Therapeutic Compliance and Its Related Factors of Lung Cancer Patients (폐암환자의 치료순응도와 관련요인)

  • Objectives : To investigate the therapeutic compliance and its related factors in lung cancer patients. Methods : The subjects of this study comprised 277 patients first diagnosed with lung cancer at Kyungpook National University Hospital between Jan 1999 and Sept 1999. Of these, 141(50.9%) participated in the study by properly replying to structured questionnaires. The data was analyzed using a simplified Health Decision Model. This model includes categories of variables covering therapeutic compliance, health beliefs, patient preferences, knowledge and experience, social interaction, sociodemographic and clinical characteristics. Results : The therapeutic compliance rate of the 141 study subjects was 78.0%. An analysis of health beliefs and patient preferences revealed health concern (p<0.05), dependency on medicine (p<0.05), perceived susceptibility and severity (p<0.05) as well as preferred treatment (p<0.01) as factors related to therapeutic compliance. Factors from the sociodemographic characteristics and clinical factors that were related to therapeutic compliance were age (p<0.01), monthly income (p<0.05), histological type (p<0.05) and clinical stage (p<0.05) of cancer. Conclusions : In order to improve therapeutic compliance in lung cancer patients it is necessary to educate the aged, low-income patients, or patients who have small cell lung cancer or lune cancer of an advanced stage for which surgery is not indicated. Additionally, it is essential for medical personnel to have a deep concern about patients who have poor lifestyles, a low dependency on medicine, or a high perceived susceptibility and severity. Practically, early diagnosis of lung cancer and thoughtful considerations of low-income patients are important. By means of population-based education in a community, we may promote attention to health and enhance the early diagnosis of lung cancer.

  • Objective: This study was conducted in order to investigate the effects of individual patient behavior regarding the health and medical care level on their therapeutic compliance for people diagnosed with high blood pressure by a physician, in order to evaluate the impact of treatment compliance. In this study, the therapeutic compliance was defined as the hypertension drug cure rate. The current blood pressure controls were persons about 30 years of age diagnosed with high blood pressure by their doctor one month fraction of the people who take more than 20 days (30 days). Methods: The data was analyzed by using the X2-test for different comparisons of the therapeutic compliance in the individual characteristics and using a two-stage multilevel logistic regression to identify the community variance of the related index of high blood pressure therapeutic compliance using the data from 229 adults over the age of 19 in a community health survey conducted in 2010. Quantitative Analysis of DNA Single-strand Breaks in EL 4 cells and Mouse Spleen Lymphocytes after Irradiation (방사선에 의한 EL 4 백서 백혈병 세포 및 정상 백서 비장 임파구 DNA Single-Strand Breaks의 정량적 분석과 측정)

  • The filter elution technique was used to assay Co-60 $\gamma$ ray-induced DNA single-strand breaks(SSB) in EL 4 mouse leukemia cell and mouse spleen lymphocyte. The lymphocytes were stimulated with lipopolysaccharide (LPS, 20 $\mug/ml$) to label [${^3}H$] thymidine. EL 4 cells and lymphocytes in suspension were exposed at $0^{\circ}C$ to 0 Gy, 1 Gy, 5 Gy,10 Gy of Co-60 radiation and elution procedure was performed at PH 12.1. The number of DNA single-strand breaks increased with increasing doses of $\gamma$ rays. The strand scission factor (SSF) was estimated in each experiment (eluted volume 21 ml). The slope for EL 4 cells was $0.01301\pm0.00096\;Gy^{-1}(n=5)$ and the slope for lymphocytes was $0.01097\pm0.00091\;Gy^{-1}(n=5)$. The slopes were significantly different (P<0.005). Thus EL 4 cells were more sensitive to induction of DNA SSB by ionizing radiation than lymphocytes.

The Role of Single-contrast CT for Management in Hemodynamically Stable Anterior Abdominal Stab wound Patients (혈역학적으로 안정된 복부 전벽 자상 환자의 개복 수술 결정에서 Single-contrast CT의 역할)

  • Purpose: To assess the accuracy and role of single-contrast computed tomography (CT) in the management of anterior abdominal stab wound (AASW). Methods: During 8-years period, single-contrast CT was performed in 21 hemodynamically stable AASW patients (age range, 22-64 years; median age, 45 years), including 19 men and 2 women. CT scans were evaluated by one trauma surgeon and one senior resident to determine the depth of injury(peritoneal violation or not), and abnormal findings of intraperitoneal cavity associated with stab injury. We retrospectively reviewed medical records regarding operative findings. Results: Nine patients underwent abdominal surgery and 12 patients nonoperative management. In the abdominal surgery group, abnormal CT findings included peritoneal violation in 14 patients and abnormal intraperitoneal cavity findings in 5 patients. There was no statistical significant difference regarding abnormal CT findings between abdominal surgery group and nonoperative management group. Among the nine abdominal surgery patients, therapeutic laparotomy was performed on 4 patients. The positive predictive value (PPV) and negative predictive value (NPV) of peritoneal violation to predictive therapeutic laparotomy were 28.6% and 100%, respectively. In addition, the PPV and NPV of abnormal intraperitoneal cavity CT findings to predict therapeutic laparotomy were 40.0% and 87.5%, respectively. There was no statistical significant difference regarding the abnormal CT findings between therapeutic laparotomy group and non therapeutic laparotomy group. Conclusion: CT is a good adjunctive method to evaluate hemodynamicaly stable AASW patients. If peritoneal violation is not seen on CT scan, conservative treatment on local wound may be safely performed without additional abdominal surgery. However, further study is warranted to evaluate the exact role of CT in the diagnostic workup of AASW patients.

Complete Recovery of Perfusion Abnormalities in a Cardiac Arrest Patient Treated with Hypothermia: Results of Cerebral Perfusion MR Imaging

  • Therapeutic hypothermia in cardiac arrest patients is associated with favorable outcomes mediated via neuroprotective mechanisms. We report a rare case of a 32-year-old male who demonstrated complete recovery of signal changes on perfusion-weighted imaging after therapeutic hypothermia due to cardiac arrest. Brain MRI with perfusion-weighted imaging, performed three days after ending the hypothermia therapy, showed a marked decrease in relative cerebral blood flow (rCBF) and delay in mean transit time (MTT) in the bilateral basal ganglia, thalami, brain stem, cerebellum, occipitoparietal cortex, and frontotemporal cortex. However, no cerebral ischemia was not noted on diffusion-weighted imaging (DWI) or fluid-attenuated inversion recovery (FLAIR) sequences. A follow-up brain MRI after one week showed complete resolution of the perfusion deficit and the patient was discharged without any neurologic sequelae. The mechanism and interpretation of the perfusion changes in cardiac arrest patients treated with therapeutic hypothermia are discussed.

  • A retrospective study was carried out of 20 patients with histologically proven invasive carcinoma of the vagina from 1979 to 1993; 17 cases with curative primary radiotherapy and three cases with postoperative radiotherapy. The five and ten year overall survival rates were $79\%$ and $53\%$, respectively. Survival was strongly correlated with stage. Five year survival rates of stage I, stage II, and stage III, IV were $100\%$, $78\%$, and $0\%$, respectively. Eight patients experienced recurrences: five within the irradiated volume, two distant metastasis, and one combined both local and distant metastasis. There was no significant late complication. From these data, radiation is effective in the management of the vaginal cancer patients and optimum treatment modality and total dose recommendation are made.

  • Radiopharmaceuticals include therapeutic radiopharmaceuticals and diagnostic radiopharmaceuticals. Therapeutic radiopharmaceuticals are administered to the body and ingested at specific organs to detect radiation emitted from the site and to construct an image to diagnose the disease. Diagnostic radiopharmaceuticals are used to treat diseases by killing cells with radiation emitted from radiopharmaceuticals, such as cancer cells, vascular endothelial cells, arthritis, and Alzheimer's disease. The application possibilities of terahertz imaging technology for the combination of radiopharmaceuticals and molecular imaging medicine are discussed and experimental methods are presented. Terahertz imaging is expected to be a powerful technique because of the effective piercing feasibility, which enables to perform safe and high resolutive imaging. To investigate the response of cell to the terahertz wave, both the pulsed and CW THz wave systems are employed. THz imaging of a rat's paraffin-embedded epithelial cell with tumor is studied in advance.

  • Purpose: Home health nurses could affect patients' choice of alternative therapy. This study was carried out to investigate home health nurses' attitudes and cognition toward alternative therapy. Method: The subjects were 92 nurses who preparing the home health nurses at 2 university in Seoul and Suwon. Using the questionnaire. we measured the subjects' attitudes and cognition toward alternative therapy. Result: The mean score of attitudes about application was 3.14. and the score of attitudes about therapeutic effect was 2.57. The score of attitudes about therapeutic effect were significantly positive correlation to the score of cognition about alternative therapy. The attitudes about therapeutic effect were different significantly according to number of working years. But they commonly wanted to apply the soojichim. aroma-therapy. massage. danjon breathing. music therapy. plant therapy. and foot reflexology to their home health care subjects. Conclusion: Alternative therapies became a popular source of health care in Korea. This study showed pre-home health nurses had positive attitudes toward alternative therapies and they had wide variations of knowledge in how to use each therapy. But they acknowledge the value of alternative therapies as useful nursing interventions. so we all nurses should pay attention to develope the nursing intervention using alternative therapy.

  • Human chorionic gonadotropin(hCG) and intravenous immunoglobulin(IVIG) treatment were attempted as a novel therapeutic approach for unexplained recurrent spontaneous abortion(RSA). Forty-four and 3 women with a history of RSA were treated with hCG and IVIG, respectively, during pregnancy. Of these patients, serum blocking factor assay was performed before and after each treatment, in 15 patients; 12 cases with hCG and 3 cases with IVIG. The results were as follows: 1. Of 44 women who receive hCG during pregnancy, 24 delivered healthy infants at term, 10 patients suffered repeat abortion, and 10 women are still pregnant under 28 weeks. Over all success rate of hCG treatment was 70.6% (24/34). Although there is no statistical significance, absolute serm blocking level was decreased after treatment(N=12). 2. Of 3 women who receive IVIG during pregnancy, all 3 women are still pregnant under 28 weeks. Serum blocking level was increased after treatment, however, this increment was not statistically significant. Although no conclusion could be extracted from the patients who received IVIG, the therapeutic effect of hCG is comparable to that of the other therapeutic regimens, such as allogeneic leukocytes. It was postulated that actual etiology of unknown RSA would be classified as hormonal origin although combined etiologies are common in Korean women.

Walking along Seoul City Wall as Therapeutic Mobilities (치유적 모빌리티로서 서울 한양도성길 걷기)

  • Human life has been inextricably linked with healing. Recently, as the demands of healing are higher, it is more necessary to study spaces, places and mobilities related to healing in geographical studies. This paper draws on a case study of urban peoples walks along Seoul City Wall to explore what healing factors affect the people walking along historical landscapes in urban center and understand geographical implications of the healing process. Based on the concept and analytical lens of 'therapeutic mobilities' introduced by Gatrell, this research analyzes texts written on 181 blogs. The results show the main factors that affect their healing are 'weather climate', 'walk stroll', and 'nightscape'. Futhermore, the blogs' narratives focusing on these main factors imply healing place-making, time as limiting factor, and the social body within nightscape in the healing process.
