Clinical Characteristics of Patients Showing Fissured Tongue with Burning Mouth Syndrome: Case Series (균열설을 보인 구강작열감증후군 환자의 임상적 특징 : 환자군 연구)

  • Objective: The aim of this study was to investigate the clinical characteristics of patients showing fissured tongue (FT) with burning mouth syndrome (BMS). Methods: We reviewed the clinical records of 16 FT patients with BMS who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from March 1, 2017 to February 28, 2018. The subjects were graded by FT classification and compared with each group on a numeral rating scale (NRS) of tongue pain, proportion of coated tongue, unstimulated salivary flow rate (USFR), Ryodoraku, and heart rate variability (HRV) results. Results: FT grade had no significant correlation with NRS of tongue pain and USFR. FT grade had a substantially significant positive correlation with BMI and LF/HF ratio in HRV. FT grade had a substantially significant negative correlation with proportion of coated tongue. The most frequent accompanying symptoms were xerostomia and insomnia. Conclusions: The results of this study suggest that FT has no correlation with tongue pain in BMS. Rather, FT seems to correlate with sympathetic function acceleration.

A Case Report on the Use of Korean Medicine Treatment for a Patient with Dysarthria Caused by Progressive Bulbar Palsy (PBP)

  • Objectives : To introduce a rare case of a patient with Progressive Bulbar Palsy (PBP) and suggest the possibility of treatment using electroacupuncture and Korean Medicine. Methods : A 61-year-old man with PBP, complaining of dysarthria, was treated with electroacupuncture and Korean Medicine, from June 16 to July 10, 2015. Improvements in symptoms were measured using the speech mechanism screening test (SMST), measurement of tongue and orbicularis oris motility, and speech handicap index (SHI). Results : The scores of SMST, motility of tongue and orbicularis oris showed a tendency for gradual improvement with 25 days of Korean Medicine Treatment after admission, but conversation was still impossible. In SHI scores, one point increased in the speech domain and one point decreased in the psycho-social domain, and three points increased in the other domain. Conclusion : Electroacupuncture and Korean Medicine Treatment improved dysarthria caused by PBP, but not completely. Korean Medicine Treatment seems effective in the management of accompanying symptoms such as black hairy tongue, dry mouth, and general condition. The symptoms of PBP are similar to those of amyotrophic lateral sclerosis and there is controversy regarding the classification of PBP. The most important aspect of treating a patient with PBP is an early diagnosis and devising appropriate rehabilitation strategies.

A Literature Study on the Diagnostic Factors and Value as a Syndrome of Damjeok (담적의 진단요소 및 증후군으로서의 가치에 대한 문헌고찰)

  • Objectives: The purpose of this study is to investigate the diagnostic factors of Damjeok in the literature. Methods: This study used the databases of five (Mediclassics, RISS, KMbase, OASIS, CNKI) to analyze diagnostic factors of Damjeok. Literatures published by May 2023 were assessed and we classified 91 literatures dealing with diagnostic factors by 5 categories, 'characteristics', 'location', 'symptom', 'pulse wave & tongue feature', and 'eating habits & lifestyle'. Results: Damjeok is mainly formed at the stomach in the form of lump or hard seed, and it can be observed in other regions or even the whole body by its trait of floating. Damjeok mainly shows symptoms of the digestive system and also causes various symptoms associated with the respiratory system and infection, musculoskeletal system, urogenital system, neuropsychiatric system and cardiovascular system. Regarding pulse wave and tongue feature, a slippery wave and greasy coated tongue are typical features of Damjeok. Additionally, bad eating habits and a lack of exercise have been described as a factor that cause and aggravate Damjeok. Conclusions: The two most important factors in the diagnosis of Damjeok are abdominal stiffness by physical examination and digestive symptoms, and these correspond to the definition of a syndrome. Diagnostic factors based on literature evidence will provide clues to the clinical diagnosis of Damjeok syndrome.

The Electrophysiological Studies of the Trapezius Muscle in Patients with Amyotrophic Lateral Sclerosis (근위축성 측상경화증 환자에서 등세모근의 신경생리학적 검사)

  • Background: Needle electromyography (EMG) and motor evoked potential (MEP) of the genioglossus (tongue) are difficult to perform in evaluations of the craniobulbar region in amyotrophic lateral sclerosis (ALS). Therefore, we investigated the yields of needle EMG and MEP recorded from the upper trapezius, since it receives innervation from the lower medulla and upper cervical cord. Methods: Needle EMG and MEP of the upper trapezius were obtained in 17 consecutive ALS patients. The needle EMG parameters recorded included abnormal spontaneous activity and motor unit action potential (MUAP) morphology. An upper motor neuron (UMN) lesion was presumed when either response to cortical stimulation was absent, or the central conduction time was delayed (>mean+2SD). Results: Of the five patients with bulbar-onset ALS, four had abnormalities in the upper trapezius and four in the tongue by needle EMG. In contrast, of the 12 patients with limb-onset ALS, 11 had abnormalities in the upper trapezius, and only five in the tongue. When MEP was performed, it was found that three of the five patients with bulbar symptoms and three of the six patients with isolated limb involvement had abnormal MEP findings. Conclusions: Electrophysiological studies of the upper trapezius are more sensitive those of the tongue in patients without bulbar symptoms. Thus, needle EMG and MEP of the upper trapezius are alternative tools for assessing bulbar and rostral neuraxial involvement in the diagnosis of ALS.

  • General health exanimation comes into operation to focus on physical inspection for industrial workers and doesn't contain oriental health care. Thus we need information of health status and disease prevention, so develop a questionnaire software for health examination in oriental medicine. Items of this soft ware consists of personal information and symptoms to could check oneself , pulse and tongue diagnosis by oriental medical doctor. Symptoms are made up of syndrome differentiation about Qi and blood, Yin and Yang, body fluid, five Zang organs, Sasang Constitutional Medicine. And we reconstruct 116 items by whole body, chest and abdomen, urine and feces, head, limbs, waist and back, five sensory organs, objective signs. A subject enter symptoms and a oriental medical doctor input diagnosis of pulse and tongue, then this software return the result of health index and explanation for oriental health care. This software would be used as tool not only health examination but also clinical research.

Two Case Reports of Taeumin Delirium Patients (태음인(太陰人) 담망 환자 치험 2례)

  • 1. Objectives The purpose of this case report is to evaluate the effects of Sasang constitutional diagnosis and treatment of two delirium patients who were diagnosed Taeumin Dry-Heat Syndrome(燥熱證). 2. Methods Two delirium patients were diagnosed Taeumin Dry-Heat Syndrome(燥熱證) based on their Nature & Emotion (性情), physical characteristics, symptoms. They were medicated Chungpyesagan-tang(淸肺瀉肝湯) through of sleep, complexion, thirst, stool, coating of the tongue, and MMSE(Mini-Mental State Exanmination) score were used of measure improvement. 3. Results and Conclusions Two delirium patients who were treated with Chungpyesagan-tang(淸肺瀉肝湯) showed improvement in delirium symptoms, thirst, constipation, redness of complexion, white coating of tongue, and general condition. These case reports describe the effects on delirium and symptoms of Sasang Constitutional Medication. The necessity of managing the psychological aspects of the pathology through Nature & Emation(性情) is also mentioned.

Symptomatic Indications of Chongpesagan-tang according to Objective Analysis of Clinical Usage in Oriental Medicine Hospital - In Case of Stroke Patients - (태음인(太陰人) 청폐사간탕(淸肺瀉肝湯)의 응용예를 통한 증후분석(證候分析) - 중풍환자를 중심으로 -)

  • In Oriental medicine, the subjective process of diagnosis often makes it difficult to explain the indications objectively, in spite of its excellent therapeutic value. The authors' objective Was to present more objective indications of Chongpesagan-tang(淸肺瀉肝湯), a prescription predominantly used to treat stroke in Oriental Medicine, We analyzed clinical symptoms of 105 patients who were hospitalized in the Department of Circulatory Internal Medicine, College of Oriental Medicine. Kyung Hee University, Seoul, who were given Chongpesagan-tang and improved without side effects, The symptoms analyzed included general symptoms which are usually checked in the process of Oriental Medicine Diagnosis: the so called Four Diagnosis(四診). We rearranged and selected symptoms which were predominantly found among other symptoms. We found some symptoms of which the patients frequently complained: sweating 84%, red tongue 74%, white tongue coat 65%, constipation 35%, tight pulse 21%, headache 20%, thirst 20%, chest discomport 17%, etc, We believe that these suggest an objective indication of Chongpesagantang.

An Epidemiologic Study on the Frequency of Stress Symptoms in the Orofacial Region (스트레스에 의한 구강안면증상의 발혀에 관한 역학적 연구)

  • Stress is recognized as a major predisposing and/ or precipitating factor in long-lastig intractable chronic pain, such as temporomandibular disorders, headache, and other psychophysiological disorders. So it is necessary to detect physical and psychological changes induced by stress as soon as possible for positive treatment outcome. This study was performed to investigate the occurrence rate of stress symptoms according to anatomic region, type of symptom, and other personal and social factors. 859 subjects from general population answered the stress symptom questionnaire devised by the author and composed of 50 items. Data from the questionnaire were analyzed statistically with SPSS program and the results obtained were as follows : 1. Oral symptom which showed the highest frequency rate of 38.8% was vesicular lesion of the lip and cheek. The other symptoms with more 20% occurrence rate were ulcerative leion of lip and cheek, toothache, paresthesia of teeth, eruption of tongue, tongue coating and taste change in descending order. 2. In extraoral symptoms, ?데 disturbance was the item which showed the highest frequency rate of 62.0%, and the items for stiffness of suboccipital region and neck, headache, facial swelling, furuncle of face were answered more than 45% of the subjects whereas only 14.0% of the subjucts complained jaw pain under stress. 3. The better one who thought his or her health status was, the fewer items were answered and the difference of symptom frequency by dwelling place and by having hobby were shown in extraoral symptoms only. 4. For relief of stress symptoms, 79.2% of the subjects replied only to take a rest whereas not more than 13, 5% of the subjects visited dental clinic. Correlationship between symptom sites were very high.

A Literature Review on Syndrome Differentiation of Dysmenorrhea (월경통의 변증별 증상특징에 대한 문헌 연구)

  • Objectives: This study was conducted to comprehend the syndrome differentiations of dysmenorrhea and find out their clinical symptoms, tongue images and pulse patterns by analyzing previous studies. Methods: The following researches were collected by searching the medical journals published from November, 2007 to October, 2017, from KISS, OASIS, CNKI. : researches on the syndrome differentiation of dysmenorrhea, researches on the criteria of diagnosis of syndrome differentiation of dysmenorrhea, randomized controlled trials (RCT) used syndrome differentiation for treating dysmenorrhea. Results: By investigating the frequency of syndrome differentiations used in RCT studies, the frequent ones were chosen. They were qi stagnation and blood stasis (氣滯血瘀), qi-blood deficiency (氣血虛弱), congealing cold with blood stasis (寒凝血瘀), liver-kidney depletion (肝腎虧損), blood stasis with dampness-heat (濕熱瘀阻). Conclusion: 4 syndrome differentiations were frequently used in RCT studies. And the frequency of clinical symptoms on each syndrome differentiations from each RCT study was analyzed and compared. Clinical symptoms chosen as chief symptoms in more than one reference, appeared in more than half of the references, most frequent tongue images and pulse conditions were organized. The most frequent clinical symptoms included the period and pattern of pain, the accompanying symptoms of whole-body and the pattern of menstrual bleeding.

  • We have plenty of study of adults diseases, but not much has been said about children. 79 children who had visited in the weak children of Dongeui Oriental Medical Hospital from March, 2002 to October, 2002, were the respondents of the Weak Child Questionnaire. The results obtained were as follows: 1. Their symptoms have been observed with sequences of respiratory diseases, Digestive diseases, Psycho-neurological diseases. 2. Distribution of sex and ages : male 38 cases, female 41 cases. $0{\sim}6$ years 60 cases, $7{\sim}12$ years 17 cases, $13{\sim}18$ years 2 cases. 3. The texture of complexion was resulted in face color and the region of the face : whitish-2/ cases. yellowish-27 cases, darkish-22 cases, blue-2 cases, flushed face-l case. Sangeun(山根)-blue : 59 cases, a lower eyelid- dark or light brown : 54 cases. the rests of color on face region didn't mean. 4. The texture of tongue was ended in tongue state : tongue form-not plump not haggard patten(57 cases), a pricky patten(12), a map patten(7), a fissured patten(3). tongue color- pink (57 cases), red(16), light white(6). coating color- thin white(61 cases), thin yellow(l0), lack(6), deep and white(2). tongue coating substance-thin(72 cases), moisten(5), dry(1), deep(1).
