The purpose of this study was to evaluate the stress distribution developed in the supporting structures by mandibular distal extension removable partial dentures with 2 different direct retainer designs and with or without indirect retainer and abutment splinting. The examined direct retainers on the second bicuspid abutment tooth were Akers clasp and RPA clasp, the indirect retainer was located on the mesial fossa of the first bicuspid, and the first and second bicuspid were splinted in case of tooth splinting. Total 8 cases were compared and analyzed with 3-dimensional finite element method. 150N were applied vertically on the artificial teeth of the removable partial denture, and then stress distribution patterns were analyzed and compared. The results were as follows : 1. The forces transmitted to the abutment tooth were primarily from the occlusal rests. 2. The abutment tooth was displaced distally when the force was applied. The compressive stress was observed at the distal root surface of the abutment tooth and the tensile stress, at the mesial root surface. 3. The denture base was displaced posteriorly and inferiorly when the force was applied. At the more distal portion of the denture base, the greater displacement was observed.And the anterior portion of the major connector was displaced superiorly. 4. The occlusal rest placed on the distal part of the abutment tooth tended to tip the tooth more posteriorly than did one on the mesial part of that tooth. 5. Severe superior displacement was observed at the anterior portion of the major connector in case of removable partial dentures without indirect retainer. 6. In case of tooth-splinting, the stress was distributed through all the root surface of both abuments. In case of no tooth-splinting, the stress was concentrated on the distal root surface of the primary abutment.
Purpose: Root resection can be a valuable procedure when the tooth in question has a high strategic value. The prognosis of root resection has been well documented in previous studies, but the results focused on the palatal root resection have not been discussed in depth. I represent here the short term effectiveness of palatal root resection of maxillary first molars. Methods: Palatal root resection was performed on maxillary first molars of three patients. All the palatal roots were floating state on the radiographic finding and showed full probing depth and purulent exudation at initial examination. Reduction of palatal cusp and occlusal table was performed concomitantly. Endodontic therapy was completed after root resection. Results: Compromised maxillary first molars were treated successfully by palatal root resection in 3 cases. The mobility of resected tooth was decreased a little bit. The probing pocket depth of remaining buccal roots was not increased compared to initial depth. All the patients satisfied with comfort and cost effective results and the fact they could save their natural teeth. Conclusions: Within the above results, palatal root resection is an effective procedure treating compromised maxillary first molar showing advanced palatal bone loss to root apex with or without pulp involvement when proper case selection is performed.
Double tooth is a dental anomaly consequent to fusion of two or more teeth or gemination of a single tooth. This report describes a unique case of double tooth in relation to a mandibular lateral incisor exhibiting the presence of four root canals. The role of conventional radiography and advanced three-dimensional imaging techniques in the better assessment of complex root canal systems and their aid in endodontic management has also been highlighted.
When the tooth avulsion occur in accidents the drying damage to the periodontal ligament has extremely detrimental effects on healing. Pulp necrosis always occurs after an avulsion injury, but revascularization can only take place in teeth with immature apexes. Therefore complications after avulsion injuries are common, and treatment must be carried out in a timely and correct fashion to prevent or limit these complications. Every effort should be made to replant the tooth within the first 15 to 20 minutes. If doubt exists that the tooth can be replanted adequately, the tooth should quickly be stored in an appropriate medium until the patient can get to the dental office for replantation. A complication of inflammatory root resorption is occurred by bacterial infection of periodontal ligament and dental pulp. Therefore aseptic endodontic treatment must be carried out in a timely and systemic antibiotics given at the time of replantation and before endodontic treatment are effective in preventing bacterial invasion. Further studies are needed to establish the clinical importance of preparation of the socket and root.
Purpose: This study investigated the prevalence of developmental and acquired pathologic conditions associated with impacted third molars (3Ms) in a Southern Brazilian population and evaluated whether demographic and tooth characteristics were correlated with the presence of bone or tooth lesions. Materials and Methods: Panoramic radiographs were assessed for developmental (bone-related) or acquired (tooth-related) pathoses associated with impacted upper or lower 3Ms. Data on tooth positioning, tooth development, and patient demographics were collected. A trained, calibrated postgraduate student evaluated all images. Binary and multivariate logistic regression models were used to assess associations between outcomes and the demographic and radiographic variables. The threshold for statistical significance was set at 5% (P<0.05). Results: The sample comprised panoramic radiographs from 2054 patients, predominantly female (59.2%), with a mean age of 27.2±11.5 years. Overall, 4066 impacted 3Ms were evaluated, revealing 471 (11.6%) developmental and 710 (17.5%) acquired pathoses. Among the developmental pathoses, 460 (95.2%) were indicative of dentigerous cysts. Male sex, lower 3M location, vertical or distoangular positioning, and incomplete root formation were associated with an elevated likelihood of developmental pathology. Lower tooth position, complete root formation, and partial eruption were linked to an increased probability of an acquired pathology in the third or second molar. Conclusion: The prevalence of pathologic conditions associated with impacted 3Ms was low. Male sex, lower 3M placement, horizontal or distoangular positioning, and incomplete root formation were associated with developmental pathoses, while lower tooth position, complete root formation, and partial eruption were related to acquired pathoses.
Objective: To investigate the dental phenotypes and treatment modalities (Tx-Mod) in Korean patients with Parry-Romberg syndrome (PRS) using longitudinal data. Methods: The samples consisted of 10 PRS patients, who were treated and/or followed-up at Seoul National University Dental Hospital between 1998 and 2019. Using a novel PRS severity index based on the numbers of the atrophy-involved area and asymmetry-involved item, we classified them into mild (n = 3), moderate (n = 2), and severe (n = 5). Dental phenotypes, including congenitally missing tooth (Con-Missing-Tooth), microdontia, tooth with short root (Short-Root), tooth with dilacerated root, and delayed eruption/impacted tooth, were investigated along with Tx-Mod. Results: The side of occurrence of all dental phenotypes showed 100% concordance with the side of PRS involvement. The most two common dental phenotypes were Con-Missing-Tooth and Short-Root (n = 29 and n = 17 in six patients). The sums of the average number of Con-Missing-Tooth and Short-Root increased from mild PRS to moderate PRS and severe PRS cases (1.0, 6.0, and 6.2). In terms of Tx-Mod, growth observation due to mild atrophy, fixed orthodontic treatment, and grafting were used for mild PRS cases. Tx-Mod for moderate PRS cases involved growth observation for surgery due to an early age at the initial visit. For severe PRS cases, diverse Tx-Mod combinations including unilateral functional appliance, fixed orthodontic treatment, growth observation, grafting, and orthognathic surgery were used. Conclusions: The novel PRS severity index may be useful to provide primary data for individualized diagnosis and treatment planning for PRS patients.
Among dental traumas, horizontal root fractures are relatively uncommon injuries. Proper initial management and periodical evaluation is essential for the successful treatment of a root-fractured tooth. If pulpal necrosis develops, endodontic treatment is indicated, exclusively for the coronal fragment. Fragment diastases exert a great influence on healing at the fracture line and on pulpal necrosis. An adequately treated root-fractured tooth has a good prognosis. This case report describes the treatment and 2-yr follow up of 3 maxillary central incisors, first with horizontal root fracture, second with horizontal root fracture and avulsion, and third with horizontal root fracture and lateral luxation. All three cases were treated with mineral trioxide aggregate (ProRoot, Dentsply). During 2 yr of follow-up evaluation, the root-fractured teeth of the present patients were well retained in the arch, showing periodontal healing, even after endodontic treatment.
Autotransplantation is the transplantation of embeded, impacted, or unerupted tooth, into extraction socket or surgically prepared socket in the same individual. Clinically, successful autotransplantation must show radiolucent space(periodontal ligament space) between transplanted tooth and supporting bone, lamina dura, no root resorption, no ankylosis, no inflammatory change, and physiologic tooth mobility. It is important that procedure is atraumatic, and the instruments should not contact the root surface during procedure. We performed autotransplantation of impacted mandibular canine that transversely located beneath the apices of the mandibular incisors with uncompletely developed apex. In radiographs and clinical evaluation, this transplant showed successful clinical finding except irregularity of mesial root surface after 14 months. It is conclued that transplantation of canine with $\frac{1}{2}{\sim}\frac{3}{4}$ root development provides a good chance of pulp survival, limited risk of root resorption and ensures sufficient final length, and is thus recommended.
Face gear transmission is widely used in aerospace shunt-confluence transmission system. Tooth wear is one of the main factors affecting its bearing transmission performance. Furthermore, the installation errors of face gear are inevitable. In order to study the wear mechanism of face gear tooth surface with installation errors, based on tooth contact analysis numerical method and Archard wear theory, the UMESHMOTION subroutine in ABAQUS is developed.Combining with Arbitrary Lagrangian-Eulerian adaptive mesh technology, the finite element mesh wear model of abraded face gear pair is established.The preprocessing conditions are set to generate the inp files.Then,the inp files for each corner are imported and batch processed in ABAQUS.The loading tooth contact problem at each rotation angle is solved and the load distribution coefficient among gear tooth, tooth root bending stress, tooth surface contact stress and loaded transmission error are obtained. Results show that the tooth root wear is the most serious and the wear at the pitch cone is close to 0.The wear law of tooth surface along tooth width direction is convex parabola and the wear law along tooth height direction is concave parabola.

Objective: This study investigated the impact of a single piezocision in the maxillary alveolar process on the speed of tooth movement. The null hypothesis was that the speed of tooth movement will be equal with and without piezocision. Methods: All maxillary molars on one side were moved against the combined incisors in 10 ten-week-old male Wistar rats. Under general anesthesia, a force of 25 cN was applied on either side using a Sentalloy closed coil spring. After placing the orthodontic appliance, vertical corticision was performed using a piezotome under local anesthesia, 2 mm mesial from the mesial root of the first molar on a randomly selected side; the other side served as the control. At the beginning of the treatment, and 2 and 4 weeks later, skull micro-computed tomography was performed. After image reconstruction, the distance between the mesial root of the first molar and the incisive canal, and the length of the mesial root of the first maxillary molar were measured. Moreover, the root resorption score was determined as described by Lu et al. Results: Significantly higher speed of tooth movement was observed on the corticision side; thus, the null hypothesis was rejected. The loss of root length and root resorption score were significantly more pronounced after piezocision than before. A strong correlation was observed between the speed of tooth movement and root resorption on the surgical side, but the control side only showed a weak correlation. Conclusions: Piezocision accelerates orthodontic tooth movement and causes increased root resorption.
