in the clinical field.

Preliminary Study to Develop the Instrument of Oriental Medical Evaluation for Lung Cancer (폐암 한의 평가도구 개발 기초 연구)

  • Objectives: The aim of study was to develop a standard instrument of oriental medical evaluation lung cancer. Methods: For this study, the committee advisor was organized by 10 Korean Medicine professors of the Korean Association of Traditional Oncology. The items and structure of the instrument were quoted from the instrument of pattern identification for lung cancer. We had a consultation with the committee twice and took additional advice from it via E-mail. Results: According to the research, we determined the Korean oriental medical assessment tool for lung cancer comprised of the 6 types of patterns of syndrome differentiation. The advisor committee gave the answers about discrimination between bian-zheng and su-zheng, mean weight of each symptom and sign. The final weight was calculated from the combination of the ratio of bian-zheng and mean weight. Conclusions: The instrument of oriental medical evaluation for lung cancer was developed through experts' discussion. If the validity and the reliability of this instrument are validated through additional clinical trial, the instrument of oriental medical evaluation for lung cancer is expected to be applied to the later research.

  • Objectives : Pulse diagnosis is one of the main diagnostic methods of Korean Medicine that understands the patient's condition and illness by reading changes in the patient's pulse, which is described in terms of pulse condition While they are described in detail in medical texts, it is difficult to grasp their true nature, as the written descriptions fail to do justice to the experience of pulse taking it tries to convey. As a way to approach pulse condition the effect of the body's tension on the radial vein was measured using an ultrasonic device, after which the measured changes and how they could be reflected in pulse condition were studied. In other words, changes in the radial vein following induced tension were analyzed. Methods : 1) The thickness of the subject's radial vein was measured using a linear probe of an ultrasonic device[LOGIQ 5 Basic, GE, USA]. 2) Fatigue level was increased through artificial stimulation using the Gripmeter[ks-301, Lavisen, Korea]. 3) Thickness of the radial vein post tension induction was measured. 4) The results were analyzed with the Tukey test or paired t-test as post hoc tests. Results : Thickness of the radial vein of the subject pre- and post- Gripmeter stimulation decreased with significance. Conclusions : Constriction of the radial vein that happened after tension induction could be linked to the Tight Pulse[緊脈] that is related to patterns of contraction and pulling.

A Study on the Locational Patterns of Wedding Shops in Seoul : A Case of Gangnam-Gu (서울시 웨딩 업체의 입지 패턴에 관한 연구 : 강남구를 사례로)

  • Wedding shops, which had been located in Jung-gu, Jongno-gu, Seodaemun-gu, and Mapo-gu in 1990, have been shown a rapid change in light of location since the year of 1995. Thus, most of shops are concentrated on Gangnam-gu in 2010. Wedding dress shops in Gangnam-gu were distributed between Kanglim Church and Apgujeong station in the early 1990s, and then moved gradually to Dosan park and then to Cheongdam-dong after passing through Hakdong intersection. Luxury image in Cheongdam-dong was the main reason for integration of wedding businesses. This intergration of shops showed the peak at the Cheongdam intersection. A motive of having come to start business in Cheongdam-dong was selected the high awareness and image in district, and was taken the ease for business-based cooperation and connection. High rent and inconvenient transportation were reasons for dissatisfaction. The business-based cooperation and connection were mostly formed through a consulting company. Level and reputation of relevant shops were important elements. Even the connective and associative shops are located the most in Cheongdam-dong followed by Nonhyeon-dong, Shinsa Apgujeong-dong in order. The cluster relationship was most noticeable in wedding studio, wedding dress, wedding consulting, and hanbok(Korean traditional clothes) business. Frequency of vertical cluster was indicated to be more than horizontal cluster. User behavior of wedding businesses in Seoul has been collecting wedding information through internet and wedding consulting business.

A Transcultural Study for Testing Models of the Treatment-seeking Behaviors in Patients with Rheumatoid Arthritis (류마티스 관절염 환자의 치료행위 모형 검증을 위한 횡문화적 비교연구)

  • Patients with chronic disease have various treatment patterns because it shows a progressive degenerative feature. Especially various physical and emotional problems of the rheumatoid arthritis patients leave them shopping around various types of treatment. According to previous studies, over 70% of patients with arthritis experienced the traditional oriental medicine or folk remedies simultaneously with medical treatment within one year after the onset of disease. The purposes of this study are 1) to compare the patterns of treatment-seeking behaviors between Korean arthritis patients and Americans ; and 2) test two models of treatment-seeking behaviors by path analysis, one for early treatment-seeking behavior model(ETBM) and the other is chronic treatment-seeking behavior model (CTBM) in Korean sample. The interview survey was performed to 133 RA patients with structured questionnaire at out-patient clinic or public health center. Patients characteristics such as age, duration of disease were similar in two countries except higher educational background in Americans. There were no patients using only alternative therapies or no medical treatment in the US. Most of the American patients have chosen both medical treatment and alternative therapy, while the Koreans less than American. In Korea, combined treatment group usually consists of the people who are younger, more educated and higher economic status than the characters of other groups in early or chronic stages. In early stage, they tend to have strong belief of curing from the disease, satisfy the relationship with their physicians and comply with direction of the medical professional. The paths of two models were explained by 70% in ETBM and 33% in CTBM. When the models were modified, almost all paths of the CTBM were the same as the previous one, but direct determinant factor was changed from the relationship with physicians to the lay referral system in chronic model. These two models' explanation powers became 94% and 88%, respectively. The attitude or perception of disease, lay referral system and the relationship with medical personnel are the main determinants of treatment-seeking behaviors.

Identification of Psychrotrophic Lactic Acid Bacteria Isolated from Kimchi (김치에서 분리한 저온성 젖산균의 동정)

  • The purpose of this study was to identify the psychrotrophic lactic acid bacteria isolated from kimchi, a Korean traditional fermented vegetable food. Thirty isolates of psychrotrophic lactic acid bacteria were isolated randomly from kimchi-A and kimchi-B which were fermented at $5{\sim}7^{\circ}C$ for 20 days and 50 days, respectively. Among 30 isolates of lactic acid bacteria isolated from kimchi-A, 14 isolates were identified as Leuconostoc mesenteroides subsp. mesenteroides, 12 as Leuconostoc mesenteroides subsp. dextranicum and 4 as Lactobacillus bavaricus. Among 30 isolates isolated from kimchi-B, 20 isolates were identified as Lactobacillus bavaricus, 3 as Leuconostoc mesenteroides subsp. mesenteroides, 3 as Leuconostoc lactis, 2 as Leuconostoc paramesenteroides and 2 as Lactobacillus homohiochii. Though these strains were identified as above, there were many strains whose sugar fermenting patterns and $NH_3$ producing ability from arginine were inconsistent with those described in Bergey's Manual of Systematic Bacteriology, and some strains identified as Leuconostoc mesenteroides subsp. mesenteroides and Leuconostoc mesenteroides subsp. dextranicum even disclosed such contradictions as the comparisons of sugar fermenting patterns between the strains of different subspecies were much more coincident than those between the same subspecies. As there were difficulties in classifying these psychrotrophic lactic acid bacteria according to the current taxonomic system, further studies were needed to solve these problems.

Analysis and Prediction Methods of Marine Accident Patterns related to Vessel Traffic using Long Short-Term Memory Networks (장단기 기억 신경망을 활용한 선박교통 해양사고 패턴 분석 및 예측)

  • Quantitative risk levels must be presented by analyzing the causes and consequences of accidents and predicting the occurrence patterns of the accidents. For the analysis of marine accidents related to vessel traffic, research on the traffic such as collision risk analysis and navigational path finding has been mainly conducted. The analysis of the occurrence pattern of marine accidents has been presented according to the traditional statistical analysis. This study intends to present a marine accident prediction model using the statistics on marine accidents related to vessel traffic. Statistical data from 1998 to 2021, which can be accumulated by month and hourly data among the Korean domestic marine accidents, were converted into structured time series data. The predictive model was built using a long short-term memory network, which is a representative artificial intelligence model. As a result of verifying the performance of the proposed model through the validation data, the RMSEs were noted to be 52.5471 and 126.5893 in the initial neural network model, and as a result of the updated model with observed datasets, the RMSEs were improved to 31.3680 and 36.3967, respectively. Based on the proposed model, the occurrence pattern of marine accidents could be predicted by learning the features of various marine accidents. In further research, a quantitative presentation of the risk of marine accidents and the development of region-based hazard maps are required.

Comparison of Ingredient and Efficacy of Galgeun-tang (Gegen-tang) Mix Extract Powder and Decoction (갈근탕 혼합단미엑스제 및 전탕액의 성분 및 생리활성 비교 연구)

  • Galgeun-tang (GGT) is a traditional herbal medicine generally used to treat cold, fever, headache, migraine and convulsion. To investigate the difference of ingredient and efficacy of GGT according to production method, mix extract powder and decoction, the quantities of index components, antioxidant and anti-inflammatory effects of two kinds of GGT formulation were compared. The contents of ten index components were simultaneously analyzed with HPLC. A HPLC method was developed and validated for the simultaneous determination. The GGT mix extract powder contained higher puerarin and daidzin content than the decoction, while the GGT decoction contained higher cinnamic acid and glycyrrhizic acid content than the mix extract powder. But ingredient of GGT mix extract powder and decoction was statistically very similar (r=0.8335). GGT decoction showed higher antioxidant effect than GGT mix extract powder, but the difference was low. The patterns of productions for NO, iNOS, COX-2, TNF-${\alpha}$, IL-$1{\beta}$ and IL-6 were decreased along the increasing dose of GGT formulation in LPS-induced RAW 264.7 macrophages. The inhibitory effects on NO, iNOS, COX-2, IL-$1{\beta}$ and IL-6 in Raw 264.7 cell were very similar to each other. The results obtained in this study could be used as fundamental data for verifying the equivalence and effectiveness of the herbal medicines.
