자세조절 증진이 편마비 환자의 보행과 균형에 미치는 영향 -증례 보고- (Effects on Walking & Balance upon Improvement of Postural Control Therapy for Hemiplegic Patient)

  • Purpose : The neurologically impaired patients have sensory, motor, cognition, perception problems which cause reduction of body schema, balance control and postural control provoke disfunction. The purpose of this case report was to evaluate effects on gait&balance upon improvement of postural control for left hemiplegia. Methods : This study has performed single subject design from March to April 2013 for 6 weeks. The subject of this study was a 67years old female patient with left hemiplegia. Timed Up and Go (TUG) test and Functional Reach (FR) test were used as evaluation tool. The subject was treated 5 times a week for 30 minutes each. Results : The result is that the walking velocity was decreased, the ability of gait was improved than before the training. The length of FR was increased from 8.33cm to 22.67cm. Conclusion : According to the results, the treatment improves subject's ability of walking in this study.

Clinical Feasibility of Mental Practice for Gait Ability Improvement of Parkinson Disease Patients: ABA Single Subject Design

  • Purpose: This study was carried out in order to verify actual applicability of mental practice in the clinical field by applying it to Parkinson disease patients. Methods: : The number of subjects was three and they were aged between 65 and 69. One year or longer period has passed since they were diagnosed with Parkinson disease. The baseline A consisted of three sessions. After the baseline period, the researcher applied mental practice to the subjects once per day, for 20 minutes per each time, for 10 sessions. The no-intervention withdrawal phase was composed of three sessions. Outcome measures 10 meter walking test(10MWT), timed up and go (TUG) test, 8-figure walk test (8FWT). Results: All results indicated that 10 MWT, TUG, F8WT was improved when compared mental practice(B) to pre base line(A). base line(A) after mental practice intervention(B) was also sustained. Conclusion: As a result of the mental practice, we can know that it helps straight gait, dynamic balance, curved gait to improve.

The Effect of Lower Extremity Strengthening Exercise Using Sliding Stander on Balance and Spasticity in Chronic Stroke: A Randomized Clinical Trial

  • Purpose: Generally, patients with stroke present with decreased balance and increased spasticity following weakness of the paralyzed muscles. Muscle weakness caused by stroke has two causes. This is caused by a decrease in motor output and an adaptive muscle change, resulting in muscle weakness and muscle paralysis. The purpose of this study was to investigate the effect of strengthening exercise on balance and spasticity in chronic stroke patients and to suggest the basis of clinical treatment. Methods: Twenty subjects were divided into two groups: a lower-extremity strengthening group (experimental group) and a general physical therapy group (control group). The sliding stander equipment was used for the experimental group and a regimen of warm-up exercise, the main exercise routine, and cool-down exercise were used for the muscle strengthening exercise program. Balance and spasticity were measured before and after the training period. Balance ability was measured by the Berg balance scale, the Timed up and Go test and the weight distribution of the paralyzed muscles by the Spacebalance 3D. Spasticity was measured by the Biodex system. Results: After the training periods, the experimental group showed a significant improvement in BBS, weight distribution of the paralyzed muscles, and decreased spasticity when compared to the control group (p<0.05). Conclusion: This study supported the hypothesis that lower-extremity strengthening exercise improves the balance and decreases the spasticity of stroke patients. If it is combined with conventional neurologic physiotherapy, it would be effective rehabilitation for stroke patients.

Physiological Approach on the Physical Fitness and Postural Balance Effects of a Whole-Body Vertical Vibration Intervention in Young Women

  • With the advent of westernized diet and a lack of exercise, young female college students are paying more attention to their bodyweight and health. Whole-body vibration has been demonstrated to be a suitable training method for improving knee extension maximal strength in young female athletes, as well as the gait performance in elderly women. This study aimed to evaluate the effects of a vertical vibration intervention on the physical fitness and postural balance in young females. Fifty-four young women were recruited; all subjects were randomly assigned to the intervention group and control group. The intervention group underwent vertical vibration with a platform for 12 weeks. The results showed that body mass index and body fat percentage had decreased (P<0.05). In addition, their muscle endurance as indicated by a sit-up test and their flexibility as indicated by a sit-and-reach test were both increased. With regard to postural balance, their 30-second sit-to-stand and timed up and go test results were improved. At the same time, their mean single-leg stance with eyes closed time increased (P<0.05). However, there were no significant differences, meanwhile, for the control group. Overall, the results showed that the whole body vibration (WBV) intervention had some beneficial effects on physical fitness and postural balance in young women.

비 마비 측과 마비 측에 적용한 탄력밴드를 결합한 고유수용성 신경근 촉진법 하지패턴이 만성 뇌졸중 환자의 균형에 미치는 영향 (Effect of Non-paralyzed side and Paralyzed side of Elastic Band Combined with Proprioceptive Neuromuscular Facilitation Lower Extremity Patterns on Balance in Chronic Stroke Patients)

  • Purpose: This study aimed to examine the effects of proprioceptive neuromuscular facilitation (PNF) lower extremity patterns combined with elastic bands applied to stroke patients diagnosed with hemiplegia through self-training using the non-paralyzed side approach and the paralyzed side approach, and to investigate the differences in the effects. Methods: Nine chronic stroke patients who were being treated not more than twice a week at H, K, R, and C hospitals located in Gangwon-do, performed self-training for 16 minutes, two times per day for four weeks between August and October 2015. The subjects' balance ability was measured using the Tetrax stability index, the weight distribution index, the Berg Balance Scale (BBS), and the Timed Up and Go (TUG) test before the experiment and four weeks later. Among the statistical methods, paired t-tests were conducted for intra-group comparison of the measurements taken before and after the experiment, and independent t-tests were conducted for inter-group comparison of the ex post facto values. The statistical significance level was set to 0.05. Results: When the lower extremity patterns were applied to the non-paralyzed side group and the paralyzed side group, significant intra-group differences were observed for the Tetrax stability index, the weight distribution index, the Berg Balance Scale (BBS), and Timed Up and Go (TUG) tests (p<0.05); however, the inter-group comparisons showed no significant differences. Conclusion: The non-paralyzed side approach was found to be easy for patients to participate in and it also affected the patients' paralyzed side. Although the paralyzed side approach produced good exercise effects in a short period of time, it could result in adverse effects, such as a decrease in motivation and self-confidence. Therefore, these approaches are considered to be more effective when they are selectively applied depending on the purpose of the intervention and the degree of a patients' participation.

Effects of Gait Training Using a Shoulder-Back Orthosis on Balance and Gait in Patients with Stroke

  • Purpose: The purpose of this study is to find out how gait training with shoulder-back assistive device affects dynamic and static balance, gait of patients with stroke and to help improve body alignment, balance, and gait ability in stroke patients. Methods: Measurements were taken of the 20 subjects before intervention without shoulder-back assistive device, after intervention with device, and follow up after an hour compared. Berg balance scale used to evaluate dynamic balance; wii balance board was used to measure static balance; and gait ability were measured by timed up and go test and 10-meter walk test. To analyze the results, a one-way repeated measures analysis of variance was implemented to compare the measurements. Results: The results showed that, after wearing the shoulder-back assistive device, the subjects' dynamic balance statistically significantly improved; no statistically significant difference was observed in static balance, although their balance ability was enhanced; and their increase in gait ability was statistically significant. Conclusion: This study proved that gait training combined with a shoulder-back assistive device positively impacted dynamic and static balance, gait of patients with stroke.

순환운동과 전통적 운동이 만성 뇌졸중환자의 보행능력에 미치는 효과 (The Effect of the Circuit Exercise and Conventional Exercise on Walking Ability in Chronic Stroke)

  • Purpose : This study achieved to search the effect of the circuit exercise and conventional exercise on walking ability(walking speed, endurance, dynamic balance, speed, endurance and pedestrian crossing) in chronic stroke. Methods : Since is diagnosed by stroke, to 30 chronic stroke patients who more than 1 year past the 15 circuit exercise group, the 15 conventional exercise group random the circuit exercise group applied circuit exercise 3th 8 weeks each week after neurological treatment because assigning and the conventional exercise group executed round trip walk exercise in parallel bar 3th 8 weeks each week after neurological treatment. The data of 25 patients who complete experimental course were statistically analysed. Results : The results of this dissertation were as following : 1) There were significantly increased after experimental of 10 meter walk test, 6 minutes walk test and Timed "Up and Go" test in circuit exercise group (p<.001). 2) There were significantly increased after experimental of 2, 4 and 6 lane road crossing mobility in Walking circuit exercise group(p<.01). 3) There were significantly differences after experimental of 10 meter walk test, 6 minutes walk test and Timed "Up and Go" test change quantity between circuit exercise group and conventional exercise group(p<.05). 4) There were correlations were found between the TUG test and 2, 4 and 6 lane road (2 lane road; r=.463, p<.01., 4 lane road; r=515, p<.01., 6lane road; r=.710, p<.01), and there were correlations were found between the 10 meter walk test and 6 minutes walk test(r=.595, p<.01), TUG test(r=.662, p<.01) and 6 lane road(r=.527, p<.01). Conclusion : Even if improvement of walk function through training consists in room, transfer of actuality pedestrian crossing is no change outside the room. Because it is much variable of the weather, seasonal factor, temperature, pedestrian number, state of underneath etc. outside the room. Then, in room after direction promotion of walk function to be promotion of walk function in actuality life and need development of connectable training method consider.

체성감각계 지원이 여성노인의 보행 및 평형감각 유지에 미치는 효과 (Effect of somatosensory input on the gait ability and equilibrium sensory of elderly women)

  • 평형능력은 보행에 영향을 미치고, 보행속도는 노인의 건강한 척도를 말해주는 기준이 된다. 이 연구의 목적은 체성감각계 지원이 여성 노인의 보행 및 평형감각에 미치는 영향을 보고자 하였다. 평소 지팡이를 사용하지 않고, 규칙적으로 운동을 하지 않으며, 이비인후과적으로 이상이 없는 노인으로 평균 연령이 75세 여성 노인 61명을 실험군 31명과 통제군 30명으로 분류하였다. 종속변인으로는 2.44m왕복걷기, 10m 일반걷기 및 빠르게 걷기, 6분 걷기를 실시하였다. 평형감각 검사로는 뉴로컴사의 EquiTest를 이용하여 분석하였다. 연구 결과 2.44m 왕복걷기와 10m 일반걷기에서는 유의한 차이가 없었으나 10m 빠르게 걷기, 6분걷기에서는 유의한 효과를 보이는 것으로 나타났다. 평형감각에서는 조건 2, 3, 4, 6에서 유의한 효과를 보이는 것으로 나타났다. 결론적으로 체성감각계 지원이 노인 여성들의 보행 및 평형유지 능력에 유의한 효과를 보이는 것으로 나타나 지팡이의 적극적 사용이 권장된다 할 수 있겠다.

치매노인을 위한 운동프로그램이 보행능력에 미치는 영향 (The Effects of Exercise Programs on Gait Ability of Elderly Dementia Patients)

  • The purpose of this study is to investigate the effects of the practice exercising and cycling on the gait abilities of dementia elderly patients. Forty forty elderly patients with dementia were participants. Participants were divided into three groups as A, B, and C, Group A was a standard group which had been treated with electric treatment, hot pack, and exercise. The second group B had been treated with cycling added to the treatments of group A. Finally, treatments of group C were arranged by subtraction of exercise from those of group A. The test was a 'timed up and go test' used for measuring the rates of the gait ability through four months. The gait ability of each group was measured after each month. The results of this study are as following: 1. After first month, the averages of changes in gait ability of group C, A and B were -1.69, -1.67, and -1.13 seconds respectively. That means that dementia control was achieved significantly in group A and B (p<0.05)whereas it was not in group C. 2. The averages after two months were -4.00 seconds for group C, -2.60 seconds for group A, and -1.56 seconds for B, respectively. All groups acquired significant effects in treating dementia (p<0.05). 3. After three months, -6.38, -3.00 and -2.31 seconds were average values of group C, A and B respectively. Same as results after two months, there were significant effects in all groups (p<0.05). 4. After four months, the averages of changes in gait ability of three groups were -8.00. -3.93 and -3.00 seconds for group C, A and B respectively. In the all patients treated analysis, treatments showed significant results (p<0.05). 5. Compared with each other through four months. three groups testing the timed up and go test showed more efficient in maintaining gait ability, in order of group B. A, and C

뇌졸중 및 노인 환자의 퇴원 평가도구에 대한 체계적 고찰 (Discharge Assessment Tool in Stroke and Elderly Patients: A Systematic Review)

  • 목적 : 본 연구는 뇌졸중 환자와 노인 환자를 대상으로 퇴원 시기에 이루어지는 평가도구에 대한 체계적 문헌고찰을 통해 평가 항목과 평가도구들을 분석하여 효율적인 퇴원계획을 위한 기초자료를 제공하고자 한다. 연구방법 : 문헌 선정은 PubMed, Medline Complete(EBSCOhost), Scopus를 사용하여 2009년 1월부터 2018년 12월까지의 온라인 데이터에 등록된 연구를 대상으로 최종 22편의 연구를 선정하였고, 연구 설계 및 퇴원 시 주요 평가영역, 하위영역, 평가도구들을 분석하였다. 결과 : 선택된 연구들의 연구 설계 형태는 기술적 연구가 13편(59.2%)으로 가장 많았다. 퇴원 시 평가 영역은 의학적 상태, 일상생활활동, 삼킴 기능, 감각, 이동성, 인지·지각, 의사소통, 감정상태, 가정환경, 환자의 지식과 퇴원을 위한 준비, 사회적지지, 안녕감 등 12개 영역으로 나타났다. 평가 영역에 따라 사용된 평가도구를 분석한 결과, 가장 높은 빈도의 사용을 보이는 평가도구는 일상생활활동을 평가하는 Barthel Index, Functional Independence Measure, 안녕감을 평가하는 Short Form Health Survey, 이동성을 평가하는 Timed Up and Go test 등으로 나타났다. 결론 : 본 연구를 통해 뇌졸중, 노인 환자의 퇴원 시 이루어지는 다양한 평가 영역과 평가도구의 사용과 제한적이나 여러 영역을 포괄하는 평가도구들을 확인할 수 있었다. 이는 뇌졸중 환자의 퇴원 시 고려되는 평가 영역과 효과적인 평가도구의 선택을 돕고 적절한 퇴원계획 중재와 제안을 위한 포괄적인 평가도구의 개발의 기초자료가 될 것이다.
