설소대단축증의 수술적 치료로서의 Z-plasty 술식 1례 (A Case of Z-plasty as a Surgical Treatment in Ankyloglossia)

  • Ankyloglossia is the presence of a lingual frenulum, which can range from a mucous membrane band to a short and thick band and, in extreme cases, to fusion of the tongue to the floor of the mouth. The effects of such a condition, in addition to speech defects and occasionally restriction of sucking, including dental deformities, such as open bite, or even prognathism. Treatment is surgical. The preferred treatment is horizontal sectioning of the frenulum down to the lingual septum and then suturing of the mucosa. The main problem after the healing of surgical wound is adhesion and contracture. Adhesion restrict the movement of tongue like tongue-tie. Z-plasty at the site of incision can solve this problem by changing the direction of scar. We have experienced a patient with ankyloglossia with speech defect, who underwent frenuloomy by Z-plasty. So we present a surgical treatment of Ankyloglossia using Z-plasty and discuss the treatment with a review of literature.

춤연극 제작과정을 통한 움직임의 언어적 소통연구 - 작품 'The tongue'를 중심으로 - (Research of movement language communication through the dancetheater production process - dancetheater 'The tongue' -)

  • 본 연구는 낭만주의 화가 프란시스코 고야의 작품 <아들을 먹어치우는 사투르누스>의 영감을 토대로 춤연극 를 제작하였다. 따라서 제작과정에 나타난 움직임의 상징적 의미와 언어적 소통의 다섯 가지 분석을 시도하였다. 연구결과 첫째, 표현주의를 대표하는 고야의 작품 <아들을 먹어치우는 사투르누스>에 나타난 공포와 괴기 속에 드러난 소통과 상징성은 움직임의 표현적 이미지에 하나의 'signal'적(的) 소통으로 직결되었다. 둘째, 프란시스코 고야가 나타낸 인간의 잔인성, 참혹함이 춤연극 제작을 통해 다른 의미로의 해석이 가능했다. 이것은 움직임 상징성과 역동적 에너지가 갖는 의미와 해석의 다양성으로 인식된다. 셋째, 정적이미지의 그림요소와 색채가 갖는 상징성은 춤연극의 언어적 유희와 움직임의 다섯 가지 상징적 이미지 체계로 정리되었다.

Oral Appliance as a Sensory Trick to Manage Tardive Dyskinesia of Tongue: A Case Report

  • Tardive dyskinesia (TD) is continuous, repetitive movement disorder of tongue, lip or jaw, induced by medication. It causes pain and dysfunction of oral structures but also interferes with dental treatment and overall social life of patients. As a dentist, it is imperative to recognize and manage TD, although currently, there is no definitive treatment for TD. This article reports a patient with TD of tongue, successfully managed with an oral appliance mimicking sensory tricks. Considering the limited treatment options for TD, an oral appliance, a simple and conservative approach, can be a meaningful treatment for some patients with orofacial dyskinesia.

CHANGES IN HYOID BONE AND TONGUE POSITIONS, AND ORAL CAVITY VOLUME AFTER MANDIBULAR SETBACK BY SAGITTAL SPLIT RAMUS OSTEOTOMY

  • Purpose: This retrospective study evaluated the changes in hyoid bone and tongue positions as well as oral cavity volume after mandibular setback by BSSRO. Materials and Methods: 18 Koreans who underwent BSSRO to correct mandibular prognathism were studied. Lateral cephalograms were taken and traced preoperatively (T0), immediately (T1) and approximately 6 months postoperatively (T2). Submentovertex radiograghs were taken and traced before surgery (T0) and about 6 months after surgery (T2). The area and volume of oral cavity, the vertical and horizontal dimensions of the hyoid bone and tongue dorsum were measured. Results: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume. The hyoid bone displaced posteroinferiorly immediately after surgery, and it tended to return to its original vertical position at 6 month after mandibular setback by BSSRO. The retropalatal space around tongue was maintained and the retrolingual space around tongue was reduced immediately postoperatively. The readaptation of tongue was not evident for that the follow up period was not long enough. No significant statistical correlations between the amounts of mandibular setback and the changes of oral cavity volume were observed. Conclusion: Mandibular setback surgery resulted in a significant reduction of lower oral cavity volume, which was most likely attributable to the posterior movement of the mandible. More subjects and long-term observations should be performed to assess the changes of oropharyngeal configuration following mandibular setback surgery.

  • Introduction. In patients with mandibular prognathism, Bilateral Sagittal Split Ramus Osteotomy(BSSRO) combined with orthodontic treatment reduces oral volume and influences tongue and other surrounding tissues. Purpose of this study was to analyze post-operative tongue position and airway dimension, as well as mandibular changes in vertical, horizontal, and angular dimensions. Materials and methods. Height of dorsum of tongue, width of airway, gonial angle and lower facial height of mandibular prognathic patients who visited Kangdong Sacred Heart Hospital from Jan. 2001 to Dec. 2006 were anaylzed via pre-operative and post-operative cephalograms. T-test was used to compare pre-operative and post-operative measurements. Also, correlations among pre-operative measurements of the patients were analyzed. Results and conclusion. A significant correlation was shown between ANS-Xi-PM area and location of dorsum of tongue in pre-operative patients. A significant superior movement of tongue and decrease of airway width was observed in post-operative patients. Also the upper gonial angle decreased significantly.

폐쇄성 수면 무호흡 환자에 있어서 구내장치 반응성에 대한 두부 방사선적 연구 (A cephalometric study in patients with obstructive sleep apnea after use of oral appliance)

  • OSA가 의심되는 환자에 대하여 수면 다원 검사를 시행하여 RDI가 5이상이고 Arousal Index가 20이상이어서 OSA로 진단된 환자 24명을 무작위로 선택하여 구내장치에 대한 반응검사를 위한 수면검사를 다시 시행하여 장치 장착 전후의 RDI의 변화를 기록하였다. 구내장치 장착전과 후에 두장의 두부 방사선을 호기 말에서 채득하여 안모 형태에 대한 변수를 첫번째 두부 방사선상에서 측정하였고, 두 장의 두부 방사선사진을 중첩하여 연구개,후 인두벽, 혀, 설골, 상기도, 하악의 구내장치에 의한 이동을 나타내는 두부 방사선상의 변수를 측정하였다. RDI 개선도에 따라 가장 양호하게 반응하는 8명과 가장 효과가 낮게 반응하는 8명을 선택하여 두 군으로 분류하여 군 비교를 시행하였다. 군 비교를 위하여 Wilcoxon rank test를 시행 하였고 구내장치에 의한 각 변수의 차이와 RDI 개선도와의 상관관계, 안모 형태와 RDI 개선도와의 상관관계를 알아보기 위하여 Spearmann correlation analysis를 시행하였다. 본 연구의 결론은 다음과 같다. 본 연구에서 사용되어진 구내장치는 장착전 평균 RDI=45.8에서 장착 후 평균 RDI=13.3으로 감소하는 효과를 나타냈고 상관 관계에 있어서 하악의 전방이동량과 설골의 상방 이동량은 구내장치 효과와 유의성 있는 상관관계를 나타냈다. 치료 효과가 양호한 군에서 연구개 끝과 후두 덮개 사이 수준(level)에서 유의성 있는 혀의 전방 이동이 관찰되었으며 이러한 구내장치에 의한 혀의 전방 이동은 RDI 개선도와도 유의성 있는 상관관계를 나타냈다. 연구개 끝과 후두 덮개사이 수준(level)에서 상기도의 전후방 길이인 AW는 RDI 개선도와 유의성 있는 상관관계를 나타냈다. 본 연구에 의하면 혀의 전방 이동과 설골의 상방 이동, 상기도의 전후길이 증가등은 RDI의 양호한 개선을 나타내는 지표 였다.

Defatting Vestibuloplasty for Functional and Esthetic Reconstruction of Tongue

  • The radial forearm free flap (RFFF) is a thin and pliable tissue with many advantages for tongue reconstruction. However, tongues reconstructed with RFFF occasionally need revision surgery because inadequate defect measurement at primary surgery can lead to bulkiness and limited movement of reconstructed tongue. In this case, the patient underwent partial glossectomy and RFFF reconstruction for treatment of tongue cancer five years prior. We could not make a lower denture for the patient, because the alveolo-lingual sulcus of tongue was almost lost. So we performed vestibuloplasty with a modified Kazanjian method on the lingual vestibule of the mandibular right posterior area, and defatting surgery to debulk the flap. After surgery, we observed that the color and texture of the revised tongue changed to become similar with adjacent tissue. The patient obtained a more functional and esthetic outcome. Accordingly, we present a case report with a review of relevant literature.

Normalized gestural overlap measures and spatial properties of lingual movements in Korean non-assimilating contexts

  • The current electromagnetic articulography study analyzes several articulatory measures and examines whether, and if so, how they are interconnected, with a focus on cluster types and an additional consideration of speech rates and morphosyntactic contexts. Using articulatory data on non-assimilating contexts from three Seoul-Korean speakers, we examine how speaker-dependent gestural overlap between C1 and C2 in a low vowel context (/a/-to-/a/) and their resulting intergestural coordination are realized. Examining three C1C2 sequences (/k(#)t/, /k(#)p/, and /p(#)t/), we found that three normalized gestural overlap measures (movement onset lag, constriction onset lag, and constriction plateau lag) were correlated with one another for all speakers. Limiting the scope of analysis to C1 velar stop (/k(#)t/ and /k(#)p/), the results are recapitulated as follows. First, for two speakers (K1 and K3), i) longer normalized constriction plateau lags (i.e., less gestural overlap) were observed in the pre-/t/ context, compared to the pre-/p/ (/k(#)t/>/k(#)p/), ii) the tongue dorsum at the constriction offset of C1 in the pre-/t/ contexts was more anterior, and iii) these two variables are correlated. Second, the three speakers consistently showed greater horizontal distance between the vertical tongue dorsum and the vertical tongue tip position in /k(#)t/ sequences when it was measured at the time of constriction onset of C2 (/k(#)t/>/k(#)p/): the tongue tip completed its constriction onset by extending further forward in the pre-/t/ contexts than the uncontrolled tongue tip articulator in the pre-/p/ contexts (/k(#)t/>/k(#)p/). Finally, most speakers demonstrated less variability in the horizontal distance of the lingual-lingual sequences, which were taken as the active articulators (/k(#)t/=/k(#)p/ for K1; /k(#)t/

Risperidone 유발 지연성 운동장애 1례(一例) (A Case on Risperidone-Induced Tardive Dyskinesia)

  • Objective : Base on clinical practice, the authors report a case of tardive dyskinesia arising during the course of treatment with resperidal. Methods : This article was review and analysis of a case on risperidone-induced tardive dyskinea. Results : Mrs K, a 51-year-old woman with a 1-year history of schizophrenic disorder, gradually developed tardive dyskinetic movement of the mouth, lip, and tongue over a 4 month period(From July, 1996 to June, 1997) while taking risperidone. Initially she was treated with haloperidol and alprazolam. However, the haloperidol was subsequently discontinued because of EPS developed. From 11th March, 1997, she was observed to have a severe form of tardive dyskinesia involving her tongue, lip, and mouth. After risperidone was withdrawn at 9th May 1997, her tardive dyskinetic movement was disappeared. Conclusions : This is, to our knowledge, the first report of the onset of tardive dyskinesia in a patient taking risperidone. However, additional controlled studies of specific questions are needed ; e.g., the dose-response curves for produce tardive dyskinesia and the mechanism of producing risperidone-induced tardive dyskinea and so on.

뇌손상 후 발생한 이갈이증과 근육긴장이상에 대한 보튤리눔 독소 A와 교합안정장치를 이용한 치료 증례 -증례 보고- (Bruxism and Oromandibular Dystonia after Brain Injury Treated with Botulinum Toxin A and Occlusal Appliance -A Case Report-)

  • Bruxism is nonfunctional jaw movement that includes clenching, grinding and gnashing of teeth. It usually occurs during sleep, but with functional abnormality of brain, it can be seen during consciousness. Oromandibular dystonia (OMD) can involve the masticatory, lower facial, and tongue muscles and may result in trismus, bruxism, involuntary jaw opening or closure, and involuntary tongue movement. Its prevalence in the general population is 21%, but its incidence after brain injury is unknown, Untreated, bruxism and OMD cause masseter hypertrophy, headache, temporomandibular joint destruction and total dental wear. We report a case of successful treatment of bruxism and OMD after brain injury treated with botulinum toxin A and occlusal appliance. The patient was a 59-year-old man with operation history of frontal craniotomy and removal of malformed vessel secondary to cerebral arteriovenous malfomation. We injected with a total 60 units of botulinum toxin A each masseteric muscle and took impression for occlusal appliance fabrication under general anesthesia. On follow up 2 weeks and 2 months, the patient remained almost free of bruxism. We propose that botulinum toxin A and occlusal appliances be considered as a treatment for bruxism and OMD after brain injury.
