교정적 치아 이동 중 치은열구액의 Interleukin-$1\beta$의 발현 (Interleukin-$1\beta$ levels in human gingiva1 crevicular fluid during orthodontic tooth movement)

  • 교정적 치아이동 중 골개조는 수종의 매개물질에 의하여 조절된다. 특히 IL-$1\beta$는 실험동물의 교정적 치아이동 중 압박측과 긴장측 모두의 치근막과 치조골에서 골흡수를 촉진하고 골형성을 억제하는 효과를 보이는 것으로 알려져 왔다. 이 연구는 인체에서의 교정적 치아이동 중 압박측과 긴장측 치은열구액에서 IL-$1\beta$의 발현을 찾고 발현양의 시간적 변화를 관찰하였다. 전신질환인 없고 임상적으로 건강한 치주조직을 가진 평균 19.2$\pm$4.2세, 12명의 남녀환자에서 편측 견치를 원심으로 견인하였다. 실험군(원심 견인측)의 근심과 원심, 대조군(동일환자의 반대편 견치)의 근심 치은열구액을 견치 후방 이동 직전, 1시간 후, 24시간 후, 168시간 후의 4개의 시간대 별로 수집하여 총단백질 농도(BCA법)와 IL-$1\beta$(ELISA법) 의 농도를 비교, 관찰하여 다음과 같은 결과를 얻었다. 1. 실험군에서의 IL-$1\beta$ 농도는 교정력을 가한 후 증가하여 24시간 전후에 최대값에 도달하였다. 2. 대조군에서의 IL-$1\beta$ 농도는 시간에 따라 유의한 차이를 보이지 않았다. 3. 교정력을 가한 24시간 후에서 IL-$1\beta$의 농도는 압박측에서 가장 높고, 이어 긴장측과 대조군의 순이었다.

  • The prevalence of congenital teeth missing is about 2-10% of population and it is more common in permanent dentition than primary dentition. In the patients with congenital missing teeth symptoms such as conical tooth, prolonged retention of primary tooth, tooth attrition. and deficient alveolar bone growth can be accompanied. In this paper the results of clinical observation of the congenital teeth missing brothers and their familial history are presented.

매복 하악 견치의 자가이식술을 이용한 치험례 (Autotransplantation of impacted mandibular canine)

  • Autotransplantation is the transplantation of embeded, impacted, or unerupted tooth, into extraction socket or surgically prepared socket in the same individual. Clinically, successful autotransplantation must show radiolucent space(periodontal ligament space) between transplanted tooth and supporting bone, lamina dura, no root resorption, no ankylosis, no inflammatory change, and physiologic tooth mobility. It is important that procedure is atraumatic, and the instruments should not contact the root surface during procedure. We performed autotransplantation of impacted mandibular canine that transversely located beneath the apices of the mandibular incisors with uncompletely developed apex. In radiographs and clinical evaluation, this transplant showed successful clinical finding except irregularity of mesial root surface after 14 months. It is conclued that transplantation of canine with $\frac{1}{2}{\sim}\frac{3}{4}$ root development provides a good chance of pulp survival, limited risk of root resorption and ensures sufficient final length, and is thus recommended.

조대술을 이용한 함치성 낭종의 치료증례 (TREATMENT OF DENTIGEROUS CYST USING MARSUPIALIZATION : A CASE REPORT)

  • A dentigerous cyst is an epithelium-lined sac that surrounds the crown of an impacted, embedded, or unerupted tooth. Many surgical procedures have been described for the elimination of dentigerous cyst, but they can be devided into two basic group: enucleation and marsupialization. Marsupialization is a conservative technique which allows the reduction or elimination of a cystic lesion by making it an accessory compartment to the oral cavity and it is the best way to conserve a tooth affected by dentigerous cyst and to permit its eruption, especially in young patient. After using marsupialization to treat dentigerous cyst associated with the crown of unerupted premolar in young patient, the results were as follows: 1. Reduction of bony expansion and rapid bone regeneration without infection and recurrence were observed. 2. Normally spontaneous eruption of involved teeth were permitted, as well as loss of affected tooth was avoided. 3. Follow-up examinations revealed no complication and recurrence.

  • the preliminary work on the prepartion of fine hydroxyapatite powder and the synthesis of dense hydroxyapatite ceramic from the powder were investigated for the development of artificial bone and tooth materials for implants. The effects of the process variables such as compositions of the raw materials the initial pH of the solutions and sintering temperature on the physical properties were investigated in order to determine the optimum conditions for the fabrication of tooth implant material. As the initial pH of the solutions was increased in the range of 10-11.6 the initial particle size of precipitates was decreased thus the sinterability of precipitates was improved. It was found that the composition of calcium rich of 1.75 in Ca/P mole ratio exhibited better sinterability and mechanical strength of the apatite ceramics for example the highest value of Vicker's hardness obtained from the compositions of 1.75 in Ca/P mole ratio was 764±30 Kg/mm2 compared to the maximum value of 600 kg/mm2 obtained from the compositions of 1.67 in Ca/P mole ratio.

  • During orthodontic treatment of impacted teeth, use of appropriate anchorage against the traction force is important. Tooth anchorage with multi-bracket appliances is commonly used but sometimes it causes unwanted movements of adjacent teeth. Skeletal anchorage devices are therefore considered to minimize such side effects. Still their survival rate and positioning are highly limited according to the bone density and the interradicular space. This case report presents a case of two impacted teeth, one of which is dilacerated and horizontally angulated. Using the microplate with short screws and a bendable neck, negative effects on adjacent teeth were minimized and impacted teeth were repositioned with good stability.

The influence of bone graft procedures on primary stability and bone change of implants placed in fresh extraction sockets

  • Background: This study was to evaluate the effect of bone graft procedure on the primary stability of implants installed in fresh sockets and assess the vertical alteration of peri-implant bone radiographically. Methods: Twenty-three implants were inserted in 18 patients immediately after tooth extraction. The horizontal gap between the implant and bony walls of the extraction socket was grafted with xenografts. The implant stability before and after graft procedure was measured by Osstell Mentor as implant stability quotient before bone graft (ISQ bbg) and implant stability quotient after bone graft (ISQ abg). Peri-apical radiographs were taken to measure peri-implant bone change immediately after implant surgery and 12 months after implant placement. Data were analyzed by independent t test; the relationships between stability parameters (insertion torque value (ITV), ISQ abg, and ISQ bbg) and peri-implant bone changes were analyzed according to Pearson correlation coefficients. Results: The increase of ISQ in low primary stability group (LPSG) was 6.87 ± 3.62, which was significantly higher than the increase in high primary stability group (HPSG). A significant correlation between ITV and ISQ bbg (R = 0.606, P = 0.002) was found; however, age and peri-implant bone change were not found significantly related to implant stability parameters. It was presented that there were no significant peri-implant bone changes at 1 year after bone graft surgery. Conclusions: Bone graft procedure is beneficial for increasing the primary stability of immediately placed implants, especially when the ISQ of implants is below 65 and that bone grafts have some effects on peri-implant bone maintenance.

Randomized clinical trial on the efficacy of Escherichia coli-derived rhBMP-2 with ${\beta}$-TCP/HA in extraction socket

  • PURPOSE. This randomized clinical trial was conducted to assess the safety and effectiveness of the ErhBMP-2 in alveolar bone regeneration as well as preservation of the ${\beta}$-TCP bone graft material that contains ErhBMP-2. MATERIALS AND METHODS. This study involved 72 patients at the 3 study centers. The patients, who were divided into 2 groups: the experiment group who had ErhBMP-2 coated TCP/HA and the control group who had TCP/HA graft material alone transplanted immediately after tooth extraction. CT was taken before and 3 months after the transplantation and healing status was compared between the two groups. The efficacy endpoints that were used to measure the degree of bone induction included alveolar bone height and 3 measurements of bone width. The paired t test was used to determine the significance of the changes (P<.05). RESULTS. Changes in alveolar bone height were $-1.087{\pm}1.413$ mm in the control group and $-.059{\pm}0.960$ mm in the experimental group (P<.01). At 25% extraction socket length [ESL], the changes were $0.006{\pm}1.149$ mm in the control group and $1.279{\pm}1.387$ mm in the experimental group. At 50% ESL, the changes were $0.542{\pm}1.157$ mm and $1.239{\pm}1.249$ mm, respectively (P<.01 for 25% ESL, and P<.05 for 50% ESL). During the experiment, no adverse reactions to the graft material were observed. CONCLUSION. ErhBMP-2 coated ${\beta}$-TCP/HA were found to be more effective in preserving alveolar bone than conventional ${\beta}$-TCP/HA alloplastic bone graft materials.

Ginsenosides Have a Suppressive Effect on c-Fos Expression in Brain and Reduce Cardiovascular Responses Increased by Noxious Stimulation to the Rat Tooth

  • The purpose of this study is to investigate the antinociceptive effects of ginsenosides on toothache. c-Fos immunoreactive (IR) neurons were examined after noxious intrapulpal stimulation (NS) by intrapulpal injection of 2 M KCl into upper and lower incisor pulps exposed by bone cutter in Sprague Dawley rats. The number of Fos-IR neurons was increased in the trigeminal subnucleus caudalis (Vc) and the transitional region between Vc and subnucleus interpolaris (Vi) by NS to tooth. The intradental NS raised arterial blood pressure (BP) and heart rate (HR). The number of Fos-IR neurons was also enhanced in thalamic ventral posteromedial nucleus (VPMN) and centrolateral nucleus (CLN) by NS to tooth. The intradental NS increased the number of Fos-IR neurons in the nucleus tractus solitarius (NTS) and rostral ventrolateral medulla (RVLM), hypothalamic supraoptic nucleus (SON) and paraventricular nucleus (PVN), central cardiovascular regulation centers. Ginsenosides reduced the number of c-Fos-IR increased by NS to tooth in the trigeminal Vc and thalamic VPMN and CLN. Naloxone, an opioid antagonist, did not block the effect of ginsenoside on the number of Fos-IR neurons enhanced by NS to tooth in the trigeminal Vc and thalamic VPMN and CLN. Ginsenosides ameliorated arterial BP and HR raised by NS to tooth and reduced the number of Fos-IR neurons increased by NS to tooth in the NTS, RVLM, hypothalamic SON, and PVN. These results suggest that ginsenosides have an antinociceptive effect on toothache through non-opioid system and attenuates BP and HR increased by NS to tooth.

변형 Widman 판막술 후 치아 동요도 변화에 관한 연구 (Comparative Study on the Tooth Mobility after Modified Widman Flap)

  • Tooth mobility is an important part of a periodontal examination and represents a function of the persisting height of the alveolar bone and the width of the periodontal ligament. The purpose of this study was to evaluate the changes of the tooth mobility over 4 week-period following surgical therapy on the periodontal disease. Thirty five patients presenting with moderate periodontal pockets were selected and tooth mobility was measured at weekly intervals using Periotest (Siemens Co., Germany) beginning at the pre-operation examination and ending four weeks following the modified Widman Flap. All data were statistically analyzed using the one-way ANOVA test. The results were obtained as follows; 1. All teeth exhibited the greatest change in mobility at 1 week post-op, mobility generally decreasing with time. 2. Comparison of the weekly tooth mobility data regarding the 1st premolars showed significant differences only between weeks 1 (9.94) and 4 (6.14) (p<0.05). 3. Comparison of the weekly tooth mobility data regarding the 1st molar showed significant changes in the intervals between pre-op (6.49) and week 1 (11.22), pre-op and week 2 (9.37), weeks 1 and 3 (7.65), weeks 1 and 4 (5.62), and weeks 2 and 4 (p<0.05). 4. Comparison of the weekly tooth mobility data regarding the 2nd premolar and 2nd molar showed significant differences between pre-op (6.91, 8.60) and week 1 (11.02, 12.62), weeks 1 and 3 (8.00, 8.05), weeks 3 ad 4 (6.22, 6.71), and weeks 2 (9.34, 11.01) and 4 (p<0.05).
