Autogenous Transplantation of Third Molar to Replace Hopeless Tooth (예후 불량한 치아를 대체하는 제3대구치를 이용한 자가치아이식술)

  • Autotransplantation is the surgical repositioning of an autogenous erupted or unerupted tooth from one site to another in the same individual. This treatment is indicated in traumatic tooth loss, teeth with severe caries, congenitally missing teeth, teeth with bad prognosis and in case of developmental anomalies of teeth. The following 2 cases describe the potential to utilize autotransplantation to replace hopeless teeth with sound wisdom teeth.

  • Cultures of primary human alveolar bone-derived cells were established from alveolar bone chips obtained from normal individuals undergoing tooth extraction. These cells were expanded in vitro until passage 3 and used for the in vivo assays. Cells were loaded into transplantation vehicles, and transplanted subcutaneously into immunodeficient mice to study the capacities of human alveolar bone-derived cells to form bone in vivo. Transplants were harvested 12 weeks after transplantation and evaluated histologically. Of 10 human alveolar bone-derived cell transplants, two formed a bone-like tissue that featured osteocytes and mineral. Eight of the ten formed no osseous tissue. These results show that cells from normal human alveolar bone are capable of forming bone-like tissue when transplanted into immunodeficient mice.

자가 치아 이식술에 사용되는 Computer Aided Rapid Prototyping model(CARP model)의 실제 치아에 대한 오차

  • Objective : The purpose of this study was to evaluate the dimensional errors between real tooth, 3D CT image and CARP model. Materials and Methods : Two maxilla and two mandible block bones with intact teeth were taken from two cadavers. Computed tomography was taken either in dry state and in wet state. After then, all teeth were extracted and the dimensions of the real teeth were measured using a digital caliper at mesio-distal and bucco-lingual width both in crown and cervical portion. 3D CT image was generated using the V-works $4.0^{TM}$ (Cybemed Inc., Seoul, Korea) software. Twelve teeth were randomly selected for CARP model fabrication. All the measurements of 3D Ct images and CARP models were made in the same manner of the real tooth group. Dimensional errors between real tooth, 3D CT image model and CARP model was calculated. Results : 1) Average of absolute error was 0.199 mm between real teeth and 3D CT image model, 0.169 mm between 3D CT image model and CARP model and 0.291 mm between real teeth and CARP model, respectively. 2) Average size of 3D CT image was smaller than real teeth by 0.149 mm and that of CARP model was smalier than 3D CT image model by 0.067mm. Conclusion : Within the scope of this study, CARP model with the 0.291 mm average of absolute eror can aid to enhance the success rate cf autogenous tooth transplantation due to the increased accuracy of recipient bone and donor tooth.

  • Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

Long-term evaluation of autotransplanted third molars (제 3대구치를 이용한 자가치아이식술의 장기적 예후 관찰)

  • Purpose: The purpose of this study is to evaluate the long term clinical and radiographic outcome and stability after transplantation of third molar with complete root formation. Methods: The subjects were 31 teeth (male 17, female 14, aged 22-55, average 39.9 yr old) of 31 patients who visited the department of periodontics and passed more than two years after autotransplantation procedure and still under regular check up. Modified success criteria of Chamberlin and Goerig was applied to determine the success of autotransplantation. Results: Three out of 31 teeth failed and resulted 90.3% of success rate. When compared according to sex, 15 out of 17 teeth had succeeded in male, 13 out of 14 succeeded in female. When compared the success rate according to cause of extraction, tooth loss due to caries and root fracture had all succeeded but 3 out of 24 had failed in tooth loss due to periodontal disease. When compared according to donor teeth, 12 out of 14 maxillary third molars and 16 out of 17 mandibular third molars had succeeded. Conclusions: In long term evaluation over two years, if appropriate surgical procedure and proper case selection is made, autotransplantation of the third molar with complete root formation can be the alternative choice that substitutes prosthetic or implant treatment and it is a functionally acceptable procedure.

DEVELOPMENT OF ALLOTRANSPLANTED TOOTH GERMS AT VARIOUS DEVELOPMENTAL STAGE INTO THE WHITE RAT'S EXTRACTION SOCKET (흰쥐의 발치와에 이식한 단계별 치아싹의 발육 과정)

  • The purpose of this study is to evaluate at which stage of tooth germ would develop into normal calcification and hence to increase the success rate of transplantation. Therefore, tooth germs on the 15th, 17th embryonic day and the 3rd day of birth were separated for allotransplantation into maxilla of adult rat of 11 weeks. Calcification processes were analyzed radiographically and histopathologically at 4 weeks and 8 weeks after allotransplantation. The results are as follows: 1. Allotransplanted tooth germ at 4 weeks and 8 weeks showed delayed calcification compared to that of normal odontogenesis. 2. At 4 weeks, abnormal calcified tissue, such as odontoma and ankylosis of osteodentin with surrounding alveolar bone were observed. 3. At 8 weeks, allotransplanted tooth germs of the 15th and 17th embryonic day showed calcification and osteodentin surrounded by periodontal ligament. 4. At 8 weeks, allotransplanted tooth germs of the 3rd day of birth showed calcification composed of cementum and osteodentin. In this study, we observed small sized and amorphous calcified tissue from allotropic allotransplantation of tooth germs. Since these calcified tissue were underdeveloped and shaped irregularly, for calcification into normal tooth form, further study needs consideration about the reduction of surgical trauma, developmental stage of transplanted tooth germ, blood supply from recipient site, fixation method in transplanted site and period of transplantation.

INTRA-ALVEOLAR TRANSPLANTATION OF COMPLETELY CROWN-ROOT FRACTURED TOOTH WITH DEMINERALIZED FREEZED DRIED BONE GRAFT (치은연 하방으로 파절된 치아의 탈회냉동건조골을 이용한 Intra-alveolar transplantation)

  • Incidence of crown-root fracture due to traumatic injury, have been reported 3% in the permanent dentiton, 2% in the deciduous dentition. There are two treatment methods for crown-root fractured teeth with pulp exposure, when the fracture line was located under the alveolar crest. One way is the extrusion by orthodontic force the other way is intra-alveolar transplantation which occlusally repositioning of apical fragment in the alveolar socket. Since intra-alveolar transplantation has introduced in 1970s, it was practiced as alternative to orthodontic extrusion. As the result, this method may thoughted that had a good prognosis. As a result of trauma, completely crown-root fracture was occured in the maxillary right central incisor in this case. We couldn't reposition the deepest fracture line above the alveolar crest by the conventional surgical extrusion, because apical fragment was too short. Thus, after extraction of apical fragment, we repositioned it to the socket following demineralized freezed dried bone graft, which possible to support the apical fragment. At the 15-month recall examination, the root still showed normal mobility and there was not observed any in flammatory or replacement root resorption in the periapical radiograph.

  • Ectopic eruption should be understood as a change in the course of the normal eruption path of a dental bud at any moment in its origin. Transposition is a unique and extreme form of ectopic eruption. The treatment for ectopic eruption and transposition is various from simple observation to surgical exposure and orthodontic traction, according to direction of erupting tooth degree of developing root apex and eruption space etc. Autotransplantation is transplantation of tooth from one area of the mouth to another in the same individual or is moving a eruption tooth into extraction socket or surgically prepared socket, and autotransplantation is considered as a treatment of choice for the ectopic eruption when orthodontic traction is unable or when tooth movement is limited. These cases which were treated with autotransplantation of maxillary lateral incisor and maxillary canine were reported, and good esthetic and functional result were induced.

THE SPONTANEOUS ERUPTION OF DISPLACED PERMANENT TOOTH BY PERIAPICAL LESION OF PRIMARY TEETH (유치의 치근단 병소에 의해 변위된 영구치의 자발적 맹출)

  • The infection of primary teeth occurs by caries or trauma. But, if it is not treated on time, the complication may occur such as enamel hypoplasia, discoloration, impaction, displacement and dilaceration of permanent successors. The periapical lesion on primary tooth could displace the permanent successors in any direction. Treatment options of displaced tooth are observation after extracting the primary teeth, surgical exposure orthodontic traction, transplantation and extraction. In these cases, displacement of permanent central incisor caused by the periapical lesion of primary teeth was observed. The spontaneous eruption after extraction of primary teeth was expected considering degree of displacement, development of root and direction of eruption. This case report shows spontaneous eruption of displaced permanent tooth germ was achieved with periodic examination after extraction of primary teeth.

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.
