Clinical Study on Lobaplatin Combined with 5-Fu and Concurrent Radiotherapy in Treating Patients with Inoperable Esophageal Cancer

  • Objective: To investigate short- and long-term treatment effects and side reactions of lobaplatin plus 5-Fu combined and concurrent radiotherapy in treating patients with inoperable middle-advanced stage esophageal cancer. Methods: Sixty patients with middle-advanced stage esophageal squamous cell cancer were retrospectively analyzed. All patients were administered lobaplatin (50 mg intravenously) for 2 h on day 1, and 5-Fu ($500mg/m^2$) injected intravenously from day 1 to 5 for 1 cycle, in an interval of 21 days for totally 4 cycles. At the same time, late-course accelerated hyperfractionated three-dimensional conformal radiotherapy was performed. Patients were firstly treated with conventional fractionated irradiation (1.8 Gy/d, 5 times/week, a total of 23 treatments, and DT41.4 Gy), and then treated with accelerated hyperfractionated irradiation (1.5 Gy, 2 times/d, a total of 27 Gy in 9 days, an entire course of 6-7 weeks, and DT 68.4Gy). Results: All patients completed treatment, including 10 complete response (CR), 41 partial response (PR), 7 stable disease (SD), and 2 progressive disease (PD). The total effective rate was 85.0% (51/60). Thirty-nine patients had an increased KPS score. One-, 2-, and 3-year survival rates were 85.3%, 57.5%, and 41.7%, respectively. The median survival time was 27 months. The adverse reactions included myelosuppression, which was mainly degree I and II. The occurrence rate of radiation esophagitis was 17.5%. No significant hepatic or renal toxicity was observed. Conclusion: Lobaplatin plus 5-Fu combined with concurrent radiotherapy is safe and effective in treating patients with middle-advanced stage esophageal cancer. However, this result warrants further evaluation by randomized clinical studies.

Sleep Quality and Attention May Correlate With Hand Grip Strength: FARM Study

  • Objective To determine the socio-demographic, psychologic, hematologic, or other relevant factors associated with hand grip strength in Korean farmers. Methods A total of 528 healthy Korean farmers were enrolled. Hand grip strength was measured in both hands using a hydraulic dynamometer. Socio-demographic characteristics were assessed and anthropometric measurements were obtained. Psycho-cognitive measurements such as sleep quality (Pittsburgh Sleep Quality Index) and Go/No-Go test response time were conducted. In addition to physical measurements, serologic parameters including insulin-like growth factor 1 were measured. The factors associated with hand grip strength were analyzed using multiple linear regression analysis after adjusting for age, height, and weight. Results The mean hand grip strength was associated with the Pittsburgh Sleep Quality Index total score (${\beta}=-0.12$, p=0.01), the Go/No-Go test response time (${\beta}=-0.18$, p=0.001), vitamin D (${\beta}=0.12$, p=0.02), and insulin-like growth factor 1 levels (${\beta}=0.1$, p=0.045). In female farmers, hand grip strength was only associated with the Pittsburgh Sleep Quality Index total score (${\beta}=-0.32$, p<0.001). Conclusion The results of this study demonstrate that hand grip strength was associated with sleep quality and attention in Korean farmers.

Correlations of Infant Temperament, Mother-Infant Interaction, and Child-rearing Environment (영아 기질과 모아상호작용, 양육환경과의 관계)

  • The purpose of this correlational study was to find the correlation between the infant temperament, mother-infant interaction, and child-rearing environment. The subjects of this study were 37 dyads of healthy mothers and healthy infants. Data were collected from 15th of March to 3rd of September, 1999. Convenient sampling was done at Obstetric wards of one University hospital, and demographic data were collected before discharge. At one month and three month postpartum, we visited subject's home, and collected the data on the infant temperament, and also video taped the mother-infant interaction during feeding. In addition, child-rearing environment was checked by researcher according to HOME(Home Observation for Measurement of the Environment) at three months postpartum. Mother-infant interactions were rated according to the NCAST Feeding Scale later. Data were analyzed by window SPSS program, and correlations between the infant temperament, mother-infant interaction, and child-rearing environment were analyzed by Pearson's correlational coefficient. The results were as follows: 1. Infant temperament. 1) Among the subscales of infant temperament, mothers perceived cuddliness and amenability most positively at one month, and responsivity and amenability most positively at three months. 2) In subscale analysis of stability, amenability, responsivity, and persistence were stable with the time. 3) Significant relationships were found between the malleability and amenability, between the malleability and responsivity at one month, and also between the malleability and amenability, and between the malleability and cuddliness at three months. 2. Correlations of infant temperament and mother-infant interaction. 1) There was no significant relationship between the infant temperament and mother-infant interaction at one month. 2) There was a significant relationship between the infant temperament and mother-infant interaction at three months(r=.335, p<.05). In subscale analysis, there were significnt relationships between the total score of infant temperament and maternal sensitivity to infant's cues(r=.372, p<.05), and between the total score of infant temperament and maternal response to infant's distress (r=.331, p<.05). 3. Correlations of infant temperament and child-rearing environment. 1) There was no significant relationship between the total score of infant temperament at one month and total score of HOME at three months. In subscal analysis, the total score of infant temperament at one showed significant relationships with the organization of environment(r=.413, p<.05), and the emotional, verbal response at three months(r=.337, p<.05). 2) There was a significant relationship between the total score of infant temperament at three months and the total score of HOME at three months (r=.599, p<.01). In subscal analysis, the total score of infant temperament at three months showed significant relationships with the organization of environment(r=.410, p<.05), maternal involvement(r=.482, p,.01), and the emotional, verbal response(r=.695, p<.01) at three months. 4. Correlations of mother-infant interaction and child-rearing environment. There was a significant relationship between the maternal score of mother-infant interaction at one month and three months and the total score of HOME at three months (r=.474, p<.01; r=.452, p<.01). In conclusion, it was proved that infant temperament had significant relationships with mother-infant interaction and child-rearing environment, especially when the infants were getting older. This showed the possibility for changeability of infant teperament by the maternal factors.

A HMM-based Method of Reducing the Time for Processing Sound Commands in Computer Games (컴퓨터 게임에서 HMM 기반의 명령어 신호 처리 시간 단축을 위한 방법)

  • In computer games, most of GUI methods are keyboards, mouses and touch screens. The total time of processing the sound commands for games is the sum of input time and recognition time. In this paper, we propose a method for taking only the prefixes of the input signals for sound commands, resulting in the reduced the total processing time, instead of taking the whole input signals. In our method, command sounds are recognized using HMM(Hidden Markov Model), where separate HMM's are built for the whole input signals and their prefix signals. We experiment our proposed method with representative commands of platform games. The experiment shows that the total processing time of input command signals reduces without decreasing recognition rate significantly. The study will contribute to enhance the versatility of GUI for computer games.

Evaluation Model of Optimal Operating Conditions for Aquaponics Pretreatment Using Response Surface Methodology (반응표면법을 이용한 아쿠아포닉스 전처리조 최적 운전 조건 평가 모델)

  • The aim of this research was to apply a method designed to derive the factors influencing total ammonia removal when operating an additional pretreatment system at Aquaponics. The Box-Behnken method, among response surface analysis methods was used to characterize and determine the optimal nitrification conditions when operating the pretreatment system. Among the mathematically and statistically calculated prediction equations, the total ammonia nitrogen concentration Y1 measured on day 8 was derived as Y1=-195.8+2.23X1+42.9X2+47.5X3+0.1856X12-1.380X1X2-1.770X1X3, and the time taken to reach the maximum total ammonia nitrogen concentration during the experiment period was derived as Y2=271-5.04X1+60.5X2-64.8X3+0.1654X12+6.54X32-0.600X1X3-9.00X2X3. The coefficients of determination of the regression models of Y1 and Y2 were 93.99% and 94.46%, respectively. The modified coefficients of determination were also high, at 89.48% and 88.91%, respectively. The prediction coefficients of determination of Y1 and Y2, were 70.68% and 62.11%, respectively, which was relatively lower than that of Y1, but still indicated a reliable prediction performance.

How Background Music Affects Consumer Perception of Waiting Time? -A Mediating Role of Emotions-

  • The present study examines whether consumers' perception of waiting time and their behavioral intentions are influenced by the presence of background music in an online shopping environment. In particular, this study aims to explain the underlying mechanism for the effects of background music on consumers' perceived wait and behavioral intentions toward the online retailer by proposing the mediating role of emotions drawing on the Stimulus-Organism-Response (S-O-R) framework. A lab experiment was employed to test the hypotheses. A total of 102 college students were used for data analyses. Results show that the presence of music has a significant impact on participants' emotions, perceived waiting time, and approach behavior. Moreover, the findings show that pleasure and the perceived waiting time serve as mediators in the relationship between the presence of music and approach behavior. Implications of the model for theory, practice, and further research are discussed.

A Study on the Algorithm for Nonlinear Dynamic Response Analysis of Shell Structure (쉘 구조물의 비선형 동적응답 해석을 위한 Algorithm에 관한 연구)

  • The main intention of this paper is to develop and compare the algorithm based on finite element procedures for nonlinear transient dynamic analysis which has combined effects of material and geometric nonlinearities. Incremental equilibrium equations based on the principle of virtual work are derived by the finite element approach. For the elasto - plastic large deformation analysis of shells and the determination of the displacement-time configuration under time-varying loads, the explicit, implicit and combined explicit-implicit time integration algorithm is adopted. In the time structure is selected and the results are compared with each others. Isoparametric 8-noded quadrilateral curved elements are used for shell structure in the analysis and for geometrically nonlinear elastic behaviour, a total Lagrangian coordinate system was adopted. On the other hands, material nonlinearity is based on elasto-plastic models with Von-Mises yield criteria. Thus, the combined explicit-implicit time integration algorithm is benefit in general case of shell structure, which is the result of this paper.

  • In the present study, the injection molding optimization of a large thick LH type elastic frames for the reduction of warpage was performed. Two kinds of fine and coarse finite element models were prepared to investigate the efficiency of analysis time and quality on simulation results. In order to derive injection molding conditions that can minimize distortion of parts, it was investigated that the effects of mold temperature, resin temperature, injection time, hold pressure switching time, holding pressure and the hold time on deformation characteristics using the design of experiments. The main influential factors on the warpage were found from the optimization simulation and the geometry data of the warpage result was converted into an initial model for injection simulation. It was shown that a coarse model with good mesh quality could be adapted for mold design since the total analysis time using the proposed model was reduced to 1/10. The suggested inversed warpage model produced the best minimized result of warpage.

Inactivation of Microorganisms and Browning Enzymes in Angelica keiskei Juice Using High Hydrostatic Pressure (초고압을 이용한 신선초 녹즙의 살균 및 갈색화 효소의 불활성화)

  • Effects of high hydrostatic pressure on microorganisms and browning enzymes in Angelica keiskei juice were investigated using response surface methodology. The optimum process condition for maximum reduction of total aerobes was $5700\;kg_f/cm^2$ (558.6 MPa) pressure and 7.16 min process time, and 3.44 log cycle reduction of total aerobes was predicted at the optimum condition. E. coli, initially $8.8{\times}10^3\;CFU/ml$, was completely inactivated by high hydrostatic pressure at all process conditions ($3800{\sim}6700\;kg_f/cm^2\;pressure;\;3{\sim}17\;min\;process\;time$). Polyphenol oxidase and peroxidase were partly inactivated by the high hydrostatic pressure. It was also indicated that inactivation of microorganisms and browning enzymes by hydrostatic pressure is dependent on pressure rather than process time.

Efficacy and Safety of Deflazacort in Korean Children with Nephrotic Syndrome (신증후군 환아에 대한 Deflazacort의 효과와 안전성)

  • Deflazacort, an oxazoline derivative of prednisolone, has been claimed to have anti-inflammatory effects with fewer side effects compared to prednisone. The objectives of the study were to evaluate efficacy and safety of deflazacort in children with nephrotic syndrome. Eligible Patients were the children with nephrotic syndrome who were treated with deflazacort from October. 1994 to April. 1999. Nephrotic syndrome was defined as having albumin level of less than 2.5 mg/dL and 24-hour urinary protein excretion of greater than $40\;mg/m^2/hr$. The primary parameters evaluating the efficacy of deflazacort were response rate, time to respond and relapse frequency. The safety profiles were the impact on children's growth, calcium sparing effect, glucose metabolism, lipid profile and adverse drug reactions. As results, total of 60 children were evaluated (47 boys, 13 girls). Response rate was $95\%$ (57/60) for initial and late responders. Median time to respond was 12 days (range 7-110 days) and median relapse frequency was one time (range 0-6). Weight/height ratio increased from $22.05\pm3.47\;to\23.20\pm3.44\;kg/m$ (p<0.001) and plasma calcium level, from $7.55\pm3.86\;to\;9.98\pm3.77\;mg/dL$ after treatment (p<0.001). Change of fasting glucose level was not statistically significant $(91.92\pm3.53\;vs.\;98.19\pm4.78\;mg/dL,\;p=0.072)$, while change of total cholesterol was significant $(362.3\pm12.0\;vs\;251.4\pm11.5\;mg/dL$, p<0.001). In conclusion, patients on deflazacort showed similar efficacy in treatment of nephrotic syndrome as reported for prednisone with less impact on growth inhibition and metabolic side effects of hyperglycemia and hyperlipidemia.
