뇌졸중 후 강직(spasticity) 평가를 위한 Tone Assessment Scale의 신뢰도 (Reliability of the Tone Assessment Scale for Poststroke Spasticity)

  • The purpose of this study was to establish reliability of the Tone Assessment Scale (TAS) translated into Korean in patients with stroke. The TAS consists of resting posture, response to passive movement, and associated reaction to active effort. Fifteen patients (14 males, 1 female) were examined by two raters. Surface electromyography (EMG) data at elbow flexor muscle and joint excursion were collected from 6 patients. To identify the correlation between muscle activity and angular changes of elbow muscle, Pearson product moment correlation was used. The inter-rater and intra-rater reliability of the TAS ranged from very good to good (K/Kw=.61~1.00 for intra-rater and K/Kw=.73~1.00 for intra-rater comparisons) in the sections of resting posture and associated reaction. However, in the section of response to passive movement, the reliability coefficients ranged from very good to fair (Kw=.29~1.00). In the 11th item, correlation between EMG ratio of elbow flexor and angular changes of elbow joint showed statistically strong positive relationship (r=.94, p<.05). These results indicate that the TAS is selectively reliable in the sections of resting posture and associated reaction.

Sanguisorba officinalis Inhibits Immediate-type Allergic Reactions

  • The effect of aqueous extract of Sanguisorba officinalis L. (Rosaceae) root (SOAE) on the immediate-type allergic reactions by anal therapy was investigated. SOAE (0.01 to 1 g/kg) dose-dependently inhibited systemic anaphylaxis induced by compound 48/80 in mice. When SOAE was pretreated at the same concentrations with systemic anaphylaxis, the plasma histamine levels were reduced in a dose-dependent manner. SOAE (0.1 and 1 g/kg) also significantly inhibited local anaphylaxis activated by anti-DNP IgE. SOAE (0.001 to 1 mg/ml) dose-dependently inhibited the histamine release from rat peritoneal mast cells (RPMC) activated by compound 48/80 or anti-DNP IgE. The level of cyclic AMP (cAMP) in RPMC, when SOAE (1 mg/ml) was added, transiently and significantly increased compared with that of basal cells. These results provide evidence that anal therapy of SOAE may be beneficial in the treatment of allergic diseases.

뇌졸중 환자의 균형, 기능적 보행, 시지각, 일상생활 평가도구의 상관성 (The Correlations between the Balance Test, functional movement, Visual Perception Test and Functional Independent Measure in Stroke Patients)

  • Purpose: The purpose of this study was to determine correlations between the Berg Balance Scale (BBS), Functional Reach Test (FRT), Timed Up & Go (TUG), Motor-Free Visual Perception Reach Test Vertical format (MVPT-V), Functional Independence Measure (FIM). Methods: The subjects used in this study were 35 stroke patients from Cheongju ST. Mary's hospital. Balance was measured by BBS, FRT. Functional mobility was measured by TUG. Visual perception was measured by MVPT-V. FIM was used to evaluate the activities of daily living. Data was analyzed using pearson product correlation. The TUG and MVPT-V index were analyzed by linear regression. Results: There was a statistically significant difference between FRT and BBS (r=0.89, p<0.01), FIM (r=0.74, p<0.05), MVPT-V (r=0.40, p<0.05), and TUG (r=-0.36, p<0.05). There was significant statistical differences between TUG and MVPT-V (r=-0.64, p<.01). However, statistically significant differences were observed between BBS and FIM (r=0.79, p<0.01). The visual close item of the MVPT-V showed the strongest variance in predicting TUG. Conclusion: The use of both quantitative and qualitive scales was shown to be a good measuring instrument for the classification of general clinical performances of stroke patients. In particular, the results suggest that the visual perception test may be able to predict functional locomotion in stroke patients.

만성 뇌졸중 환자의 자세 조절과 일상생활동작, 근긴장도, 그리고 기능증진과의 관계 (The Relationship Between Postural Control, ADL Function, Muscle Tone, and Functional Improvement in Chronic Stroke Patients)

  • The purpose of this study was to find any correlations among Postural Assessment Scale for Stroke (PASS), Modified Barthel Index (MBI), Tone Assessment Scale (TAS), Motor Assessment Scale-Gait (MAS-G), Fugl Meyer-Balance (FM-B), and to predict MBI from subscales of the PASS. The subjects were 41 stroke patients of the Korea National Rehabilitation Center in Seoul. The main outcome measures were postural control (PASS), gait (MAS-G), Balance (FM-B), Tone (TAS), ADL (MBI). The data was analyzed using the Pearson product correlation. PASS scale was used between other clinical and instrumental indexes, multiple stepwise regression analyses were performed to identify prognostic factors for ADL incline, and Cronbach's alpha coefficient was used to identify internal consistency on PASS scale. The results of this study areas follows: 1. The highest level was sitting without support, the lowest level was standing on paretic leg on PASS scale. The highest level was chair/bed transfer, the lowest level was bathing on MBI. 2. All items of the PASS, except postural tone were significantly correlated with Gait, Balance, MBI (p<.01), 3. The Internal Consistency (Cronbach's alpha coefficient=.85) was very high, indicating that the PASS is homogeneous and is likely to produce consistent response. Furthermore, the sums of maintaining position items and of changing-position items were strongly correlated (r=.64, p<.05) and there were significant correlations between sums of PASS, sums of maintaining position items (r=.87, p<.01), and changing-position items (r=.93, p<.01). 4. The standing without support of the PASS items was the strongest variance ($R^2$=.85) of the predicting ADL function. These findings provide strong evidence of the predictive value of the postural control on gait, Balance, ADL function in stroke patients and to can provide a reference for the successful therapeutic program and more improved functional recovery.

  • The purposes of this study were to determine correlations between the Berg Balance Test (BBS), Timed -UP & Go Test, Fugl Meyer-L/E, Balance, Sensory (FM-L/E, B, S), Motor Assessment Scale-Gait (MAS-G), Comfortable maximal Gait Speed (C MGS), and the Modified Barthel Index (MBI). Main outcome measures were Balance control (BBS, FM-B), Gait (TUG, C MGS, MAS-G), ADL (MBI) and Motor Function of Lower Extremities (FM-L/E, S). FM scales between other clinical and instrumental indexes and multiple stepwise regression analyses were performed to identify prognostic factors for Balance, Gait and ADL Motor Function of Lower Extremity inclinations. The results of this study were as follows: The BBS, FM-L/E, balance, sensory and MBI showed positive correlation relations, but TUG and C MGS showed negative correlations. The sensory factor of the FM-scale showed the strongest variance in predicting BBS. However the FM-balance showed the strongest variance in predicting TUG, MAS-G and C MGS. The use of both quantitative and qualitative scales was shown to be a good measuring instrument for the classification of the general clinical performance of the patients.

Neuroprotective Effect of Astersaponin I against Parkinson's Disease through Autophagy Induction

  • An active compound, triterpene saponin, astersaponin I (AKNS-2) was isolated from Aster koraiensis Nakai (AKNS) and the autophagy activation and neuroprotective effect was investigated on in vitro and in vivo Parkinson's disease (PD) models. The autophagy-regulating effect of AKNS-2 was monitored by analyzing the expression of autophagy-related protein markers in SH-SY5Y cells using Western blot and fluorescent protein quenching assays. The neuroprotection of AKNS-2 was tested by using a 1-methyl-4-phenyl-2,3-dihydropyridium ion (MPP+)-induced in vitro PD model in SH-SY5Y cells and an MPTP-induced in vivo PD model in mice. The compound-treated SH-SY5Y cells not only showed enhanced microtubule-associated protein 1A/1B-light chain 3-II (LC3-II) and decreased sequestosome 1 (p62) expression but also showed increased phosphorylated extracellular signal-regulated kinases (p-Erk), phosphorylated AMP-activated protein kinase (p-AMPK) and phosphorylated unc-51-like kinase (p-ULK) and decreased phosphorylated mammalian target of rapamycin (p-mTOR) expression. AKNS-2-activated autophagy could be inhibited by the Erk inhibitor U0126 and by AMPK siRNA. In the MPP+-induced in vitro PD model, AKNS-2 reversed the reduced cell viability and tyrosine hydroxylase (TH) levels and reduced the induced α-synuclein level. In an MPTP-induced in vivo PD model, AKNS-2 improved mice behavioral performance, and it restored dopamine synthesis and TH and α-synuclein expression in mouse brain tissues. Consistently, AKNS-2 also modulated the expressions of autophagy related markers in mouse brain tissue. Thus, AKNS-2 upregulates autophagy by activating the Erk/mTOR and AMPK/mTOR pathways. AKNS-2 exerts its neuroprotective effect through autophagy activation and may serve as a potential candidate for PD therapy.

요통환자의 천장관절에 대한 도수교정 전과 후의 골반경사 비교 (A Comparison of Pelvic Tilt Before and After Manipulation of Sacroiliac Joint in the Patients with Low Back Pain)

  • The purposes of this study were to compare pelvic tilt before and after manipulation of sacroiliac joint in 31 low back pain patients (11 males, 20 females) with sacroiliac joint dysfunction. The sacroiliac joint of patients was manipulated on the side of anterior pelvic tilt, using the technique described by Stoddard (1962) and Greenmann (1996). I used this technique because it usually eliminated sacroiliac joint dysfunction in one treatment session. SPSS for window computer system was used to analyze the data. Also t-test was performed for comparison of pelvic tilt angle before and after manipulation, and Pearson product-moment correlation analysis was performed for intratester reliability for measurements of pelvic tilt angle before and after manipulation. The result were as follows: 1. Intratester reliability was good for measures of pelvic tilt (r=.98). 2. The pelvic tilt after manipulation was significantly decreased (mean=$3.40^{\circ}$) compared with the pelvic tilt before manipulation (p=.001). All subjects showed asymmetrical right versus left pelvic tilt before manipulation. 40% of subjects showed decreased asymmetrical right versus left pelvic tilt after manipulation, and 60% of subjects showed symmetrical right versus left pelvic tilt after manipulation. I think that pelvic tilt asymmetry with hypomobility due to loss of joint play could be symmetrized by manipulation or mobilization, but pelvic tilt asymmetry with unilateral pelvic muscle shortening could not be symmetrized by manipulation or mobilization without relaxation and stretching of shortened muscles.

시열반응과제의 운동학습이 대뇌피질 활성화의 변화에 미치는 영향 (Changes of Cortical Activation Pattern Induced by Motor Learning with Serial Reaction Time Task)

  • Purpose: Numerous investigators demonstrated that adaptative changes were induced by motor skill acquisition in the central nervous system. We investigated the changes of neuroelectric potential following motor learning with serial reaction time task in young healthy subjects, using electroencephalography (EEG). Methods: Twelve right-handed normal volunteers were recruited, who have no history of neurological dysfunction and were given to written the informed consent. All subjects were assigned to flex to extend the wrist joint or flex the thumb for pressing the matched button as quickly and accurately as possible, when one of five colored lights was displayed on computer screen (red, yellow, green, blue, white). EEG was measured, whenfive types simulations ware presented randomly with equal probabilities of 20% in total 200 times at the pre and post test. And they were scheduled for 30 minutes practice session during two consecutive days in the laboratory. Results: The results showed that the reaction time at the post test was significantly reduced, compared to one of the pre test in serial reaction time task. In EEG map analysis, the broaden bilateral activation tended to be changed to the focused contralateral activation in the frontoparietal area. Conclusion: These findings showed that acquisition of motor skill led to product more fast motor execution, and that motor learning could change cortical activation pattern, from the broaden bilateral activation to the focused contralateral activation. Thus we concluded that the adaptative change was induced by motor learning in healthy subjects.

Adverse drug reaction monitoring during antimicrobial therapy for septicemia patients at a university hospital in New Delhi

  • Background/Aims: Adverse drug reaction (ADR) is an appreciably harmful or unpleasant reaction, resulting from an intervention related to the use of a medicinal product. The present study was conducted in order to monitor the frequency and severity of ADR during antimicrobial therapy of septicemia. Methods: A prospective, observational, and noncomparative study was conducted over a period of 6 months on patients of septicemia admitted at a university hospital. Naranjo algorithm scale was used for causality assessment. Severity assessment was done by Hartwig severity scale. Results: ADRs in selected hospitalized patients of septicemia was found to be in 26.5% of the study population. During the study period, 12 ADRs were confirmed occurring in 9, out of 34 admitted patients. Pediatric patients experienced maximum ADRs, 44.4%. Females experienced a significantly higher incidence of ADRs, 66.7%. According to Naranjo's probability scale, 8.3% of ADRs were found to be definite, 58.3% as probable, and 33.3% as possible. A higher proportion of these ADRs, 66.7% were preventable in nature. Severity assessment showed that more than half of ADRs were moderate. Teicoplanin was found to be the commonest antimicrobial agent associated with ADRs, followed by gemifloxacin and ofloxacin. Conclusions: The incidence and severity of ADRs observed in the present study was substantially high indicating the need of extra vigilant during the antimicrobial therapy of septicemia.

The Camellia sinensis Inhalation Effects of Pulmonary Structure Protection and Anti-oxidants Enzyme from Cigarette Smoking

  • Cigarette smoke causes atypical structure of pulmonary and oxidative damage. Therefore, we carried out to determine if exposure to cigarette smoke alters pulmonary structure and anti-oxidant related enzyme in a animal model, when natural product extracts using by Nebulizer. The rat were divided into four groups: $H_2O-treated$ (Control), natural product (Camellia sinensis) extracts-treated (CS), natural product extracts-treated with cigarette smoke-exposed (CS+SM) and cigarette smoke-expose (SM). All groups are similar to Control group in weight, but SM group is lower than the other groups. Microscopic image of the pulmonary structure in SM group showed deleterious alterations in the morphology, but the other groups are maintained in normal structure. In anti-oxidant related enzymes, SOD (superoxide dismutase) and catalase, SM group represents the lowest enzyme activity among all groups. But G6PD (glucose-6-phosphate dehydrogenase) and LPO (lipid peroxidation) is SM group represents the highest enzyme activity among all groups. These result indicate that the natural product extracts is an efficient tissue protective substance against smoke-induced lung injury.
