Postherpetic neuralgia, the most feared complication of acute herpes zoster, may agonize the pain clinician because the appropriate management of intractable pain can fail inspite of various and prolonged therapeutic techniques. Of all patients with herpes zoster, approximately 5~10% will develop postherpetic neuralgia. Postherpetic neuralgia is very rare in young patient but very common in patients over 60. In other words, the older, the higher incidence. In our pain clinics, 13 postherpetic neuralgic patients were treated with sympathetic blocks, local infiltration with local anesthetics and steroids, TENS, Laser and various drugs including antiderpressant. The results of management of pain in 13 patents were as follows. 1. Excellent pain control :4(30.8%) 2. Good pain control :3(23.0%) 3. Fair pain control :2(15.4%) 4. No effect :4(30.8%).

Necrobiosis lipoidica (NL) is a rare, idiopathic, chronic granulomatous inflammatory disease of collagen degeneration with the risk of ulceration. Many procedures have been proposed to treat this rare disease. In this study, we applied LDM®-MED for the management of NL, and this condition in our patient was chronic and refractory to other therapeutic options. To the best of our knowledge, no study has explored treatment of NL using ultrasound. Our results suggest that application of LDM®-MED seems to be an effective treatment option for NL. Long-term and systematic studies are needed to determine whether such application of LDM®-MED will be an innovative and effective treatment option for NL and various kinds of chronic wounds.
The present experiment was performed in order to know the therapeutic effect of laserpuncture and aquapuncture with methionine on recovery in artificially induced hepatic damaged rats by carbon tetrachloride. The animals were divided into control, laserpuncture and aquapuncture groups. The changes of serum enzyme activities(ALT and AST), serum total protein contents, protein fractions(ALB and GLB) and A/G ratio were examined before and after application of laserpuncture and aquapuncture. In change of serum ALT activity, recovery with treatment in laser and aquapuncture groups was more rapid than that of control. The significant decreased value was shown on 3rd(P < 0.05) and 7th day(P < 0.01) after treatment in laserpuncture group and on 7th day(P < 0.05) after treatment in aquapuncture group. In addition, significance was detected on 2nd day(P < 0.05) between laserpuncture and aquapuncture groups. In change of serum AST activity, recovery with treatment in laserpuncture and aquapuncture groups was more rapid than that of control. The significant lower values on 7th day(P<0.01) in aquapuncture groups. In addition, significant low value was detected on 7th day(P < 0.05) in aquapuncture group comparing with that of laserpuncture group. The change of serum TP contents showed similar pattern in control, laser and aquapuncture groups. The significances were detected on 7th day(P < 0.05) in both experimental groups. In addition, significant high value was detected on 2nd day(P < 0.05) in laserpuncture group comparing with that of aquapuncture group. The change of serum ALB content with treatment in laser and aquapuncture groups was more rapid than that of control. The significant high value was shown on 2nd(P < 0.01), 5th(P < 0.05) and on 7th day(P < 0.01) in laserpuncture group. In addition, significant high value was detected on 3rd day(P<0.01) only in aquapuncture group comparing with laserpuncture group. The change of serum GLB content showed similar pattern among groups. The significant low values were detected on 2nd day(P < 0.05) in aquapuncture group. In addition significant high value was detected on 5th day(P < 0.01) only in laserpuncture group comparing with that of aquapuncture group. In change of A/G ratio laser and aquapuncture groups showed higher values than that of control. The significant high values were detected on 1st day(P < 0.Of) and 2nd day(P < 0.05) in laserpuncture group, and on 1st day(P < 0.05) and 2nd day(P < 0.01) in aquapuncture group. In addition, significant high value was detected on 5th day(P < 0.05) in aquapuncture group comparing with that of laserpuncture. Considering above findings collectively, it was considered that both laserpuncture and aquapuncture were effective; aquapuncture was more effective than laserpuncture for recovery of hepatic damage.
광역학 요법(photodynamic therapy)은 특정 파장의 빛에 의해 활성화되는 광민감제(photosensitizer)를 사용하여 세포 내 산소를 활성화시키는 치료 방법으로, 항생제 내성균에 의한 상처 감염의 치료에 유망한 접근 방법이다. 일반적으로 건강한 사람에게 비병원성인 녹농균(Pseudomonas aeruginosa, P. aeruginosa)은 특정 병원성 징후를 보이지 않지만, 피부 손상이나 면역력이 저하된 사람들에서는 패혈증과 같은 심각한 질병을 유발할 수 있다. 항생제는 P. aeruginosa 감염에 대한 전통적인 치료법이나 약물 오용으로 인한 항생제 내성의 증가는 이러한 감염을 관리하는 데 큰 어려움을 준다. 본 연구에서는 P. aeruginosa의 억제제로서 광민감제(PhotoMed, Methyl pheophorbide A, Radachlorin®)와 다이오드 레이저를 이용한 광역학 치료 효과를 조사하는 것을 목표로 했다. P. aeruginosa 현탁액과 광민감제(PhotoMed, Methyl pheophorbide A, Radachlorin®)를 페트리 접시에 접종하여 30분 후 다이오드 레이저를 사용하여 3 J/cm2의 에너지 밀도로 조사했다. 그 결과, P. aeruginosa는 PhotoMed에서 79.65%, Methyl pheophorbide A에서 47.36%, Radachlorin®에서 40.91%의 사멸률을 보였다. 이번 연구는 P. aeruginosa를 억제하기 위한 가장 효과적인 접근법이 PhotoMed와 다이오드 레이저를 이용한 광역학 치료임을 보여준다.
비전리 방사선의 인체에 대한 작용은 최근 우리 주위에서 많이 사용되는 microwave, radio wave, laser, ultrasoundwave, ultravioletradiation 등의 문제가 될 수 있으며 본고에서는 주로 Radio Frequency Electromagnetic (RFEM) spectrum의 shortwave와 microwave의 생물학적인 작용을 정리 발표하고자 한다. RFEM에 의한 carcinogenesis에 관해서는 아직 RFEM이 인체에서는 발암물질로서 작용한다는 보고는 없으나, cancer promotion에는 작용할 것이라는 보고는 있다. chromosomal effect에 관해서는 체외실험에서 microwave에 의한 chromosome의 stickiness의 증가와 mitotic index의 변화에 관한 보고가 있다. 인체에서는 RFEM에 많이 피폭되었다고 알려진 Moscow의 미 대사관 직원 5000명을 대상으로 한 역학조사에서는 특별한 염색채 이상은 발견되지 않았다. 동구에서의 보고에 의하면 RFEM에 의한 선천성 기형아의 출산이 보고된바 있으나 출산시의 통증을 줄이기 위한 2450 MHz의 microwave를 조사한 산모 2000명 출산아에서 기형은 없었다고 한다. 인간의 행동과 정서적인 측면에서 RFEM에 의한 변화는 1974년 Sadcikoda에 의해 기술된바 있는 µwave sickness&는 irritability, 두통, 전신쇄약, 불면증, 성욕 감퇴 및 발기부전 등을 호소하는 증후군으로 주로 동유럽의 저자들에 의하여 보고되고 있으며 구 소련에서는 microwave sickness 또는 radio wave sickness는 확실한 하나의 임상병명으로 간주되고 있다. cataractogenesis에 관해서는 실험 동물에서 잘 연구되고 있으며, 사람에서도 1952년 레이다 작업종사자 에서 보고된 이후 50여례 이상의 보고가 있으나 백내장 자체가 그 발생 빈도가 높은 질환이고 많은 원인이 있으므로 아직 인간에서의 확실한 cataractogenesis에 관해서는 더욱 연구가 필요한 분야이다. radar wave에 노출된 적이 있는 군인을 대상으로 한 미 해군의 역학조사에 의하여 radar wave에 의한 uveitis, retinitis와 비진행성 백내장등이 보고되고 있으나 딴 연구에서는 입증된바 없다.
광 역학 치료법은 그 효과가 특정 위치의 암세포에 국한되어있어 주변의 정상 세포에 큰 영향을 미치지 않기 때문에 다양한 부작용이 존재하는 현재의 암 치료법의 대안으로 제안되어 왔다. 이러한 광 역학 치료에는 광감응제를 선택적으로 자극시키기 위해 레이저를 이용하고 있으나, 고가의 치료비용과 높은 발열현상으로 사용에 제한적이다. 그러나 다양한 파장의 빛을 발할 수 있는 소형 발광 다이오드가 저가로 개발됨에 따라 대체가 가능하며, 이는 광 역학 치료 장비의 소형화 및 저가의 치료 장비 개발에 큰 영향을 미치고 있다. 현재 활발하게 연구되는 광 역학 치료법은 다양한 광감응제를 이용하여 암세포의 사멸을 유도하는데 목적이 있으나, 특정한 파장을 중심으로 진행된 연구는 상대적으로 미비한 실정이다. 따라서 본 논문에서는 저렴하며, 발열현상이 작고, 다양한 파장대의 광원지원이 가능한 발광 다이오드를 사용하고 이를 아두이노로 제어하여 새로운 저가의 암세포 증식 억제 모듈을 개발함으로써, 그 효과를 정량적으로 분석하고 새로운 항암치료법을 제시하고자 한다.
Jaeumgenby-tang(JGT) have been used in oriental medicine for many centries as a a therapeutic agent of vertigo caused by deficiency of qi and blood. The effects of JGT on the regional cerebral blood flow(rCBF), mean arterial blood pressure(MABP) and cardiac muscle contractile force(CMF) is not known. The purpose of this Study was to investigate effects of JGT on the rCBF, MABP, CMF and mechanism of JGT induced changed rCBF, MABP, CMF. The changes of rCBF, MABP and CMF were determinated by Laser-Doppler Flowmetry(LDF). The results were as follows; JGT extract was increased rCBF, MABP and CMF in a dose-dependent, specially JGT extract was significantly increased rCBF and MABP. Pretreatment with propranolol was significantly inhibited JGT induced increase of rCBF but pretreatment with indomethacin and methylene blue were accelerated JGT induced increase of rCBF. Pretreatment with propranolol and indomethacin were inhibited JGT induced increase of MABP, but pretreatment with methylene blue was accelerated JGT induced increase of MABP. Pretreatment with propranolol was significantly inhibited JGT induced increase of CMF but pretreatment with indomethacin and methylene blue were accelerated JGT induced increase of CMF. This results suggest that JGT increased rCBF by increasing MABP and CMF and the action of JGT is mediated by adrenergic β-receptor.
The aim of this study was to evaluate the bacterial effects of Moraxella catarrhalis in otitis media with effusion (OME) by photodynamic therapy (PDT). Bacterial suspensions (10000 CFU/mL) were prepared. The colony forming units (CFU) of Moraxella catarrhalis have been measured after an application of photogem plus 632 nm diode laser irradiation. One ml of the bacterial suspensions have been incubated in the dark for 3h with various concentrations of photogem ($0.625{\sim}5.0_{\mu}g/mL$) and then irradiated with 632 nm diode laser ($15J/cm^2$). After, the PDT Moraxella catarrhalis suspensions ($50{\mu}L$) were inoculated on chocolate agar plate and cultured in the dark at $37^{\circ}C$, 5% $CO_2$ condition for 18h. The colony forming units off the bacteria were measured. Also transmission electron microscopy (TEM) was employed to evaluate the effect of otitis media pathogens by PDT. The nucleus of Moraxella catarrhalis was stained using green fluorescent nucleic acid dye thiazole orange and the fluorescence intensity of the nucleus was measured by flow cytometry. The PDT was effective in killing Moraxella catarrhalis at the photogem dose of $5.0_{\mu}g/mL$, respectively, As assessed by flow cytometry analysis the fluorescence intensity of the nucleus got lower after PDT. TEM result appeared to able to cause damage to the bacterial membranes. On the basis of these findings, bacterial photodynamic therapy with photogem can be considered to be a promising new therapeutic approach for OME.

Background: Low back pain secondary to discopathy is a common pain disorder. Multiple minimally invasive therapeutic modalities have been proposed; however, to date no study has compared percutaneous laser disc decompression (PLDD) with intradiscal injection of radiopaque gelified ethanol (DiscoGel®). We are introducing the first study on patient-reported outcomes of DiscoGel® vs. PLDD for radiculopathy. Methods: Seventy-two patients were randomly selected from either a previous strategy of PLDD or DiscoGel®, which had been performed in our center during 2016-2017. Participants were asked about their numeric rating scale (NRS) scores, Oswestry disability index (ODI) scores, and progression to secondary treatment. Results: The mean NRS scores in the total cohort before intervention was 8.0, and was reduced to 4.3 in the DiscoGel® group and 4.2 in the PLDD group after 12 months, which was statistically significant. The mean ODI score before intervention was 81.25% which was reduced to 41.14% in the DiscoGel® group and 52.86% in the PLDD group after 12 months, which was statistically significant. Between-group comparison of NRS scores after two follow-ups were not statistically different (P = 0.62) but the ODI score in DiscoGel® was statistically lower (P = 0.001). Six cases (16.67%) from each group reported undergoing surgery after the follow-up period which was not statistically different. Conclusions: Both techniques were equivalent in pain reduction but DiscoGel® had a greater effect on decreasing disability after 12 months, although the rate of progression to secondary treatments and/or surgery was almost equal in the two groups.

Background and Objectives Low-level light therapy (LLLT) is an application of low-power light for various purposes such as promoting tissue repair, reducing inflammation, causing analgesia, etc. A previous study suggested the effect of light emitting diode (LED) light with the wavelength of 740 nm for promoting wound healing of corneal epithelial cells. This current study aimed to confirm the effect of LLLT for managing inflammation of a dry eye disease (DED) mouse model. Materials and Methods A total of 50C57BL/6 female mice were randomly grouped into 5 groups to compare the effect of LLLT:1) Control group, 2) Only LLLT group, 3) Dry eye group, 4) LLLT in dry eye group, and 5) Early treatment group. DED was induced with 4 daily injections of scopolamine hydrobromide and desiccation stress for 17 days, and LLLT at 740 nm was conducted once every 3 days. To analyze the effect of LLLT on the DED mouse model, tear volume, corneal surface irregularities, and fluorescence in stained cores were measured, and the level of inflammation was assessed with immunohistochemistry. Results The DED mouse model showed significant deterioration in the overall eye condition. After LLLT, the amount of tear volume was increased, and corneal surface irregularities were restored. Also, the number of neutrophils and the level of inflammatory cytokines significantly decreased as well. Conclusion This study showed that LLLT at 740 nm was effective in controlling the corneal conditions and the degree of inflammation in DED. Such findings may suggest therapeutic effects of LLLT at 740 nm on DED.
