무치악 구개결손 환자를 위한 폐쇄장치의 삼차원 유한요소 분석 (A three-dimensional finite element analysis of obturator prosthesis for edentulous maxilla)

  • 연구 목적: 이 연구의 목적은 삼차원 유한요소 분석법을 이용하여 무치악 구개결손 환자에 사용되는 폐쇄장치의 응력 및 변위를 측정하고 이를 총의치 모형과 비교하는 것이다. 연구 재료 및 방법: 무치악 환자의 컴퓨터단층촬영 영상을 기반으로 구개 중앙에 상악절제술 후 발생한 결손부를 가지는 무치악 상악모형과, 무치악 3차원 유한요소 모형을 제작하였다. 각각의 모형에 적합한 총의치와 폐쇄장치를 제작하고 좌측 소구치와 구치 부위에 200 N의 수직하중을 가하였다. 3차원 유한요소 분석법을 이용하여 두 모형에서 보철물과 잔존 상악골조직의 von Mises 응력값과 변위량을 측정하였다. 결과: 두 모형 모두 하중을 가한 쪽의 피질골에서 최대 von Mises 응력 값이 관찰되었다. 총의치 모형의 피질골에서 가장 높은 값은 2.73 MPa 이었으며 폐쇄장치 모형의 피질골에서 가장 높은 값은 2.69 MPa 이었다. 보철물의 조직면에서도 높은 응력값이 관찰되었다. 최대 변위량은 폐쇄장치에서 총의치보다 더 높은 값이 관찰되었다. 결론: 폐쇄장치는 총의치에 비해 교합력 전달 면에서 불리했고, 더 큰 변위량이 관찰되어 상악절제술을 받은 무치악 환자의 보철치료 시 해부학적 지식에 바탕을 둔 정확한 인상채득과 보철치료 원칙의 적용이 고려되어야 한다.

  • Statement of problem. The implant prosthesis has been utilized in various clinical cases thanks to its increase in scientific effective application. The relevant implant therapy should have the high success rate in osseointegration, and the implant prosthesis should last for a long period of time without failure. Resorption of the peri-implant alveolar bone is the most frequent and serious problem in implant prosthesis. Excessive concentration of stress from the occlusal force and biopressure around the implant has been known to be the main cause of the bone destruction. Therefore, to decide the location and angulation of the implant is one of the major considering factors for the stress around the implant fixture to be dispersed in the limit of bio-capacity of load support for the successful and long-lasting clinical result. Yet, the detailed mechanism of this phenomenon is not well understood. To some extent, this is related to the paucity of basic science research. Purpose. The purpose of this study is to perform the stress analysis of the implant prosthesis in the partially edentulous mandible according to the different nature locations and angulations using three dimensional finite element method. Material and methods, Three 3.75mm standard implants were placed in the area of first and second bicuspids, and first molar in the mandible Thereafter, implant prostheses were fabricated using UCLA abutments. Five experimental groups were designed as follows : 1) straight placement of three implants, 2) 5$^{\circ}$ buccal and lingual angulation of straightly aligned three implants, 3) 10$^{\circ}$ buccal and lingual angulation of straightly aligned three implants. 4) lingual offset placement of three implants, and 5) buccal offset placement of three implants. Average occlusal force with a variation of perpendicular and 30$^{\circ}$ angulation was applied on the buccal cusp of each implant prosthesis, followed by the measurement of alteration and amount of stress on each configurational implant part and peri-implant bio-structures. The results of this study are extracted from the comparison between the distribution of Von mises stress and the maximum Von mises stress using three dimensional finite element stress analysis for each experimental group. Conclusion. The conclusions were as follows : 1. Providing angulations of the fixture did not help in stress dispersion in the restoration of partially edentulous mandible. 2. It is beneficial to place the fixture in a straight vertical direction, since bio-pressure in the peri-implant bone increases when the fixture is implanted in an angle. 3. It is important to select an appropriate prosthodontic material that prevents fractures, since the bio-pressure is concentrated on the prosthodontic structures when the fixture is implanted in an angle. 4. Offset placement of the fixtures is effective in stress dispersion in the restoration of partially edentulous mandible.

  • Particles reinforced MMCs have higher specific modulus, higher specific strength, better properties at elevated temperatures and better wear resistance than monolithic metals. But the coefficient of thermal expansion(CTE) of Al6061 is 5 times larger than that of SiCp. The discrepancy of CTE makes some residual stresses inside of MMCs. This work investigates Si$C_p$/Al6061 composites at high temperatures in the microscopic view by three-dimensional elasto-plastic finite element analyses and compares the analytical results with the experimental ones. The theoretical model is not able to consider the nonuniform shape of particle. So the shape of particle is assumed to be perfect global shape. And also particle distribution is not homogeneous in experimental specimen. It is assumed to be homogeneous in simulation model. The type of particle distribution is face-centered cubic array(FCC array). Furthermore, non-homogeneous distribution is modeled by combination of several volume fractions.

  • The success of porcelain laminate veneer depends on the bond strength between tooth structure and ceramic restoration and the design of tooth preparation. In particular, incisal coverage and incisal finish line are the two most important factors in long-term fracture resistance. Although the majority of clinicians are practicing incisal coverage and there are various opinions on the geo-metrical ratio between the clinical crown length of the remaining tooth structure and the length of incisal extension in porcelain laminate veneer and the optimal incisal finish lines. scientific evidence still loaves much to be desired. The purpose of this study was to determine the effects of the amounts of incisal coverage and the types of incisal finish line on the stress distribution in maxillary anterior porcelain laminate veneers under two different loading conditions. Three-dimensional finite element models of a maxillary anterior porcelain veneer with differ-ent amounts of incisal coverage ; 0, 1, 2, and 3mm and different incisal finish lines feathered edge, incisal bevel, reverse bevel and lingual chamfer with various amounts of lingual extension were developed. 300N force was applied at the point 0.5mm cervical of the linguoincisal edge in two loading conditions ; A) 125 degrees, B) 132 degrees. Tensile and compressive stress in ceramic and shear stress in the resin cement layer were analyzed using three-dimensional finite element method. The results were as follows : 1. The types of incisal finish line had more influence on the stress distribution in porcelain laminate veneer than the amounts of incisal coverage. 2. In case of no incisal coverage, incisal beveled laminate exhibited more evenly distributed tensile stress than feathered edged laminate. And in case of incisal coverage, reverse beveled laminate and lingual chamfered laminate with 1mm lingual extension exhibited more evenly distributed tensile stress than lingual chamfered laminates with 2mm and 3mm lingual extension. 3. As long as the lingual chamfer goes, less tensile stress was found at the incisal edge, while much more tensile stress was found at the lingual margin area in proportion to the length of lingual extension. 4. Under 125 degree load, tensile stress in porcelain laminate veneer had increased compared with that under 132 degree load and the difference exhibited by the change of the amount of tooth support was larger. 5. The types of incisal finish line and the distance from the incisal finish line to the loading point had more influence on the shear stress distribution in the resin cement layer than the amounts of incisal coverage. In contrast loading condition had little influence.

Three-dimensional analysis of stress and strain transmission through line joints of spatial linkage of plates

  • The examined model consists of two substructures linked by a right angle rigid line joint. One element is a wall loaded externally along its upper edge by an uneven vertical load. The other element, defined as a plate, is not loaded. Stresses and displacements in the vicinity of the joint are analysed, considering the lateral distribution which leads to three-dimensional effects. The proposed solution combines classical approach with numerical means, using appropriate stress distribution polynomial functions along the joint. Space structure constructions supply cases of interest.

A three-dimensional finite element analysis of the relationship between masticatory performance and skeletal malocclusion

  • Purpose: The aim of this study was to evaluate the transfer of different occlusal forces in various skeletal malocclusions using finite element analysis (FEA). Methods: Three representative human cone-beam computed tomography (CBCT) images of three skeletal malocclusions were obtained from the Department of Orthodontics, Yonsei University Dental Hospital, Seoul, South Korea. The CBCT scans were read into the visualization software after separating bones and muscles by uploading the CBCT images into Mimics (Materialise). Two separate three-dimensional (3D) files were exported to visualize the solid morphology of skeletal outlines without considering the inner structures. Individual dental impressions were taken and stone models were scanned with a 3D scanner. These images were integrated and occlusal motions were simulated. Displacement and Von Mises stress were measured at the nodes of the FEA models. The displacement and stress distribution were analyzed. FEA was performed to obtain the 3D deformation of the mandibles under loads of 100, 150, 200, and 225 kg. Results: The distortion in all three skeletal malocclusions was comparable. Greater forces resulted in observing more distortion in FEA. Conclusions: Further studies are warranted to fully evaluate the impact of skeletal malocclusion on masticatory performance using information on muscle attachment and 3D temporomandibular joint movements.

  • Finite element alternating method have been suggested and used for assessing the integrity of cracked structures. In the paper, in order to analyze arbitrarily shaped three dimensional cracks, the finite element alternating method is extended. The cracks are modeled as a distribution of displacement discontinuities by the displacement discontinuity method and the symmetric Galerkin boundary element method. Applied the proposed method to three dimensional crack included in the elbow, the efficiency and applicability of the method were demonstrated.

세라믹/금속 접합재의 잔류응력 해석 (Analysis of Residual Stress of Ceramic/Metal Joint)

  • 동을 중간재로 하는 $Si_3N_4/SUS304$ 접합재의 접합계면 근방의 잔류응력 분포를 유한요소법과 X선 응력측정법을 이용하여 해석을 하였다. 그 결과, 접합재의 세라믹부 계면 근방의 잔류응력 분포를 정량적으로 밝혀낼 수 있었다. 세라믹부에 발생되는 접합 잔류응력은 접합계면 근방에서 대단히 크게 나타났으며, 특히 최대인장 잔류응력 ${\sigma}_x$는 단부에서 발생하였다. 한편, ${\sigma}_x$는 접합계면 근방에서 3차원분포를 하고 있기 때문에 2차원 유한요소 해석결과와는 대단히 다른 값을 나타내고 있으며, 특히 시험편 중앙부의 계면 근방에서는 X선 실측결과가 인장 잔류응력임에 반하여 2차원 유한요소 해석결과는 압축 잔류응력으로 계산되어짐을 알았다. 따라서, 이와같은 3차원 분포를 하고 있는 접합계면 근방의 잔류응력 ${\sigma}_x$보다 간편하고 정확하게 예측할 수 있는 유한요소 해석모델에 대하여 서로 검토하였다.

  • It has been held that excessive mechanical forces to the osseous and soft tissues of the TMJ result in joint dysfunction. Understanding the stress pattern on TMJ is very important in TMJ research. But, it is very difficult to measure directly the biomechanical stress distribution in the TMJ when the mandible is loaded. Therefore, stress distribution in the TMJ during functional movement was studied through animal experiment or mathematical model. It was observed and compared the stress distribution occuring in the working and balancing condyle when lower right canine, lower right first molar and lower right second molar were clenched by the three dimensional finite element analysis. Also, stress distribution in the working and balancing condyles were observed and compared when $20^{\circ}$ forward and buccal bite forces were applied to the first molar. The results were as follows : 1. Stress distribution in the condyles during unilateral clenching of the first molar, second molar, canine showed no difference. In the working condyle, tensile force was concentrated on the lateral aspect of the condylar articular surface and condylar neck. And compressive force was concentrated on the anteromedial and lateral aspect of condyle. In the balancing condyle, tensile and compressive forces were concentrated on the lateral aspect of the condylar articular surface and stress transmission to the temporal bone was not observed. 2. When lateral forces were applied to the first molar, tensile forces were concentrated on the medial aspect of the condylar neck and condylar posterior surface in working and balancing condyle. Compressive force was concentrated on the anteromedial and lateral surface of the condyle and stress transmission to the temporal bone was not observed. 3. During unilateral clenching, stress in the working condyle decreased as the occlusal load moved posteriorly while the stress in the balancing condyle increased. when lateral force was applied to first molar, the incremental amount of stress was greater than vertical load. 4. During unilateral clenching, the average balancing/working condyle stress ratio was 2.52. There was a greater concentration of stress in the balancing condyle. The ratio increased as the occlusal load moved posteriorly and decreased considerably when lateral forces were applied to the first molar.

  • Statement of problem: Currently, there are some 20 different geometric variations in implant/abutment interface available. The geometry is important because it is one of the primary determinants of joint strength, joint stability, locational and rotational stability. Purpose: As the effects of the various implant-abutment connections and the prosthesis height variation on stress distribution are not yet examined this study is to focus on the different types of implant-abutment connection and the prosthesis height using three dimensional finite element analysis. Material and method. The models were constructed with ITI, 3i TG, Bicon, Frialit-2 fixtures and solid abutment, TG post, Bicon post, EstheticBase abutment respectively. And the super structures were constructed as mandibular second premolar shapes with 8.5 mm, 11 mm, 13.5 mm of crown height. In each model, 244 N of vertical load and 244 N of $30^{\circ}$ oblique load were placed on the central pit of an occlusal surface. von Mises stresses were recorded and compared in the crowns, abutments, fixtures. Results: 1. Under the oblique loading, von Mises stresses were larger in the crown, abutment, fixture compared to the vertical loading condition. 2. The stresses were increased proportionally to the crown height under oblique loading but showed little differences with three different crown heights under vertical loading. 3. In the crown, the highest stress areas were loading points under vertical loading, and the finish lines under oblique loading. 4. Under the oblique loading, the higher stresses were located in the fixture/abutment interface of the Bicon and Frialit-2 systems compared to the ITI and TG systems. Conclusions: The stress distribution patterns of each implant-abutment system had difference among them and adequate crown height/implant ratio was important to reduce the stresses around the implants.
