Affinity Maturation of an Anti-Hepatitis B Virus PreS1 Humanized Antibody by Phage Display

  • In a previous study we generated an anti-Hepatitis B Virus (HBV) preS1 humanized antibody (HzKR127) that showed in vivo HBV-neutralizing activity in chimpanzees. However, the antigen-binding affinity of the humanized antibody may not be sufficient for clinical use and thus affinity maturation is required for better therapeutic efficacy. In this study, phage display technique was employed to increase the affinity of HzKR127. All six amino acid residues (Glu95-Tyr96-Asp97-Glu98-Ala99-Tyr100) in the heavy (H) chain complementary-determining region 3 (HCDR3) of HzKR127 were randomized and phage-displayed single chain Fv (scFv) library was constructed. After three rounds of panning, 12 different clones exhibiting higher antigen-binding activity than the wild type ScFv were selected and their antigen-binding specificity for the preS1 confirmed. Subsequently, five ScFv clones were converted to whole IgG and subjected to affinity determination. The results showed that two clones (B3 and A19) exhibited an approximately 6 fold higher affinities than that of HzKR127. The affinity-matured humanized antibodies may be useful in anti-HBV immunotherapy.

THE CLIMICAL EFFICACY OF CHYMORAL IN ORAL SURGERY (구강외과 영역에 있어 Chymoral 임상응용 치험예)

  • The authors have reported on the evaluation of remarkable therapeutic results from the use of chymoral against postextracted and post operative swelling, pain, 2nd hemorrhage and muscle spasm. The drug was administered to 8 tablets per day in the oral method and to 47 cases of 9 different kinds of oral diseases. The following are the results of the clinical efficacy of the drugs. 1) As results of the use of chymoral preparation, marked efficacy was observed in 30 cases(70%), moderate efficacy in 14 cases(24%), total efficacy in 47 cases (94%) and obscure efficacy in 3 cases(6%). 2) Postoperative infection by extraction and its complications were nearly all cured within a short time by use of chymoral. 3) The comparatively satisfactory results obtained from the use of antibiotic drugs (especially, penbritin or tetracycline) combined with chymoral.

Nursing Frequency, Nursing Time, and Nursing Intervention Priorities depending on Neonatal Therapeutic Hypothermia Methods (신생아 저체온치료방법에 따른 간호활동 빈도와 간호시간 및 간호중재)

  • Purpose: This study compared nursing frequency, nursing time, and nursing intervention priorities depending on the method of neonatal induced hypothermia. Methods: We observed 15 neonatal subjects receiving therapeutic hypothermia for 3 days each. Forty-five nurses experienced with nursing neonatal patients under therapeutic hypothermia provided responses about nursing intervention priorities. Analyses with the chi-square, the Fisher exact test, the paired t-test, the Wilcoxon signed rank test, and the Wilcoxon rank-sum test were performed on the data using SAS version 9.4. Results: The frequency of nursing activities was higher for selective head therapeutic hypothermia (SHTH) than for systemic therapeutic hypothermia (STH), and nursing time was also significantly longer. In terms of nursing intervention priorities, there were priority differences in "risk for ineffective thermoregulation" and "risks for impaired skin integrity" for SHTH compared to STH. Conclusion: Since SHTH for neonatal therapeutic hypothermia requires more nursing time and frequent nursing activities than STH, STH is therefore recommended if the therapeutic efficacy is similar. Appropriate nursing personnel should be allocated for neonatal SHTH nursing. Nurses should be aware of nursing interventions for therapeutic hypothermia as the priorities are different for different methods of neonatal therapeutic hypothermia.

Injectable hydrogels delivering therapeutic agents for disease treatment and tissue engineering

  • Background: Injectable hydrogels have been extensively researched for the use as scaffolds or as carriers of therapeutic agents such as drugs, cells, proteins, and bioactive molecules in the treatment of diseases and cancers and the repair and regeneration of tissues. It is because they have the injectability with minimal invasiveness and usability for irregularly shaped sites, in addition to typical advantages of conventional hydrogels such as biocompatibility, permeability to oxygen and nutrient, properties similar to the characteristics of the native extracellular matrix, and porous structure allowing therapeutic agents to be loaded. Main body: In this article, recent studies of injectable hydrogel systems applicable for therapeutic agent delivery, disease/cancer therapy, and tissue engineering have reviewed in terms of the various factors physically and chemically contributing to sol-gel transition via which gels have been formed. The various factors are as follows: several different non-covalent interactions resulting in physical crosslinking (the electrostatic interactions (e.g., the ionic and hydrogen bonds), hydrophobic interactions, ${\pi}$-interactions, and van der Waals forces), in-situ chemical reactions inducing chemical crosslinking (the Diels Alder click reactions, Michael reactions, Schiff base reactions, or enzyme-or photo-mediated reactions), and external stimuli (temperatures, pHs, lights, electric/magnetic fields, ultrasounds, or biomolecular species (e.g., enzyme)). Finally, their applications with accompanying therapeutic agents and notable properties used were reviewed as well. Conclusion: Injectable hydrogels, of which network morphology and properties could be tuned, have shown to control the load and release of therapeutic agents, consequently producing significant therapeutic efficacy. Accordingly, they are believed to be successful and promising biomaterials as scaffolds and carriers of therapeutic agents for disease and cancer therapy and tissue engineering.

Study of Recognition and Necessity of Oriental Medicinal Menu, and Development of Oriental Therapeutic Menu for Stroke Patients (뇌졸중 환자에서 한방치료식의 인지도 및 요구도 조사와 문헌고찰을 통한 한방 치료식단(藥膳) 개발)

  • The purpose of this study was to develop the oriental therapeutic menu(Yak-sun) through the recognition and necessity of oriental medicine diet in stroke patients admitted to K Oriental Medical Hospital. From May to July 2007, the 73 stroke patients were surveyed for the recognition and necessity of oriental therapeutic menu. The oriental therapeutic menu was developed on the basis of literature of oriental medicine books and research papers. The developed two week cycle of menus were then reviewed by the experts and specialists. The sensory evaluation and nutritional analysis of the menu were followed. The stroke patients who participated in the survey did not know about the oriental medical diet, however, 61.6% of the patients had interest. Most of the patients expected the efficacy of oriental therapeutic diets. Total of forty three medical herbs and ten herb-combined prescriptions were used in the menu. The daily average calorie of developed menu was 2,031 $\pm$ 57.6 kcal and the most of nutrients met Dietary Reference Intakes for Korean(KDRIs). The caloric ratios of carbohydrates, protein, and lipid were 61.3%, 18.7%, 20.0%, respectively. The average values of cholesterol(194 $\pm$ 57.5 mg/day), sodium(3,078 $\pm$ 678.5 mg/day), calcium(1,099 $\pm$ 140.1 mg/day), and dietary fiber(33 $\pm$ 7.0 g/day) were met Korean dietary guide line for stroke patients. Oriental therapeutic diet might be unfamiliar to the hospital patients even though they had interest in and favorable thought about it. Implements of oriental therapeutic diet to the hospital patients might be useful to improve the quality of service in the hospitals.

  • Purpose: The purpose of this study was to identify effects of a self-management program on self-efficacy and compliance in patients with CHF. Hypothesis: 1) Patients with CHF who are provided with a self-management program will show higher self-efficacy scores than a control group. 2) Patients who are provided with a self-management program will show higher compliance scores than a control group. Method: This study was designed as a nonequivalent non-synchronized pre-posttest control group. There were eight patients in the experimental group, and twelve in the control group. According to NYHA classification, all patients belonged under the classesII to IV. Data were collected using the instruments developed by the researchers. Data were analyzed using descriptive statistics and Mann Whitney U test. Result: There were significant differences in self-efficacy scores and compliance scores between the experimental and control group. Conclusion: By utilizing the program, patients were able to monitor their symptoms routinely, comply with therapeutic regimen, and feel better able to positively influence their disease. Therefore, better compliance means fewer readmissions of patients with CHF.

The Efficacy of Opuntia ficus-indica for the Treatment of Chronic Otitis Externa in Dogs (백년초 추출물(Opuntia ficus-indica)을 이용한 개의 만성 외이도염 치료 효과)

  • To determine the antimicrobial and anti-inflammatory efficacy of Opuntia ficus-indica for chronic otitis, we evaluated the effects of topical applications of the methanol extracts with 20 cases of dogs which had chronic malignant otitis by pathogens with antibiotic resistance. The dogs had revealed recurrent symptoms of malignant otitis and were not treated by conventional therapeutic agents. However, in this study, the clinical cure rates of Opuntia ficus-indica was 75% and the average alleviation period was $1.21{\pm}0.42$ week, and the mean recovery period was $1.06{\pm}1.06$ week after the initiation of treatment. As the results of this study, topical Opuntia ficus-indica extracts was found to be highly effective for the treatment of chronic malignant otitis with clinical cure rates of 75% within 1.06 weeks of therapy. Further evaluation of Opuntia ficus-indica extracts will allow us to establish and to optimize the therapeutic strategy for the malignant otitis in veterinary practice, and the potential usefulness of this complementary treatment on recurrent infectious pathology.
