Diagnosis of Coxofemoral Joint Luxation in a Whooper Swan (Cygnus Cygnus) Using Computed Tomography and Radiography

  • A wild Whooper swan (Cygnus Cygnus) with limping due to an injured left pelvic limb in an accident was rescued on the seashore and transferred to the Jeju Wildlife Rescue Center on November 23rd, 2020. On physical examination, its body condition score was 1 out of 5 due to starvation and dehydration. The left coxofemoral joint was also examined by careful palpating and estimating the damage. Moderated soft tissue swelling and crepitus surrounding the hip joint were confirmed. Radiography and computed tomography (CT) were used together for an accurate diagnosis of the joint. By radiographs readings, it was difficult to accurately confirm the condition of the proximal femur due to superimposition of the synsacrum and internal organs. However, signs such as avulsion fracture of the femoral head and a few fragments around the joint were revealed by CT imaging. Besides, through three-dimensional (3D) image analysis of CT, the dislocated area and condition of the left hip joint could be accurately and easily confirmed. The diagnostic process showing in this paper could be used as a good reference for diagnosing coxofemoral joint luxation in wild swan.

A comparative study of the deviation of the menton on posteroanterior cephalograms and three-dimensional computed tomography

  • Purpose: Facial asymmetry has been measured by the severity of deviation of the menton (Me) on posteroanterior (PA) cephalograms and three-dimensional (3D) computed tomography (CT). This study aimed to compare PA cephalograms and 3D CT regarding the severity of Me deviation and the direction of the Me. Materials and Methods: PA cephalograms and 3D CT images of 35 patients who underwent orthognathic surgery (19 males and 16 females, with an average age of $22.1{\pm}3.3years$) were retrospectively reviewed in this study. By measuring the distance and direction of the Me from the midfacial reference line and the midsagittal plane in the cephalograms and 3D CT, respectively, the x-coordinates ($x_1$ and $x_2$) of the Me were obtained in each image. The difference between the x-coordinates was calculated and statistical analysis was performed to compare the severity of Me deviation and the direction of the Me in the two imaging modalities. Results: A statistically significant difference in the severity of Me deviation was found between the two imaging modalities (${\Delta}x=2.45{\pm}2.03mm$, p<0.05) using the one-sample t-test. Statistically significant agreement was observed in the presence of deviation (k=0.64, p<0.05) and in the severity of Me deviation (k=0.27, p<0.05). A difference in the direction of the Me was detected in three patients (8.6%). The severity of the Me deviation was found to vary according to the imaging modality in 16 patients (45.7%). Conclusion: The measurement of Me deviation may be different between PA cephalograms and 3D CT in some patients.

Novel Resectable Myocardial Model Using Hybrid Three-Dimensional Printing and Silicone Molding for Mock Myectomy for Apical Hypertrophic Cardiomyopathy

  • Objective: We implemented a novel resectable myocardial model for mock myectomy using a hybrid method of three-dimensional (3D) printing and silicone molding for patients with apical hypertrophic cardiomyopathy (ApHCM). Materials and Methods: From January 2019 through May 2020, 3D models from three patients with ApHCM were generated using the end-diastolic cardiac CT phase image. After computer-aided designing of measures to prevent structural deformation during silicone injection into molding, 3D printing was performed to reproduce anatomic details and molds for the left ventricular (LV) myocardial mass. We compared the myocardial thickness of each cardiac segment and the LV myocardial mass and cavity volumes between the myocardial model images and cardiac CT images. The surgeon performed mock surgery, and we compared the volume and weight of the resected silicone and myocardium. Results: During the mock surgery, the surgeon could determine an ideal site for the incision and the optimal extent of myocardial resection. The mean differences in the measured myocardial thickness of the model (0.3, 1.0, 6.9, and 7.3 mm in the basal, midventricular, apical segments, and apex, respectively) and volume of the LV myocardial mass and chamber (36.9 mL and 14.8 mL, 2.9 mL and -9.4 mL, and 6.0 mL and -3.0 mL in basal, mid-ventricular and apical segments, respectively) were consistent with cardiac CT. The volume and weight of the resected silicone were similar to those of the resected myocardium (6 mL [6.2 g] of silicone and 5 mL [5.3 g] of the myocardium in patient 2; 12 mL [12.5 g] of silicone and 11.2 mL [11.8 g] of the myocardium in patient 3). Conclusion: Our 3D model created using hybrid 3D printing and silicone molding may be useful for determining the extent of surgery and planning surgery guided by a rehearsal platform for ApHCM.

3차원 X-ray CT 영상을 이용한 화성암 불균질 지수와 역학적 특성과의 상관관계에 대한 연구 (A Study on Correlation between Heterogeneity Index and Mechanical Properties of Igneous Rocks using 3D X-ray Computed Tomography Image)

  • 본 연구에서는 국내의 다양한 화성암에 대해 내부구조의 불균질성을 정량화하고, 이와 탄성파속도 및 점하중강도간의 상관관계를 분석하였다. 3차원 X-ray Computed Tomography(CT)를 통해 암석 시편 내부 구조에 대한 정보를 획득하였으며, 3차원 영상에 통계적 기법을 적용하여 뷸균질성 대표계수(representative unit length, LR)를 계산하였다. 또한 암석의 탄성파 속도 및 점하중강도와 LR간의 상관관계로부터 암석의 역학적 특성치를 예측하는 추정식을 제안하였다. 본 연구에서 제안한 방법을 통해 3차원 X-ray 영상에 기반한 내부 특성 분석값을 이용해 실내실험을 수행하지 않고도 암석의 역학적 물성을 평가할 수 있는 간접적인 인자를 도출할 수 있는 가능성을 확인하였다.

3차원 컴퓨터단층촬영상을 이용한 정상 성인의 경추 신경공 면적 비교 (A Comparison for Cervical Neural Foraminal Area by 3-dimensional CT in Normal Adults)

  • Cervical foraminal stenosis is a disease in which the nerves that pass from the spinal canal to the limbs are narrowed and the nerves are compressed or damaged. Due to the lack of an imaging method that provides quantitatively stenosis, this study attempted to evaluate the area of the cervical vertebrae by reconstructing a three-dimensional computed tomography image, and to determine the area of the neural foramen in normal adults to calculate the stenosis rate. Using a three-dimensional image processing program, the surrounding bones including the posterior spinous process, lateral process, and lamellar bones of the cervical vertebra were removed so that the neural foramen could be observed well. A region of interest including the neural foraminal area of the three-dimensional image was set using ImageJ, and the number of pixels in the neural foraminal area was measured. The neural foraminal area was calculated by multiplying the number of measured pixels by the pixel size. To measure the largest neural foraminal area, it was measured between 40~50 degrees in the opposite direction and 15~20 degrees toward the head. The average area of the right C2-3 foramen was 44.32 mm2, C3-4 area was 34.69 mm2, C4-5 area was 36.41 mm2, C5-6 area was 35.22 mm2, C6-7 area was 36.03 mm2. The average area of the left C2-3 foramen was 42.71 mm2, C3-4 area was 32.23 mm2, C5-6 area was 34.56 mm2, and C6-7 area was 31.89 mm2. By creating a reference table based on the neural foramen area of normal adults, the stenosis rate of patients with neural foraminal stenosis could be quantitatively calculated. It is expected that this method can be used as basic data for the diagnosis of cervical vertebral foraminal stenosis.

  • Occlusal loading is considered as the main factor of noncarious cervical lesions. The goal of this study is to identify stress distribution using three dimensional finite element analysis, when occlusal loading is applied on the cervical lesion of human tooth. A finite element model was constructed from micro-CT image and three kinds of static force(500 N) were assumed. In all cases stress concentrates on the same area in the cement-enamel junction. This finding is consistent with published experimental results.

Diagnosis of split fractures of the mandible in adults

  • Background: Mandibular split fractures, in which the fracture occurs exclusively in the posterior wall, are uncommon. This study aimed to enhance clinicians' understanding of mandibular split fractures and offer insights for future research. Methods: This study included six patients who visited our hospital between January 2020 and June 2023 and were diagnosed with mandibular split fractures. We retrospectively collected data from patients' medical records on their age, sex, symptoms, mechanism, impact site, associated injuries, and treatment method, as well as the location, pattern, and number of fractures observed on computed tomography (CT) and panoramic images. The frequency of split fractures among all mandibular fractures was calculated. Results: The six patients included three men (50%) and three women (50%), ranging in age from 20 to 71 years (mean age, 49.8 years). The split fractures were located in the symphysis in one patient (16.7%), symphysis to parasymphysis in two patients (33.3%), parasymphysis in one patient (16.7%), and parasymphysis to the body in two patients (33.3%). Four patients (66.7%) had condylar head fractures, while two patients (33.3%) had single split fractures. The mechanism of trauma was a slip-down incident in four cases (66.7%), while two cases (33.3%) were caused by motorcycle traffic accidents. Four patients (67%) underwent intermaxillary fixation, while two patients (33%) improved with conservative treatment. Split fractures were diagnosed in all six patients on CT, whereas the fracture line was not clearly visible on panoramic images. Mandibular split fractures accounted for 5.6% of all mandibular fractures. Conclusion: This study provides insights into the clinical characteristics of rare mandibular split fractures and the diagnostic imaging findings. Furthermore, CT scans and three-dimensional image synthesis-instead of panoramic images-may be essential for accurately diagnosing mandibular fractures, including mandibular split fractures, in the future.

정모두부방사선사진에서 하악골 우각부 영상확대 및 이의 보정에 관한 3차원 CT영상 연구 (Three-dimensional CT image study on the correction of gonial angle width enlarged on frontal cephalogram)

  • 두부방사선규격사진은 작은 크기의 방사선원에서 방사상으로 방사선이 퍼져나감으로써 피사체의 확대와 왜곡이 불가피하다는 단점이 있다. 본 연구는 두 장의 방사선사진을 서로 직각으로 촬영한 후 방사선의 기하학적 성질을 이용하여 좌우 우각부간 폭경을 계산할 경우 실제 폭경의 산출이 가능한지 알아보고자 시행되었다. 성인 40명을 연구대상으로 하여 두부자세재현기를 이용하여 측모두부방사선사진과 정모두부방사선사진을 서로 직각으로 촬영한 후 측모사진과 정모사진에서의 확대율을 이용하여 실제 우각부 폭경을 산출한 다음 계측치 및 산출치를 3차원 CT영상에서의 계측치와 각각 비교 분석하였다. 연구결과 정모두부방사선사진에서 우각부 폭경은 작게는 7.92mm 크게는 11.31mm까지. 평균 9.10mm의 확대를 보였다. 측모 및 정모두부방사선사진을 이용하여 얻은 우각부 폭경 보정치를 3차원 CT영상에서 얻은 기준치와 비교한 결과 평균 0.14mm의 작은 오차를 보였으며 통계적으로 유의한 차이를 보이지 않아 측모 및 정모두부방사선사진 촬영 시 두부자세재현기를 이용하여 서로 직각으로 촬영할 경우 실제 우각부 폭경을 계산을 통해 산출할 수 있음을 보여주었다. 한편 이부편위량과 보정오차는 통계적으로 유의한 상관성을 보이지 않아 안면비대칭이 심한 경우에도 본 보정 방법이 유용하게 사용될 수 있음을 보여 주었다.

Comparative Analysis of Surgical Outcomes of C1-2 Fusion Spine Surgery between Intraoperative Computed Tomography Image Based Navigation-Guided Operation and Fluoroscopy-Guided Operation

  • Objective : Fixation of the C1-2 segment is challenging because of the complex anatomy in the region and the need for a high degree of accuracy to avoid complications. Preoperative 3D-computed tomography (CT) scans can help reduce the risk of complications in the vertebral artery, spinal cord, and nerve roots. However, the patient may be susceptible to injury if the patient's anatomy does not match the preoperative CT scans. The intraoperative 3D image-based navigation systems have reduced complications in instrument-assisted techniques due to greater accuracy. This study aimed to compare the radiologic outcomes of C1-2 fusion surgery between intraoperative CT image-guided operation and fluoroscopy-guided operation. Methods : We retrospectively reviewed the radiologic images of 34 patients who underwent C1-2 fusion spine surgery from January 2009 to November 2018 at our hospital. We assessed 17 cases each of degenerative cervical disease and trauma in a study population of 18 males and 16 females. The mean age was 54.8 years. A total of 139 screws were used and the surgical procedures included 68 screws in the C1 lateral mass, 58 screws in C2 pedicle, nine screws in C2 lamina and C2 pars screws, four lateral mass screws in sub-axial level. Of the 34 patients, 19 patients underwent screw insertion using intraoperative mobile CT. Other patients underwent atlantoaxial fusion with a standard fluoroscopy-guided device. Results : A total of 139 screws were correctly positioned. We analyzed the positions of 135 screws except for the four screws that performed the lateral mass screws in C3 vertebra. Minor screw penetration was observed in seven cases (5.2%), and major pedicle screw penetration was observed in three cases (2.2%). In one case, the malposition of a C2 pedicle screw was confirmed, which was subsequently corrected. There were no complications regarding vertebral artery injury or onset of new neurologic deficits. The screw malposition rate was lower (5.3%) in patients who underwent intraoperative CT-based navigation than that for fluoroscopy-guided cases (10.2%). And we confirmed that the operation time can be significantly reduced by surgery using intraoperative O-arm device. Conclusion : Spinal navigation using intraoperative cone-beam CT scans is reliable for posterior fixation in unstable C1-2 pathologies and can be reduced the operative time.

In-House Developed Surface-Guided Repositioning and Monitoring System to Complement In-Room Patient Positioning System for Spine Radiosurgery

  • Purpose: This study aimed to develop a surface-guided radiosurgery system customized for a neurosurgery clinic that could be used as an auxiliary system for improving the accuracy, monitoring the movements of patients while performing hypofractionated radiosurgery, and minimizing the geometric misses. Methods: RGB-D cameras were installed in the treatment room and a monitoring system was constructed to perform a three-dimensional (3D) scan of the body surface of the patient and to express it as a point cloud. This could be used to confirm the exact position of the body of the patient and monitor their movements during radiosurgery. The image from the system was matched with the computed tomography (CT) image, and the positional accuracy was compared and analyzed in relation to the existing system to evaluate the accuracy of the setup. Results: The user interface was configured to register the patient and display the setup image to position the setup location by matching the 3D points on the body of the patient with the CT image. The error rate for the position difference was within 1-mm distance (min, -0.21 mm; max, 0.63 mm). Compared with the existing system, the differences were found to be as follows: x=0.08 mm, y=0.13 mm, and z=0.26 mm. Conclusions: We developed a surface-guided repositioning and monitoring system that can be customized and applied in a radiation surgery environment with an existing linear accelerator. It was confirmed that this system could be easily applied for accurate patient repositioning and inter-treatment motion monitoring.
