Background: Severe pain always develops after thoracotomy; intrapleural regional analgesia is used as a simple, safe technique to control it. This study was performed to evaluate whether a small dose of naloxone with local anesthetics prolongs sensory blockade. Methods: A prospective, randomized double-blinded controlled study was conducted on 60 patients of American Society of Anesthesiologists statuses I and II, aged 18 to 60 years, scheduled for unilateral thoracotomy surgery. After surgery, patients were randomly divided into two groups: through the intrapleural catheter, group B received 30 ml of 0.5% bupivacaine, while group N received 30 ml of 0.5% bupivacaine with 100 ng of naloxone. Postoperative pain was assessed using the visual analog pain scale (VAS). Time for the first request for rescue analgesia, total amount consumed, and incidence of postoperative complications were also recorded. Results: The VAS score significantly decreased in group N, at 6 h and 8 h after operation (P < 0.001 for both). At 12 h after injection, the VAS score increased significantly in group N (P < 0.001). The time for the first request of rescue analgesia was significantly longer in group N compared to group B (P < 0.001). The total amount of morphine consumed was significantly lower in group N than in the bupivacaine group (P < 0.001). Conclusions: Addition of a small dose of naloxone to bupivacaine in intrapleural regional analgesia significantly prolonged pain relief after thoracotomy and delayed the first request for rescue analgesia, without significant adverse effects.

Background: With recent advances in cardiac surgery through minimal access, mini-thoracotomy has emerged as an excellent alternative for cardiac myxoma resection. This study analyzed the surgical results of this approach, focusing on postoperative cerebral embolism and tumor recurrence. Methods: We retrospectively reviewed 64 patients (mean age, 56.0±12.1 years; 40 women) who underwent myxoma resection through mini-thoracotomy from October 2008 to July 2020. We conducted femoral cannulation and antegrade cardioplegic arrest in all patients. Patient characteristics and perioperative data, including brain diffusion-weighted magnetic resonance imaging (DWI) findings, were collected. Medium-term echocardiographic follow-up was performed. Results: Thirteen patients (20.3%) had a history of preoperative stroke, and 7 (11.7%) had dyspnea with New York Heart Association functional class III or IV. Sixty-one cases (95.3%) had myxomas in the left atrium. The mean cardiopulmonary bypass and cardiac ischemic times were 69.0±28.6 and 34.1±15.0 minutes, respectively. Sternotomy conversion was not performed in any case, and 50 patients (78.1%) were extubated in the operating room. No early mortality or postoperative clinical stroke occurred. Postoperative DWI was performed in 32 (53%) patients, and 7 (22%) showed silent cerebral embolisms. One patient underwent reoperation for tumor recurrence during the study period; in that patient, a genetic study confirmed the Carney complex. Conclusion: Mini-thoracotomy for cardiac myxoma resection showed acceptable clinical and neurological outcomes. In the medium-term echocardiographic follow-up, reliable resection was proven, with few recurrences. This approach is a promising alternative for cardiac myxoma resection.
Approximately 9.8% of neurogenic tumors of the mediastinum extend into the spinal column so that the composite neoplastic mass was dumbbell shaped. We experienced three patients confirmed by Dumbbell shaped mediastinal tumor radiologically and tried surgical resection by posterolateral thoracotomy only corpectomy and costotransversectomy was performed simultaneously in three patients and interbody fusion in two. In one patient the diagnosis was liposarcoma and in 1 neuroblastoma and in 1 neurilemmoma. then followed by radiation theraphy in case I and radiation and chemotheraphy in case II. All three cases showed satisfactory results clinically and radiographically.
To assess the therapy of spontaneous pneumothorax in the aged patients, the treatment methods and results in 60 patients 50 years of age or older were retrospectively reviewed. Most of the patients were male [56 of 60 patients] and the major underlying lung diseases associated with spontaneous pneumothorax were tuberculosis [32 patients] and chronic obstructive lung disease [20 patients]. The recurrence rate of thoracostomy tube drainage with or without chemical pleurodesis using tetracycline was 39.6% [21 of 53 patients], but there was no recurrence in the patients treated with open thoracotomy, pleural abrasion, and chemical pleurodesis using talcum powder[Asbestosis free]. In the patients treated with open thoracotomy, the bullous or bleb lesions were placed in the various sites of both lungs. We concluded that even though thoracostomy tube drainage is the first choice of therapy for spontaneous pneumothorax in the aged patients, the recurrence rate is high, especially in the patients with persistent air leakage for more than 2 days, and the open thoracotomy with pleural abrasion and chemical pleurodesis using talcum powder can prevent the recurrence in the selected patients.
An approach to the treatment of post-operative pain by the injection of bupivacaine into the pleural space through an intrapleural cathter has been studied. Among 24 thoracotomy patients, bupivacaine was injected only to experimental group[ 12 patients ] when the patient was able to head up for oneself during recovery from anesthesia. The pain and ROM[ range of motion ] scores, respiration rate, PaCO2 level of both experimental and control group were measured at the time of head-up and 30 and 120 minutes thereafter. The scores of pain and ROM of experimental group were significantly[ P value < 0.05 ] decreased in 30 minutes and 120 minutes after bupivacaine injection compared with those of control group but respiration rate and PaCO2 level were not changed significantly. With this result, we can suggest that intrapleural injection of bupivacaine is useful for pain relief following thoracotomy.
Children`s small airway precludes the use of standard methods of bronchial separation. So, we performed the posterior thoracotomy under the prone position in 3 cases to avoid endobronchial gravity spillage of secretion and infected debris from the diseased lung to the contralateral sound lung. The advantages of the posterior thoracotomy under the prone position was discussed. In two cases, empyema with total collapse of left lung and congenital cystic adenomatoid malformation [CCAM] of right lung, copious secretion was spilled through the endotracheal tube but could be removed successfully by the endotracheal suction. In the third case of bilateral peripleural abscess, bilateral posterior thoractomy was done without position change. All procedures were performed without any technical difficulty and complication.
Fifty-two cases of spontaneous pneumothorax encountered in Seoul National University Hospital during the period from 1961 to 1968 were reviewed. The incidence was highest in the adult between 21 and 40 years of age, showing 50%. Four cases of bilateral spontaneous pneumothorax [7.7%] were noted. Pulmonary tuberculosis was the most common cause, 50% of the cases. Non-tuberculous group was 26 cases [50%], of which 15 cases were idiopathic, 5 emphysematous bullae or blebs, 5 inflammatory lung disease, mostly pneumonia, and one pulmonary paragonimiasis. Among 52 cases, the lung expanded completely with absolute bed rest in 7 cases, 3 out of 9 with needle aspiration and bed rest, 34 out of 41 with closed thoracotomy and underwater seal Stedman suction, and 7 cases were treated with open thoracotomy with resection of the lesions without complication. Among these cases treated with closed thoracotomy it took about 3 days in non-tuberculous group to expand the collapsed lung and more than 2 weeks in tuberculous group.
The mediastinoscopy was a well known useful diagnostic tool for detection of mediastinal lymph nodes invasion by bronchogenic carcinoma, and also useful means for histologic diagnosis of metastatic carcinoma and certain mediastinal tumors. 31 cases of mediastinoscopies were reviewed which were experienced at Kyung Hee University Hospital from July, 1979 to June, 1985. We experienced 20 cervical mediastinoscopies, 10 left anterior mediastinotomy, and 1 both procedures. Of the 31 cases, 22 cases were used for preoperative staging of bronchogenic carcinoma, 7 cases for mediastinal tumor diagnosis, and 2 cases for histologic diagnosis of metastatic carcinoma. In 22 mediastinoscopies which were used for preoperative staging, 10 cases were revealed positive mediastinal nodes, and could avoid meaningless thoracotomy. All 12 mediastinoscopy negative patients were received thoracotomy, and 10 of them were resectable. The resectability in bronchogenic carcinoma was 83%, on the contrary, the other series at premediastinoscopic era revealed only 65% resectability. Other mediastinal lesions such as tuberculous granuloma [4], sarcoidosis [2], malignant thymoma [1], and metastatic carcinoma [2] were also diagnosed successfully. Mediastinoscopy is very useful tool for determination of treating method of bronchogenic carcinoma, and for diagnosis of certain mediastinal tumors which, otherwise, need a thoracotomy for confirmatory diagnosis.
One hundred and twenty three patients underwent 137 thoracostomies for spontaneous pneumothorax in the department of Thoracic and Cardiovascular Surgery, Gyeongsang National University from January 1987 to December 1994. There were 118 men and 6 women and average age was )2.4 years. The two most common surgical Indications were recurrent pneumothorax and continuous air leakage. Other indications were visible bullae on simple X-ray, previous contralateral pneumothorax, incomplete expansion of the lung, and bilateral pneumothoraces. Methods of thoracotomy were subaxillary thoracotomy in 82 cases, lateral minithoracotomy in 12 cases and posterolateral thoracotomy in 43 cases. Operation time was 63.0 $\pm$ 30.8, 98.3 $\pm$ 37.9, 186.9 $\pm$ 87.9 minutes respectively, and postoperative chest tube keeping time was 5.2 $\pm$ 4.1 days in subaxillary thoracotomy, 6.2 $\pm$ 5.0 days in minithoracotomy and 10.0 $\pm$ 5.8 days in posterolateral thoracotomy Bullae were present mostly at the apex in spontaneous and tuberculous pneumothorax comparred to the cases of chronic obstructive or emphysematous lung disease, where there were no redilection of presence of bullae (p< 0.01). Operative procedures were wedge resection, bullae obliteration and lobectomy. Postoperative complications were continuous air leakage, bleeding, brachial plexus injury, empyema, and wound infection, but all the complications were cured by the time of discharge. There was no mortality.

Background: We evaluated the feasibility and outcomes of pulmonary resection and mediastinal node dissection (MND) by video-assisted thoracoscopic surgery (VATS) following neoadjuvant therapy for stage IIIA N2 non-small cell lung cancer (NSCLC). Methods: From November 2009 to December 2013, a total of 35 consecutive patients with pathologically or radiologically confirmed stage IIIA N2 lung cancer underwent pulmonary resection and MND, performed by a single surgeon, following neoadjuvant chemoradiation. Preoperative patient characteristics, surgical outcomes, postoperative drainage, postoperative complications, and mortality were retrospectively analyzed. Results: VATS was completed in 17 patients. Thoracotomy was performed in 18 patients, with 13 planned thoracotomies and 5 conversions from the VATS approach. The median age was $62.7{\pm}7.9years$ in the VATS group and $60{\pm}8.7years$ in the thoracotomy group. The patients in the VATS group tended to have a lower diffusing capacity for carbon monoxide (p=0.077). There were no differences between the 2 groups in the method of diagnosing the N stage, tumor response and size after induction, tumor location, or histologic type. Complete resection was achieved in all patients. More total and mediastinal nodes were dissected in the VATS group than in the thoracotomy group (p<0.05). The median chest tube duration was 5.3 days (range, 1 to 33 days) for the VATS group and 7.2 days (range, 2 to 28 days) for the thoracotomy group. The median follow-up duration was 36.3 months. The 5-year survival rates were 76% in the VATS group and 57.8% in the thoracotomy group (p=0.39). The 5-year disease-free survival rates were 40.3% and 38.9% in the VATS and thoracotomy groups, respectively (p=0.8). Conclusion: The VATS approach following neoadjuvant treatment was safe and feasible in selected patients for the treatment of stage IIIA N2 NSCLC, with no compromise of oncologic efficacy.
