Influence of size and insertion depth of irrigation needle on debris extrusion and sealer penetration

  • Objectives: To determine the effect of size and insertion depth of irrigation needle on the amount of apical extruded debris and the amount of penetration depth of sealer using a confocal laser scanning microscope (CLSM). Materials and Methods: Twenty maxillary premolars were assigned to 2 groups (n = 10), according to the size of needle tip, 28 G or 30 G. Buccal roots of samples were irrigated with respective needle type inserted 1 mm short of the working length (WL), while palatal roots were irrigated with respective needle type inserted 3 mm short of the WL. Prepared teeth were removed from the pre-weighed Eppendorf tubes. Canals were filled with F3 gutta-percha cone and rhodamine B dye-labeled AH 26 sealer. Teeth were transversally sectioned at 1 and 3 mm levels from the apex and observed under a CLSM. Eppendorf tubes were incubated to evaporate the irrigant and were weighed again. The difference between pre- and post-weights was calculated, and statistical evaluation was performed. Results: Inserting needles closer to the apex and using needles with wider diameters were associated with significantly more debris extrusion (p < 0.05). The position of needles and level of sections had statistically significant effects on sealer penetration depth (p < 0.05 for both). Conclusions: Following preparation, inserting narrower needles compatible with the final apical diameter of the prepared root canal at 3 mm short of WL during final irrigation might prevent debris extrusion and improve sealer penetration in the apical third.

하악 임플랜트 유지형 피개의치의 안정성과 하악골 응력분포에 대한 3차원 유한요소법적 연구 (3D FINITE ELEMENT ANALYSIS OF OVERDENTURE STABILITY AND STRESS DISTRIBUTION ON MANDIBULAR IMPLANT-RETAINED OVERDENTURE)

  • Statement of problem: Recently there are on an increasing trend of using implants-especially in edentulous mandible of severly alveolar bone recessed. Purpose: The aim of this study was to analyze the displacement and stress distribution of various mandibular implant-retained overdenture models supported by two implants in interforaminal region under the occlusion scheme load. Material and method: FEA models were made by the 3D scanning of the edentulous mandibular dentiform. The three models were named as Model M1, M2, and M3 accord ing to the position of implants: M1, Lt. incisor area, M2, Canine area, and M3, 1st Premolar area. Inter-implant angulation model was named as M4. Conventional complete denture was named M5 and used as a control group. Ball implant and Gold matrice were used as a retentive anchors. The occlusion type loads were applied horizontally over each tooth. Results: 1. In mandibular implant retained overdenture Canine Protected Occlusion type load resulted in higher levels of stress to the implants and female matrices than other types of loads. 2. The overdenture model M1, with implants in lateral incisor areas resulted in lower stress concentration to the implants and female matrices than other models. 3. In mandibular implant retained overdenture the stresses of the implant and female matrice were lower in mesially inclined implant than these of parallel installed implant. Conclusion: Lateral incisor areas could be the best site for the implants in mandibular implant-retained overdenture. The mandibular implant retained overdenture models mentioned above showed to the lowest stress to the implants and female matrices.

CT사진을 이용한 한국인의 C형 치근관의 분포에 대한 연구 (The distribution of C-shaped canal system in Korean population with CT image)

  • Purpose: To investigate the incidence and prevalence of C-shaped root canal using computed tomographic images of head and neck in Korean population. Materials and Methods : Regardless of each examination purpose, randomly selected 268 examples which have serial axial computed tomographic images with 8 normal molars in maxilla and mandible were selected and investigated. Defined C-shaped root canal and we could get 82 proper image examples in view of this definition. These were detected and investigated of incidence and prevalence of C-shaped root canal. Results: C-shaped root canals were found in 82 examples among 268 (30.6%) and 147 teeth. They were only found on molar area, and the highest incidence was 37.41% of left mandibular second molars, and the lowest was 2.04% of right maxillary second molars. On prevalence of tooth position, mandibular second molar was the highest, 65% of C-shaped root canal teeth and maxillary second molar was 6% the lowest. Conclusion: 31% of randomly selected CT images no considering check-up reason have C-shaped root canals. The C-shaped root canals of mandibular second molar are found most frequently and they are also detectable on maxilla. On Korean population, C-shaped root canals are one of conditions that have to concern with dental treatment for it appears at a high prevalence relatively. 교합과 치은 퇴축과의 관계 (Relationship of occlusion and gingival recession)

  • Many factors have been implicated in the etiology of gingival recession, including faulty toothbrushing, the position of the tooth in the arch(malalignment), the presence of inflammation, frenal attachment, impingement of restoration margins, orthodontic treatment and trauma from occlusion. Among the many factors, this study was to evaluate the relationship of occlusion and gingival recession. 640 teeth without other etiologic factors of gingival recession were evaluated in 40 subjects aged 21-59 years. Only 1st, 2nd premolar and molar were included in this study. We recorded nonworking contacts, working contacts, cervical abrasion, sex, gingival recession and evaluated that relation of occlusion and gingival recession. The results of this study were as follows; 1. Teeth with nonworking contacts were significantly more gingival recession than teeth without nonworking contacts.(p<0.01) 2. Teeth with working contacts were significantly more gingival recession than teeth without working contacts.(p<0.01) 3. Teeth with cervical lesion were significantly more gingival recession than teeth without cervical lesion.(p<0.01) 4. Men's teeth were more gingival recession than women's teeth but it was not significant.(p>0.01)

Evaluation of biogeneric design techniques with CEREC CAD/CAM system

  • PURPOSE. The aim of this study was to evaluate occlusal contacts generated by 3 different biogeneric design modes (individual (BI), copy (BC), reference (BR)) of CEREC software and to assess the designs subjectively. MATERIALS AND METHODS. Ten pairs of maxillary and mandibular casts were obtained from full dentate individuals. Gypsum cast contacts were quantified with articulating paper and digital impressions were taken. Then, all ceramic crown preparation was performed on the left first molar teeth and digital impressions of prepared teeth were made. BI, BC, and BR crowns were designed. Occlusal images of designs including occlusal contacts were superimposed on the gypsum cast images and corresponding contacts were determined. Three designs were evaluated by the students. RESULTS. The results of the study revealed that there was significant difference among the number of contacts of gypsum cast and digital models (P<.05). The comparison of the percentage of virtual contacts of three crown designs which were identical to the contacts of original gypsum cast revealed that BI and BR designs showed significantly higher percentages of identical contacts compared with BC design (P<.05). Subjective assessment revealed that students generally found BI designs and BR designs natural regarding naturalness of fissure morphology and cusp shape and cusp tip position. For general occlusal morphology, student groups generally found BI design "too strong" or "perfect", BC design "too weak", and BR design "perfect". CONCLUSION. On a prepared tooth, three different biogeneric design modes of a CAD/CAM software reveals different crown designs regarding occlusal contacts and morphology.

The influence of various core designs on stress distribution in the veneered zirconia crown: a finite element analysis study

  • PURPOSE. The purpose of this study was to evaluate various core designs on stress distribution within zirconia crowns. MATERIALS AND METHODS. Three-dimensional finite element models, representing mandibular molars, comprising a prepared tooth, cement layer, zirconia core, and veneer porcelain were designed by computer software. The shoulder (1 mm in width) variations in core were incremental increases of 1 mm, 2 mm and 3 mm in proximal and lingual height, and buccal height respectively. To simulate masticatory force, loads of 280 N were applied from three directions (vertical, at a $45^{\circ}$ angle, and horizontal). To simulate maximum bite force, a load of 700 N was applied vertically to the crowns. Maximum principal stress (MPS) was determined for each model, loading condition, and position. RESULTS. In the maximum bite force simulation test, the MPSs on all crowns observed around the shoulder region and loading points. The compressive stresses were located in the shoulder region of the veneer-zirconia interface and at the occlusal region. In the test simulating masticatory force, the MPS was concentrated around the loading points, and the compressive stresses were located at the 3 mm height lingual shoulder region, when the load was applied horizontally. MPS increased in the shoulder region as the shoulder height increased. CONCLUSION. This study suggested that reinforced shoulder play an essential role in the success of the zirconia restoration, and veneer fracture due to occlusal loading can be prevented by proper core design, such as shoulder.

치아 정출술을 이용한 전치부 외상치의 치험례 (TREATMENT OF ANTERIOR TEETH FRACTURE BY FORCED ERUPTION)

  • 영구 전치부에 발생한 외상성 치은연하 파절과 자연 맹출을 기대하기 어려운 함입탈구가 발생한 경우에 여러 가지 치료방법이 알려져 있으나, 교정적 치아 정출술이 골수술을 동반한 치관길이 연장술이나 의도적 재식술에 비해 양호한 치료 성과를 나타내는 것으로 보고되고 있다. 본 증례는 외상으로 인해 치은연하로 치관파절을 보이는 환아들에서 근관치료와 함께 고정성 장치를 이용하여 치주수술을 동반한 교정적 치아 정출술을 시행한 후 복합레진 수복술로 치료하였다. 또한 치아함입이 있는 환아에서는 6개월 동안 자연 맹출을 기다린 후 맹출되지 않아 가철성 장치를 이용한 치아 정출술을 시행하여 치아를 원래의 위치로 회복시킬 수 있었는데, 이 환아의 경우 치료도중 실활의 증상을 보여 근첨성형술을 시행하였고 근첨이 형성된 후에는 근관충전을 통하여 비교적 양호한 임상적 결과를 얻을 수 있었다.

치면열구전색제 도포 후 광중합 할 때까지의 경과시간이 전색제 침투에 미치는 영향 (EFFECT OF TIME ELAPSED FROM APPLICATION TO CURING ON THE PENETRATION OF SEALANT)

  • 치면열구전색제 도포 후 시간에 따른 열구 내 침투율을 알아보고자 발치된 소구치 192개를 이용하여 전색제 도포 후 광중합 할 때까지 경과시간 3초, 5초, 10초, 20초별로 침투율을 비교 분석하였다. 여기에 추가적으로 상악과 하악, filled sealant와 unfilled sealant군으로 나누어 시간대별로 침투율에 차이가 있는지도 비교분석하였다. 그 결과 시간이 경과할수록, filled sealant군보다 unfilled sealant군에서, 상악보다는 하악에서 전색제의 침투율이 증가하였다. 이상의 결과에서 임상에서 치면열구전색을 시행할 때 이 결과를 임상적 지침으로 적용하면 유용하리라고 여겨진다.

  • 리니어 펄스 모터(LPM)는 유연한 선형운동이 필요한 분야에 사용되며 LPM의 위치정밀도는 리드스쿠류 보다 매우 높다. LPM은 간단한 기계적 구조, 고 반복성, 정밀한 오픈루프 동작 및 저 관성 등의 장점을 지니고 있으므로 공장 자동화 및 고속 위치제어용과 같은 응용분야에서 광범위하게 활용된다. 본 논문에서는 고 정밀성과 반복성을 지닌 LPM의 힘 특성을 해석하였다. 추력과 수직력은 고정자와 가동자의 치 피치와 공극에 매우 민감하다. 이 때 수직력은 추력에 비해서 매우 높지만 추력도 상당히 크다. 본 LPM의 자기회로는 복잡한 구조이기 때문에, 힘을 계산하기 위한 적합한 방법으로는 유한요소법(FEM)을 적용한다. 여기서는 가상변위와 맥스웰 응력을 이용하였다.

C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

  • Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.
