ESR dosimetry and Dating toward $21^{st}$ Century

  • Dating and dosimetry using electron spin resonance (ESR) in 20th Century developed at both Yamaguchi University and Osaka University have been reviewed with emphasis on new prospects and strategies in 21th century. Natural radiation have been generating radicals that accumulated in archaeological and geological materials. ESR detects these radicals and the ESR signal intensity is proportional to the radiation dose and therefore the age. The assessment of the total dose of natural radiation and the annual dose rate give their ESR ages. The ESR dating of stalactites and stalagmites ant Akiyoshi cave in Yamaguchi prefecture in 1975 was extended to anthropological dating using bones and tooth enamel excavated in Greek Petralona cave. Fossils of shells and corals gave the ages of marine terraces and sea-level changes. Quartz grains gave the ages of geothermal alteration and fault movements. Future ESR dating of ices at outer planets anf their satellite are also investigated as basic studies for ices od $H_2O,\;CO_2,\;SO_2$ as well as terrestrial hydrates in laboratory. Atomic bomb radiation dosimetry at Hiroshima and Nagasaki using ESR lead to the dosimetry of personnel, Chemobyl and JCO criticality accidents. Monitoring of radiation dose with sensitive materials with tissue equivalence are being developed. finally a new scanning ESR imaging apparatus (a near field microwave microscope) developed in our laboratory gave ESR images of Radicals from fossils to Si-CVD and diamond films as summarized in my book in 2002.

위장내 음식물에 따른 방사선 감약 및 화질에 관한 연구 (The Study on the Attenuation of X-ray and Imaging Quality by Contents in Stomach)

  • 인체의 $40{\sim}50%$인 연부조직에 X선을 조사하면 연부조직의 두께에 따라 투과선량이 감소하며 영상에도 질적 저하를 가져온다. 본 연구는 복부 촬영시 위장내 내용물에 따라 X선 감약 및 화질에 차이가 나타날 수 있다는 가정 하에 실제 위의 조직과 비슷한 조직등가물질 phantom을 제작하여 실제 위장내 내용물에 따른 X선의 감약 변화와 DR (Digital Ridiography)의 ROI수치를 비교해보았다. 단백질 두께가 증가할수록 X-선 감약에 의한 투과선량이 감소되었으며(p < 0.001), Film과 DR 영상에서의 ROI Mean값의 변화도 감소하였다(p <0.001). Film과 DR 영상에서의 ROI Mean값에 대한 비교는 Film이 DR 영상에 비해 단백질 두께에 따른 농도의 변화 값이 크게 나타났다(p < 0.001). 이와 같은 결과를 종합해 볼 때 DR system 촬영 보다는 필름을 사용하는 단순촬영에서 금식(NPO ; nothing by mouth)의 필요성이 더 중요함을 알 수 있다.

COPD 동물 모델에서 소청룡탕 흡입제형의 효과 (Effects of Inhalable Microparticles of Socheongryong-tang on Chronic Obstructive Pulmonary Disease in a Mouse Model)

  • Objectives: This study aimed to evaluate the effects of microparticles of Socheongryong-tang (SCRT) on chronic obstructive pulmonary disease (COPD) in a mouse model. Methods: The inhalable microparticles containing SCRT were produced by spray-drying with leucine as an excipient, and evaluated with respect to the aerodynamic properties of the powder by Andersen cascade impactor (ACI). Its equivalence to SCRT extract was evaluated using lipopolysaccharide (LPS) and a cigarette-smoking (CS)-induced murine COPD model. Results: SCRT microparticles provided desirable aerodynamic properties (fine particle fraction of $49.6{\pm}5.5%$ and mass median aerodynamic diameter of $4.8{\pm}0.3{\mu}m$). SCRT microparticles did not show mortality or clinical signs over 14 days. Also there were no significant differences in body weight, organ weights or serum chemical parameters between SCRT microparticle-treated and non-treated groups. After 14 days the platelet count significantly increased compared with the non-treated group, but the values were within the normal range. Inhalation of SCRT microparticles decreased the rate of neutrophils in blood, granulocytes in peripheral blood mononuclear cells (PBMC) and bronchoalveolar lavage fluid (BALF) and level of TNF-${\alpha}$ and IL-6 in BALF on COPD mouse model induced by LPS plus CS. This effect was verified by histological findings including immunofluorescence staining of elastin, collagen, and caspase 3 protein in lung tissue. Conclusions: These data demonstrate that SCRT microparticles are equivalent to SCRT extract in pharmaceutical properties for COPD. This study suggests that SCRT microparticles would be a potential agent of inhalation therapy for the treatment of COPD.

아크릴 팬톰에서 GafChromic 필름을 이용한 고선량률 근접 치료용 Ir-192 선원의 근접 거리에서 비등방성 측정 (Anisotropy in a Few mm Regions from an Ir192 High Dose Rate Source Measured with a GafChromic Film in Acrylic Phantom)

  • GafChromic 필름은 높은 분해능과 낮은 민감도 그리고 조직 등가 물질에 유사하게 구성되어 있다. 본 연구의 목적은 GafChromic 필름을 이용하여 고선량률 근접치료에 사용하는 Ir-192선원 근접 거리에서의 비등방성을 측정하는 것이다. GafChromic 필름은 0 Gy 105 Gy까지 4 MV 선형가속기를 이용하여 흑화도를 구하였다. 비등방성 값은 아크릴 팬톰에서 GafChromic 필름을 이용하여 측정하였고, 측정된 값은 J. F. Williamson이 몬데칼로 계산한 값과 비교하였다. 비교 결과 거리(r) 2.5 mm, 각도 50 ∼ 130도까지 영역에서 4.4% 이내로 일치하였고 반면 140도 부근에서 17.6% 차이가 나는 것을 알 수 있었고, 한편 거리(r) 5 mm, 각도 35 ∼ 160도까지 3.7% 이내로 일치하였고, 30도 부근에서는 7.6% 정도 차이가 나는 것을 알 수 있었다. GafChromic필름은 고선량률 근접치료 Ir-192선원의 근접 거리에서 비등방성 측정 시 기존에 사용되고 있는 측정 장비보다 매우 유용한 것으로 생각된다.

X선 촬영시 산란선 방향 의존성에 관한 연구 (A Study on the Directional Dependence of Scatter Ray in Radiography)

  • In this pauper, the back, forward, side and $45^{\circ}$ oblique scatter dose were measured the X-ray exposure conditions 60, 80, 100, 120kV, FFD 100cm, FS $20\times20cm$, toward the $25\times25cm\times10\sim20cm$ of solid water, paraffin and MiX-DP phantom, and Pb, Cu, Al, and styrofoam meterials, by the electrometer and 5.3 cc ionization chamber. The obtained results are summarized as following. 1. The percentage depth dose(PDD) at the range of the diagnostic x-ray energy were appeared 50 % depth dose at the 2 cm depth with 60 kV, and 5 cm depth with 120 kV X-ray, 10 % depth dose at the 10 cm depth with 60 kV and 14 cm depth with 120 kV X-ray, 5 % below depth dose at the 20 cm depth. 2. The back scatter dose which were generated the surface of Pb, Cu and Al metal plates were 10 % below, and than the back scatter dose at the Pb plate were a most amount of these which were about 10 %, and were appeared the order of Cu and Al. 3. The percentage forward scatter were appeared from 50 % to 65 %, and the more phantom thicknees become, the more forward scatter were increased with the ratio of 5 % per 5 cm thickness. 4. The percentage back scatter which were generated the tissue equivalence meterials solid water, paraffin and MiX-DP were from 20 % to 40 %, and than the back scatter dose at the solid water were a mest amount of those, and paraffin and MiX-DP were appeared with the next values. 5. The percentage $90^{\circ}$ lateral and $45^{\circ}$ oblique side scatter dose were measured from 4 % to 12 %. a most amount of scatter dose which were generated from the patient in radiography were the forward scatter, the next values were the back scatter, the third values were the $90^{\circ}$ lateral scatter.

중환자의 욕창 예방 연구 : 욕창 예방 QI팀을 중심으로 (CQI Action Team Approach to Prevent Pressure Sores in Intensive Care Unit of an Acute Hospital Korea)

  • Background : A pressure sore was defined as any skin lesion caused by unrelieved pressure and resulting in damage to underlying tissue. The health care institutions in the United States were reported the incident rate of pressure sores ranging from 6 to 14 %. Intensive Care Unit needed highest quality of care has been found over 40% incidence rate of pressure sore. Also, Annual expenditures for the care of pressure sores in patients in the United States have been estimated to be $7.5 billion; furthermore, 50 percent more nursing time is required to care for patients with pressure sore in comparison to the time needed to implement preventive measures against pressure sore formation. However, In Korea, there were little reliable reports, or researches, about incidence rates of pressure sore in health care institution including intensive care unit and about the integrated approach like CQI action team for risk assessment, prevention and treatment of pressure ulcers. Therefore, this study was to develop pressure sore risk assessment tool and the protocol for prevention of pressure sore formation through CQI action team activities, to monitor incident rate of pressure sore and the length of sore formation for patients at high risk, and to approximately estimate nursing time for sore dressing during research period as the effect of CQI action team. Method : CQI action team in intensive care unit, launched since early 1996, reviewed the literature for the standardized risk assessment tool, developed the pressure sore assessment tool based on the Braden Scale, tested its validity, compared on statistics including incidence rate of pressure sore for patients at high risk. Throughout these activities, CQI action team was developed the protocol, called as St. Marys hospital Intensive Care Unit Pressure Sore Protocol, shifted the emphasis from wound treatment to wound prevention. After applied the protocol to patients at high risk, the incident rate and the period of prevention against pressure development were tested with those for patients who received care before implementation of protocol by Chi-square and Kaplan-Meier Method of Survival Analysis. Result : The CQI action team found that these was significant difference of in incidence rate of pressure sores between patients at high risk (control group) who received care before implementation of protocol and those (experimental group) who received it after implementation of protocol (p<.05). 25% possibility of pressure sore formation was shown for the patients with 6th hospital day in ICU in control group. In experimental group, the patients with 10th hospital day had 10% possibility of pressure sore. Therefore, there was significant difference(p<.05) in survival rate between two groups. Also, nursing time for dressing on pressure sore in experimental group was decreased as much as 50% of it in control group. Conclusion : The collaborative team effort led to reduced incidence, increased the length of prevention against pressure sore, and declined nursing care times for sore dressing. However, there have had several suggestions for future study. The preventive care system for pressure sore should be applied to patients at moderate, or low risk throughout continuous CQI team activities based on Bed Sore Indicator Fact Sheet. Hospital-wide supports, such as incentives, would be offered to participants for keeping strong commitment to CQI team. Also, Quality Information System monitoring incidents and estimating cost of poor quality, like workload (full time equivalence) or financial loss, regularly in a hospital has to be developed first for supporting CQI team activities as well as empowering hospital-wide QI implementation. Being several limitations, this study would be one of the report cards for the CQI team activities in intensive care unit of an acute hospital and a trial of quality improvement of health care in Korea.
