Primary Tumors of the Chest Wall (원발성 흉벽종양)

  • Primary tumors of the chest wall are rare than those of other portions of the body. Soft tissue tumors of the chest wall, though these are benign or malignant, should not be paid special attentions about their management than other soft tissue tumors of the body. Thoracic skeletal tumors, however, have some problems in the treatment because of defect in chest wall leading to herniation of lung and paradoxical movement of thoracic cage. The authors experienced 10 case of primary chest wall tumors at the department of thoracic and cardiovascular surgery, the national medical center, during last 15 years. Five of 10 cases were soft tissue tumors, and they were 2 case of lipoma and each one case of myxosarcoma and leiomyosarcoma. Among 5 bone tumors there no cases of sternal tumor, and their histopathological diagnosis were each one of fibrous dysplasia, giant cell tumor, osteochondroma, Ewing`s sarcoma and osteogenic sarcoma. Wide excision, though it was palliative one in certain case, was performed in 9 cases and only diagnostic incisional biopsy in one case, There were no postoperative deaths during admission to the hospital and all cases were missed during short term follow up after discharge from the hospital.

Experience of Reconstruction of an Extensive Chest Wall Defect with Extended External Oblique Musculocutaneous Flap (거대 흉벽 결손에 대한 확장 외복사근 근피판을 이용한 흉벽재건 치험례)

  • Purpose: The resection of locally advanced or recurred breast cancers frequently result in large chest wall defects and it leads to a great challenges to cover. Generally simple skin grafts are not a practical option for patients because of their poor cosmetic appearance and prognosis. The latissimus dorsi and rectus abdominis musculocutaneous flap have traditionally been recommended for closure of these large defects. Though the cosmetic result of reconstruction using these flaps is often excellent, but has significant drawbacks. Therefore, we thought that chest wall reconstruction using the external oblique musculocutaneous flap can be an alternative method for extensive chest wall defect related to large, locally advanced breast carcinoma. Methods & Results: We present a case of a 50-year-old Korean female, refered to our department with a left breast tumor for 10 months. CT demonstrate a large tumor on the left anterior chest wall and multiple nodules of varying size in the cervical areas and liver. FDG-PET showed areas of hot uptake throughout the left chest wall, mediastinum and liver. Biopsy was consistent with invasive ductal carcinoma (Grade III). The initial tumor was considered inoperable, so a series of chemotherapy was initiated. Though the size of the breast mass was slightly decreased, the patient continued to suffer from purulent discharge, unpleasant odor and contact bleeding of the mass, the salvage mastectomy was performed. Conclusion: We could reconstruct $23{\times}16\;cm$ sized large chest wall defect, resulting from the resection of a locally advanced breast carcinoma, using an external oblique musculocutaneous flap successfully. Immediate postoperatively checked flap was healthy. Overall result was good without any significant complications and discharged 3 weeks after operation.

Immunocytolocalization of Cell Wall Peroxidase and Other Wall Antigens from Maize Seedlings

  • Immunocytochemistry utilizes the specificity of the antigen-antibody reaction to localize specific antigens in cells or cellular organelles. Here we report the use of monoclonal antibodies, in conjunction with gold-labeled second antibodies to study the ultrastructural localization and tissue distribution of the Mr 98, 000 anionic peroxidase and other wall antigens. The antibody specific for this wall peroxidase, mWP3, labeled mainly the cell wall area. At the tissue level, the Mr 98, 000 peroxidase is located predominantly in the leaf mesophyll, internal coleoptile and sieve elements, but not in the root, as assayed with these procedures. The coleoptile walls were less heavily stained than the walls of leaf mesophyll cells. At the subcellular level, it is localized mainly in intercellular regions of the cell walls. A similar staining pattern was revealed by mWP19, one of anti-$\beta$ glucosidase antibody, though it looked less heavily stained than one with mWP3. In order to serve as a control wall staining using IgM monoclonal antibodies, mWP18 was used. Most of the label is localized over wall regions of cells of the young leaf mesophyll and coleoptile.

Application of a Regular Wave Model to Calculation of Irregular Wave Reflection from Perforated-Wall Caisson Breakwaters (불규칙파의 유공 케이슨 방파제로부터의 반사율 산정시 규칙파 모델 적용)

  • Numerous studies have been performed to develop an analytical model that can predict the reflection of regular or irregular waves from a perforated-wall caisson breakwater. Though such irregular wave models as Suh et at. (2001) become available, regular wave models are still in extensive use because of their simplicity. In the present study, using the regular wave model of Fuggazza and Natale(1992), the reflection of irregular waves from a perforated-wall caisson breakwater was calculated in several different methods. First, the regular wave model was re-validated by the hydraulic model tests. Though the model somewhat over-predicted the reflection coefficients at larger values and under-predicted them at smaller values, overall agreement was pretty good between calculation and measurement. Then, the regular wave model was applied to calculate the irregular wave reflection in the experiments of Suh et at.(2001) and Bennett et al. (1992). In applying the regular wave model to irregular wave reflection, several different methods were used. The results showed that it is the most reasonable to use the regular wave model repeatedly for each frequency component of the irregular wave specuum with the root-mean-squared wave height for all the frequencies .

  • The objective of this study is to investigate the effect of torsional eccentricity on the seismic response of high-rise RC bearing-wall structures with vertical irregularity. For this purpose, two 1:12 scale 17-story RC model structures, the one has concentric shear wall and the other has eccentric shear wall, were constructed and then subjected to a series of earthquake excitations. The test result shows the followings: 1) the layout of shear wall has the negligible effect on the natural period and the base shear coefficient, 2) the eccentric model behaves in the first and second mode while the concentric model has the first mode predominantly, 3) the stiff frame in the eccentric model resists most of overturning moment in the severe earthquake though both frames (the stiff and flexible frames) resist almost equally in the design earthquake.

  • Ground penetrating radar(GPR) was applied for measuring depths, sizes and intervals of rebars embedded in concrete. A concrete wall was constructed for this study and a sand pool and a concrete block were used for simulation. Result of this study shows that GPR can be used for measuring rebar depths and intervals, even though it is limitary, but that measuring sizes is almost impossible. Simulation with the sand pool was helpful for research on the versatile rebar arrays though signal was not clear as real concrete wall. A concrete block with many cylindrical holes for inserting different sized rebars could not be used for simulator due to many unknown reflective waves. Antenna orientation must be perpendicular to rebars for large reflection signal.

  • Due to the nature of the existing load, the criteria of assessing the intensity of the lightweight wall's impact resistance has been though of as obscure. The current study, therefore, focuses on the standardized assessment of the impact resistance to the force of the large soft body applying to the lightweight wall. The gypsum board wall showed a low level of the maximum residual displacement. It is, however, required to be careful about the selection of the finishing process since the high level of the maximum displacement is likely to cause harm to finishing materials. Unlike the gypsum board, the ALC block wall displayed a considerable rigidity while showing almost no maximum residual displacement. Even with the low level of the maximum displacement due to the stiffness, the ALC block wall is still likely to be affected by the vibration derived from any impact on the surface, which demands a need for additional study. The future experimental study, accordingly, will focus on the impact of the vibration on finishing materials, consequently leading to the accurate prediction of the possibility of potential damage to the lightweight wall caused by the large soft body.

  • Application of anchored or strutted wall system for the earth retention of excavation works in a populated urban area or a poor soil deposit can be limited due to various restrictions. Since the strut becomes longer in a wide excavation site, the stability of an earth retaining wall is decreased, the wall deformation is increased, and the ground settlement is also increased due to an increased buckling or bending deformation of struts. Especially, in a populated urban area, the installation of anchors can be problematic due to the property line of adjacent structures or facilities. Thus, a new concept of earth retaining system like Self-Supported diaphragm Wall can solve several problems expected to occur during excavation in the urban area. Application of self-supported counterfort diaphragm wall was verified in this paper though comparing the design of self-supported counterfort diaphragm wall with the data monitored during excavation in Singapore.

Numerical Study on the Leakage Safety of the Membrane LNG Tank Wall (멤브레인식 LNG 탱크벽체의 누설안전에 관한 수치해석적 연구)

  • In this numerical study, the leakage safety of the LNG tank in which is constructed by membrane inner tank-plywood-polyurethane form-plywood-prestressed concrete structures has been presented for four leakage analysis models. The LNG leak criterion of the tank wall with a storage capacity of $200,000\;m^3$ is analyzed based on the thermal resistance technique. This means that if the cryogenic temperature of a leaked LNG is detected at the outer side of the PC wall, it may be leaked through the wall thickness of the tank. The calculated results based on the thermal resistance method between two walls show that the plywood, PUF, and another plywood walls may block the leakage of the leaked LNG even though the strength of these walls is already collapsed by a leaked LNG pressure. But, the leaked LNG may pass the thickness of the prestressed concrete wall for a period of elapsed time even though the PC outer tank supports the leaked LNG pressure. Thus, the PC outer tank may extend the leakage time of a leaked LNG.

  • Failure behaviors of thin cylinder with corrosion are very important for the integrity of boiler and pressure vessel system. In this study, FEM with internal pressure are conducted on 1000 mm diameter (length 3000 mm and thickness, 5.9 mm) SS400 carbon steel. Failure behaviors of locally wall thinned cylinders were calculated by elasto-plastic analysis using finite element method. The elasto-plastic analysis was performed by FE code ANSYS. We simulated various types of local wall thinning that can be occurred at cylinder surface due to corrosion. Locally wall thinned shapes were machined to be different in size along the circumferential or axial direction of straight cylinder. In case of local wall thinned length 30 mm, internal pressure, when the crack initiation and the plastic collapse occur, didn't decrease dramatically even though local wall thinned depth was deep. In 400 mm, the more local wall thinned depth is deep, the more internal pressure decreased dramatically. In degraded materials, crack is easily initiation but plastic collapse was difficult.
