Optimal Location Problem for Constrained Number of Emergency Medical Service (한정된 응급시설의 최적위치 문제)

  • This paper proposes an EMS algorithm designed to determine the optimal locations for Emergency Medical Service centers that both satisfy the maximum ambulance response time T in case of emergency and cover the largest possible number of residents given a limited number of emergency medical services p in a city divided into different zones. This methodology generally applies integer programming whereby cases are categorized into 1 if the distance between two zones is within the response time and 0 if not and subsequently employs linear programming to obtain the optimal solution. In this paper, where p=1, the algorithm determines a node with maximum coverage. In cases where $p{\geq}2$, the algorithm selects top 5 nodes with maximum coverage. Based on inclusion-exclusion method, this selection entails repeatedly selecting a node with the maximum coverage when nodes with lower numbers are deleted. Among these 5 selected nodes, the algorithm selects a single node set with the greatest coverage and thereby as the optimal EMS location. The proposed algorithm has proven to accurately and expeditiously obtain the optimal solutions for 12-node network, 21-node network, and Swain's 55-node network.

  • Air transport today is a means of transport that is optimized for exchanges between nations. Around the world, has experienced an increase in operating and the number of airline route expansion that has entered into the international aviation agreements in order to take advantage of the air transport efficient, but the possibility of the occurrence of air transport accidents increased. When compared to the accident of other means of transport, development of air transport accidents, not high, but it leads to catastrophe aviation accident occurs. Air Transport accident many international transportation accident than domestic transportation accident, in the event of an accident, the analysis of the legal responsibility of the shipper or the like is necessary or passenger air carrier. Judgment of the legal order of discipline of air transport accident is a classification of the type of air transport agreement. Depending on the object, air transport agreements are classified into the contract of carriage of aviation of the air passenger transportation contract. For casualties occurs, air passenger transportation accident is a need more discussion of legal discipline for this particular. Korean Commercial Code, it is possible to reflect in accordance with the actual situation of South Korea the contents of the treaty, which is utilized worldwide in international air transport, even on the system, to control land, sea, air transport and welcoming to international standards. However, Korean Commercial Code, the problem of the Montreal Convention has occurred as it is primarily reflecting the Montreal Convention. As a cause of liability for damages, under the Commercial Code of Korea and the contents of the treaty precedent is reflected, the concept of accident is necessary definition of the exact concept for damages of passengers in particular.   • Recently data centric routing or application dependent routing protocols are emerged in mobile ad hoc networks. In this paper, we propose a routing method for query processing in MANET(Mobile Ad hoc NETwork) environment, called road-based query routing, with consideration on real time traffic information of large number of vehicles. In particular, we focus on the method that process arrival time dependent shortest path query in MANET without a central server on the road networks. The main idea of our approach lies in a routing message that includes query predicates based on the road connectivity and on data gathering method in real time from vehicles on the road by ad-hoc network. We unify route discovery phase and data delivery(query processing) phase in our mechanism and reduce unnecessary flooding messages by pruning mobile nodes which are not on the same or neighboring road segments. In order to evaluate the performances of the proposed method, we established a model of road networks and mobile nodes which travel along the roads. The measurement factor is the number of nodes to whom route request is propagated according to each pruning strategy. Simulation result shows that road information is a dominant factor to reduce the number of messages.

  • Construction sites are becoming larger and complex with the growth of national economy. Accordingly, it is important to identify real-time information about materials, equipments, and manpower during construction at sites. Even though research utilizing position tracking sensors has been conducted in architectural engineering fields, this area of research is almost nil in civil engineering fields. Therefore, a feasibility study to find a way to apply position tracking sensors to an in-situ tunnel construction site adopting conventional tunnelling method is performed in this study. A methodology is proposed that the progress management of the tunnelling work can be monitored by checking construction materials needed at job site and the safety management system can be assessed by checking distance between in-situ workers and construction equipments. The most representative materials were identified so that IT technology can be applied by attaching and monitoring sensors to the selected materials. Also, time of arrival (TOA) for a position determination technology along with a wireless network technology was chosen and build wireless network system. The adopted methodology was applied to an in-situ tunnelling site, and verified the usefulness of the proposed system.

  • When Catastrophic extreme flood occurs due to dam break, the response time for flood warning is much shorter than for natural floods. Numerical models can be powerful tools to predict behaviors in flood wave propagation and to provide the information about the flooded area, wave front arrival time and water depth and so on. But flood wave propagation due to dam break can be a process of difficult mathematical characterization since the flood wave includes discontinuous flow and dry bed propagation. Nevertheless, a lot of numerical models using finite volume method have been recently developed to simulate flood inundation due to dam break. As Finite volume methods are based on the integral form of the conservation equations, finite volume model can easily capture discontinuous flows and shock wave. In this study the numerical model using Riemann approximate solvers and finite volume method applied to the conservative form for two-dimensional shallow water equation was developed. The MUSCL scheme with surface gradient method for reconstruction of conservation variables in continuity and momentum equations is used in the predictor-corrector procedure and the scheme is second order accurate both in space and time. The developed finite volume model is applied to 2D partial dam break flows and dam break flows with triangular bump and validated by comparing numerical solution with laboratory measurements data and other researcher's data.

  • ' Healthy Japan 21 ' is a new health policy that has been proposed for the 21st century: it embodies a totally new concept for its viewpoints and methods. To start with, for its goal, the focus is placed on the ' quality of life ' or a life that is tree of diseases, rather than mere prolongation of life. For its doctrine, the emphasis has shifted dramatically from improving the health of the entire population (the traditional approach for health improvement) to ' achieving an ideal health status for each individual. The ultimate aged society that arrives first in Japan is a society in its ultimate form for human being. Why did Japan become westernized, giving up her traditional culture? Why did she go through industrialization, sacrificing her nature? And why does she try so hard to industrialize the developing countries? These efforts are all preparation for the arrival of a ultimate aged society. During the 20th century, we believed in unlimited possibilities and expanded our social frontier. 1n the 21st century, on the other hand, a super-aged society (the ultimate society), a glimpse of which we have witnessed from time to time, will descend on us sooner or later. It is expected to arrive first in Japan. ' Healthy Japan 21 ' is intended to prepare for the arrival of the hitherto unheard of super-aged society by building the physiological basis of people. This policy is social experimentation on an immense social scale, in which questions are posed on the understanding of health, the relationship between individuals and society, the relationship between administration and citizens, the manner by which central and local governments operate, and the new relationship between prevention and therapy. ' Healthy Japan 21 ' may be summarized as an experiment on a huge scale directed to the ultimate form of human society, in which Japan and each of her citizens playa role and set an example for the rest of the world. Even just by considering various approaches newly suggested for this venture, one may be convinced that it is a policy with features suitable for a country that has already achieved the world's highest longevity.

  • 임진강 수계는 북측 지역이 다수를 차지하는 유역 특성으로 예고 없는 상류 급방류, 강우 등으로 인해 댐 운영에 근본적 어려움이 있으며, 이에 따라 홍수조절지 및 댐 하류 계측 가능 지역의 취득 자료를 고려한 하천 수위 변화에 대한 사전 예측을 필요로 하고 있다. 홍수기 하천 도달시간 및 수위예측 기법으로는 물리 기반 및 데이터 기반 모델들이 다양하게 연구되어 왔으며, 일부 연구성과들은 현업에 활용하고 있다. 물리기반 모델은 하천 지형 변화에 대한 자료 취득 및 분석에 많은 시간을 요하는 단점은 있으나, 설명 가능한 모델을 구현할 수 있을 것으로 사료 된다. 반면, 데이터 기반 인공지능 모델은 짧은 시간 및 비용으로 모델을 개발할 수 있으나, 복잡한 알고리즘구현 시 설명이 불가하여 일관성을 의심 받을 수 있다. 본 논문에서는 홍수 도달시간과 하류 수위 상승에 대하여 설명 가능한 인공지능 알고리즘 및 시뮬레이션 프로그램을 개발하고자 하였다. 홍수 도달시간 예측은 기존 조견표 방식에서 고려하지 않았던 홍수파의 영향을 추가 변수화 하고, 데이터의 전후처리를 통하여 도달시간을 예측하였다. 실시간 하류 수위 예측은 댐 방류량, 주변 강우, 조위 등을 고려하여 도달시간 후 수위를 예측할 수 있도록 구현하였으며, 자료 동화 기술을 일부 적용하였다. 미래 방류조건에 대한 시뮬레이션을 위해서는 미래 방류량, 예상 강우 입력 시 하천 지점별 수위 상승을 예측할 수 있도록 알고리즘 및 프로그램을 개발하였다. 이를 구현하기 위하여 다양한 인공지능 알고리즘을 이용한 학습, 유전자 알고리즘을 이용한 가중치 학습 제한 조건내 최적화, 수위파와 조위파의 중첩의 정리 등을 이용하여 예측 정확도 및 신뢰성을 제고 하였다. 인공지능 분석결과의 현업활용성 제고를 위하여 시뮬레이터 프로그램을 개발하여 현업에 적용하였다.

The Variation of Scan Time According to Patient's Breast Size and Body Mass Index in Breast Sentinel lymphangiography (유방암의 감시림프절 검사에서 유방크기와 체질량지수에 따른 검사시간 변화)

  • Purpose : At this time, the sentinel lymph node mapping using radioisotope and blue dye is preceded for breast cancer patient's sentinel lymph node biopsy. But all patients were applied the same protocol without consideration of physical specific character like the breast sizes and body mass indexes. The purpose of this study is search the optimized scan time in breast sentinel lymphangiography by observing how much the body mass index and breast size influence speed of lymphatic flow. Materials and Methods : The Object of this study was 100 breast cancer patients(Female, 100 persons, average age $50.34{\pm}10.26$ years old)at Severance hospital from October 2011 to December 2011. They were scanned breast sentinel lymphangiography before operation. This study was performed on Forte dual heads gamma camera (Philips Medical Systems, Nederland B.V.). All patients were intra-dermal injected $^{99m}Tc$-Phytate 18.5 MBq, 0.5 ml. For 80 patients, we have scanned without limitation of scan time until the lymphatic flow from the lymph node since injection. We measured how long the lymphatic flow time between departures from injects site and arrival to lymph node using stopwatch. After we calculated patient's Body mass Index and classified as 4 groups. And we measured patient's breast size and classified 3 groups. The modified breast lymphangiography that changing scan time according to comparison study's result was performed on 20 patients and was estimated. Results : The mean scan time as breast size was A group 2.48 minutes, B group 7.69 minutes, C group 10.43 minutes. The mean scan time as body mass index was under weight 1.35 minutes, normal weight 2.56 minutes, slightly over 5.62 minutes, over weighted 5.62 minutes. The success rate of modified breast lymphangiography was 85%. Conclusion : As the Body mass index became higher and breast size became bigger, the total scan time is increased. Based on the obtained information, we designed modified breast lymphangiography protocol. At the cases applying that protocol, most of sentinel lymph nodes were visualized as lymphatic pool. In conclusion, we found that the more success rate in modified protocol considering physical individuality than study carrying out in the same protocol.

The Clinical Investigation of Gastric Lavage in Patients with Acute Poisoning (급성 약물 중독 환자에서 위세척의 치료 실태)

  • The majority of acute toxic poisoning occur via oral route. The most important emergency treatment of acute poisoning are gastric lavage. Gastric lavage should be considered a patient has ingested a potentially life-threatening amount of a poison and the procedure can be undertaken within 60 mins of ingestion. But, gastric lavage does not consist properly in the cases of emergency situation or an inexperienced doctors treat. The purpose of this study was to determine whether gastric lavage is performed properly. Eighty patients were enrolled in the study in 12-month period from January to December 2002. A retrospective chart review was performed on patients identified as drug overdose who admitted to ER. To assess whether there was a subgroup of patients who may have been candidates for the initiation of gastric lavage in the ER, the patients divided in two groups by time interval from toxin ingestion to ER arrival. The group 1 that admit within 60 minutes after drug ingestion was 38 cases ($47.5\%$), and the group 2 patient who admitted after 60 minutes was 42 cases ($52.5\%$). The average age was $44\pm19$ years in group 1, and $48\pm24$ years in group 2. There were no differences in sexual distribution of two groups. The mean time interval was $49\pm20$ minutes in the group $1,258\pm190$ minutes in the group 2. Only thirty ($37.5\%$) of the patients had an overdose for which the treatment of gastric lavage was potentially feasible according to guideline. The correctly performed gastric lavage was 18 ($47.4\%$) in group 1, 12 ($28.6\%$) in group 2. We must enforce education about the gastric lavage, and do so that may treat according to guideline.

Incidence and Features of Cognitive Dysfunction Identified by Using Mini-mental State Examination at the Emergency Department among Carbon Monoxide-poisoned Patients with an Alert Mental Status (의식이 명료한 일산화탄소 중독환자를 대상으로 응급실에서 시행한 간이정신상태검사의 임상적 의의)

  • Purpose: Because carbon monoxide (CO)-intoxicated patients with an alert mental status and only mild cognitive dysfunction may be inadequately assessed by traditional bedside neurologic examination in the emergency department (ED), they may not receive appropriate treatment. Methods: We retrospectively investigated the incidence and features of cognitive dysfunction using the Korean version of the Mini-Mental State Examination (MMSE-K) in ED patients with CO poisoning with alert mental status. We conducted a retrospective review of 43 consecutive mild CO poisoned patients with a Glasgow Coma Scale score of 15 based on documentation by the treating emergency physician in the ED between July 2014 and August 2015. Results: Cognitive dysfunction, defined as a score of less than 24 in the MMSE-K, was diagnosed in six patients (14%) in the ED. In the MMSE-K, orientation to time, memory recall, and concentration/calculation showed greater impairments. The mean age was significantly older in the cognitive dysfunction group than the non-cognitive dysfunction group (45.3 yrs vs. 66.5 yrs, p<0.001). Among the initial symptoms, experience of a transient change in mental status before ED arrival was significantly more common in the cognitive dysfunction group (32.4% vs. 100%, p=0.003). Conclusion: Patients with CO poisoning and an alert mental status may experience cognitive dysfunction as assessed using the MMSE-K during the early stages of evaluation in the ED. In the MMSE-K, orientation to time, memory recall, and concentration/calculation showed the greatest impairment.
