A STUDY OF THE CORRELATIONS BETWEEN THE TONGUE AND ORAL CAVITY VOLUME IN THE SKELETAL MANDIBULAR PROGNATHISM (골격성 하악전돌자의 혀와 구강용적의 상관관계에 대한 연구)

  • When mandibular prognathic patients are operated orthognathic surgery which reduce the oral cavity volume, tongue volume should be harmonized with oral cavity volume to be changed because prevent the relapse of malocclusion. To decide the need for tongue resection, the author measured the tongue volume and oral cavity volume with stone model which were taken by impression method and study the difference between the ratio of tongue and oral cavity volume in mandibular prognathic group and normal group. The samples were consisted of four groups, the 40 subjects of the control group, 40 subjects of the experimental group. Each group was subdivided into male and female group respectively. The results were as follows : 1. The tongue volume and oral cavity volume measurements are $20.7cm^3,\;32.7cm^3$ in the control group respectively, and $24.9cm^3,\;42.9cm^3$ in the experimental group respectively. 2. There is no difference in the ratio of the tongue volume to oral cavity volume in control group and experimental group. 3. Correlation coefficients between the tongue volume and oral cavity volume are 0.11, 0.29 in experimental group and control group respectively, and 0.43 in gross total group. 4. The tongue volume of male is larger than female(p<0.05).

  • The measuring tests on the rest position capacity and the tongue volume were conducted on the one hundred normal dental college students and staffs of Seoul National University in order to study the relationships between the est position Capacity and tongue volume. The results were as follows : 1) The correlation between the rest position capacity and tongue volume was hardly recognized. 2) The correlation between tongue volume and the weight was recognized. 3) Physiologic reflex was coused by the water injected in the mouth in the rest position. 4) Each correlation of the height and cheek thickness to the tongue volume was not recognized.

Head & neck 환자의 방사선치료 시 tongue displacer 사용의 유용성 평가

  • I. 목적 : 방사선 치료 시 최적화된 체내 선량분포를 얻는 것은 정상조직의 장애를 줄이고 종양선량을 높여 치료 효율을 극대화하는데 매우 중요하다. 본원에서는 병변 부위가 한쪽으로 치우친 head&neck 환자 치료 시 정상조직(tongue)을 보호하기 위해 tongue displacer를 만들어 사용한다. 이에 본 저자는 tongue displacer사용의 치료 유용성을 평가 하고자 한다. II. 대상 및 방법 : head & neck 치료 환자 중 병변 부위가 인체의 정중선(MSP)을 기준으로 한쪽으로 치우친 환자를 대상으로 하였다. 사용된 실험재료로는 C-T (high speed advantage, GE,US), RTP System (3D RTP system, prowess, US), 치과용 인상제 주입기(caulk system, quixx, japan), tongue displacer 등이 있다. 실험 방법은 모의 치료나 planning C-T를 시행하기 전에 치료 환자에게 사용할 개인용 tongue displacer를 치과용 인상제로 자체 제작하였다. 제작 후 모의 치료를 시행하고 3D plan을 하기 위해 planning C-T를 촬영하게 되는데 이때 tongue displacer사용 유. 무에 따라 각각 촬영을 하였다. 촬영된 두 가지의 CT영상을 prowess를 이용하여 3D plan을 하게 되는데 이때의 plan parameter나 beam direction등 plan에서의 모든 조건은 모두 동일시하고 선량 분포 및 DVH(dose volume histogram)값을 비교하였다. III. 결과 : tongue displace의 사용 유. 무에 따른 3D plan상의 DVH 비교 결과 tumor volume 주위의 다른 organ들은 모두 비슷한 양상의 DVH를 보였으나 tongue에 있어서 큰 변화를 보였다. tongue displacer를 사용 시, 미 사용시 보다 tongue의 위치를 변화시켜 치료 부위 외의 tongue에 받는 방사선 피폭 면적을 줄일 수 있었고 그 결과 DVH상의 $50\%$ volume이 $16\%$ 정도 줄어드는 것이 확인되었다. IV. 결론 : tongue에 방사선을 조사하면 방사선 부작용으로 mucositis, ulcer, hemorrhage등의 pain(동통)이 수반되므로 치료환자의 음식물 섭취불량으로 체증감소 등 전신 쇠약으로 이어질 수 있다. head & neck 환자 중에서 병소 위치가 한쪽으로 치우쳐서 있을 경우 인상제를 이용하여 tongue displacer를 만들어서 사용하면 tongue 의 위치를 변화시켜 방사선 조사 야에서 제외시켜준다. 그러므로 방사선 치료 시 tongue의 부작용을 최소화 할 수 있고 환자의 방사선 치료 만족도를 높일 수 있다고 사료된다.

Three-dimensional evaluation of the association between tongue position and upper airway morphology in adults: A cross-sectional study

  • Objective: This study aimed to evaluate the association between low tongue position (LTP) and the volume and dimensions of the nasopharyngeal, retropalatal, retroglossal, and hypopharyngeal segments of the upper airway. Methods: A total of 194 subjects, including 91 males and 103 females were divided into a resting tongue position (RTP) group and a LTP group according to their tongue position. Subjects in the LTP group were divided into four subgroups (Q1, Q2, Q3, and Q4) according to the intraoral space volume. The 3D slicer software was used to measure the volume and minimum and average cross-sectional areas of each group. Airway differences between the RTP and LTP groups were analyzed to explore the association between tongue position and the upper airway. Results: No significant differences were found in the airway dimensions between the RTP and LTP groups. For both retropalatal and retroglossal segments, the volume and average cross-sectional area were significantly greater in the patients with extremely low tongue position. Regression analysis showed that the retroglossal airway dimensions were positively correlated with the intraoral space volume and negatively correlated with A point-nasion-B point and palatal plane to mandibular plane. Males generally had larger retroglossal and hypopharyngeal airways than females. Conclusions: Tongue position did not significantly influence upper airway volume or dimensions, except in the extremely LTP subgroup.

Salivary secretion and salivary stress hormone level changes induced by tongue rotation exercise

  • PURPOSE. Prevention of xerostomia and stress is important to prolong healthy life expectancy and improve the quality of life. We aimed to investigate the effects of tongue rotation exercise for increasing salivary secretions and stabilizing salivary stress hormone levels. MATERIALS AND METHODS. Twenty four participants without subjective oral dryness were enrolled. The exercises comprised tongue rotation exercise and empty chewing. The salivary stress hormone level was measured using a Salivary Amylase Monitor. Unstimulated whole saliva volume and salivary amylase activity were measured before tongue rotation exercise or empty chewing and subsequently 5, 10, and 15 minutes after these exercises. Differences in the rates of change of unstimulated whole saliva volume and salivary amylase activity were analyzed by repeated measure analysis of variance. RESULTS. Statistically significant differences among the rates of change were not observed after empty chewing for unstimulated whole saliva volume and salivary amylase activity at the four measurement times. However, the rate of change of unstimulated whole saliva volume and salivary amylase activity were statistically significantly different among the four time points: before the tongue rotation exercise and 5, 10, and 15 minutes post-exercise (P<.05 and P<.01, respectively). CONCLUSION. Tongue rotation is effective in increasing saliva secretion, reducing stress, improving oral function, and extending healthy life expectancy.

CHANGES IN HYOID BONE AND TONGUE POSITIONS, AND ORAL CAVITY VOLUME AFTER MANDIBULAR SETBACK BY SAGITTAL SPLIT RAMUS OSTEOTOMY

  • Purpose: This retrospective study evaluated the changes in hyoid bone and tongue positions as well as oral cavity volume after mandibular setback by BSSRO. Materials and Methods: 18 Koreans who underwent BSSRO to correct mandibular prognathism were studied. Lateral cephalograms were taken and traced preoperatively (T0), immediately (T1) and approximately 6 months postoperatively (T2). Submentovertex radiograghs were taken and traced before surgery (T0) and about 6 months after surgery (T2). The area and volume of oral cavity, the vertical and horizontal dimensions of the hyoid bone and tongue dorsum were measured. Results: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume. The hyoid bone displaced posteroinferiorly immediately after surgery, and it tended to return to its original vertical position at 6 month after mandibular setback by BSSRO. The retropalatal space around tongue was maintained and the retrolingual space around tongue was reduced immediately postoperatively. The readaptation of tongue was not evident for that the follow up period was not long enough. No significant statistical correlations between the amounts of mandibular setback and the changes of oral cavity volume were observed. Conclusion: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume, which was most likely attributable to the posterior movement of the mandible. More subjects and long-term observations should be performed to assess the changes of oropharyngeal configuration following mandibular setback surgery.

Estimation of Volume and Surface Area for Reconstruction of Tongue (혀의 재건을 위한 부피 및 표면적의 측정)

  • Purpose : Anterolateral thigh and radial forearm flap is the most important fasciocutaneous flap widely used for reconstruction of tongue. One important purpose of flap is replacing the volume of tongue but still there is no data about the surface area and volume to be reconstructed after glossectomy. In this paper, surface area and volume is estimated from the 3-dimensionally reconstructed MRI images to see which flap is more ideal and to give the reference value for reconstruction. Materials and Methods : With coronal MRI image, tongue including only the intrinsic muscle is delineated in every section and reconstructed 3-dimensionally and calculated the volume and surface area to be reconstructed according to the degree of glossectomy. This volume and surface area was compared with the volume of anterolateral thigh and radial forearm flap. Results : The volume and surface area to be reconstructed in hemiglossectomy was $39.0{\pm}4.0cm^3$ and $31.8{\pm}2.7cm^2$ respectively. The average thickness of anterolateral thigh flap is $9.4{\pm}2.8mm$ and that of radial forearm is $3.8{\pm}1.0mm$. Comparing the curve of tongue surface area and volume with the volume of flap, the anterolateral thigh flap has more ideal volume to replace the defect. Conclusions : The surface area and volume requested for reconstruction could be suggested and the anterolateral thigh flap has more ideal volume for reconstruction of glossectomy defect.

Total Tongue Reconstruction with Reinnervated Rectus Abdominis Musculocutaneous Flap (재신경화된 복직근 근피판을 이용한 혀 전체 재건술)

  • After total glossectomy, recovery of swallowing and speech function can greatly improve quality of life. The reconstructed tongue must be thick enough to contact with the hard palate for articulation. If the free flap is denervation, it may procede to have atrophy postoperatively. Therefor it is difficult to maintain the tongue volume for a long period of time. To resolve this problem, we have used a innervated rectus abdominis musculocutaneous flap and maintaining the volume through a neurorrhaphy. 7 patients underwent immediate reconstruction using a reinnervated rectus abdominis musculocutaneous free flap in which included intercostal nerve was anastomosed to the remaining hypoglossal nerve. The reinnervated rectus abdominis musculocutaneous free flap has provided good tongue contour with sufficient bulk and shown no obvious atrophy in all patients even though postoperative 9 months later. Considering swallowing and articulation, we concluded that reinnervated rectus abdominis musculocutaneous flap is a viable method after total glossectomy

Effect of Luteinising Hormone Releasing Hormone Analogue and Human Chorionic Gonadotropin on Spermiation in the Tongue Sole, Cynoglossus semilaevis (Luteinising Hormone Releasing Hormone Analogue과 Human Chorionic Gonadotropin 처리가 박대(Cynoglossus semilaevis)의 배정에 미치는 영향)

  • Experiments were carried out examine short-term effects of human chorionic gonadotropin (hCG) and long-term effects of luteinising hormone releasing hormone analogue (LHRHa) on milt production of the tongue sole Cynoglossus semilaevis. In the first experiment, each fish was implanted with a blank cholesterol pellet (control), 100 and $200{\mu}g$ LHRHa per kg body weight. In the second experiment, fish were injected with either 100, 200, 400 and 800 IU hCG per kg body weight or same volume of marine fish Ringer's solution. In the first experiment, milt volume was increased in male implanted with $200{\mu}g$ LHRHa pellet compared with other groups at day 10. Injection of 400 and 800 IU hCG resulted in an increase in the milt volume at hour 96 after the treatment. Although statistical difference is unable to confirm because of small milt volume, compared with the control group, hormone pellet-treated groups had a reduction in the mean spermatocrit (Sct) and sperm concentration (Sc). The results suggest that the increase in milt volume is at least partially gonadotropin (GtH)-dependent and increased milt volume has a relationship with milt hydration.

Home Based Dysphagia Rehabilitation for Stroke Patients Using Information and Communication Technology

  • Background: Recently, a new home-based dysphagia rehabilitation method using information and communications technology (ICT) has been reported, but clinical evidence is still lacking. Objectives: To investigate the effects of home-based dysphagia rehabilitation using ICT on tongue muscle strength and volume in patients with developed dysphagia after stroke. Design: Randomized controlled trial design. Methods: Twenty patients who developed dysphagia after stroke were enrolled. The experimental group received dysphagia rehabilitation in the form of ICT-based home care. In contrast, the control group received traditional rehabilitation based on swallowing under the supervision of occupational therapists. All interventions were conducted five times a week for four weeks. Results: Both groups showed statistically significant increases in tongue muscle strength and volume after the intervention (P<.05, both). However, there were no significant differences in tongue strength or volume between the two groups after the intervention (P>.05, both). Conclusion: Home-based dysphagia rehabilitation using ICT showed effects similar to those of conventional swallowing rehabilitation in patients who developed dysphagia after stroke. These findings suggest that dysphagia rehabilitation can be conducted at home without the help of a therapist.
