2022학년도 작업치료(학)과 신입생의 학과 결정에 관한 연구 (A Study on Factors that Influence Department Selection for Freshmen Majoring in Occupational Therapy in 2022)

  • Purpose : This study aimed to identify factors that influence selection of occupational therapy as a college major by freshmen. Methods : A survey was conducted on 308 freshmen majoring in occupational therapy from March 2022 to September 2022. The questionnaire consisted of five items about general characteristics, two about the choice of college and major, three about the timing of major selection, three about information acquisition, and one about the university application process. Results : While choosing a university, 37% of the students primarily considered the available choice of majors, whereas 41% considered the college application period. Employment rate was another important factor that was considered when choosing a department. Most students learned about occupational therapy through research on physical therapy and primarily gathered information by searching on "Naver and blogs." Among the descriptions of occupational therapy, the most interesting keyword was "hospital" (54 %). Conclusion : This study investigated the factors that influenced the college major selection by new students in the department of occupational therapy. This study provides meaningful basic data that can be referred to when promoting the department of occupational therapy. A limitation of this study is that a high percentage of students from Gyeongnam were included, and hence, further research that investigates according to region is needed. Moreover, detailed investigations on factors related to university applications in each region are required. It is also necessary to investigate the relationship between the characteristics of freshmen and the determining factors of the department and the admission process.

DYNAMIC ANALYSIS OF ATTENDANCE BEHAVIOR WITHIN CONSTRUCTION CREWS

  • Although individual-level factors (e.g., attitudes, personality) have long been associated with day-to-day attendance decisions, increasingly researchers have recognized "the social nature of attendance dynamics and their susceptibility to social control (Johns, 2008)." Implications of this social approach for research would be to focus attention on the causes and effects of absence culture (i.e., absence-related perceptions, beliefs, values), and the effects of absence culture on individual and group attendance within social units. Construction projects typically require workers to work in teams or crews on highly interdependent projects, and, thus, are particularly relevant contexts to study absence culture. In this paper we apply a system dynamics (SD) model to study absence culture by utilizing the advantages of SD in capturing a feedback process and state changes. We were particularly interested in: (a) the awareness of social norms within construction crews that pertained to attendance, (b) the interplay between formal attendance rules (policy) and these social norms, and (c) how these sources of influence affected the decision-making process of construction crew members. We expect that the results of this work will help construction organizations evaluate (or re-consider) the effects of their attendance control policies (e.g., timing, strength, and frequency) within a social context. Moreover, our findings suggest that the key to reducing excessive absences might be to invest time in influencing absence culture directly rather than imposing frequent and strict regulations - which, in turn, may inadvertently fortify a culture that works against the organization's interests.

Outcomes of Extracorporeal Membrane Oxygenation in COVID-19: A Single-Center Study

  • Background: Coronavirus disease 2019 (COVID-19) can lead to acute respiratory failure, which frequently necessitates invasive mechanical ventilation and extracorporeal membrane oxygenation (ECMO). However, the limited availability of ECMO resources poses challenges to patient selection and associated decision-making. Consequently, this retrospective single-center study was undertaken to evaluate the characteristics and clinical outcomes of patients with COVID-19 receiving ECMO. Methods: Between March 2020 and July 2022, 65 patients with COVID-19 were treated with ECMO and were subsequently reviewed. Patient demographics, laboratory data, and clinical outcomes were examined, and statistical analyses were performed to identify risk factors associated with mortality. Results: Of the patients studied, 15 (23.1%) survived and were discharged from the hospital, while 50 (76.9%) died during their hospitalization. The survival group had a significantly lower median age, at 52 years (interquartile range [IQR], 47.5-61.5 years), compared to 64 years (IQR, 60.0-68.0 years) among mortality group (p=0.016). However, no significant differences were observed in other underlying conditions or in factors related to intervention timing. Multivariable analysis revealed that the requirement of a change in ECMO mode (odds ratio [OR], 366.77; 95% confidence interval [CI], 1.92-69911.92; p=0.0275) and the initiation of continuous renal replacement therapy (CRRT) (OR, 139.15; 95% CI, 1.95-9,910.14; p=0.0233) were independent predictors of mortality. Conclusion: Changes in ECMO mode and the initiation of CRRT during management were associated with mortality in patients with COVID-19 who were supported by ECMO. Patients exhibiting these factors require careful monitoring due to the potential for adverse outcomes.

Solid pseudopapillary epithelial neoplasm of pancreas in pregnancy: A case report and review of literature

  • The solid pseudopapillary epithelial neoplasm (SPEN) of the pancreas is an uncommon tumor that accounts for approximately 1%-2% of exocrine pancreatic neoplasms. It predominantly affects female in their second and third decades of life. In this case report, we present a clinical scenario of a 21-year-old pregnant woman who incidentally discovered a solid cystic lesion in her pancreas, exhibiting features suggestive of SPEN. The patient underwent surgery during the second trimester. Management of pregnant females with SPEN poses challenges due to the absence of definitive treatment guidelines, particularly in determining the ideal timing for surgical intervention. Notably, during pregnancy, the presence of a small SPEN does not necessarily require immediate resection. However, if the tumor is of significant size, it can give rise to complications such as tumor rupture, multivisceral resection, recurrence, spontaneous abortion, intrauterine growth restriction, or premature delivery if not addressed. In the existing literature, a common finding is that approximately two-thirds of pregnant females with SPEN underwent surgery in the second trimester, often without complications for the mother or fetus. All these tumors were larger than 8 cm. The decision to operate before or after birth can be individualized based on team discussion. However, delay in surgery may lead to larger tumors and higher risks like bleeding, rupture, multivisceral resection, and recurrence. Therefore, second-trimester surgery seems safer, and lessens dangers, emergency surgery, and tumor recurrence.

수학적 모델을 이용한 신종인플루엔자 환자 예측 및 대응 전략 평가 (Mathematical Modeling of the Novel Influenza A (H1N1) Virus and Evaluation of the Epidemic Response Strategies in the Republic of Korea)

  • Objectives: The pandemic of novel influenza A (H1N1) virus has required decision-makers to act in the face of the substantial uncertainties. In this study, we evaluated the potential impact of the pandemic response strategies in the Republic of Korea using a mathematical model. Methods: We developed a deterministic model of a pandemic (H1N1) 2009 in a structured population using the demographic data from the Korean population and the epidemiological feature of the pandemic (H1N1) 2009. To estimate the parameter values for the deterministic model, we used the available data from the previous studies on pandemic influenza. The pandemic response strategies of the Republic of Korea for novel influenza A (H1N1) virus such as school closure, mass vaccination (70% of population in 30 days), and a policy for anti-viral drug (treatment or prophylaxis) were applied to the deterministic model. Results: The effect of two-week school closure on the attack rate was low regardless of the timing of the intervention. The earlier vaccination showed the effect of greater delays in reaching the peak of outbreaks. When it was no vaccination, vaccination at initiation of outbreak, vaccination 90 days after the initiation of outbreak and vaccination at the epidemic peak point, the total number of clinical cases for 400 days were 20.8 million, 4.4 million, 4.7 million and 12.6 million, respectively. The pandemic response strategies of the Republic of Korea delayed the peak of outbreaks (about 40 days) and decreased the number of cumulative clinical cases (8 million). Conclusions: Rapid vaccination was the most important factor to control the spread of pandemic influenza, and the response strategies of the Republic of Korea were shown to delay the spread of pandemic influenza in this deterministic model.

Comparison of the Outcomes between Surgical Aortic Valve Replacement and Transcatheter Aortic Valve Replacement in Patients Aged above 80

  • Background: Transcatheter aortic valve replacement (TAVR) has been suggested as a less invasive treatment for high-risk patients with aortic valve disease. I n this study, we compared the outcomes of conventional surgical aortic valve replacement (AVR) and TAVR in elderly patients aged over 80. Methods: A total of 108 patients aged 80 years or older who underwent isolated AVR (n=35) or TAVR (n=73) from 2010 through 2015 at Asan Medical Center were identified. Early and late clinical outcomes, including echocardiographic findings, were evaluated in both groups. The mean follow-up duration was $766.4{\pm}528.7days$ in the AVR group and $755.2{\pm}546.6days$ in the TAVR group, and the average timing of the last follow-up echocardiography was at $492.6{\pm}512.5days$ in the AVR group and $515.7{\pm}526.8days$ in the TAVR group. Results: The overall early mortality was 2.8% (0 of 35, 0% in the AVR group vs. 3 of 73, 4.1% in the TAVR group). Permanent pacemaker insertion was significantly more common in the TAVR group (p=0.010). Renal failure requiring dialysis and new-onset atrial fibrillation was more frequent and the length of hospital stay was longer in the AVR group; however, this difference did not reach statistical significance. In the TAVR group, 14 patients (19.2%) were rehospitalized due to cardiac problems, and 13 patients (17.8%) had developed significant paravalvular leakage by the time of the last follow-up echocardiography. Conclusion: TAVR could be a good alternative to conventional surgical AVR in elderly patients. However, TAVR has several shortcomings, such as frequent significant paravalvular leakage or readmission, which should be considered in decision-making.

가구의 정보통신수용도가 DTV 구매에 미치는 영향 micro data를 이용한 실증연구 (Effects of IT-related Household Demand Characteristics on DTV Adoption: An Empirical Analysis Using Micro Data)

  • 본 연구는 국가 경제적으로 매우 중요한 의미를 지니는 기술혁신적인 제품인 디지털 TV의 보급에 영향을 미치는 요인들에 대해 정보통신수용도를 중심으로 실증적으로 분석하였다. 이를 위해 1,000개의 가구 설문조사를 토대로 구축된 미시 데이터(micro data)를 이용하여 가구의 DTV 구매선택에 어떤 요인들이 영향을 미쳤는지를 실증적으로 분석하였으며, 이로부터 디지털 TV보급관련 정책에 함의를 찾고자 하였다. 분석 결과, 다른 설명변수의 조건이 일정할 때 가구 세대주의 학력이 높은 가구가, 소득이 높은 가구가, 통신 및 방송, 인터넷 등 정보통신 여러 부문에 대한 수용도가 높은 가구가 각각 DTV의 구매의향이 높은 것으로 나타났다. 아울러 2010년에 아날로그 방송이 중단되며 디지털방송으로 전환이 이루어진다는 정책방향을 아는 가구가 이를 알지 못하는 가구에 비해 훨씬 구매의향이 높은 것으로 나타나, 정부의 디지털전환 정책 홍보가 디지털 TV의 보급에 매우 큰 영향을 미친다는 점을 시사하고 있다. 또한 TV를 구입한지 오래될수록 디지털 TV를 구매할 확률이 높은 것으로 나타나 TV의 대체수요에 대한 인센티브를 줄 경우 더 많은 가구들이 구매할 것임을 보여 주었다. 끝으로, 디지털위성방송 시청가구가 그렇지 않은 가구에 비해 디지털 TV구매 확률이 월등히 높은 것으로 나타나 DTV 확산을 위하여는 위성방송에 대한 정책과의 연계가 효과적이라는 점을 시사하고 있다.

다중 언폴딩 기법을 이용한 SHA-1 해쉬 알고리즘 고속 구현 (Implementation of High-Throughput SHA-1 Hash Algorithm using Multiple Unfolding Technique)

  • 본 논문은 다중 언폴딩 기법을 이용한 고속 SHA-1 해쉬 알고리즘 구현 방법을 제시하였다. 제안된 SHA-1 해쉬 구조는 알고리즘의 반복적인 단계 연산을 언폴딩한 후 연산의 순서를 재 배열하고, 임계경로에 포함된 연산의 일부를 이전 단계에서 선행연산하여 임계경로의 길이를 줄였다. 제안된 SHA-1 해쉬 구조는 최대 118 MHz의 동작주파수에서 5.9 Gbps 처리량을 나타낸다. 이는 기존의 SHA-1 보다 전송량이 26% 증가하였고, 회로 크기가 32% 감소하는 결과를 얻었다. 또한 이 논문에서는 여러 개의 SHA-1 모듈을 시스템 레벨에서 병렬로 연결하여 여러 개의 SHA-1을 다중 처리하여 고속화를 할 수 있는 모델을 제안했다. 이 모델은 하나의 SHA-1을 사용하는 것보다 빠르게 데이터를 처리할 수 있고 입력되는 데이터의 최소한의 지연으로 처리 가능하다. 제안된 모델은 입력되는 데이터가 지연 없이 처리 되도록 하기 위해 필요로 하는 SHA-1의 FPGA 수를 구할 수도 있다. 고속화된 SHA-1은 압축된 메시지에 유용하게 사용될 수 있고 모바일 통신이나 인터넷 서비스 등의 강한 보안에 널리 이용가능하다.

잣나무 임분의 개체목 거리독립생장모델을 이용한 간벌효과 분석모델 개발 (Development of Thinning Effect Analysis Model (TEAM) Using Individual-Tree Distance-Independent Growth Model of Pinus koraiensis Stands)

  • 본 연구의 목적은 거리독립생장모델을 이용한 잣나무 임분의 간벌효과 분석모델을 개발하는데 있다. 이 모델은 간벌시업계획의 주요 인자로 간벌의 횟수와 시기, 강도 그리고 간벌방법을 고려할 수 있도록 설계되었다. 개발된 모델의 적용성 검토를 위해 간벌시업계획에 따른 7개의 시나리오를 작성하여 임분생장 효과를 분석하였다. 연구결과, 개발된 모델을 이용하여 간벌시업 형태에 따른 개체목의 직경급별 본수, 수고, 재적과 임분의 평균흉고직경, 평균수고, ha당 본수 및 재적변화에 관한 생장을 예측할 수 있었다. 1개소의 잣나무 현실임분을 대상으로한 모델의 적용성 검토에 있어서, 간벌시나리오별 임분생장량을 비교한 결과, 간벌시업에 의한 임분밀도조절로 주벌시점의 재적량 증가효과를 기대할 수는 없는 것으로 나타났다. 그러나 간벌량과 주벌량을 포함하는 총수확량은 무간벌의 경우보다 간벌시업을 한 경우에 ha당 약 $40{\sim}75m^3$의 증가효과를, 그리고 임분의 평균흉고직경에서는 5 cm이내, 평균 수고에서는 1 m이내의 증가효과를 보였다. 본 연구를 통해 개발된 모델은 현실임분의 구성여건에 따라 간별의 실시 여부 및 간벌시업체계의 선택문제를 결정하는 의사결정지원도구로서 활용가능 할 것이다.

통행연계 변수를 중심으로 한 경기도 버스정류장 유형 구분 (Classification and Profiling of Bus Stops in Gyeong-gi Province on the Basis of Trip Chain Variables)

  • 본 연구는 합리적 버스 환승체계 구축을 위한 버스정류장의 환승센터로서의 유형 구분에 목적을 두고 있다. 이 분야 기존 연구는 주로 버스정류장 이용 수요의 특성과 버스정류장을 역세권으로 보았을 때 주변의 토지이용 특성을 확인하고, 이를 근거로 정류장의 유형을 구분한 후 구분된 유형들 각각의 특성을 요약하는 방식으로 진행되었다. 이러한 전통적 역세권 연구들의 공통적인 문제점은 대표적 연구 변수인 토지이용 특성과 정류장 이용 특성 등 횡단면적 snapshot의 특성만을 고려하여 환승의 본질인 통행 연계의 detail을 확보하지 못한다는 데 있다. 이에 본 연구에서는 환승통행 발생 정류장의 기능 구분을 위해 통행연계 변수로서 교통수단연계, 경유지연계, 이용시간연계 등의 변수 값이 각 정류장이 환승센터로서의 특성을 고려할 때 어느 정도와 방향인지를 확인토록 한다. 이를 위해 2009년 4월 20일 월요일 하루 동안 경기도 버스에 승하차한 통행 샘플을 분석하였다. 분석 결과 버스정류장은 연계성 관련 변수를 기준으로 비교적 뚜렷한 구분이 가능하였다. 이러한 결과는 환승센터 입지 선정이나 설계에 유용한 정보를 줄 것으로 기대된다.
