Lung Cancer Screening: Subsequent Evidences of National Lung Screening Trial

  • The US National Lung Screening Trial (NLST) demonstrated a 20% reduction in lung cancer mortality and a 6.7% decrease in all-cause mortality. The NLST is the only trial showing positive results in a high-risk population, such as in patients with old age and heavy ever smokers. Lung cancer screening using a low-dose chest computed tomography might be beneficial for the high-risk group. However, there may also be potential adverse outcomes in terms of over diagnosis, bias and cost-effectiveness. Until now, lung cancer screening remains controversial. In this review, we wish to discuss the evolution of lung cancer screening and summarize existing evidences and recommendations.

  • Subgena and clypeus are a continuous plate, so should be collectively called as peristoma. Subsequently, subgenal sulcus is revised into peristomal sulcus or cranio-stomal sulcus. Ptilinal suture on dipteran adult head is revised into ptilinal fissure. Notal and pleural wing processes are revised into notal and pleural process, respectively. The lower plate of abdomen is ventum, while that of thorax is sternum.

  • Acute mediastinitis is almost always secondary to some other condition, and most cases are due to esophageal perforation. Although acute mediastinitis from odontogenic infection is extremely rare in the era of antibiotic drugs, some more fulminant odontogenic infections can produce complications including airway obstruction, necrotizing fascitis and extension of the infection to thorax. Irrespective of the changing incidence of etiologic factors, unless the pathophysiology of acute mediastinitis and its causes are understood and the conditions promptly recognized and properly treated, the result may be prolonged illness and even death. We experienced a case of odontogenic infection followed by acute mediastinitis and present review of literature.

  • A 38 years old man had been treated as a pulmonary tuberculosis by the positive result of acid fast stain of bronchial washing from the focal infiltrative lesion at left lower lobe. On radiologic examination after one year treatment, there was an aggravation of lesion at left lower lobe with moderate amount of pleural effusion at the same side. After 11 weeks, follow up chest film disclosed bilateral pleural effusion. The pleural fluid of both side was pus in gross appearance with low pH, high LDH, low glucose and high protein. Pleurodectomy was performed to remove the loculated empyema with the thickened pleura of right thorax. This pleuro-pulmonary lesion can be easily misdiagnosed as a tuberculous lesion if it is not taken into consideration as a possible diagnosis.

  • We have developed a bioimpedance measurement system for impedance cardiography and pneumography. The system injects 50kHz, $200mA_{p-p}$ curreng into the thorax and measures the voltage changes using body surface electrodes. We used the four-electrode method for tile measurement of cardiac singnals and two-electrode method for respiratory signals. We developed a Microsoft Windows program for the acquisition, display, storage, and processing of impedance signals.

  • Worldwide cardiovascular diseases are the major cause of death. Aside from heart transplants, which are limited due to the availability of human donor hearts, artificial hearts are the only therapy available for terminal heart diseases. For various reasons, a total implantable artificial heart is desirable. But the limited space in the human thorax sets rigorous restrictions on the weight and dimensions of the device. Nevertheless, the appropriate functionality of the artificial heart must be ensured and blood damage must be prevented. These requirements set further restrictions to the drive of this device. In the this paper, two optimization methods, namely, the manual parameter variation and Differential Evolution algorithm, are presented and applied to match the specifications of an artificial heart.

  • We evaluated sixteen patients of traumatic diaphragmatic injuries that we have experienced from Jan. 1987 to Aug 1993. Age was ranged from 6 to 71 years, predominantly in the fourth and fifth decades. 13 were male and 3 were female, a ratio of 4.3: 1. Blunt trauma was develped in 11 [Lt 7, Rt 4], penetrating trauma in 5 [Lt 2, Rt 3]. Preoperative diagnosis of diaphragmatic injury was possible in 8 patients [72.2 %] in blunt trauma, and 1 patient [20 %] in penetrating trauma. 8 cases[54.5%] in blunt trauma, and 4 cases in penetrating trauma were treated within 24 hours,meanwhile, patients treated after 10 days were 3, all by blunt trauma.The repair of 16 cases were performed with thoracic approach in 4 cases, thoracoabdominal approach in 3 cases, and abdominal approach in 9 cases. The herniated organs in thorax were stomach [5], colon [3], liver [2], and pancreas [1]. Postoperative complication were developed in 9cases[56.3%] significantly related with delayed operation time [p < 0.01 ]. Hospital mortality was 12.5 % [2/16], and the causes of death were hypovolemic shock in one and hepatic failure due to portal vein rupture in another.

Lymphangioma of the Chest Wall (흉벽의 임파관종)

  • Lymphangioma(or cystic hygroma) of the chest wall is rare case. We experienced one such case in a 16-year-old girl who complained of a large recurrent mass on her right upper post erolateral chest wall which had developed several years ago. The diagnosis was made following a physical examination, chest magnetic resonance imaging(MRI), and radio isotope (RI) lymphangiogrphy and was confirmed by a histopathological examination. We performed total excision of the lesion followed by a repeated sclerosing therapy with intralesional injection of Vibramycin.

  • The intractable pneumothorax with continuous air leakage, emphysematous lung and high operative risk treated by selective bronchial occlusion has been seldomly reported abroad. The bronchus responsible for air leakage was occluded with such materials as fibrin glue, gelatin sponge and oxidized regenerated cellulose(surgicel). We performed selective bronchial occlusion by flexible fiberoptic bronchoscopy with gelfoam in two cases. There was no complication after the procedure; therefore, we report the treatment for intractable pneumo- thorax by bronchoscopy with gelfoam packing.

  • A great variety of penetrating injuries is happening due to the increasing population and violence today. An optimal surgical approach is the key factor for successful repair of a complicated penetrating injury. A 23-year-old woman fell down the stairs from the second floor and received cervico-thoracic penetration injury due to a metalic bar. The metalic bar ruptured the right jugular vein and penetrated the left upper and lower lung. Under cervico-median sternotomy, neck vessels were repaired and the left thorax was successfully entered to repair the damaged lung through the mediastinal pleura. With this approach, the patient's position did not need to be changed during operation, while reduced the operation time compared to the conventional approach (cervical incision and standard thoracotomy).
