Surgery First Approach in Orthognathic Surgery : Indication and limitation (임상가를 위한 특집 4 - 턱교정수술에서 선수술접근 :적응증과 한계)

  • Surgery first approach in orthognathic surgery is to proceed the orthognathic surgery without preoperative orthodontic treatment. This approach has many advantages, which include a shorter total treatment period, a high level of patient satisfaction due to immediate post-surgical facial improvement, easy postoperative orthodontic treatment due to early normalization of skeletal muscle, and the rapid tooth movement reflecting the regional accelerated phenomenon. However instability due to transient occlusal interference after surgery make worse of long-term skeletal stability. Especially increasing of vertical occlusion caused by interference of interbicupid and molar happen postsurgical skeletal change. Until now, there is no common consensus about treatment protocol of surgery first approach in orthognathic surgery. The purpose of this paper is to introduce our treatment protocol of the surgery first approach and to evaluate indication and limitation with case analysis.

The Experimental Investigation of the Spherical Involute Bevel Gear (구형 인볼루트 베벨기어에 대한 실험적 연구)

  • For exact spherical involute bevel gear, serveral researchers had developed the mathematical models. The solid models of straight bevel gears are obtained and inspected to avoid interference by computer graphics. Furthermore, A gearbox is assembled by spherical involute bevel gears, which are manufactured by CNC machine. The transmission errors in the tooth mesh are measured by Laser sensor, are compared with the AGMA standard. This gearbox is found to be ranked AGMA Q10(JIS 3)

  • Elliptical gear is used as a rotor of oval flowmeter. The elliptical gear can assist water flow-rate measurement by the space of the elliptical shape. Presently, elliptical gears have been processed using hob tool imported from Japan. But if it were not for the technical cooperation with Japan company, the Elliptical gears can't be processed. The purpose of this study is developing a domestic technology for elliptical gears and overcoming the dependence of foreign technical skills. It will bring the import substitution effect. In case of pressure angle $20^{\circ}$, the elliptical gears have an interference of tooth. The increase of pressure angle makes the root of a tooth thick and the top of tooth thin. By results of this study, tooth of elliptical gear was designed and verified through the 2D and 3D simulation.

  • This case report demonstrates two different uprighting mechanics separately applied to mesially tipped mandibular first and second molars. The biomechanical considerations for application of these mechanisms are also discussed. For repositioning of the first molar, which was severely tipped and deeply impacted, a novel cantilever mechanics was used. The molar tube was bonded in the buccolingual direction to facilitate insertion of a cantilever from the buccal side. By twisting the distal end of the cantilever, sufficient uprighting moment was generated. The mesial end of the cantilever was hooked over the miniscrew placed between the canine and first premolar, which could prevent exertion of an intrusive force to the anterior portion of the dentition as a side effect. For repositioning of the second molar, an uprighting mechanics using a compression force with two step bends incorporated into a nickel-titanium archwire was employed. This generated an uprighting moment as well as a distal force acting on the tipped second molar to regain the lost space for the first molar and bring it into its normal position. This epoch-making uprighting mechanics could also minimize the extrusion of the molar, thereby preventing occlusal interference by increasing interocclusal clearance between the inferiorly placed two step bends and the antagonist tooth. Consequently, the two step bends could help prevent occlusal interference. After 2 years and 11 months of active treatment, a desirable Class I occlusion was successfully achieved without permanent tooth extraction.

A Study on Automatic Generation for 3-Dimensional Geometry of Cylindrical Gear and Hob (원통 치차와 호브의 3차원 형상 자동 생성에 관한 연구)

  • When designing a gear, designers determine basic dimensions of a gear with transmitted power considering strength, interference and so on. But, designers can not easily obtain the tooth profile generated by dimensions as well as the geometry of generating hob for cutting the tooth profile. In order to resolve these problems, an automatic design system creating not only the solid model of a gear but also that of the generating hob using the design parameters of dimensions is developed. Through the developed system, designers system, designers can improve the efficiency of design and satisfy the variable requirements of design as well. In this research, the three-dimensional solid models for spur and helical gears are generated considering the design parameters. Besides, those for generating hob regarding the design parameters of hob is created automatically. The system is developed by using Visual Basic and its three-dimensional geometric modeling module is constructed by using SolidWorks.

  • In this paper, by utilizing the iBeacon is BLE (BlueTooth Low Energy) protocol devices that are supported by over the Bluetooth 4.0, and have implemented a system to improve the accuracy in the system for measuring the position of the user. After measuring through the system according to the state of the iBeacon, the interference factors are analyzed through analysis of the collected data, and applying the extended Kalman filter for calibration. Compared with the data after applying a filter with existing data, it was confirmed an increase in accuracy. Improvement techniques for providing the less complexity to the actual implementation, is effective in improving the accuracy of vulnerable services basic iBeacon.

  • Supernumerary teeth are frequently found in the anterior portion of the maxilla and develop as a result of abnormal proliferation of the dental lamina during tooth germ formation, caused by genetic or environmental factors. They may result in various complications, such as eruption interference, displacement, rotation of adjacent teeth, diastema, eruption into the nasal cavity, and development of dentigerous cyst. The optimal time for surgical extraction of supernumerary teeth has been a controversial issue. Someone prefer early surgical extraction because supernumerary teeth can cause eruption interference and displacement of adjacent teeth, eventually altering occlusion. Others prefer to delay surgical extraction until $8{\sim}10$ years of age in consideration of root maturation of the adjacent teeth and also patient's behavior. When surgical extraction of supernumerary teeth is postponed, there is possibility that impacted supernumerary teeth in the inverted or horizontal position move toward the nasal cavity, hard palate, or premolar area. When such intraosseous tooth migration is combined with the vertical growth of the maxilla, surgical approach becomes even harder. Therefore, possibility of intraosseous tooth migration should be considered as an important factor when deciding appropriate time for surgical extraction. We are presenting cases of mesiodens which showed intraosseous migration during $6{\sim}7$ years of follow-up period since the first diagnosis had been made at the $2{\sim}3$ years of age.

A INFLUENCE OF ARTIFICIAL OCCLUSAL INTERFERENCE ON THE ACTIVE OF ANTERIOR TEMPERAL AND MASSETER MUSCLES (인위적 교합장애가 측두근 전염 및 교근의 활성도에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate the influence of the occlusal interference on the activity of anterior temporal and masseter muscles during maximal biting. In seven subjects, cast hard gold cap providing new occlusal surface approximately 0.5mm above its occlusal level was cemented on the mandibular right first molar during aquired experimental period. EMG recordings were taken bilaterally during one second hardest possible clenching four times. This procedure was done not only before, immediately after, 48 hours after and one week after cementation of interfering crown but also immediately after and one week after removal of occlusal interfering crown. The results were as follows : 1. Immediately after cementation of interfering crown, the activity was significantly decreased in all muscles studied. 2. One week after cementaion of interfering crown, the lowest muscle activity was recorded. But clinical sign and symptoms were significantly subsided. 3. Immediately after removal of interfering crown, the activity was increased in all muscles studied. 4. One week after removal of interfering crown, the activity of masseter muscles was increased to the level of before cementation of interfering crown. 5. There was no significant activity difference between right and left in the muscle studied. 6. Masseter muscle activity showed significant change than anterior temporalis with the unilateral mandibular posterior tooth interference.

  • EHD(elastohydrodynamic) lubrication theories are applied to analyse the contact characteristies between the circular are profiled teeth of the plano-wheel gear and the cylindrical pin type teeth of the inner gear for the planocentric gears unit, For improving lubrication characteristic and contact frictional interference between the teeth, a new design method of optimising continuous meshing position is introduced, and the new tooth profile which is modified as a rounded curvature of the edge of circular arc is also suggested. The results of mathematical computation from conventional and the modified gear unit are compared, respectively.

  • In this study, we designed tooth profile of the fly-cutter for antisymmetric rotor which is used in screw compressor. In order to verify this profile, we manufactured three different pairs(J46, N46, P46) of antisymmetric rotor using fly-cutter. We got the following conclusions from this study. (1) We obtained better contact condition using 3pairs of rotor which are manufactured by the fly-cutter. (2) We could prevent the cutter interference near bottom point of the robe of screw rotor.
