Mediating Effects of Resilience on the Relationship between Stress and Therapeutic Compliance in Patients with Systemic Lupus Erythematosus (루푸스 환자의 스트레스와 치료지시이행의 관계에 미치는 극복력의 매개효과)

  • Purpose: This study was conducted to explore the mediating effect of resilience in the relationship between stress and therapeutic compliance in patients with systemic lupus erythematosus. Methods: The subject (N=145) was a systemic lupus erythematosus patient who visited the D Hospital in B city. Data collection was conducted from January 2016 to August 2016. Twenty-five resilience measurement tools, 20 stress measurement tools, 39 therapeutic compliance measurement tools used to measure resilience, stress and therapeutic compliance levels. The collected data were analyzed using descriptive analysis, Pearson correlation, 3-step regression analysis of Baron and Kenny, and Sobel test in SPSS/WIN 21.0. Results: There was a significant negative correlation between stress and therapeutic compliance, a significant negative correlation between stress and resilience, and significant positive correlation between resilience and therapeutic compliance. Resilience showed a direct effect on therapeutic compliance through stress as a mediating variable. Conclusion: It is necessary to develop appropriate resilience enhancement program to improve therapeutic compliance with systemic lupus erythematosus.

The Influencing Factors of the Compliance Level with Therapeutic Regimen after the Bone Mineral Densitometry (골밀도 검사 후 치료지시 이행정도에 영향을 미치는 요인)

  • Purpose: This study was to identify the influencing factors of the compliance level to a therapeutic regimen after a bone mineral densitometry test. Method: The sample for the study was 95 people who took the bone mineral densitometry test from March, 2002 to July, 2002. Data was collected by mail using aself reporting questionnaire on the selected variables such as the compliance level, self efficacy, health locus of control, susceptibility, severity, usefulness, barrier, and self esteem. Results: The average compliance level was 63.93. Through multiple regression, three independent variables including chance health locus of control on personality, the result of bone mineral density and self-efficacy were entered in the model as the significant determinants of the compliance level after a bone mineral densitometry test. The coefficients of determination of each variable were 10.9%, 8.3% and 8.1% respectively. Conclusion: The identification of the determinants of the compliance level to the therapeutic regimen after bone mineral densitometry is expected to contribute to the development of an intervention program to improve the compliance level to the therapeutic regimen in osteoporosis patients.

A Study of the Relationship between Compliance with Therapeutic Regimens and Physiological Parameters of Hemodialysis Patients (혈액투석 환자의 치료지시 이행정도와 생리적 지표와의 관계)

  • Purpose: This study was done to investigate correlations between compliance and physiological parameters of hemodialysis patients. Method: The subjects were 102 patients on hemodialysis at 3 hospitals in B city. Data was collected using Shon(l986)'s questionnaire and measuring physiological parameters (serum urea nitrogen, creatinine, hemoglobin, albumin, potassium, phosphorus, interdialytic weight gain). Result: Mean scores of compliance with the therapeutic regimen was $4.00\pm$ 0.55 on a 5 point scale. The area of visiting hospitals and taking medicines . were shown to have high compliance with therapeutic regimens; on the other hand, the areas concerning diet and symptoms were shown to be low. Interdialytic weight gain and phosphorus were significantly related to the compliance with therapeutic regimens. Conclusion: Hemodialysis patients' therapeutic compliance was related to the physiological parameters(potassium, phosphorus, interdialytic weight gain). Therefore, these findings give hemodialysis patients useful information for raising their therapeutic compliance.

Effects of Self-efficacy and Transplant-related Knowledge on Compliance with a Therapeutic Regimen for Recipients of Liver Transplant (간이식 수혜자의 자기효능감과 이식 관련 지식이 치료지시이행에 미치는 영향)

  • Purpose: This study was done to investigate the degree of self-efficacy, transplant-related knowledge, and other factors affecting compliance with a therapeutic regimen for liver transplant recipients. Methods: Participants were 140 patients who had received a liver transplant at a tertiary hospital in Y City, Gyeongnam Province and made regular hospital visits as outpatients. A self-report questionnaire was used to collect the data and collection was done from December 4, 2017 to January 26, 2018. Data were analyzed using frequencies, percentages, means, and standard deviations, and t-test, ANOVA, Pearson's correlation coefficients. Multiple linear regression was performed using SPSS/WIN 21.0 program. Results: Participants scored $113.29{\pm}20.95$ (out of 150) on self-efficacy, $16.38{\pm}3.62$ (out of 18) on transplant-related knowledge, and $148.30{\pm}31.06$ (out of 200) on compliance with the therapeutic regimen. Analyzed of correlations among participant's self-efficacy, transplant-related knowledge, and compliance with the therapeutic regimen showed a significant positive correlation between self-efficacy and compliance with the therapeutic regimen (r=.64, p=.001), but no significant correlations were found between self-efficacy and transplant-related knowledge (r=-.01, p=.912) or between transplant-related knowledge and compliance with the therapeutic regimen (r=.06, p=.458). Multiple regression analysis showed that factors affecting compliance with the therapeutic regimen were state of re-transplantation (${\beta}=.17$, p=.016) and self-efficacy (${\beta}=.53$, p=.001). Conclusion: There is a need to apply a differentiated nursing intervention program considering the differences in patients' self-efficacy, transplant-related knowledge, and compliance with the therapeutic regimen.

Factors Related to Therapeutic Compliance of Hypertensive Patients in Small and Medium Scale Industry (중소규모 사업장 고혈압환자의 치료순응과 관련요인)

  • In order to evaluate the factors related to therapeutic compliance of hypertensive patients in small and medium scale industry, the questionnaire survey and the blood pressure measurement were performed on 138 hypertensive patients who were reported to have C or $D_2$ result of hypertension at the workers' periodic health examination from March to November 2001. The contents of questionnaire included the informations of factors related to therapeutic compliance of hypertensive patients such as, age, sex, marital status, income, education levels, scale, occupation, work duration, smoking and drinking habits, exercise, family history, stress and A type personality, employer's concern, organizational culture, health status, awareness, knowledge and attitude toward the needs of hypertension treatment, and experience of health education. The results of the study were as follows: 1. The proportion of those who were compliant to the treatment of hypertension was 41.3% of subjects. Among small scale industry less than 50 employees, the rate of therapeutic compliance group was 27.5% and therapeutic noncompliance group was 72.5%. Among medium scale industry more than 50 employees, the rate of therapeutic compliance group was 60.3% and therapeutic noncompliance group was 39.7%. 2. Among therapeutic noncompliance group, 95.1% of patients were aware of the fact that workers have hypertension, and 77.8% of patients got 6-10 marks of hypertension related knowledge. For health education, 27.2% were experienced and 34.6% said no intention to participate. And for hypertension treatment, 9.9% said no need to get the treatment and 44.4% said have no idea whether get treatment or not. 3. The significant factors related to therapeutic compliance of hypertensive patients in small scale industry were work duration, A type personality(anger), health status, attitude toward the needs of hypertension treatment, and experience of health education. 4. The significant factors related to therapeutic compliance of hypertensive patients in medium scale industry were age, occupation, subjective symptom, attitude toward the needs of hypertension treatment, and experience of health education. In consideration of above findings, it was suggested that in order to improve the therapeutic compliance in small and medium scale industry hypertensives, it be necessary to change attitude, perception, knowledge and treatment of hypertension by various methods such as effective health education and individually consulting programs by occupational health professional.

Therapeutic Compliance and Its Related Factors of Lung Cancer Patients (폐암환자의 치료순응도와 관련요인)

  • Objectives : To investigate the therapeutic compliance and its related factors in lung cancer patients. Methods : The subjects of this study comprised 277 patients first diagnosed with lung cancer at Kyungpook National University Hospital between Jan 1999 and Sept 1999. Of these, 141(50.9%) participated in the study by properly replying to structured questionnaires. The data was analyzed using a simplified Health Decision Model. This model includes categories of variables covering therapeutic compliance, health beliefs, patient preferences, knowledge and experience, social interaction, sociodemographic and clinical characteristics. Results : The therapeutic compliance rate of the 141 study subjects was 78.0%. An analysis of health beliefs and patient preferences revealed health concern (p<0.05), dependency on medicine (p<0.05), perceived susceptibility and severity (p<0.05) as well as preferred treatment (p<0.01) as factors related to therapeutic compliance. Factors from the sociodemographic characteristics and clinical factors that were related to therapeutic compliance were age (p<0.01), monthly income (p<0.05), histological type (p<0.05) and clinical stage (p<0.05) of cancer. Conclusions : In order to improve therapeutic compliance in lung cancer patients it is necessary to educate the aged, low-income patients, or patients who have small cell lung cancer or lune cancer of an advanced stage for which surgery is not indicated. Additionally, it is essential for medical personnel to have a deep concern about patients who have poor lifestyles, a low dependency on medicine, or a high perceived susceptibility and severity. Practically, early diagnosis of lung cancer and thoughtful considerations of low-income patients are important. By means of population-based education in a community, we may promote attention to health and enhance the early diagnosis of lung cancer.

  • Objective: This study was conducted in order to investigate the effects of individual patient behavior regarding the health and medical care level on their therapeutic compliance for people diagnosed with high blood pressure by a physician, in order to evaluate the impact of treatment compliance. In this study, the therapeutic compliance was defined as the hypertension drug cure rate. The current blood pressure controls were persons about 30 years of age diagnosed with high blood pressure by their doctor one month fraction of the people who take more than 20 days (30 days). Methods: The data was analyzed by using the X2-test for different comparisons of the therapeutic compliance in the individual characteristics and using a two-stage multilevel logistic regression to identify the community variance of the related index of high blood pressure therapeutic compliance using the data from 229 adults over the age of 19 in a community health survey conducted in 2010. The Spss 18.0 statistical program was used with HLM 7 (hierarchical linear model) Results: It was found that the rate of therapeutic compliance was affected by the individual health behavior and health and medical care levels. Correlation between Knowledge of Hepatitis C and Compliance with therapeutic guidelines in Chronic Hepatitis C Patients (만성 C형간염환자의 질병에 대한 지식과 치료지시이행 간의 관계)

  • Purposes: The purpose of the study was to evaluate knowledge of hepatitis C and compliance with therapeutic guidelines and their correlation. Methods: Participants included a total of 197 subjects with chronic hepatitis C from 4 general hospitals. Subjects were asked 25 items of knowledge on hepatitis C and 17 items of compliance with therapeutic guidelines. The collected data was analyzed for frequency, percentage, average, standard deviation, t-test, ANOVA and Pearson's correlation coefficient. with the help of SPSS 21.0 program. Results: The subject's knowledge on chronic hepatitis C expressed as percentage was 67.1%. Compliance of the research participants gained $3.96({\pm}0.76)$ points in general on a 5-point scale. Learned compliance was higher when the patients were 45 years old and over. female and with spouse. There was a statistically significantly positive correlation between knowledge and compliances. Conclusions: Development of educational programs requires consideration of patient characteristics, particularly education on the male patients under 45 years of age.

  • Compared to other variables being considered in therapeutics, patient compliance has long been given minor attention although it affects every aspect of medical care; Limited methodology of compliance measurement, in particular, has hampered major progress in research, and pre-conceptions have been reiterated. However, there is a recent surge in interest derived from new data revealed by reliable methods, i.g. continuous medication (compliance) monitoring. The visualization of dynamics in drug regimen compliance over time offers unique opportunities, both to scientific drug evaluation and therapeutics in medical practice. New perspectives related to the descriptive and explanatory side of the problem are outlined by giving examples from various therapeutic fields.

  • Objectives : Generally, it seemed that the therapeutic result in diabetic patients was changed by compliance. This study was conducted on the basis of assumption that the therapeutic result id diabetic patients could control according to compliance. This study was conducted to analyze the related factors in association with compliance to drug, diet and exercise therapy. Methods : 224 diabetic patients in Kyungpook National University Hospital were selected through the interviews and HbA1c values from 1 Jan. to 28 Feb.1997. The drug compliance was tested by regularity of drug administration, the diet compliance was tested by restriction of food, exactly allocation, balance of nutrient, measuring food and the exercise compliance was tested by regularity of exercise per day. We assessed compliance by percentage, $x^2-test$ and generalized logit regression model(method:enter). Results : The significant variable was the satisfaction to medical personnels in drug, the knowledge to disease in diet, the participation of the diabetic education in exercise therapy and the satisfaction to medical personnels in HbA1c. Using the generalized logit model(method : enter) in compliance change, the significant variables were the satisfaction to medical personnels and the complication in drug; the significant variables were the age at the first diagnosis, the family history, the concern of health, the knowledge of disease, the self-exertion for therapy and the complication in diet: the only significant variable was the gender in exercise therapy. Conclusions : The degree of glycemic control in diabetic patients was influenced by compliance. In order to improve patient's compliance, we must foster the knowledge on the diseases, lead participation for diabetic education. Because the satisfaction to medical personnels was the important variables, we must build up good relationship between doctors and patients.
