Clinical Features of Wrist Drop Caused by Compressive Radial Neuropathy and Its Anatomical Considerations

  • Objective : Posture-induced radial neuropathy, known as Saturday night palsy, occurs because of compression of the radial nerve. The clinical symptoms of radial neuropathy are similar to stroke or a herniated cervical disk, which makes it difficult to diagnose and sometimes leads to inappropriate evaluations. The purpose of our study was to establish the clinical characteristics and diagnostic assessment of compressive radial neuropathy. Methods : Retrospectively, we reviewed neurophysiologic studies on 25 patients diagnosed with radial nerve palsy, who experienced wrist drop after maintaining a certain posture for an extended period. The neurologic presentations, clinical prognosis, and electrophysiology of the patients were obtained from medical records. Results : Subjects were 19 males and 6 females. The median age at diagnosis was 46 years. The right arm was affected in 13 patients and the left arm in 12 patients. The condition was induced by sleeping with the arms hanging over the armrest of a chair because of drunkenness, sleeping while bending the arm under the pillow, during drinking, and unknown. The most common clinical presentation was a wrist drop and paresthesia on the dorsum of the 1st to 3rd fingers. Improvement began after a mean of 2.4 weeks. Electrophysiologic evaluation was performed after 2 weeks that revealed delayed nerve conduction velocity in all patients. Conclusion : Wrist drop is an entrapment syndrome that has a good prognosis within several weeks. Awareness of its clinical characteristics and diagnostic assessment methods may help clinicians make diagnosis of radial neuropathy and exclude irrelevant evaluations.

  • The purpose of this study is to report the effects of Korean medicine treatment in wrist drop patient with thoracic outlet syndrome. We treated the patient for 8 days using Korean medicine treatment such as herbal medicine, acupuncture, Chuna manual therapy and pharmacopuncture. Changes in the numerical rating scale (NRS), EuroQol five dimension (EQ-5D) index, and symptoms were measured for assessment. After 8 days hospitalization, NRS decreased from 5 to 3, EQ-5D index and the symptoms of the patient also were improved. In conclusion, this case shows that Korean medicine treatment can be an effective treatment for wrist drop with thoracic outlet syndrome.

Two Cases of Radial Nerve Palsy with Chuna Treatment on Cervical Vertebrae (경추 추나치료를 적용한 압박성 요골신경마비 환자 치험 2례)

  • Objective : The purpose of this study is to evaluate the korean medical treatment for compressive neuropathy of radial nerve, especially chuna treatment of cervical vertebrae. Methods : Two patients were treated with acupuncture and moxibustion, herbal medication, chuna treatment on cervical vertebrae. To evaluate the wrist drop and numbness of hand, Coding Result of arbitrary values and manual muscle test(MMT), visual analogue scale(VAS) were used. Results : After 2 weeks-treatment(in case 1) & 5 weeks-treatment(in case 2), the movement and power of wrist were restored to nearly normal range, also the numbness of hands was removed. All cases show the improvement in the movement of wrist and the numbness of hands. Conclusions : Korean medical treatment including cervical manual therapy as chuna treatment is remarkably effective for radial nerve palsy. But further studies are required to concretely prove the effectiveness of chuna treatment on central vertebrae for peripheral neuropathy.

A Kinematic Analysis on Clear & Drop Motion of Badminton (배드민턴 클리어와 드롭 동작에 대한 운동학적 분석)

  • The purpose of this study was to present the basic data on improving the skills for 3 junior high school national badminton players in clear and drop motion through the 3-dimensional image analysis. Therefore, the results of this study are as follows: 1. In the duration times per phase, subject C relatively showed a similar time between clear and drop motion. Accordingly, C took a more effective motion than A and B. 2. In the velocities of racket head, subject A and C showed similar changes relatively. However, in case of subject B, the velocity was decreased before back swing(E2) and increased until impact(E3). 3. Regardless of clear and drop motion, the changing phases of joint angle for wrist and elbow showed similar changes comparatively. 4. In the angles of upper body, clear motion was average 85.0 degree and drop one was average 80.7 degree during the impact(E3). Hence, it showed that drop motion hit the ball bowing the upper body more than clear one. 5. In the angles of racket head, clear motion was average 87.7 degree and drop one was average 85.6 degree during the impact(E3). Consequently, drop motion was impacted forward more than clear one.

  • Posterior interosseous nerve(PIN) syndrome is an entrapment of the deep branch of the radial nerve just distal to the elbow joint. It is caused by acute trauma or masses compressing the nerve. We report an unusual case of PIN syndrome with wrist drop caused by compression of the nerve by anomalous vascular leash. The patient has recovered with the surgical decompression of the offending vessels and arcade of Frohse.

  • After treating a patient suffering from metacarpophalangeal joint extension disturbance which is caused by posterior interosseous nerve palsy, some results are gained as follows. The symptom of posterior interosseous nerve palsy is simillar to the it of radial nerve palsy. But posterior interosseous nerve palsy isn't accompany with wrist drop. posterior interosseous nerve palsy is accompany with metacarpophalangeal joint extension disturbance. This symptom is caused by posterior interosseous nerve palsy. Posterior interosseous nerve palsy is correspond to MAMOKBULIN(麻木不仁), SUTONG(手痛), SUGI(手氣) in oriental medicine. The cause of this case on oriental medicine is Deficiency of qi and blood. Treatment which based on cause of oriental medicine-herb medication, acupuncture treatment- have a good effect to patient.

  • Thiamine deficiency can cause peripheral polyneuropathy and Wernicke's encephalopathy. Wernicke's encephalopathy is characterized by ataxia, ophthalmoplegia, nystagmus, and confusion, and typically presents acute and rapidly progressive course, whereas peripheral neuropathy associated with thiamine deficiency manifests chronic and slowly progressive one. However, acute and rapidly progressive axonal polyneuropathy combined with Wernicke's encephalopathy is quite rare and unusual. Here, we describe a patient with Wernicke's encephalopathy who presented with acute bilateral axonal neuropathy.

  • Objectives : The purpose of this study is to report the patients with radial nerve palsy, who inproved by oriental medical treatment Methods & Results : The patients were hospitalized Dept. of Acupunture & Moxibustion, College of Oriental Medicine, Daejeon University from 1st, Mar. 2005 to 31th. Aug. 2005, and treated with Acupuncture and Electrical Stimulation theraphy, Herbal medication, Oriental physical therapy. To evaluate the wrist drop and numbness of fingers, Visual Analogue Scale(VAS) and the measurement of wrist circumstance are used. As the result, symptoms are improved remarkably Conclusion : Patients were treated for 4.1 weeks(average). 6 cases were excellent and 1 case was good.

  • For the purpose of providing basic data for the development of size specifications for middle-aged women in Busan, one dimensional measurements were made with the subjects. ranging 45 to 59 in age. The following are the conclusions : 1) According to ages, the middle and elder groups of the middle-aged women shared similar body types, having shorter height, more lowered bust. and increased upper-body depth and girth factors, compared with the earlier middle-aged women group. But the thigh girth of the elder middle-aged group (age: 55∼59) was the lowest, maybe owing to the aged lower-body muscles. 2) In the comparison with national averages. armhole girth, elbow girth, wrist girth. back waist length, side neck point-B.P.-waist line, and sleeve length showed great differences. This is problematic in that the national size standards fail to reflect each legion\`s peculiar body type characteristics. 3) Even though body types were classified according to drop value criteria suggested by KS specifications, 23.05% did not belong to the criteria. They were thus classified as A, N, and H body types, following the distribution of the present experiment. 4) In consideration of the economy factor of the production and sales of the clothes industry, the combinations of height and chest garth for the middle-aged Busan women were as follows: 155cm-85cm (Body Type A) 150cm-88cm (Body Type N), and 155cm-94cm (Body Type H).

Posterior Interosseous Nerve Palsy Caused by a Ganglion of the Arcade of Frohse

  • A 51-year-old male who is right-handed visited the outpatient for right fingers-drop. The patient's fingers, including thumb, were not extended on metacarpophalangeal joint. The active motion of the right wrist was available. The electromyography and nerve conduction velocity study were consistent with the posterior interosseous neuropathy. Further evaluation was done with the magnetic resonance imaging for finding the space-occupying lesion or any possible soft tissue lesion around the radial nerve pathway. On magnetic resonance imaging, the ganglion cyst, which was about 1.8 cm in diameter, was observed on the proximal part of the superficial layer of the supinator muscle (Arcade of Frohse). The surgical excision was done on the base of ganglion cyst at the base of stalk of cyst which looked to be connected with proximal radioulnar joint capsule. The palsy had completely resolved when the patient was observed on the outpatient department a month after the operation.
