Plant-based production of therapeutic antibodies (식물기반 치료용 항체생산)

  • Antibodies are powerful and versatile tools to play a critical role in the diagnosis and treatment of many diseases. Their application has been enhanced significantly with the advanced recombinant DNA and heterologonous expression technologies, allowing to produce immunotherapeutic proteins with improved biofunctional properties. However, with currently available technologies, mammalian cell-based therapeutic antibody production, as an alternative for production in humans and animals, is often not plentiful for passive immunotherapeutics in treatment of many diseases. Recently, plant expression systems for therapeutic antibodies have become well-established. Thus, plants have been considered to provide an attractive alternative production system for therapeutic antibodies, as plants have several advantages such as the lack of human pathogens, and low cost of upstream production and flexible scale-up of highly valuable recombinant glycoproteins. Recent advances in modification of posttranslational processing for human-like glycosylation in transgenic plants will make it possible that plant can become a suitable protein expression system over the animal cellbased current production system. This review will discuss recent advances in plant expression technology and issues for their application to therapeutic antibody production.

Effects of Therapeutic Exercise on Pain, Range of Motion, Strength and Balance Ability in a Patient with Anterior Ankle Impingement: A Case Study (치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

A Study on Design and Application of Tissue Compensator for 6MV X-rays (6MV X-선에 대한 조직 보상체의 제작 및 응용에 관한 연구)

  • A radiation beam incident on an irregular or sloping surface produces the non-uniformity of absorded dose. The use of a tissue compensator can partially correct this dose inhomogeneity. The tissue compensator is designed based on the patient's three dimensional contour. After required compensator thickness was determined according to tissue deficit at $25cm\pm25cm$ field size, 10cm depth for 6MV x-rays, tissue deficit was mapped by isoheight technique using laser beam system. Compensator was constructed along the designed model using 0.8mm lead sheet or 5mm acryl plate. Dosimetric verification were peformed by film dosimetry using humanoid phantom. Dosimetric measurements were normalized to central axis full phantom readings for both compensated and non-compensated field. Without compensation, the percent differences in absorbed dose ranged as high as $12.1\%$ along transverse axis, $10.8\%$ along vertical axis. With the tissue compensators in place, the difference was reduced to $0\~43\%$ Therefore, it can be concluded that the compensator system constructed by isoheihnt technique can produce good dose distribution with acceptible inhomogeneity, and such compensator system can be effectively applied to clinical radiotherapy.

Neutron Therapy of Unresectable and Recurrent Rectal Cancer (수술불능 및 재발성 직장암에 대한 중성자선 치료)

  • Total of 53 patients of unresectable and recurrent rectal cancer treated with neutron beam during the period from Oct.1987 to Apr.1992 were analyzed. Dose fractionation for the neutron only group was 1.5 Gy per fraction,3 fraction per week,21 Gy/41/2 wks for 42 patients out of 53 ($76{\%}$). Neutron only but modified fractionation schedule ($10{\%}$ more or less of total dose) was applied for 9 patients, and mixed beam (neutron boost) was for 4 patients, Complete tumor response was obtained in 40 patients ($76{\%}$, response rate). Local control rate was 28 out of 53 ($53{\%}$). Statistically significant better prognostic factors for local control were age below 49 years old (15/22, $68{\%}$) than above 50 years old (13/31, $42{\%}$), male (20/32, $63{\%}$) than female (8/21, $38{\%}$), tumor size less than 5 cm and non-metastatic (16/24, $67{\%}$) than size more than 5 cm or metastatic (12/29, $41{\%}$). Major complication had developed in 7 patients ($13{\%}$). Two year overall survival rate by Kaplan-Meier method was $30{\%}$, but it was rised to, $47{\%}$ when the turner was less than 5 cm non-metastatic.

Comparative observational study of aminophylline with prophylactic and therapeutic uses for clinical outcomes in preterm infants

  • Objectives: Aminophylline has been used for prevention or treatment of apnea in preterm infants with idiopathic apnea of prematurity. The aim of this study was to assess the clinical usefulness of prophylactic in comparison with therapeutic aminophylline therapy. Methods: This retrospective observational study included infants born with a birth weight of < 2,500 g or at < 36 weeks of gestation. Infants born between August 2013 and July 2014 who received aminophylline therapy within 24 hr after birth were assigned to the prophylactic group, while infants born between August 2014 and July 2015 who received aminophylline therapy after obvious apnea were assigned to the therapeutic group. We compared clinical characteristics, including days of ventilator and oxygen therapy and bronchopulmonary dysplasia (BPD) between both groups. Results: Sixty-four patients and 25 infants were identified in the prophylactic and therapeutic groups, respectively. The mean gestational age and birth weight were $32.57{\pm}1.96weeks$ and $1765{\pm}205g$, respectively, in the prophylactic group and $32.46{\pm}1.82weeks$ and $1770{\pm}250g$, respectively, in the therapeutic group. No significant differences in clinical characteristics were found between the two groups. Similar clinical outcomes, including days of ventilator and oxygen therapy, intraventricular hemorrhage (IVH), periventricular leukomalacia, and BPD, were observed between the two groups. Conclusions: The present study showed that the prophylactic use of aminophylline does not improve the clinical outcomes, including BPD, IVH, and ventilator dependency as compared with therapeutic use. In other words, routine prophylactic use of aminophylline is unnecessary.

Exterior Environments for the Elderly with Dementia in the U.S.A. (치매노인을 위한 시설의 옥외공간에 관한 연구 -미국의 사례를 중심으로-)

  • The purpose of this study is to understand the importance of exterior environments and to identify the design guidelines for the therapeutic garden for cognitively impaired seniors with Alzheimer's disease and related dementias. The data was collected from 4 Adult Day Care Centers, 2 Nursing Homes, & 1 Assisted living in the U.S.A. from 2001 to 2002 by interview and the floor plan about the outdoor space. Although some may believe that patients with Alzheimer's disease and related disorders are unresponsive to environment, evidence shows that environments especially designed for cognitively impaired seniors can maintain or increase their level of functioning. The use of specially designed exterior environments nay have in reducing incidents of aggressive behavior, and contributing significantly to a wanderer management program. The checklist includes 6 dimensions: safety environment, various sensual stimulations, social interactions, wandering, privacy, and support orientation and reality. The results indicated that the exterior environment have to support wandering behavior, orientation and reality.

  • It has been known that ATM plays a central role in response of cells to ionizing radiation by enhancing DNA repair. We have investigated the feasibility of increasing radiosensitivity of tumor cells with the use of ATM inhibitors such as caffeine, pentoxifylline and wortmannin. Human colorecta1 cancer RKO.C cells and RKO-ATM cells (RKO cells overexpressing ATM) were used in the present study. (omitted)
