골결손부가 있는 발치직후 매식 임플란트에서 탈회동결건조골과 GTAM차단막이 골재생에 미치는 영향 (EFFECT OF DFDB AND GTAM BARRIERS ON BONE REGENERATION AROUND IMMEDIATE IMPLANTS PLACED IN SURGICALLY DFFECTIVE SOCKET)

  • Dental implant may be immediately placed in postextraction socket which has alveolar bone defect. The purpose of this study was to compare the bone regeneration and bone quality around defects adjacent to implants that were placed into extraction sockets according to EFEB, GTAM barrier and GTAM barrier with DFDB. Mandibular P2, P3 and P4 were extracted bilaterally in dogs, and buccal defects were created about 4mm in depth and 3.3mm in width. Screwed pure titanium implants, 3.8mm in diameter and 10mm in length, were placed into the extraction sockets. The experimental groups were divided into four groups : the G group was covered with a GTAM barrier on the defective area, the D+G group was filled with DFEB and covered with a GTAM barrier, the D group was filled with DFDB only and the control group was sutured without any special treatment on the defective area. The experimental animals were killed after 12 weeks and specimens were prepared for light microscopic evaluation and fluorescent dyes were administered daily for 2 weeks after implantation, and injected on the 4th and 11th week for fluorescent microscopic examination to observe new bone formation and bone remodeling. The new Bone height of the buccal defect was measured and compared with the another for bone gain and the removal torque for the implant was measured for the comparison of bone density and bone-implant osseointegration. Results obtained were as follows : 1. Experimental groups showed bone regeneration in oder from D+G, G, D group and control. D+G and G group was significantly from D group and control(P<0.01). 2. In the defective area of control the regenerated alveolar bone showed poorly developed lamellated structure and fibrous tissue intervention into the bone-implant interface but the others showed well developed lamellated structure and osseointegration. 3. All implant groups showed no significaant difference in the removal torque for implant(P>0.05) These results suggest that immediate implants placed in defective sockets were successfully osseointegrated and utilizing placed in defective sockets were successfully osseointegrated and utilizing not only the combination of GTAM and DFDB but also only the GTAM was favorable for the predictable regeneration of the defective area.

Bioactive characteristics of an implant surface coated with a pH buffering agent: an in vitro study

  • Purpose: The purpose of this study was to evaluate the effectiveness of conventional sandblasted, large-grit, acid-etched (SLA) surface coated with a pH buffering solution based on surface wettability, blood protein adhesion, osteoblast affinity, and platelet adhesion and activation. Methods: Titanium discs and implants with conventional SLA surface (SA), SLA surface in an aqueous calcium chloride solution (CA), and SLA surface with a pH buffering agent (SOI) were prepared. The wetting velocity was measured by the number of threads wetted by blood over an interval of time. Serum albumin adsorption was tested using the bicinchoninic acid assay and by measuring fluorescence intensity. Osteoblast activity assays (osteoblast adhesion, proliferation, differentiation, mineralization, and migration) were also performed, and platelet adhesion and activation assays were conducted. Results: In both the wetting velocity test and the serum albumin adsorption assay, the SOI surface displayed a significantly higher wetting velocity than the SA surface (P=0.000 and P=0.000, respectively). In the osteoblast adhesion, proliferation, differentiation, and mineralization tests, the mean values for SOI were all higher than those for SA and CA. On the osteoblast migration, platelet adhesion, and activation tests, SOI also showed significantly higher values than SA (P=0.040, P=0.000, and P=0.000, respectively). Conclusions: SOI exhibited higher hydrophilicity and affinity for proteins, cells, and platelets than SA. Within the limits of this study, it may be concluded that coating an implant with a pH buffering agent can induce the attachment of platelets, proteins, and cells to the implant surface. Further studies should be conducted to directly compare SOI with other conventional surfaces with regard to its safety and effectiveness in clinical settings.

표면처리가 다른 5종 임플랜트의 안정성에 관한 연구 (A STUDY ON THE STABILITY OF 5 DIFFERENT SURFACE TREATMENT METHODS TO DENIAL IMPLANT USING RESONANCE FREQUENCY AND HISTOMORPHOMETRIC ANALYSIS)

  • Purpose. The purpose of this study was to compare the effects of various surface treatments by measuring resonance frequency and histomorphometric analyses. Material and methods. In 5 adult dogs, the mandibular premolar were extracted. Six months later, 30 screw titanium implants (Dentium Co., Seoul, Korea) 6mm in length and 3.4mm in diameter, were placed in the mandibles of 5 dogs. Implants were divided into five groups following to surface treatment methods ; Group 1 is machined controls, Group 2 is sandblasted with large grit and acid-etched (SLA), Group 3 is anodized (Autoelectric Co., Korea, 660Hz, Duty10), Group 4 is hydroxyapatite(HA) coated by ion beam assisted deposition(E-beam), Group 5 is hydroxyapatite(HA) coated with Sol-gel coating process. Resonance frequency was measured implant placement immediately, and 3, 6 weeks and 10 weeks of healing perods. With the animal subject's sacrifice 10 weeks after implantation, implants were removed on bloc and histologic and computer-based histomorphometric analyses were performed. Histomorphometric analysis involved quantification of the entire bone to metal contact around the implants. Statistical analyses were performed using the SPSS for Windows (ver. 9.0 SPSS Inc.) Statistical differences were considered significant at P<0.05. Results. The results were as follows : 1) In five groups, mean value of resonance frequency analysis(RFA) were highest in group 5 (Sol-gel implant) at implantation and those of group 4 (E-beam)was highest at 10 weeks . but there was no correlation between surface treatments and RFA. 2) In all surface treatment groups, the RFA values of implants decreased until 3 weeks and increased to 10 weeks. 3) The percentage of direct bone-to-implant contact (BIC) had statistical significance between five groups in cancellous bone, (P<0.05) the percentage of bone density inside the thread had no statistical significance between five groups. (P>0.05) 4) There was a significant difference between cortical bone and cancellous bone in BIC. (P<0.05) and bone density. (P<0.05) 5) There was a correlation between the RFA value of implants at 10 weeks and BIC in cancellous bone, and between the RFA value of implants at 10 weeks and bone density in cortical bone. (P<0.05). Conclusions. These results indicate that surface treatment does not affect the implant stability in case of good bone quality.

임플랜트 지대주 나사의 텅스텐 카바이드/탄소 코팅이 전하중에 미치는 영향에 관한 연구 (INFLUENCE OF TUNGSTEN CARBIDE/CARBON COATING ON THE PRELOAD OF IMPLANT ABUTMENT SCREWS)

  • Statement of problem: In order to increase preload with reducing the friction coefficient, abutment screws coated with pure gold and Teflon as dry lubricant coatings have been introduced. But the reported data indicate that if screw repeated tightening and loosening cycle, an efficiency of increasing preload was decreased by screw surface wearing off. Purpose: This study was to evaluate the influence of tungsten carbide/carbon coating, which has superior hardness and frictional wear resistance, on the preload of abutment screws and the stability of coating surface after repeated closures. Material and method: The rotational values of abutment screws and the compressive forces between abutment and fixture were measured in implant systems with three different joint connections, one external butt joint and two internal cones. Moreover the stability and the alteration of coating surface were examined by comparison of the compressive force and the removable torque values during 10 consecutive trials, observation with scanning electron microscope and analyzed the elemental composition with energy dispersive x-ray spectroscopy Results and conclusion: 1. Application of coating resulted in significant increase of compressive force in all implant systems(P<.05). The increasing rate of compressive force by coating in external butt joint was gloater than those in internal cones (P<.05). 2. Coated screw showed the significant additional rotation compared to non-coated screw in all implant systems (P<.05). There were no significant differences in the increasing rate of rotation among implant systems (P>.05). 3. Removable torque values were greater with non-coated screw than that with coated screw (P<.05). 4. Coated screw showed insignificant variations in the compressive forces during 10 consecutive trials(P>.05) 5. After repeated trials, the surface layer of coated screw was maintained relatively well. However surface wearing and irregular titanium fragments were found in non-coated screw.

Surface characteristics of thermally treated titanium surfaces

  • Purpose: The characteristics of oxidized titanium (Ti) surfaces varied according to treatment conditions such as duration time and temperature. Thermal oxidation can change Ti surface characteristics, which affect many cellular responses such as cell adhesion, proliferation, and differentiation. Thus, this study was conducted to evaluate the surface characteristics and cell response of thermally treated Ti surfaces. Methods: The samples were divided into 4 groups. Control: machined smooth titanium (Ti-S) was untreated. Group I: Ti-S was treated in a furnace at $300^{\circ}C$ for 30 minutes. Group II: Ti-S was treated at $500^{\circ}C$ for 30 minutes. Group III: Ti-S was treated at $750^{\circ}C$ for 30 minutes. A scanning electron microscope, atomic force microscope, and X-ray diffraction were used to assess surface characteristics and chemical composition. The water contact angle and surface energy were measured to assess physical properties. Results: The titanium dioxide ($TiO_2$) thickness increased as the treatment temperature increased. Additional peaks belonging to rutile $TiO_2$ were only found in group III. The contact angle in group III was significantly lower than any of the other groups. The surface energy significantly increased as the treatment temperature increased, especially in group III. In the 3-(4,5-Dimethylthiazol- 2-yl)-2,5-diphenyltetrazolium bromide assay, after 24 hours of incubation, the assessment of cell viability showed that the optical density of the control had a higher tendency than any other group, but there was no significant difference. However, the alkaline phosphatase activity increased as the temperature increased, especially in group III. Conclusions: Consequently, the surface characteristics and biocompatibility increased as the temperature increased. This indicates that surface modification by thermal treatment could be another useful method for medical and dental implants.

CELLULAR RESPONSES ON ANODIZED TITANIUM DISCS COATED WITH $1{\alpha}$,25-DIHYDROXYVITAMIN D3 INCORPORATED POLY (D,L-LACTIDE-CO-GLYCOLIDE) (PLGA) NANOPARTICLES

  • STATEMENT OF PROBLEM: A biochemical approach for surface modification has offered an alternative for physicochemical and morphological methods to obtain desirable bone-implant interfaces. PURPOSE: The purpose of the present study was to investigate cell responses to poly (D,L-lactide-co-glycolide) (PLGA)/$1{\alpha}$,25-(OH)$_2D_3$ coating with reference to cellular proliferation and differentiation in vitro. MATERIAL AND METHODS: 96 titanium discs were fabricated and divided into four groups. Group 1 was anodized under 300 V as control. Group 2, 3 and 4 were anodized then coated with 3 ml PLGA/$1{\alpha}$,25-(OH)$_2D_3$ solutions. Amount of the solutions were 2 ul, 20 ul and 200ul respectively. The osteoblast-like Human Osteogenic Sarcoma (HOS) cells were seeded and cultured for 1, 3 and 7 days. MTSbased cell proliferation assay and ALPase activity test were carried out. RESULTS: PLGA nanoparticles were observed as fine, smooth and round and HOS cells attached to the anodized surfaces through strand-like and sheet-like filopodia. After 3 days of culture, the dendritic filopodia were exaggerated and sheet-like cytoplasmic projections covered the coated titanium surfaces. After 3 days of culture, all of the groups showed increased cellular proliferation and the lowest proliferation rate was measured on group 2. Higher amount of incorporated $1{\alpha}$,25-(OH)$_2D_3$ (Group 3 and 4) improved cellular proliferation but the differences were not significant statistically (P > .05). But they increased the rate of ALP activities than the control group at day 3 (P < .05). CONCLUSION: Biodegradable PLGA nanoparticles incorporated with vitamin D metabolite positively affected proliferation and differentiation of cells on the anodized titanium surface.

구리 합금 초음파 스케일러 팁이 치과 임플란트 및 수복 재료 표면에 미치는 영향 (The effect of copper alloy scaler tip on the surface roughness of dental implant and restorative materials)

  • 목적: 본 연구는 구리 합금 스케일러 팁을 비롯한 수종의 스케일러 팁이 임플란트와 수복물 재료 표면에 실제 치석제거 시와 같이 적용하였을 때 표면 거칠기에 미치는 영향을 알아보고자 하였다. 재료 및 방법: 도재, 티타늄, 지르코니아, 제3형 금합금의 지름 15 mm, 높이 1.5 mm의 원반형 시편을 준비하였고, 거칠기 형성기구로 스테인리스 스틸팁(SS), 플라스틱 수동 큐렛(PS), 구리 합금 팁(IS)을 이용하였으며, 시편의 개수는 각 재료 당 기구 별로 4개씩 총 64개를 사용하였다. 표면 거칠기는 40 g의 힘으로 초음파 스케일러는 시편의 표면과 팁이 15도, 핸드 큐렛은 시편의 표면과 큐렛의 날이 45도가 되도록 하여 1초에 1회 5 mm 수평 왕복운동을 30초 동안 시행하여 형성하였다. 각 시편을 주사전자 현미경을 이용하여 관찰하였고, 원자현미경과 표면조도 거칠기 단 차 측정기를 이용하여 표면 거칠기(Ra, ${\mu}m$)를 측정하고 분석하였다. 결과: 주사전자 현미경으로 관찰 결과 표면 거칠기의 증가는 스테인리스 스틸 팁(Group SS)에서 가장 컸으며 구리 합금 팁(Group IS)에서 가장 적게 나타났다. 원자현미경으로 표면 거칠기를 측정한 결과, 스테인리스 스틸 팁(Group SS)은 도재 군과 제3형 금합금 군에서 대조군과 플라스틱 수동 큐렛(Group PS), 구리 합금 팁(Group IS)보다 표면 거칠기가 유의성 있게 크게 나타났고, 구리 합금 팁(Group IS)은 금합금 군에서는 스테인리스 스틸 팁(Group SS)과 플라스틱 수동 큐렛(Group PS)에 비해 표면 거칠기가 유의성 있게 적게 나타났다. 표면조도 거칠기 단 차 측정기로 측정 결과, 스테인리스 스틸 팁(Group SS)은 모든 군에서 대조군 및 플라스틱 수동 큐렛(Group PS)과 구리 합금 팁(Group IS)보다 표면 거칠기가 유의성 있게 크게 나타났으며, 구리 합금 팁(Group IS)은 모든 군에서 스테인리스 스틸 팁(Group SS)에 비해 표면 거칠기가 유의성 있게 적게 나타났다. 제3형 금합금은 도재, 티타늄, 지르코니아 군에 비해 치석제거 기구에 의한 표면 거칠기의 증가가 크게 나타났다(P<.05). 결론: 이상의 연구결과 새로이 개발된 구리 합금 팁(IS)을 적용하였을 때 티타늄 및 치과 수복 재료의 표면 거칠기에 영향을 주지 않음으로써, 임플란트 및 수복치료 된 치아의 치석제거 시 전통적인 스테인리스 스틸 팁(SS)의 대용품으로 유용하게 사용될 수 있을 것으로 사료된다.

타이타늄 임플랜트 시편 내부에 설치한 자석의 자성강도에 따른 골형성 변화 (THE CHANGE OF BONE FORMATION ACCORDING TO MAGNETIC INTENSITY OF MAGNET PLACID INTO TITANIUM IMPLANT SPECIMENS)

  • Purpose. The purposes of this investigation were to discover the possibility of clinical application in the areas of dental implants and bone grafts by investigating the bone formation histologically around specimen which was depending on the intensity of magnetic field of neodymium magnet inside of the specimens. Material and method. 1. Measurement of magnetic intensity - placed the magnet inside of the specimen, and measured the intensity of magnetic field around the 1st thread and 3rd thread of specimen 20 times by using a Gaussmeter(Kanetec Co., Japan). 2. Surgical Procedure - Male rabbit was anesthetised by constant amount of Ketamine (0.25ml/kg) and Rompun (0.25ml/kg). After incising the flat part of tibia, and planted the specimens of titanium implant, control group was stitched without magnet, while experimental groups were placed a magnedisc 500(Aichi Steel Co., Japan) or magnedisc 800(Aichi Steel Co., Japan) into it, fixed by pattern resin and stitched. 3. Management after the surgery - In order to prevent it from the infection of bacteria and for antiinflammation, Gentamycin and Ketopro were injected during 1 week from operation day, and dressed with potadine. 4. Preparation of histomorphometric analysis - At 2, 4 and 8 weeks after the surgery, the animals were sacrificed by excessed Ketamine, and then, specimens were obtained including the operated part and some parts of tibia, and fixed it to 10% of PBS buffer solution. After embedding specimens in Technovit 1200 and B.P solution, made a H-E stain. Samples width was 75$\mu$m . In histological findings through the optical microscope and using Kappa image base program(Olympus Co. Japan), the bone contact ratio and bone area ratio of each parts of specimens were measured and analyzed. 5. Statistical analysis - Statistical analysis was accomplished with Mann Whitney U-test. Results and conclusion. 1. In histomorphometric findings, increased new bone formation was shown in both control & experimental groups through the experiment performed for 2, 4 & 8 weeks. After 4 weeks, more osteoblasts and osteoclasts with significant bone remodeling were shown in experimental groups. 2. In histomorphometric analysis, the bone contact ratios were 38.5% for experimental group 1, 29.5% for experimental group 2 and 11.9% for control group. Experimental groups were higher than control group(p<0.05) (Fig. 6, Table IV). The bone area ratios were 60.9% for experimental group 2, 46.4% for experimental group 1 and 36.0% for control group. There was no significantly statistical difference between experimental groups and control group(p<0.05) (Fig. 8, Table VII) 3. In comparision of the bone contact ratios at each measurement sites according to magnetic intensity, experimental group 2(5.6mT) was higher than control group at the 1st thread (p<0.05) and experimental group 1 (1.8mT) was higher than control group at the 3rd thread(p<0.05) (Fig. 7, Table V, VI). 4. In comparision of the bone area ratios at each measurement sites according to magnetic intensity, experimental group 2(5.6mT) was higher than control group and experimental group 1 (4.0mT) at the 1st thread(p<0.1) and experimental group 2(4.4mT) was higher than experimental group 1 (1.8mT) at the 3rd thread(p<0.1) (Fig. 9, Table IX, X). Experiment group 2 was largest, followed by experiment group l and control group at the 3rd thread of implant. There was a significant difference at the 1st thread of control group & experiment group 2, and at 1st thread & 3rd thread of experiment group 1 & 2, and not at control group experiment group 1.(p<0.1)

Impact on Retrievability by Cement Variety for Implant Restorations Equipped with a Lingual Slot

  • Purpose: The purpose of this study is to measure and compare the removal torques of different cements applied in attachments of zirconia restorations on titanium (Ti) abutments fitted with retrievable cement-type slot (RCS) on the lingual side for the better retrievablity by use of a slot driver. Materials and Methods: Three types of cements were used in the experiment: two permanent cements in $RelyX^{TM}$ U200 (RU) (3M ESPE) which is a resin cement and $FujiCem^{TM}$ (FC) (GC) which is a resin-modified glass ionomer cement, and a temporary cement in $Freegenol^{TM}$ temporary cement (TC) (GC). Measurements of removal torques were conducted as follows; an attached sample was fixed on the equipment customized for the experiment; a slot driver was connected to a MGT12 (Mark-10 Corp.), a torque measurement instrument; the sample had the driver fitted to its RCS and then was rotated until the it was removed; and finally, the maximum torque value was recorded. Result: As for the removal torque measurement results, the average values were $47.9{\pm}2.6Ncm$ for RU, $43.4{\pm}1.5Ncm$ for FC, and $20.9{\pm}1.0Ncm$ for TC. The statistical analysis using Kruskal-Wallis test yielded the significance probability of P<0.05 (P=0.002), which confirmed the presence of significant differences between the three groups. Conclusion: All three cements exhibit clinically acceptable levels of removal torque when applied to an upper zirconia implant restoration fitted with a lingual slot, with RU and FC, the two permanent cements, having the significantly higher values than that of TC, the temporary cement.

Displacement of scan body during screw tightening: A comparative in vitro study

  • PURPOSE. The purpose of this study was to evaluate the occurrence of displacement while tightening the screw of scan bodies, which were compared according to the material type. MATERIALS AND METHODS. Three types of scan bodies whose base regions were made up of polyether ether ketone (PEEK) material [Straumann Group, Dentium Group, and Myfit (PEEK) Group] and another scan body whose base region was made up of titanium material [Myfit (Metal) Group] were used (15 per group). The reference model was fabricated by aligning the scan body library on the central axis of the implant, and moving this position by the resin model. The screws of the scan bodies were tightened to the implant fixture with torques of 5 Ncm, 10 Ncm, and a hand tightening torque. After the application of the torque, the scan bodies were scanned using a laboratory scanner. To evaluate the vertical, horizontal, and 3-dimensional (3D) displacements, a 3D inspection software program was used. To examine the difference among groups, one-way analysis of variance and Tukey's HSD post hoc test were used (α=.05). RESULTS. There were significant differences in 3D, vertical, and horizontal displacements among the different types of scan bodies (P<.001). There was a significantly lower displacement in the Straumann group than in the Myfit (PEEK) and Dentium groups (P<.05). CONCLUSION. The horizontal displacement in all groups was less than 10 ㎛. With the hand tightening torque, a high vertical displacement of over 100 ㎛ occurred in PEEK scan bodies (Myfit and Dentium). Therefore, it is recommended to apply a tightening torque of 5 Ncm instead of a hand tightening torque.
