Traumatic Tracheobronchial Injury: Delayed Diagnosis and Treatment Outcome

  • Background: Most traumatic tracheobronchial injuries are fatal and result in death. Some milder cases are not life threatening and are often missed at the initial presentation. Tracheobronchial rupture is difficult to diagnose in the evaluation of severe multiple trauma patients. We reviewed the traumatic tracheobronchial injuries at Konyang University and Eulji University Hospital and analyzed the clinical results. Materials and Methods: From January 2001 to December 2011, 23 consecutive cases of traumatic tracheobronchial injury after blunt trauma were reviewed retrospectively. We divided them into two groups by the time to diagnosis: group I was defined as the patients who were diagnosed within 48 hours from trauma and group II was the patients who diagnosed 48 hours after trauma. We compared the clinical parameters of the two groups. Results: There was no difference in the age and gender between the two groups. The most common cause was traffic accidents (56.5%). The Injury Severity Score (ISS) was 19.6 in group I and 27.5 in group II (p=0.06), respectively. Although the difference in the ISS was not statistically significant, group II tended toward more severe injuries than group I. Computed tomography was performed in 22 cases and tracheobronchial injury was diagnosed in 5 in group I and 6 in group II, respectively (p=0.09). Eighteen patients underwent surgical treatment and all four cases of lung resection were exclusively performed in group II (p=0.03). There were two mortality cases, and the cause of death was shock and sepsis. Conclusion: We believe that close clinical observation with suspicion and rigorous bronchoscopic evaluation are necessary to perform diagnosis earlier and preserve lung parenchyma in tracheobronchial injuries from blunt trauma.

둔상에 의한 외상성 기도손상의 외과적 치료 (Surgical Treatment of Tracheobronchial Injury due to 81un1 Trauma)

  • 1990년 1월부터 1994년 12월까지 4년간의 들상에 의한 외상성 기도손상으로 10명의 환자가 응급실로 내원하였다. 7명은 자동차 사고였고 3명은 산업재 해였다. 내원시 주증상은 피하기종, 호흡부전, 각혈 등이였다. 방사선학적 소견은 주로 기흥, 종격동기흥, 혈흥 등이 었다. 기관지내시경은 내원 초기에 매우 유효한 검사이다. 파열부위는 우폐 전적출술을 한 1명을 제외하고 단순봉합하였다. 술후 1명이 폐좌상으로 인한 급성호흡부전으로 사망하였으며 9명은 별다른 합병증없이 일상생활로 복귀하였다. 외상성 기도손상환자의 치료에 있어서 중요한 것은 조기진단과적절한 치료이다.

Tracheobronchial Polyps Following Thermal Inhalation Injury

  • The early pulmonary consequences of inhalation injury are well documented; however, little is known about delayed pulmonary complications following thermal inhalation injury. Although thermal injury below the vocal cords is rare because of effective heat dissipation in the upper airway, inflammatory endobronchial polyps have previously been reported as a delayed complication associated with inhalation injury. We report an extraordinary case of tracheobronchial polyps in patients with smoke inhalation injury. This report shows the delayed development and natural course of tracheobronchial polyps following thermal injury.

The Surgical Outcome for Patients with Tracheobronchial Injury in Blunt Group and Penetrating Group

  • Purpose: Tracheobronchial injuries caused by trauma are rare, but can be life threatening. The objective of this study was to evaluate the surgical outcome for patients with tracheobronchial injuries and to determine the difference, if any, between the outcomes for patients with penetrating trauma and those for patients with blunt trauma. Methods: From January 2010 to June 2015, 40 patients underwent tracheobronchial repair surgery due to trauma. We excluded 14 patients with iatrogenic injuries, and divided the remaining 26 into two groups. Results: In the blunt trauma group, injury mechanisms were motor vehicle accident (9 cases), free falls (3 cases), flat falls (1 case) and mechanical injury (1 case). In the penetrating trauma group, injury mechanisms were stab wounds (10 cases), a gunshot wound (1 case) and a stab wound caused by metal pieces (1 case). The mean RTS (Revised Trauma Score) was $6.89{\pm}1.59$ (range: 2.40-7.84) and the mean ISS (Injury Severity Score) was $24.36{\pm}7.16$ (range: 11-34) in the blunt group; the mean RTS was $7.56{\pm}0.41$ (range: 7.11-7.84), and the mean ISS was $13{\pm}5.26$ (range: 9-25) in the penetrating trauma group. In the blunt trauma group, 9 primary repairs, 1 resection with end-end anastomosis, 2 lobectomies, 1 sleeve bronchial resection and 1 pneumonectomy were performed. In the penetrating trauma group, 10 primary repairs and 2 resections with end-end anastomosis were performed. Complications associated with surgery were found in one patient in the blunt trauma group, and one patient in the penetrating trauma group. No mortalities occurred in either groups. Conclusion: Surgical management of a traumatic tracheobronchial injury is a safe procedure for both patients with a penetrating trauma and those with a blunt trauma.

Surgical Repair of a Traumatic Tracheobronchial Injury in a Pediatric Patient Assisted with Venoarterial Extracorporeal Membrane Oxygenation

  • Tracheobronchial rupture due to blunt chest trauma is a rare but life-threatening injury in the pediatric population. Computed tomography (CT) is not always reliable in the management of these patients. An additional concern is that ventilation may be disrupted during surgical repair of these injuries. This report presents the case of a 4 -year-old boy with an injury to the lower trachea and carina due to blunt force trauma that was missed on the initial CT scan. During surgery, he was administered venoarterial extracorporeal membrane oxygenation (ECMO). Although ECMO is not generally used in children, this case demonstrated that the short-term use of ECMO during pediatric surgery is safe and can prevent intraoperative desaturation.

Robotic Intraoperative Tracheobronchial Repair during Minimally Invasive 3-Stage Esophagectomy

  • Tracheobronchial injury (TBI) is an uncommon but potentially fatal event. Iatrogenic lesions during bronchoscopy, endotracheal intubation, or thoracic surgery are considered the most common causes of TBI. When TBI is detected during surgery, concomitant surgical treatment is recommended. Herein we present a case of successful robotic primary repair of iatrogenic tracheal and left bronchial branch tears during a robot-assisted hybrid 3-stage esophagectomy after neoadjuvant chemoradiotherapy. A robotic approach can facilitate the repair of this injury while reducing both the potential risk of conversion to open surgery and the associated increased risk of postoperative respiratory complications.

외상성 기관-기관지 손상의 진단 방법 (Diagnostic Methods of Traumatic Tracheobronchial Injury)

  • 배경: 이 연구는 외상성 기관-기관지 손상의 특징적인 임상 증상 및 영상학적 소견 등에 대하여 알아보고자 하였다. 대상 및 방법: 2003년 1월부터 2009년 12월까지 본원 응급실을 통해서 내원한 외상 환자들 중에서 수술을 통해서 외상성 기관-기관지 손상으로 진단된 6명의 환자를 대상으로 하였다. 외상의 종류, 동반된 손상, 진단 방법 및 수술까지 걸린 시간, 수술 소견 및 수술 방법, 예후 등에 대해서 조사해 보고 후향적으로 진단에 중요한 인자 등을 알아 보았다. 결과: 손상의 원인으로는 교통사고가 1명, 낙상 및 흉부에 강한 압박을 받은 경우가 5명이었다. 주 증상으로 피하기종, 호흡 곤란, 통증 등이 있었으며 영상소견으로는 기흉, 종격동 기종, 혈흉, 늑골 골절, 폐좌상 등이 있었다. 기관지 내시경을 시행하지 않은 상태에서 2명에서는 흉부 CT 소견에서 기관-기관지 손상이 의심되었지만 나머지 4명에서는 의심하지 못했다. 수상 부위는 기관부위가 2예, 기관지가 4예 있었다. 수술은 개흉술을 통한 일차 문합을 시행하였고 수술 후 사망과 문합 부위 유출은 없었으며 1명에서 술 후 성대 마비가 있었다. 진단에 도움이 되는 특징적인 소견으로는 흉관 삽입 후 음압의 적용에도 불구하고 지속적인 심한 폐 허탈이 가장 중요한 소견이었으며, 흉부 CT에서의 기관-기관지의 주행 경로의 단절이 진단에 중요하였다. 결론: 외상성 기관-기관지 손상은 의심하지 않으면 진단이 쉽지 않으나, 특징적인 흉관 삽입 후의 임상 증상과 영상 소견은 진단에 큰 도움을 주었다.

외상으로 인한 엽성 기관지파열: 수술1 예 (Traumatic Rupture of Lobar Bronchus - One case report -)

  • The tracheobronchial rupture from blunt trauma is an uncommon injury with a variety of clinical presentations. Most of traumatic tracheobronchial tears occur within 2.5cm of the tracheal carina; lobar or segmental bronchi are rarely affected. Recently we experienced one case of traumatic rupture of right middle lobar bronchus in a patient who had fallen from a bicycle. Bronchoscopy showed linear disruption of the right middle lobe bronchus at its bifurcation site. Emergency right middle lobectomy was performed. The patient was recovered without any sequelae.

전통적 인공호흡과 고빈도환기시 폐의 조직.병리학적 소견의 비교 관찰 (The Comparison of Histopathology of Cats Received Conventional Mechanical Ventilation and High Frequency Oscillation Ventilation)

  • 고빈도환기법시에 생길 수 있는 기관 및 기관지의 손상의 정도를 알아 보고자 저자들은 13마리의 고양이를 대상으로 CMV와 HFOV을 각각 6시간동안 사용후 폐의 조직 병리소견을 비교하여 다음과 같은 결과를 얻었다. 1) CMV와 HFOV 사용군에서 기관 및 기관지 손상의 정도의 차이에는 통계적으로는 유의한 차이가 없었다. 2) 상피 세포내 점액의 소실은 CMV군에서 다소 더 심하였다. 3) 폐실질병변으로 폐기종은 CMV군에서 더 심하였다. 이상의 결과로 건강한 고양이에서 비교적 짧은 기간동안에 사용한 CMV와 HFOV군에서 기관 및 손상의 차이는 저명하지 않았으나, CMV군에서 다소 더 세포기능의 장애가 초래되었을 것으로 생각되며 또한 폐압손상도 더 받았을 것으로 추측된다.

외상성 기관지 파열 -1례 보고- (Tracheobronchial Rupture following Blunt Chest Trauma -1 case report-)

  • The rupture of tracheobronchial tree caused by non penetrating blunt trauma is being increased in incidence though it is still rarely occurred on now Because this type of injury is uncommon, a high index of suspicion should be maintained in all crush injuries involving the chest. Early diagnosis and primary repair not only restore normal lung function but also avoid the difficulties and complications associated with delayed diagnosis and repair. We experienced a case of tracheobronchial rupture caused by non penetrating blunt chest trauma without rib fracture. The patient was a 16 year old male who was a high school student. He was compressed on anterior chest by hand ball goal post being failed down on the morning of admission day. After this accident, he was suffered from progressively developing dyspnea and subcutaneous emphysema on face, neck and anterior chest. The diagnosis, tracheal rupture, was made by chest CT and bronchoscopy. After right thoracotomy, the ruptured site was directly closed by using interrupted suture. Post-operative course was uneventful. Thus we report this case of traumatic tracheal rupture with review of literature.
