섬유강화형 포스트를 이용한 치관-치근 파절의 치료: 증례 보고 (TREATMENT OF CROWN-ROOT FRACTURE USING FIBER-REINFORCED POST: A CASE STUDY)

  • 치관-치근 파절은 법랑질, 상아질, 백악질이 모두 포함된 치아의 파절로, 파절선이 대부분 절단연이나 순측의 변연부 치은에서 구개측 치은열구 하방으로 사선으로 진행된다. 파절선의 위치가 치은 연상이라면 파절편의 제거 및 치은연상 수복을 시행하고, 치은 연하라면 파절면의 외과적 노출술, 교정적 정출술, 외과적 정출술을 이용한 치아의 수복이 이루어진다. 그 외에 섬유 강화형 포스트를 삽입하여 치관 수복물의 유지력을 높이고, 레진 접착 시스템을 사용하여 치관 파절편을 재부착함으로써 생물학적으로 최소한으로 침습적인 치료를 하는 방법도 있다. 만약 파절의 정도가 치은 연하로 깊은 경우 발치나 치관절제술을 이용한 치근의 유지 등을 고려할 수 있다. 본 증례는 치관-치근파절로 내원한 12세 환아로 섬유강화형 포스트를 사용하여 치관 파절편 재부착을 시행하였으며 양호한 경과를 보여 이를 보고하는 바이다.

Esthetic enhancement of a traumatized anterior tooth with a combination of forced eruption and tooth alignment: a case report

  • Exposing sound structure of a subgingivally fractured tooth using orthodontic extrusion is considered to be a conservative way to re-establish biologic width without sacrificing esthetics or jeopardizing periodontal support of neighboring teeth. When a misaligned tooth is traumatically involved, a more comprehensive approach combining tooth extrusion and re-alignment may be necessary for a successful restorative outcome. This case report describes a successful esthetic management of a patient with complicated crown-root fracture on the maxillary right central incisor and pre-existing malocclusion in the maxillary anterior region. Forced eruption along with re-alignment of teeth by orthodontic movement seems to allow re-positioning of the fracture line to a favorable position and correction of crowding, providing a better esthetic result.

치아 기관배양시 골형성단백의 역할에 관한 연구 (THE ROLE OF BONE MORPHOGENETIC PROTEIN IN THE TOOTH CULTURE)

  • Objectives : The proper development of the facial structures relies upon a sequence of tightly regulated signaling interactions between the ectoderm and mesoderm involving the participation of several families of signaling molecules. Among these, bone morphogenetic proteins (BMPs) have been suggested to be a key signal that regulates the development of the mandible and the initiation and morphogenesis of the teeth. The aim of this study was to examine the artificial development of the mandibular structures and to examine the role of BMPs on tooth morphogenesis and differentiation using an organ culture system. Materials and Methods : The tooth germs from Ed 11.5, 13.5 mice were dissected, and transplanted into the diastema of the mandible primordia. The mandibles containing the transplanted tooth germs were cultured in vitro. During this period, beads soaked with BMP4 were implanted around the transplanted tooth germs. In addition, a diastema block containing the transplanted tooth germ was dissected, then transferred to an adult mouse kidney. After the organ culture, the developing mandibular explant was removed from the kidney and prepared for the tissue specimens. Odontogeneis of the transplanted tooth germs was examined after Hematoxylin-eosin, Masson-trichrome staining. Results : Proliferation and differentiation of the tooth germs cultured in the diastema was observed. In the BMP4-treated tooth germs, the formation of the first and second molars was noted. The crown of the developing tooth showed the formation of a mature cusp with the deposition of enamel and dentin matrix. In conclusion, it was confirmed that BMP4 is involved in the formation of a dental crown and the differentiation of ameloblasts and odontoblasts of the molar tooth during the development of the transplanted tooth germs.

심한 부식 환자의 디지털 치관연장술 가이드를 이용한 전악 수복 증례 (Full mouth rehabilitation of a patient with severe tooth erosion with a digital crown lengthening guide)

  • 구강이라는 특수한 환경 속에서 치아는 평생을 거쳐 마모, 부식 등을 겪게 된다. 점진적이며 일정한 치아의 마모는 노화의 자연스러운 현상이나, 특정 요소에 의한 마모나 부식 등은 병적인 요소이며 교합면의 병리적 손상과 이에 따른 교합 부조화, 심미적 문제, 턱관절 장애 등을 야기할 수 있다. 본 증례는 26세의 젊은 여성 환자로 전반적인 치아의 마모와 부식이 동반된 상태였다. 디지털 안모 분석, 생리적 안정위, 전치부 치관 길이 평가 등을 통해 진단 납형을 제작하였다. 디지털 분석을 통해 치관연장술에 필요한 가이드를 제작하여 수술을 진행하였고 임시 보철물을 통하여 저작 기능 및 심미성을 회복하였다. 지르코니아 최종 보철로 수복하여 심미적, 기능적으로 만족할만한 결과를 얻었으므로 이 증례를 보고하고자 한다.

  • The Finite Element Analysis has been used for stress analysis of prosthesis, orthodontic or orthopedic appliances and filling materials. The primary purpose of the present studying was to evaluate the effectiveness of needle post in promoting the retention and integrity of composite crown restored on the pulpotomized primary central incisor. Three finite element models-natural tooth (Sample I), composite crown with (Sample II) and without (Sample III) needle post-were constructed and the stress distribution within each model were analyzed and compared one another. The results can be summarized as follows: 1. In sample I, the stress was shown to have distributed uniformly throughout the whole tooth even to the alveolar bone. 2. In sample II, the transmission of stress from the crown to the root area was shown to be very poor and irregular. 3. In sample III, the needle post was proved to be very effective in distributing the stress well to the aveolar bone which might help in maintaining the stability of crown restoration.

  • 파절의 종류는 치관파절 치근파절, 동시에 나타난 치관-치근파절이 있고 치수노출에 따라 단순파절과 복잡파절로 나뉜다. 치아파절이 치은 연하로 침범하여 치아장축을 따라 나타난 경우나 치근의 1/3 이상을 넘어선 경우 발치가 원칙이다. 그러나 치근쪽 파절편을 교정적으로 정출시켜서 수복시켜 주는 방법과 인위적으로 발치하여 적절하게 수복 가능한 위치로 재식 시켜주는 방법을 선택적으로 이용할 수 있다. 본 증례에선 치아외상으로 상악 중절치가 치관-치근 파절된 혼합치열기의 환아에서 의도적 재식술을 이용하여 발치 후 즉일 근관치료와 역충전 후 레진수복을 시행하여 치료하였기에 보고하는 바이다.

A simple technique for impression taking of teeth and functionally generated paths

  • The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.

CAD/CAM으로 제작한 유전치 지르코니아 전장관의 두께에 따른 파절강도와 반투명도 비교 (Fracture Strength and Translucency of CAD/CAM Zirconia Crown for Primary Anterior Tooth)

  • 이 연구의 목적은 파절강도와 반투명도 측정을 통해 Computer Aided Design/Computer Aided Manufacturing (CAD/CAM)으로 제작한 유전치 지르코니아 전장관의 임상적 적용 가능성을 확인하는 것이다. 기성 유전치 모형 중 #61을 스캔하여 얻은 3차원 형상 데이터로 CAD 디자인 프로그램을 이용하여 전장관을 디자인하고, 지르코니아 블록으로 CAM 밀링 기계를 이용하여 전장관을 제작하였다. 지르코니아 전장관은 두께에 따라 0.3, 0.5, 그리고 0.7 mm의 3군으로 나누어 1.0 mm 두께의 레진 전장관과 파절강도를 비교하였다. 파절강도는 만능시험기를 이용하여 30° 각도에서 1 mm/min 속도로 절단면에 하중을 가하여 측정하였다. 반투명도는 가로와 세로가 7.0 mm, 그리고 두께가 각각 0.3, 0.5, 0.7 mm 인 사각형 지르코니아 시편을 제작하여 분광측색장치로 측정하였다. 파절강도 측정 결과 지르코니아 전장관은 두께가 증가할수록 파절강도가 높아졌으며 모든 두께의 지르코니아 군에서 대조군인 레진 전장관보다 파절강도가 높았다(p < 0.05). 반투명도는 0.3 mm 군에서 가장 높았으며 0.5, 0.7 mm로 두께가 증가할수록 감소하였다(p < 0.05). CAD/CAM으로 제작한 지르코니아 전장관은 개별 치아에 맞추어 얇게 디자인 및 제작 가능하며, 얇은 전장관을 이용한 수복은 임상적으로 치아 삭제량과 치수노출 위험성 감소 뿐만 아니라, 더 심미적인 수복을 가능하게 해줄 것이다.

전치부 ALL-CERAMIC PARTIAL CROWN 의 임상 (- Aesthetic Restoration of the Gingival Recession with All-ceramic Partial Crown -)

  • The black triangle is a common clinical finding in aged people with gingival recessions. Among other prosthetic treatment molalities such as composite resin filling and laminate venners, the all ceramic restoration procedure can lead to most successful result. With improved bonding strength and ceramic properties, the tooth preparation design for all-ceramic crown can be modified to minimize the reduction of sound tooth structure without loosing properties of conventional preparation design. Case selection is an important factor in acheiving succesful prosthesis. In this case report, the leucite reinforced pressable ceramic, $Authentic^{TM}$ [Ceramay, Germany] was used to fabricate the prosthesis.

전치폭경이 전치부 교합형태에 미치는 영향 (THE EFFECT OF MESIODISTAL CROWN WIDTHS OF ANTERIOR TEETH ON THE INCISOR RELATIONSHIP)

  • This study was intended to investigate the effect of mesiodistal crown widths of the anterior teeth on the incisor relationship and to determine whether incisor width ratio and anterior width ratio could be used to assess interarch tooth width harmony. From the casts taken from 63 subjects, 26 of open bite, 18 of deep bite and 19 of normal over bite with age of 17-20, mesiodistal crown widths of maxillary and mandibular 6 anterior teeth were measured with Boley gauge. On the basis of tooth measurements, anterior and incisor width ratio were calculated. The cephalograms were taken from same subjects to measure the degree of over bite and over jet and to compare the craniofacial bony structures between open bite, deep bite and normal over bite. Correlations among the anterior width ratio, incisor width ratio, over bite and over jet were calculated. The results were as follows. 1. There were no differences in mesiodistal widths of anterior teeth, incisor width ratio and anterior width ratio between open bite, deep bite and normal over bite. 2. The incisor width ratio and anterior width ratio can be useful in the assessment of tooth width harmony because the incisor width ratio and anterior width ratio were stable statistically and significantly correlated with each other. 3. Over bite and over jet were not correlated with incisor width ratio and anterior width ratio.
