Fusiform Aneurysm Presenting with Cervical Radiculopathy in Ehlers-Danlos Syndrome

  • Ehlers-Danlos syndrome (EDS) type IV is characterized by its clinical manifestations, which are easy bruising, thin skin with visible veins, and rupture of arteries, uterus, or intestines. Arterial complications are the leading cause of death in vascular EDS because they are unpredictable and surgical repair is difficult due to tissue fragility. The authors report a case presented with cervical radiculopathy due to a segmental fusiform aneurysm of the cervical vertebral artery. Transfemoral cerebral angiography (TFCA) was done to verify the aneurysmal dilatation. However, during TFCA, bleeding at the puncture site was not controlled, skin and underlying muscle was disrupted and profound bleeding occurred during manual compression after femoral catheter removal. Accordingly, surgical repair of the injured femoral artery was performed. At this time it was possible to diagnose it as an EDS with fusiform aneurysm on cervical vertebral artery. Particularly, cervical fusiform aneurysm is rare condition, and therefore, connective tissue disorder must be considered in such cases. If connective tissue disorder is suspected, the authors suggest that a noninvasive imaging modality, such as, high quality computed tomography angiography, be used to evaluate the vascular lesion to avoid potential arterial complications.

Reconstruction Techniques for Tissue Defects Formed after Preauricular Sinus Excision

  • Background Preauricular sinuses are congenital abnormalities caused by a failure of fusion of the primitive tubercles from which the pinna is formed. When persistent or recurring inflammation occurs, surgical excision of the infected tissue should be considered. Preauricular defects inevitably occur as a result of excisions and are often difficult to resolve with a simple suture; a more effective reconstruction technique is required for treating these defects. Methods After total excision of a preauricular sinus, the defect was closed by a plastic surgeon. Based on the depth of the defect and the degree of tension when apposing the wound margins, the surgeon determined whether to use primary closure or a posterior auricular flap. Results A total of 28 cases were examined. In 5 cases, including 2 reoperations for dehiscence after primary repair, reconstruction was performed using posterior auricular transposition flaps. In 16 cases of primary closure, the defects were closed using simple sutures, and in 7 cases, closure was performed after wide undermining. Conclusions If a preauricular defect is limited to the subcutaneous layer and the margins can be easily approximated, primary closure by only simple suturing may be used to perform the repair. If the defect is deep enough to expose the perichondrium or if there is tension when apposing the wound margins, wide undermining should be performed before primary closure. If the extent of the excision exposes cartilage, the procedure follows dehiscence of the primary repair, or the tissue is not sufficiently healthy, the surgeon should use a posterior auricular flap.

토종홍화씨의 급여가 실험동물의 늑골골절 회복중 골조직에 미치는 영향 (Effects of Diet of Korean Safflower(Carthamus tinctorious L.) Seed Powder on Bone Tissue in Rats during the Recovery of Rib Fracture)

  • This study was carried out to investigate the effects of diet of Korean safflower(Carthamus tinctorious L.) seed powder on bone tissue during the recovery of rib-fracture in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups including the control group(C group, AIN-76 semipurified diet) and safflower seed group(S group, AIN-76 semipurified diet+10% safflower seed powder) and were fed experimental diets for 12 days after adaptation period. After this period, the 9th right rib was fractured surgically and sham-operation was also performed. Rats were fed with experimental diets for up to 30 more days after rib-fracture. The degree of bone repair was evaluated during the recovery period at the 8th, 11th, 16th, 21st, 30th days after the surgical operation by microscopic observation of the fractured rib tissue. In callus formation, the portion of hyaline cartilage was noticably higher in S group than C group. The intracatilagenous ossification was observed at the 8th day in S group, but at 11th day in C group. The intramembranous ossification in callus was widely found over the 8th day to the 11th day in S group, but it was shown over the 11th day to the 16th day in C group. Bone resorption was also occured more rapidly in S group as indicated by large numbers of osteoclasts observed. At the 30th day, most of trabecular bones were disappeared in S group, whereas still shwon in C group over wide ranges of fractured ribs. These results imply that the supplementation of Korean safflower seed powder influences in the recovery of bone fracture by accelerating the process of bone repair.

  • Finding a right repair material for furcation perforation is one of the major issues in clinical endodontics. In this experiment, three materials, calcium sulfate, amalgam, and calcium hydroxide were tested for perforated furcation repair. Sixty premolars and molars of five dogs were used. A #4 round bur was used to create the perforation. All experimental teeth were divided into two repair-time groups. One was immediate-repair group, where the perforation was repaired immediately, the other was delayed-repair group, where the perforation was left open for four weeks and then repaired with the same manner as in the immediate-repiar group. All chamber openings were sealed with amalgam and then radiographed. The animals were sacrificed at eighth week following the repair procedure. Radiographic evaluation for furcal bone destruction was done. Histologic evaluation was ranked as 0,1,2,3 according to the inflammation degrees. New bone formation was also recorded. The following conclusions were drawn within the limits of the experimental results: 1. In immediate-repair group, no significant differences existed between the materials. 2. In delayed-repair group, calcium sulfate showed significantly less furcal bone destruction and lower inflammation degree than amalgam.(p<0.05) 3. Overextruded specimens showed more severe inflammation than unextruded specimens. 4. Most of the specimens showed certain degrees of inflammatory reaction and incomplete hard tissue healing. 5. In delayed-repair group, treated group showed less inflammation than untreated control group.

광범위 회전근 개 파열의 봉합술 (Massive Rotator Cuff Tear Repair)

  • 목적: 광범위 회전근 개 파열은 파열단의 내측 퇴축, 근위축 및 지방 변성과 함께 주변 조직과의 유착 등으로 해부학적 봉합이 불가능한 경우가 많다. 본 종설에서는 광범위 회전근 개 파열의 여러 가지 치료 방법 중 봉합술에 대하여 문헌 고찰과 함께 임상 결과 향상과 재파열율을 감소시키고 치유력을 높일 수 있는 방법을 알아보고자 한다. 대상 및 방법: 광범위 회전근 개 파열 봉합술의 선택은 환자의 나이, 동반 질환, 통증, 운동 범위 감소, 근력 약화 등의 임상 증상 및 재활 의지 등 환자와 관련된 요인과, 회전근 개 파열 기간, 크기, 퇴축 및 지방 변성 정도 등은 회전근 개와 관련된 요인을 고려하여 선택해야 한다. 결과: 조직의 가동성이 떨어져 해부학적 봉합이 어려운 광범위 회전근 개 파열은 주변 조직 유리술과 간격 활주 방법으로 가동성을 증가시켜 봉합할 수 있다. 주변 조직 유리술을 시행하고 회전근 개의 가동성을 증가시켜도 해부학적 봉합이 불가능한 광범위 회전근 개 파열은 부분 봉합, 변연 수렴 술식 및 상완 이두건 절제 및 고정술 등의 대체 술식을 고려할 수 있다. 광범위 회전근개 파열 환자에서 견봉하 감압술 및 회전근 개 봉합술에 관한 여러 보고는 통증 감소, 견관절 기능 및 근력 회복 등 만족할만한 임상 결과를 보고하고 있으나 장기 추시 결과 높은 재파열율이 관찰되고 있다. 결론: 광범위 회전근 개 파열의 치료는 아직까지 높은 재파열 발생율로 해결해야 할 과제가 많으나 광범위 파열의 병인, 진행 과정 및 임상 양상에 대한 이해와 재파열 예후 인자 분석 등을 통해 향상된 치료 성적을 얻을 수 있을 것으로 사료된다.

  • It has been reported that the gap between the tendon stumps in chronic Achilles tendon rupture is filled with interposed scar tissue. If it was available to use the interposed scar tissue for reconstruction or augmentation of Achilles rupture, possible damage of normal tissues could be avoided. Our results show that direct repair method using interposed scar tissue for chronic Achilles tendon rupture can successfully relieve pain and restore function of the ruptured Achilles tendon in carefully selected patients.

유성견 소구치 정출시 치아 및 치주 조직의 초기 조직학적 변화 및 재생에 관한 연구 (A HISTOLOGIC STUDY OF INITIAL CHANCE AND REPAIR OF TOOTH AND PERIODONTAL TISSUE IN EXTRUSION OF YOUNG ADULT DOGS)

  • 본 실험은 유성견에서 인위적인 정출 시 조직학적, 임상학적인 반응과 시간경과에 따른 치아 및 치아 주위조직의 회복을 연구하기 위해 생후 평균 10 개월 웅성 잡종 유성견 하악 소구치를 대상으로 Energy chain을 이용한 정출 실험으로, 실험 1 군은 힘을 작용 시킨 후 일 주일만에 희생시키고 실험 2 군은 정출력을 가한 후 일 주일 후, 정출력이 제거되고 3 주간의 회복 기간 경과 후에 희생시켰다. HE 및 Masson's Trichrome 염색법을 이용하여 치아와 주위 조직의 반응을 치경부, 치근 중앙부, 치근단부, 치수부로 분류하여 광학 현미경하에서 관찰하여 다음과 같은 결과를 얻었다. 1. 힘을 가한 후 일 주일 후, 모든 부위에서 치주인대의 주행방향이 치아를 향하여 사선 방향을 나타내 주었으며 치주인대의 간격이 전반적으로 증가되었다. 또 부분적인 치주 인대의 파열이 치근단부에서 뚜렷하게 관찰되었다. MT 소견에서는 대조군에 비해 치조능 부위에서 미성숙 골의 형성을 보여주었다. 주위 치조골에서는 편측으로 활발한 골조직의 형성이 관찰되었다. 치수에서는 치근단 부위에서 많은 혈관의 파열을 보여 주었고 공포의 형성과 충혈을 나타냈다. 2. 힘을 제거 된 후3주간의 보정을 한 군의 모든 치주인대의 주행 방향과 간격이 대조군과 같이 회복되었으며 밀도는 치밀하게 나타났다. MT소견에서 1군에서 형성된 치조능의 미성숙 골이 석회화가 이루어짐을 보여주었고 치수의 공포와 충혈의 감소를 보여주었다. 수복과정에서의 치수의 섬유화가 더욱 진행되었다. 이상의 결과를 종합해 볼 때, 3주간의 회복 기간 중에도 치주 인대 부위에서는 대조군과 유사한 조직 소견을 보여 상당한 회복능력을 보였지만 치수에서는 섬유화가 진행됨을 보여 더욱 긴 회복 기간이 필요함을 보여주었다.

복잡 대동맥 교약증 또는 대동맥 차단증의 일차적 완전교정술에 대한 연구 (One-stage total Correction for Complex Aortic Coarctation and Interrupted Aortic Arch)

  • Between March 1989 and December 1994, one-stage repair was performed for correction of the intracardiac malformations associated with aortic coarctation in 34 patients or interrupted aortic arch in 8 patients via median sternotomy. There were 26 male and 16 female patients, and their body weight ranged from 1.8 to 8kg[mean weight, 4.0$\pm$l.4kg . The age at the operation ranged from 7 days to 18 months [mean age, 3.1$\pm$3.8 months . The repair of aortic coarctation or interrupted aortic arch was performed using extended end-to-end anastomosis in most of the patients[86%, 36/42 , and six patients underwent ductal tissue excision and patch aortoplasty. Intracardiac defects were corrected concomitantly through the right atrium unless the anatomy dictated otherwise. Obstructive outlet septurn was resected whenever necessary. There were seven early deaths[16.8% , and three late deaths with a mean follow-up period of 25 months [range from 1 to 65 months . Three patients were reoperated upon residual subaortic stenosis,stenosis at the RPA origin, and subacute bacterial endocarditis respectively. None showed any significant residual or anastomotic stenosis postoperatively. One stage repair of the aortic coarctation and interrupted aortic arch associated with intracardiac defect leaves no native coarctation shelf tissue or residual hypoplasia in the repaired segment, has low incidence of recurrent or residual stenosis, minimizes reoperation and incisions, and manages arch hypoplasia easily.We conclude that surgical results of one-stage repair for the intracardiac malformation associated with aortic coarctation or interrupted aortic arch are resonable.

  • Many studies have shown that Mg-Nd-Zn-Zr (abbreviated as JDBM) alloy has good biocompatibility and biodegradability as well as promotion of cell adhesion, proliferation and differentiation, and Wnt/β-catenin signaling pathway may play a unique role in joint tissue by controlling the function of chondrocytes, osteoblasts and synoviocytes. However, it is not clear whether the JDBM alloy induces osteochondral repair through Wnt/β-catenin signaling pathway. This study aims to verify that JDBM alloy can repair osteochondral defects in rats, which is realized by Wnt/β-catenin signaling pathway. In this study, the osteochondral defect model of the right femoral condyle non-weight-bearing area in rats was established and randomly divided into three groups: Control group, JDBM alloy implantation group and JDBM alloy implantation combined with signaling pathway inhibitor drug ICRT3 injection. It was found that after JDBM alloy implantation, the bone volume fraction (BVF) became larger, the bone trabeculae were increased, the relative expression of osteogenesis gene Runx2, Bmp2, Opn, Ocn and chondrogenesis gene Collagen II, Aggrecan were increased, and the tissue repair was obvious by HE and Masson staining, which could be inhibited by ICRT3.
