Spinal Cord Partial Block Technique Using Dynamic MLC

  • The spinal cord dose is the one of the limiting factor for the radiation treatment of the head & neck (H&N) or the thorax region. Due to the fact that the cord is the elongated shaped structure, it is not an easy task to maintain the cord dose within the clinically acceptable dose range. To overcome this problem, the spinal cord partial block technique (PBT) with the dynamic Multi-Leaf Collimator (dMLC) has been developed. Three dimension (3D) conformal beam directions, which minimize the coverage of the normal organs such as the lung and the parotid gland, were chosen. The PBT field shape for each field was designed to shield the spinal cord with the dMLC. The transmission factors were determined by the forward calculation method. The plan comparisons between the conventional 3D conformal therapy plan and the PTB plan were performed to evaluate the validity of this technique. The conformity index (CI) and the dose volume histogram (DVH) were used as the plan comparison indices. A series of quality assurance (QA) was performed to guarantee the reliable treatment. The QA consisted of the film dosimetry for the verification of the dose distribution and the point measurements. The PBT plan always generated better results than the conventional 3D conformal plan. The PBT was proved to be useful for the H&N and thorax region.

Will Prone Trunk Extension Affect Scapular and Thoracic Kinematics and Muscle Activities During Scapular Posterior Tilting Exercise in Subjects With Round Shoulder and Flexed Posture?

  • Background: Round shoulder posture, results from excessive flexed posture of the thorax, is defined as a position of scapular protraction, anterior tipping, and downward rotation. However, previous studies have focused on only passive position of the thorax during scapular posterior tilting (SPT) and have not reported on SPT combined with correction of flexed posture. Objects: The aim of this study was to compare effects of SPT and SPT with prone trunk extension (SPT + PTE) on activities of the lower trapezius, serratus anterior, and thoracic erector spinae and degree of posture in subjects with round shoulder and flexed posture. Methods: Fifteen subjects with round shoulder and flexed posture were recruited. The caliper was used to measure the degree of round shoulder and flexed posture. Electromyography was performed to collect data of muscle activities. Paired t-test was used to compare two exercise (${\alpha}=.05$). Results: When SPT + PTE was applied, the degree of round shoulder posture (p=.001) and flexed posture (p=.039) significantly decreased compared with that when SPT was applied. The lower trapezius activity significantly increased in the SPT + PTE condition compared with that in the SPT condition (p=.026). There were no significant differences in serratus anterior activity between SPT + PTE and SPT. The thoracic erector spinae activity significantly increased in the SPT + PTE condition compared with that in the SPT condition (p=.014). Conclusion: SPT + PTE might be one of the effective methods to enhance activities of lower trapezius and thoracic erector spinae, and to reduce round shoulder posture and flexed posture in subjects with round shoulder and flexed posture.

  • Inverse problem in electrical impedance tomography (EIT) is highly ill-posed therefore prior information is used to mitigate the ill-posedness. Regularization methods are often adopted in solving EIT inverse problem to have satisfactory reconstruction performance. In solving the EIT inverse problem, iterative Gauss-Newton method is generally used due to its accuracy and fast convergence. However, its performance is still suboptimal and mainly depends on the selection of regularization parameter. Although, there are few methods available to determine the regularization parameter such as L-curve method they are sometimes not applicable for all cases. Moreover, regularization parameter is a scalar and it is fixed during iteration process. Therefore, in this paper, a novel method is used to determine the regularization parameter to improve reconstruction performance. Conductivity norm is calculated at each iteration step and it used to obtain the regularization parameter which is a diagonal matrix in this case. The proposed method is applied to human thorax imaging and the reconstruction performance is compared with traditional methods. From numerical results, improved performance of proposed method is seen as compared to conventional methods.

Effect of Saddle to Pedal Length in Kayak Ergometer on Rowing Motion and EMG Activation in Elite Kayak Players (엘리트 카약 선수들의 에르고미터를 이용한 로잉 동작 시 안장과 페달의 거리가 로잉 패턴 및 근 활성도에 미치는 영향)

  • The purpose of this study was to identify the saddle to pedal length contributing to successful performance in kayak using a kayak ergometer. Ten male elite kayak players participated in this study. players were tested on the kayak ergometer which was varied saddle to pedal length by the knee flexion angle(90deg; 120deg; 150deg) to measure stroke frequency, paddling amplitude, joint angle, RoM and angular velocity, foot pressure and force, iEMG using the 3D motion system, foot pressure system and EMG wireless system. At a results, rowing at 120deg on knee flexion angle showed higher stroke frequency and paddling amplitude than other knee flexion angles. RoM at upper extremity showed not significant difference between knee flexion angles. But there were significant differences in thorax and pelvis rotation RoM, knee flexion-extension RoM in each condition. In addition, foot pressure, force and iEMG were significantly different in knee flexion angles. Study showed that changed of saddle to pedal length affected rowing performance kinds of stroke frequency, paddling amplitude. The most important thing, increased range of motion in pelvic and thorax has occurred by force that generated foot-bar to seat. Not only that, but it seems to be attributed to a technical adaptation developed to maximum rowing performance.

The Effects of Breath-Counting Meditation and Deep Breathing on Heart Rate Variability

  • Objectives: This study aimed to evaluate the effects of breath-counting meditation (BCM) and deep breathing (DB) on heart rate variability (HRV). These breathing techniques have the characteristics of non-paced and self-controlled breathings, resulting in less increase of HRV. We also compared BCM and DB with usual breathing (UB) or relaxing breathing (RB) which can reveal the characteristics of those. Methods: 83 healthy volunteers sitting in chairs performed non-paced breathing; UB, RB, BCM, and DB each for 5 minutes. One minute of relaxation was permitted between breathings. Participants surfed the internet sitting in front of a computer during UB, while for RB, they remained steady with eyes closed. For BCM, they breathed inwardly counting from 1 to 10 repetitively, while they took a deep breath during DB. Physiological indices were simultaneously recorded with a biofeedback system. Results: Respiration rate, thoracic amplitude, and mean heart rate decreased in RB compared with UB, but there was no change in HRV. Respiration rate in BCM and DB was lower than that in UB or RB, and the amplitude of thorax or abdomen, and HRV all increased (p<0.05). However, mean heart rate and skin conductance decreased in BCM compared with UB (p<0.05), whereas those were no different between DB and UB. Conclusion: BCM, just concentrating mentally on breathing with counting each breath, can increase HRV with less sympathetic activation, while DB, actively moving thorax and abdomen for achieving the deepest respiration rate, can greatly raise HRV with the maintenance of mean vagal or sympathetic tone.

  • Alkaline phosphatase from Culex pipiens pallens was examined to determine the optimal assay condition and to assay the activity during ovarian development. The activity of alkaline phosphatase in a male and a nongravid female continuously were declined after eclosion. But by the stimulus of a blood meal, the enzyme activity was increased dramatically. At 30 hr. after a blood meal, the maximal activity was reached and then declined. And after 48 hr. after a blood meal, the second activity increase was revealed. This second increase was maintained up to oviposition. The first activity increase was revealed in the midgut and the second increase was done in the ovary to assay the organ distribution of alkaline phosphatase. In electrophresis data, it was shown 5 isozyme bands, ALP-1 and ALP-2 in the ovary, ALP-3 in the thorax and the midgut, and ALP-4 and ALP-5 in the thorax, the fatbody and the midgut in crude extract at 30 hr. after a blood meal. One the same ovary pattern were shown at 72 hr. after a blood meal.

Treatment of Tumor Involving Thoracic Inlet by Using Transmanubrial Osteomuscular Sparing Approach - One case report- (흉강입구를 침범한 종양의 근골보존 경흉골병 접근술을 이용한 치유 -1예 보고-)

  • The operative case of transmanubrial osteomuscular sparing approach for the tumor involving thoracic inlet is reported. A 69-year-old man visited the hospital due to right upper extremity weakness. Chest roentgenogram showed bronchiectasis, chronic pulmonary tuberculosis, and fungal ball in right upper lobe. On computed tomogram, tumor was located in epidural space of the 6th, the 7th cervical, and the 1st thoracic spine and extended to the apex of the right thorax. A neurosurgeon performed laminectomy and removed the tumor located in the spinal canal. A thoracic surgeon performed a transmanubrial osteomuscular sparing approach and removed the tumor involving thoracic inlet. The tumor was diagnosed as hemangiopericytoma. The patient recovered without complication.

Ultrasonographic Evaluation of the Thorax and Abdomen in Horse (말에서 흉복부의 초음파상)

  • Ultrasonogram for kidney, spleens liver, intestine and heart was evaluated 11 Chriu horses and 14 Thoroughbred horse. The kidney was determined at both flan and both 17th intercostal spaces. The spleen was determined at the left 13-17 intercom spacer and the liver was determined at the right 7-13 intercostal space. The heart was determined at the right 4-6 intercostal space and left 3-6 intercostal space. The length, of rig kidney at the Thoroughbred horse, Thoroughbred foul, Cheju horse and Cheju foul were 16.2, 12.6, 13.1 and 11.2 mm, respectively. The width of right kidney at the Thoroughbed horse, Thorughbred foul, Cheju horse and Cheju foul were 5.4, 4.3, 4.6 and 4.2 mm, respectively. The depth of right kidney at the Thoroughbred horsed Thoroughbred foul, Cheju horse and Chrju foul were 5.2, 4.4, 4.5 and 4.3 mm, respectively. Similar ultrasonographic measurements were obtained for the left kidney, The left ventricular end-diastolic diameter at the Thoroughbred horse, Thoroughbred fouls Cheju horst and Cheju foul were 107, 83, 85 and 73 mm, respectively. The left ventricular end-systolic diameter were at the Thoroughbred horse, Thoroughbred foul, Cheiu horse and Cheiu foul were 63, 52, 53 and 45 mm, respectively. Also, the interventricular septum in end-diastole, interventricular septum in end-systoles left ventricular wall end-diastoles left ventricular wall end-systoles right ventricular end-diastole diameter, aorta and left atrium at t Thoroughbred horse, Thoroughbred foul, Chriu horse and Chriu foul were measured. Experimental renal stone and enterolith of colon were observed by ultrasonography.

Analysis of the Bone Proportional Method for Determining Acupoints in the Upper and Lower Abdominal Region in Males and Females

  • Background: The purpose of this study was to redefine the location of acupoints in the lower abdomen by taking actual thoracic and abdominal measurements. Methods: Measurements of the length and height of the thorax, and both the upper and lower abdominal area were compared to standard values defined by the World Health Organization Western Pacific Region (WHO/WPRO), and medical text books such as Miraculous Pivot, and A-B Classic of Acupuncture and Moxibustion. These measurements were used to redefine the standard of 1 cun, and the cun value of the lower abdomen. Results: Of the 60 studies screened, all studies used the cun measurement system. Considerable variation in the localization of acupoints in the lower abdominal area were detected. The average measurement of a typical male lower abdomen was within 6.33 cun to 6.34 cun. From this data, 6.5 cun appears to be more accurate than the current standardized length of 5 cun. The standardized index values of the width of the 4 fingers, and the distance from the lateral prominence from the greater trochanter to the popliteal crease (Fm), appeared to apply to only males, as defined by the WHO/WPRO. Conclusion: Further studies on standardizing the index measurements for the lower abdominal area are necessary. For males, the more accurate standardized length of the lower abdomen was 6.5 cun, whereas for females, the measurement would typically be longer than 6.5 cun.

The Importance of Positioning in Left Lateral Chest X-Ray Examination (흉부 왼쪽 엑스선검사 시 위치 잡기의 중요성)

  • This study was conducted to ultimately reduce unnecessary radiation exposure by emphasizing the need and importance of correct positioning by examining the positioning relationship of anatomical structures in the human body and changes in X-ray images according to changes in patient positioning during the left lateral chest X-ray examination. This study investigated and analyzed previously published papers and books on the left lateral chest X-ray examination to find out the importance of positioning in the left lateral chest X-ray examination. To find out the importance of correct positioning in the left lateral chest X-ray, we compared three images of incorrectly positioned right thorax and left thorax rotated forward and the lower median surface of the body leaning against the image receptor. In the left lateral chest examination, a distorted image was obtained in which the shape of the anatomical structure observed in the image was changed according to the presence or absence of rotation of the patient and the inclination of the median visual surface. X-ray images with the most accurate and large amount of information were obtained from X-ray images with the correct positioning performed during left lateral chest X-ray examination. Therefore, It is believed that the left lateral chest X-ray examination will have beneficial effects such as providing accurate medical information, preventing misdiagnosis, reducing social costs, and ultimately reducing radiation exposure.
