The complex dose distribution and dose transfer characteristics of intensity-modulated radiotherapy increase the importance of precise beam data measurement and review in the acceptance inspection and preparation stages. In this study, we propose a process map for the introduction and installation of high-precision radiotherapy devices and present items and guidelines for risk management at the acceptance test procedure (ATP) and commissioning stages. Based on the ATP of the Varian and Elekta linear accelerators, the ATP items were checked step by step and compared with the quality assurance (QA) test items of the AAPM TG-142 described for the medical accelerator QA. Based on the commissioning procedure, dose quality control protocol, and mechanical quality control protocol presented at international conferences, step-by-step check items and commissioning guidelines were derived. The risk management items at each stage were (1) 21 ionization chamber performance test items and 9 electrometer, cable, and connector inspection items related to the dosimetry system; (2) 34 mechanical and dose-checking items during ATP, 22 multileaf collimator (MLC) items, and 36 imaging system items; and (3) 28 items in the measurement preparation stage and 32 items in the measurement stage after commissioning. Because the items presented in these guidelines are limited in terms of special treatment, items and practitioners can be modified to reflect the clinical needs of the institution. During the system installation, it is recommended that at least two clinically qualified medical physicists (CQMP) perform a double check in compliance with the two-person rule. We expect that this result will be useful as a radiation safety management tool that can prevent radiation accidents at each stage during the introduction of radiotherapy and the system installation process.
This sturdy proposed and tested whether the six core treatment processes of Acceptance and Commitment Therapy (ACT) reduce self-criticism and dichotomous thinking in Evaluative concerns perfectionists and even diminish depression and ultimately verify it. The sample consisted of 22 subjects who displayed high EC perfectionism and depression. The subjects were randomly assigned to ACT groups or control groups (n-11/each). The ACT program was administered in eight sessions. All participants completed scale about EC perfectionism, self-criticism, dichotomous thinking, depression and acceptance & action at the pre-test, post-test, and the six week follow-up. The results showed that evaluative concerns perfectionism, self-criticism, dichotomous thinking, and depression decreased more in the treatment group than in the control group, whereas acceptance & action increased in the former. Finally, the implications and limitations of this study, along with suggestions for future study were discussed.
The purpose of this study is to evaluate the effects of the acceptance commitment therapy program (ACT) on the psychological flexibility, self-compassion, recovery in alcohol dependents. The ACT program contains skills that establish acceptance and willingness, defusion, self-as-context, contact with the present moment, clarifying values, and committed action. This study was intended to enhance the effects on the psychological flexibility, self-compassion, recovery in alcohol dependents with ACT program. Fifteen inpatients with alcohol dependence were participated in the ACT group and the same number of participants were in the both CBT control group and TAU control group. The ACT program was held once a week, eight times, and each session was 90-minute long. Repeated measure of MANOVA and post-hoc were used to compare pre-test mean with post-test and with follow-up test mean. As a result of ACT program, positive changes were assured in the psychological flexibility including acceptance, cognitive defusion, being present, committed action and the increase of self-compassion and of recovery in alcoholics.

A Proton Therapy Center was established this year in National Cancer Center, Korea. We chose IBA of Belgium as the vendor of the equipment package. A 230 MeV fixed-energy cyclotron will deliver proton beams into two gantry rooms, one horizontal beam room, and one experimental station. The building for the equipment is currently under design with a special emphasis on radiation shielding. Installation of equipments is expected to begin in September next year starting with the first gantry, and the acceptance test will be performed about a year later. To generate therapeutic radiation fields the wobbling method will be a main treatment mode for the first gantry. A pencil beam scanning system on the other hand will be equipped for the second gantry relying on the availability at the time of installation. The beam scanning with intensity modulation adapted will be a most advanced form in radiation therapy known as IMPT. Some details on the project progress, scope of the system, and design of building are described.
Objectives : Mindfulness meditation has recently become a major component in mainstream modern cognitive behavioral therapy. The purpose of this study was to examine effects of a mindfulness meditation program on positive resources of outpatients with depressive disorder and anxiety disorder. Methods : Participants were 55 psychiatric clinic outpatients. Participants received eight weekly sessions in a mindfulness meditation training program, that was approximately 90 minutes each. Measures included the Positive Resources Test (POREST), Acceptance and Action Questionnaire-16 (AAQ-16), Life Satisfaction Expectancy Scales (LSES), and Subjective Happiness Scale (SHS). Variables were measured at two time points : pre- and post-implementation (eight weeks later). Results : Paired t-test results of participants before and after the mindfulness meditation program revealed statistically significant improvement in positive resources (t=-5.847, p<.001), acceptance (t=-4.090, p<.001), life satisfaction expectancy (t=-3.892, p<.001), but not in subjective happiness. Conclusion : Results suggest mindfulness meditation may be effective to enhance positive resources, acceptance, and life satisfaction in outpatients with depressive disorder and anxiety disorder. Mindfulness meditation may be a factor in improving psychological well-being and positive psychological features in outpatients with depressive disorder and anxiety disorder.

This study aims to identify the types and type-specific characteristics of subjectivity on complementary alternative therapy of nursing students using Q-methodology and to provide them as basic materials for nursing education. Q-methodology provides a scientific method for identifying perception structures that exist within certain individuals or groups. Thirty C college nursing students' classified 35 selected Q-statements into 9 points standard, according to the level of agreement or disagreement by forced normal distribution. The collected data was analyzed by using a QUNAL PC program. Resultingly, the attitude of nursing students regarding complementary alternative therapy was divided into three types. Type 1 is positive recognition. The students who think the therapy contributes to maintaining and improving health, and is a way to protect human body and improve welfare by enhancing immunity. Type 2 is objective acceptance. The students who think the complementary alternative therapy is effective when it is properly used for specific constitutions in discussion with medical teams after its effects are scientifically reviewed. Type 3 is popularity consideration. The students who put focus on its popular use and regard the therapy as very useful for not only patients but also ordinary healthy people because it is based on the natural and human approach. In conclusion, proposed are studies on influence factor of nursing students' awareness about the complementary alternative therapy and reviews on the effects of the complementary alternative therapy that is applied as nursing intervention.

Background Laryngeal allotransplantation (LA) is a technique involving transplantation of a deceased donor's larynx into a recipient, and it may be substituted for conventional laryngeal reconstruction. There are widely different views on LA, as the recipient is administered continuous, potentially life-threatening, immunosuppressive therapy for a functional or aesthetic result, which is not directly related to life extension. The purpose of this study was to analyze the difference in risk acceptance and expectations of LA between four population groups. Methods A survey was performed to examine patients' risk acceptance and expectations of LA. The survey included 287 subjects in total (general public, n=100; kidney transplant recipients, n=53; post-laryngectomy patients, n=34; doctors, n=100), using a Korean translated version of the louisville instrument for transplantation (LIFT) questionnaire. Results All four groups responded differently at various levels of their perception in risk acceptance and expectations. The kidney transplant recipients reported the highest risk acceptance and expectations, and the doctor group the lowest. Conclusions This study examined the disparate perception between specific population groups of the risks and benefits of using LA for the promotion of the quality of life. By addressing the information gaps about LA in the different populations that have been highlighted from this survey, we suggest that LA can become a more viable alternative to classical surgery with resultant improved quality of life for patients.
Image-guided radiation therapy (IGRT) is a process of incorporating imaging techniques such as computed tomography (CT), magnetic resonance imaging (MRI), Positron emission tomography (PET), and ultrasound (US) during radiation therapy (RT) to improve treatment accuracy. It allows real-time or near real-time visualization of anatomical information to ensure that the target is in its position as planned. In addition, changes in tumor volume and location due to organ motion during treatment can be also compensated. IGRT has been gaining popularity and acceptance rapidly in RT over the past 10 years, and many published data have been reported on prostate, bladder, head and neck, and gastrointestinal cancers. However, the role of IGRT in lymphoma management is not well defined as there are only very limited published data currently available. The scope of this paper is to review the current use of IGRT in the management of lymphoma. The technical and clinical aspects of IGRT, lymphoma imaging studies, the current role of IGRT in lymphoma management and future directions will be discussed.
Purpose: This study describes the effects of kicking a ball training on balance and upper limb function in chronic hemiplegia. Methods: Thirty chronic stroke patients were randomly selected; Kicking a ball (n=10), Treadmill gait (n=10), Stepping on the ground group (n=10). The Short Physical Performance Battery (SPPB) for balance and Foot Scan for weight distribution. Upper limb function was measured by the Manal Function Test (MFT). This treatment was performed five times a week for a total of 4 weeks. Results: Kicking group was significantly increased in SPPB (p<0.05) and MFT (p<0.05) between pretest vs post test and pretest vs follow-up. Conclusion: Kicking a ball was effective for improving weight acceptance on the paretic leg, balance and upper limb function. However, In terms of balance, the core stabilization that affected the upper extremity function was not evaluated. Thus, additional research may help determine correlation between core stability and upper limb function to improve balance.
The purpose of this study is to develop a program which aims at providing a better understanding of parenthood, establishing a desirable value of being a parent and preparing for being a good father positively affecting children, and determine effects of the program on unmarried men who are in the so-called stage of 'not belonged youth'. The program consisted of 5 sessions titled 'My father, 'I will be a father, too', 'Being a good father I', 'Being a good father II' and 'I will be a respectable father'. 7 unmarried men surveyed here were asked to participate in the program held at B church located in Busan from Oct. 3 to Nov. 7, 2004, with 1 session a week and 120 minutes per session based. Before and after the program, tests were carried out with the father acceptance-rejection scale and the scale of confidence in father's role'. For each of the sessions, a process evaluation was performed. Results of the study can be summarized as follows. First, the pre- and post-tests with the father acceptance-rejection scale showed some changed recognition that father's attitude of child raising would be more acceptable and unrejectable. This indicates that the program made some achievements in relation to understanding and reestablishing relations with father. Second, significant differences were found in confidence in father's role between before and after the program. Especially, anxiety about father's role as a sub-area of such confidence was most considerably reduced. In conclusion, such objectives of the program as removing vague anxiety about father's role, establishing a desirable view of fatherhood and improving confidence about being a father proved to be achievable through getting skills of child understanding with the concept of developmental perspective and narrative therapy based.
