The Existence and Role of Ji-chong for Medical Exchange in Ancient Korea (지총(知聰)의 실존(實存)과 고대 한국 의학 교류(古代 韓國 醫學 交流)에 대한 역할(役割))

  • Objectives : Considering the indigenousness of Korean medicine, the historical record was first introduced in 1946 as follows; a Chinese person, Ji-chong (知聰), brought 164 volumes of medical books to Japan via Goguryeo (高句麗) in A.D. 562. Since this event happened, Korean Oriental Medicine has been derived from Traditional Chinese Medicine because ancient Korean Medicine originated and was developed in China. The purpose of this study was to investigate the existence and role of Ji-chong in the history of medical exchanges between ancient Korea and Japan. Methods : We studied Ji-chong through ancient and modern historical literatures such as Nihon Shoki (日本書紀), the record of $Shinsen-sh{\bar{o}}jiroku$ (新撰姓氏錄), Korean Medical History (韓國醫學史), Japanese Medical History (日本醫學史), Samguk Sagi (三國史記), etc. Results : We found indications of the existence of Ji-chong and the import of Chinese medical literature to the ancient Korean peninsula by examining domestic and foreign historical literature. Especially, he was closely related to historical assumptions about the Japanese conquest of Goguryeo in A.D. 562, although without objective historical evidence and described only in modern Japanese historical records and Korean Medical History. However, substantial medical exchange toward Japan was accomplished by Korean medicine of either Goguryeo, Baekje (百濟), or Silla (新羅) dynasty until the late A.D. 6 century. Chuna Manual Therapy for Temporomandibular Joint Disorder: Systematic Review (추나요법이 턱관절 증후군에 미치는 영향: 체계적 문헌 고찰)

  • Objectives To determine the effectiveness of Tuina for temporomandibular joint disorder. Methods We searched 10 electronic databases (Pubmed, CNKI, EMBASE, Cochrane Library, KISS, KISTI, NDSL, RISS, KMBASE, DBpia) up to May 2017. We included randomized controlled trials (RCTs) using Tuina for temporomandibular joint disorder. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results 14 RCT studies were eligible in our review. 14 studies were divided into 4 groups, and meta-analysised. The meta-analysis of 6 studies showed favorable results for the use of Tuina. High risk of bias were observed in 9 studies. Conclusions Although there are favorable results with meta-analysis, our systematic review are highly dependent on the single source of Chinese electrical database, CNKI. Now limited evidence is available to support Tuina for temporomandibular joint disorder and further well-designed RCTs should be encouraged.

  • Objectives To determine the effectiveness of Chuna manual therapy combined acupuncture for trigeminal neuralgia. Methods We searched 6 electronic databases (Pubmed, CAJ, Oasis, RISS, DBPIA, KoreanTK) and 2 journals up to August 2016. We included randomized controlled trials (RCTs) using Chuna manual therapy combined acupuncture for trigeminal neuralgia. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results 3 RCT studies were eligible in our review. The meta-analysis of 3 studies showed favorable results for the use of Chuna manual therapy combined acupuncture. High risk of bias were observed in all studies. Conclusions Although there are favorable results with meta-analysis, our systematic review are highly dependent on the single source of Chinese electrical database, CAJ. Now limited evidence is available to support Chuna manual therapy combined acupuncture for trigeminal neuralgia and further well-designed RCTs should be encouraged.

Baduanjin for Hypertension : Systematic Review (팔단금 도인 운동이 고혈압에 미치는 영향 : 체계적 문헌 고찰)

  • Objectives : To determine the effectiveness of Baduanjin for hypertension Methods : We searched 8 electronic databases(Pubmed, CAJ, EMBASE, Cochrane Library, KISS, KISTI, NDSL, RISS) up to April 2017. We included randomized controlled trials(RCTs) using Badanjin for hypertension. The methodological quality of each RCT was assessed by the Cochrane risk of bias tool. Results : 14 RCT studies were eligible in our review. 14 studies were divided into 4 groups, and 2 of them were meta-analysised. The meta-analysis of 10 studies showed favorable results for the use of Baduanjin with Usual care than Usual care. But, meta-analysis of 2 studies showed Baduanjin and Usual care has no difference. High risk of bias were observed in all studies. Conclusions : Although there are favorable results with meta-analysis, our systematic review are highly dependent on the single source of Chinese electrical database, CAJ. Now limited evidence is available to support Baduanjin combined usual care for hypertension and further well-designed RCTs should be encouraged.

Analysis of Domestic and Foreign Research Trends on the Korean Medical Treatment of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사의 한의학적 치료에 대한 국내·외 연구동향 분석)

  • Objectives This paper is an analysis of domestic and foreign trends for the Korean medical treatment of avascular necrosis of the femoral head. Methods Domestic and foreign databases were searched and analyzed for Korean medical treatment interventions, research types, and treatment effects for avascular necrosis of the femoral head. Results There were 10 case reports in Korea, 2 case reports and 3 randomized controlled studies abroad. A total of 12 treatment interventions were used, and the types include acupuncture, herbal medicine, phamacopunture, electroacupuncture, cupping, physical therapy, chuna, acupotomy, moxibustion, warm acupuncture, laser acupuncture, and motion style treatment. As for the evaluation index, visual analogue scale, range of motion and harris hip score were the most common with 7 cases each. Conclusions Through this study, it was possible to understand the trends of domestic and foreign research on the treatment of avascular necrosis of the femoral head with Korean medicine. However, due to the small number of literature and no randomized control studies in Korea, treatment evidence is insufficient. It is considered that further research is needed in the future.

A Systematic Review and Meta-Analysis of Foot Bath Therapy for Insomnia Disorder (불면 장애에 대한 족욕 치료법의 체계적 문헌고찰과 메타분석 연구)

  • Objectives: The purpose of this study was to provide evidence for the clinical effects of foot bath therapy for insomnia disorder through a systematic review and meta-analysis. Methods: Studies were selected from seven domestic and international literature databases. Data on diagnostic tools, pattern identification, sample size, intervention methods, outcome measurements, results, and adverse events were extracted. The quality of the selected literature was assessed using Cochrane's risk of bias (RoB) assessment tool. Results: A total of 11 studies were included in this study. The primary diagnostic tool for insomnia dis- order was the criteria for the diagnosis and therapeutic effect of diseases and syndromes in traditional Chinese medicine (CDTDSTCM), along with the Chinese classification of mental disorders-3 (CCMD-3). Treatment effects were mainly evaluated by the effective rate and the Pittsburgh Sleep Quality Index (PSQI). The meta-analysis results showed statistically higher effectiveness rates for treating insomnia disorder in the experimental groups receiving herbal medicine foot bath therapy with sleeping or herbal medication compared to the control group. The PSQI was lower in the treatment group receiving herbal medicine foot bath therapy with sleeping medication. The herbal materials used in herbal medicine foot bath therapy mainly included Radix Angelicae Sinensis, Polygonum Multiflorum, Ziziphi Spinosae Semen, Rhizoma Coptidis, Radix Salviae Miltiorrhizae, and Cortex Albizziae. The quality of the included studies was generally poor. Conclusions: This study found that foot bath therapy had clinical efficacy for treating insomnia disorder. The research findings could provide foundational evidence for establishing the clinical basis of foot bath therapy in the treatment of insomnia.

Review of non-clinical experimental studies on precocious puberty using herbal medicine (한약을 이용한 성조숙증에 대한 비임상 연구 보고 고찰)

  • Objectives : This study aimed to provide basic data for research by investigating non-clinical experimental studies on herbal medicines and its compounds for precocious puberty. Methods : A search was conducted for all literature until October 2023 using combinations of keywords such as precocious puberty, puberty, and chinese medicine in three databases (Pubmed, OASIS, and ScienceON). Results : 1. In animal experiments, studies were mainly conducted using a model that induced precocious puberty by subcutaneously administering danazol to SD rats on the 5th day after birth, and in cell experiments, precocious puberty was induced by treating GT1-7 cells with kisspeptin 10 or estradiol. 2. Anemarrhenae Rhizoma, Phellodendri Cortex, and Prunellae Spica were mainly used as herbal medicine to evaluate their efficacy on precocious puberty in non-clinical experiments. 3. Macroscopic observation, hematological analysis, histological analysis, and genetic analysis were performed as methods to analyze the experimental results. Conclusions : In this study, the effects of herbal medicine on precocious puberty and non-clinical research methods were confirmed. Based on the results of this study, it is expected that non-clinical effectiveness and mechanism research on materials that are clinically effective in Traditional Korean Medicine will be revitalized.

A Review of Studies Using Syndrome Differentiation Questionnaire in Cancer Patients (암 환자 대상 변증 설문지 활용 현황에 대한 문헌고찰)

  • Objective: The aim of this review is to analyze the studies using syndrome differentiation questionnaire in cancer patients. Methods: We searched electronic databases including Pubmed, google scholar, Cochrane library, CNKI, KISS, RISS and OASIS. Key words used for searching were cancer, Korean medicine, pattern identification, and questionnaire. Studies using a symptom differentiation questionnaire to cancer patients were selected. Results: 35 studies were enrolled. A total of 17 questionnaires was used. Most of the types of included studies were observational studies, followed by randomized controlled trials (RCTs) and validation studies. The purposes of using questionnaires were rrelation analysis, outcome measurement, evaluating adverse events, subgroup analysis, and questionnaire development. The most used questionnaire was Body Constitution Questionnaire (BCQ), and it was used 8 times, Questionnaire for the Sasang Constitution Classification II (QSCC II) was used 5 times, Constitution in Chinese Medicine Questionnaire (CCMQ), TCM-Symptom Complex Differentiation Questionnaire (TCM-SCDQ), Yin Deficiency Questionnaire were used 4 times, and Qi Blood Yin Yang Deficiency Questionnaire was used twice. BCQ is a questionnaire diagnosing and evaluating yang deficiency, yin deficiency, and blood stasis. It has high reliability, validity, and optimal cut-off value. Conclusion: BCQ is the most used syndrome differentiation questionnaire in cancer-related studies. So, BCQ could be recommended in syndrome differentiation-related cancer studies.

Network pharmacological analysis for exploration of the potential application of Hwangryunhaedok-tang for brain diseases (황련해독탕(黃連解毒湯)의 뇌질환 응용 가능성 탐색을 위한 네트워크 약리학적 분석)

  • Objectives : To explore the associated potential pathways and molecular targets of Hwangryunhaedok-tang(HHT) by the approaches of network pharmacology and bioinformatics in traditional chinese medicine(TCM). Methods : Hwangryunhaedok-tang constituent drugs(Coptidis Rhizoma, CR; Scutellariae Radix, SR; Phellodendri Cortex, PC; Gardeniae Fructus, GF) and their processing types were searched from TCM systems pharmacology(TCMSP). The databases of TCMSP, Kyoto Encyclopedia of Genes and Genomes(KEGG), MCODE and STRING were used to gather information. The network of bioactive ingredients and target gene was constructed by Cytoscape software(version 3.8). Results : A total of 94 HHT active compounds(CR, 12; SR, 35; PC, 33; GF, 14, respectively) were found, and HHT were identified by TCMSP. Applications of KEGG and MCODE analysis indicates that total of 6 bioactive ingredients in the top 10% ranking were obtained and 32 diseases of HHT were screened. The molecular pathway analysis revealed that HHT exerts cancer, inflammation and cerebrovascular diseases effects by acting on several signaling pathway. In addition, HHT found that three genes(e.g. SPIN1, TRIM25, and APP) correlate with the aforementioned diseases. Conclusions : This study showed that network pharmacology analysis is useful to elucidate the complex mechanisms of action of HHT.

A proteomic approach reveals the differential protein expression in Drosophila melanogaster treated with red ginseng extract (Panax ginseng)

  • Background: Red ginseng is a popularly used traditional medicine with antiaging effects in Asian countries. The present study aimed to explore the changes in protein expression underlying the mechanisms of life span extension and antiaging caused by red ginseng extract (RGE) in Drosophila melanogaster. Methods: A proteomic approach of two-dimensional polyacrylamide gel electrophoresis (2-DE) was used to identify the differential abundance of possible target proteins of RGE in D. melanogaster. The reliability of the 2-DE results was confirmed via Western blotting to measure the expression levels of selected proteins. Proteins altered at the expression level after RGE treatment (1 mg/mL) were identified by matrix-assisted laser desorption/ionization-time of flight tandem mass spectrometry and by searching against the National Center for Biotechnology nonredundant and Uniprot protein databases. The differentially expressed proteins were analyzed using bioinformatics methods. Results: The average survival life span of D. melanogaster was significantly extended by 12.60% with RGE treatment (1 mg/mL) compared to untreated flies. This followed increased superoxide dismutase level and decreased methane dicarboxylic aldehyde content. Based on the searching strategy, 23 differentially expressed proteins were identified (16 up-regulated and 7 down-regulated) in the RGE-treated D. melanogaster. Transduction pathways were identified using the Kyoto Encyclopedia of Genes and Genomes database, and included the hippo and oxidative phosphorylation pathways that play important roles in life span extension and antiaging process of D. melanogaster. Conclusion: Treatment with RGE in D. melanogaster demonstrated that mechanisms of life span extension and antiaging are regulated by multiple factors and complicated signal pathways.
