A Study on the Difference between Gender Role Attitude and Dual Income Attitude: Focusing on Classification of Married Couple with Children (성역할 태도와 맞벌이 태도의 차이에 관한 연구: 기혼·유자녀 가구를 중심으로 한 유형화)

  • Objectives: The purpose of this study is to empirically verify the difference between traditional gender role attitude and realistic dual earner attitude. Most preceding studies analyzed gender role attitude by summated rating scale while few studies have been conducted on the difference between traditional gender role attitude and dual earner attitude. Method: By using the 9th Korea Welfare Panel Data (year 2014), this study extracted households of married couples and with children in working age (18 through 64 years of age) who are the main subjects in work-family relationship. First, this study classified traditional group and modern group in their gender role attitude and passive group and active group in their dual earner attitude and by crossing these attitudes, drew out 4 gender role/dual earner attitudes. For analysis, this study investigated frequency of attitude types through descriptive statistics. By conducting ANOVA, study verified if there are significant differences in the satisfaction with family relationship depending on the types of gender role/dual earner attitudes. Lastly, study examined the effect of socio-economic status and demographic characteristics on the types of gender role/dual earner attitudes by conducting multi-nominal logistic regression analysis. Results: As the result of analysis, first, half of respondents belonged to 'equal-role-oriented type' who has modern gender role attitude and active dual earner attitude. However, there were not a few 'dual-earner-avoiding type' who has modern gender role attitude but passive dual earner attitude and 'incongruous attitude type' who has traditional gender role attitude but active dual earner attitude. Next, 'incongruous attitude type' had significantly low level of satisfaction with overall family life, relationship with spouse and relationship with children than other attitude types. Lastly, those whose incomes belong to poor strata, those who are high school graduates and lower, males and those who are over the age of 60 had significantly higher probability of belonging to 'incongruous attitude type.' Conclusion: The results of analysis demonstrate that the difference between gender role attitude and dual earner attitude is an important matter and since 'incongruous attitude type' exhibits low level of satisfaction with family relationship, scrupulous policy approaches are required for those who have high possibility of belonging to 'incongruous attitude type.'

  • This study examined the effects of social capital on psychological identity and practical wellbeing for the Korean baby boom generation. To achieve this, an empirical survey was carried out on baby boomers who use elderly welfare centers and cultural centers living in D City. The overall research results showed that trust (t=6.893, p<.05), participation (t=5.157, p<.05), network (t=8.093, p<.05), and norm and reciprocity (t=4.787, p<.05), as sub-factors of social capital for baby boomers, had a significant effect on their practical wellbeing. Psychological identity was moderated (t=2.023, p<.05) in the effect of trust on practical wellbeing, adopting the hypothesis. This means that the social ties and the strong trust relationship between family members and relatives, which built up amid rapid economic growth, work with positive expectations of social capital and have a major effect on practical wellbeing. Moreover, practical welling also rated high, along with the high trust relationship and psychological identity. Consequently, various exchange programs and group and volunteer activity programs for baby boomers should be established to decrease their psychological identity due to the loss of social roles. Moreover, the decline of activities at a time of retirement can slow practical wellbeing.

The Clinical Impact of Advanced Age on the Postoperative Outcomes of Patients Undergoing Gastrectomy for Gastric Cancer: Analysis Across US Hospitals Between 2011-2017

  • Purpose: This study systematically evaluated the implications of advanced age on post-surgical outcomes following gastrectomy for gastric cancer using a national database. Materials and Methods: The 2011-2017 National Inpatient Sample was used to isolate patients who underwent gastrectomy for gastric cancer. From this, the population was stratified into those belonging to the younger age cohort (18-59 years), sexagenarians, septuagenarians, and octogenarians. The younger cohort and each advanced age category were compared in terms of the following endpoints: mortality following surgery, length of hospital stay, charges, and surgical complications. Results: This study included a total of 5,213 patients: 1,366 sexagenarians, 1,490 septuagenarians, 743 octogenarians, and 1,614 under 60 years of age. Between the younger cohort and sexagenarians, there was no difference in mortality (2.27 vs. 1.67%; P=0.30; odds ratio [OR], 1.36; 95% confidence interval [CI], 0.81-2.30), length of stay (11.0 vs. 11.1 days; P=0.86), or charges ($123,557 vs. $124,425; P=0.79). Compared to the younger cohort, septuagenarians had higher rates of in-hospital mortality (4.30% vs. 1.67%; P<0.01; OR, 2.64; 95% CI, 1.67-4.16), length of stay (12.1 vs. 11.1 days; P<0.01), and charges ($139,200 vs. $124,425; P<0.01). In the multivariate analysis, septuagenarians had higher mortality (P=0.01; adjusted odds ratio [aOR], 2.01; 95% CI, 1.18-3.43). Similarly, compared to the younger cohort, octogenarians had a higher rate of mortality (7.67% vs. 1.67%; P<0.001; OR, 4.88; 95% CI, 3.06-7.79), length of stay (12.3 vs. 11.1 days; P<0.01), and charges ($131,330 vs. $124,425; P<0.01). In the multivariate analysis, octogenarians had higher mortality (P<0.001; aOR, 4.03; 95% CI, 2.28-7.11). Conclusions: Advanced age (>70 years) is an independent risk factor for postoperative death in patients with gastric cancer undergoing gastrectomy.

  • This study examined the relationships between the blood heavy metal (Pb, Hg and Cd) concentration and exposure-related factors. The subjects were 2,042 (male: 953, female: 1,089) who were recruited through a National Health and Nutrition Examination Survey. Differences in the concentrations of mercury were observed in males, whereas females showed differences in lead and cadmium. Analyses of the blood concentrations according to body mass index and waist circumference showed significantly higher concentrations of mercury. In males, high-density lipoprotein cholesterol of dyslipidemia was higher than normal in the blood lead concentration. In females, low-density lipoprotein cholesterol of dyslipidemia was higher than normal in the blood lead concentration. The blood lead and cadmium concentrations of male smokers was higher than non-smokers, and cadmium showed the same results in females. The blood lead levels were higher in male alcohol consumers than non-drinkers. The blood concentrations of mercury and cadmium showed significant differences according to household income. Blood concentrations of heavy metals were closely related to exposure-related factors, and age, smoking, drinking alcohols, obesity, hyperlipidemia, and household income were found to be relevant.

  • The purpose of the study is to assess and evaluate methods employed in nutrition surveys and to give an information on methodological considerations for future studies in Korea. Three hundred thirty six papers of nutritional surveys published from 1960 to 1990 were classified into 9 groups according to the survey subject and analyzed for methods used in survey. The frequency of publications according to the survey subject was as the following order : 'in-fants and preschool children' 'school age children' 'adolescence' 'college students' 'preg-nant and lactating women' 'housewives' 'the elderly' 'workers' and 'patients' Eighty five percents of papers applied methods of dietary assessment. of which weighing method recall method an record method were extensively used in the order. Questionnairs included items related to nutrition such as food habit nutrition knowledge etc. Anthropometric techniques were employed in 53% of papers and in general growth measurement were undertaken for children and body composition was emphasized for adults. biochemical assessment was limited to 32% of papers and assessment of anemia was the major test. Clinical studies were applied to 26% of the studies. of which only 5% employed examination of clinical signs of malnutrition Recently self-reported health status assessed using questionnaires has been adopted frequently. Further researches are required for evaluation of the methods employed in nutrition surveys in order to develop the standard model for nutrtion survey so that obtained data can become more reliable and utilized efficiently.

The analysis of medical care behaviors influencing New Diagnosis-Related Groups (DRG) based payment - focused on hospitalized patients with medical illness (신포괄수가에 영향을 미치는 의료행태 요인 분석 - 내과 입원환자 중심으로)

  • Purpose: The purpose of this study is to investigate medical care behaviors influencing accuracy of the payment based New diagnosis-related groups (DRG) compared to fee for service (FFS) in hospitalized patients with medical illness. Methodology: In order to estimate the difference in medical costs between New DRG and FFS depending on medical care behaviors, medical records and hospital claims data (n=4,232) were utilized, which were collected from a single public hospital during the first-half of 2018. Data were analyzed by descriptive statistics, t-test, chi-square test, and multivariate binary logistic regression. Findings: The average difference in medical costs between New DRG and FFS were KRW 506,711±13,945 with incentives and KRW -51,506±12,979 without incentives, respectively. Forty-four point two percent (44.2%, n=1,872) of total subjects were shown to have negative compensation in overall medical costs with New DRG compared to the costs with FFS. Medical care behaviors that affected on the negative compensation were the presence of severe bed sores on admission, medical consultations, death, operations, medications and laboratory or imaging tests with unit price over KRW 100,000, hospital-acquired complications or underlying comorbidities, elderly patients (≧65 years), and hospitalized for more than average inpatient days defined by New DRG (p<0.001). The difference in average medical cost between New DRG and FFS for a group with mild illness was KRW -11,900±10,544, whereas it was KRW -196,800±46,364 for a group with severe illness (p<0.0001). Practical Implications: These findings suggest that New DRG payment model without incentives may incompletely cover the variation of medical costs in real clinical practice. Therefore, policy makers need to consider that the current New DRG reimbursement should be focused and refined to improve accuracy of payment on medical care resources utilized in severe and complex medical conditions.

Design and Evaluation of Multisensory Interactions to Improve Artwork Appreciation Accessibility for the Visually Impaired People (시각장애인의 미술 작품 감상 접근성을 높이는 다중감각 인터랙션의 설계 및 평가)

  • This study suggests multisensory interaction techniques to help visually impaired people appreciate and understand artworks through non-visual senses such as tactile, auditory, and olfactory senses. A user study was conducted on the basis of a qualitative interview about the experience of appreciating artwork through the multisensory interaction system to visually impaired people so as to verify the development of the interaction techniques. The user test shows that the multisensory interactions in artwork generally not only help them appreciate and understand it but also give them satisfaction through artwork appreciation. However, it also indicates that some multisensory interactions caused the visually impaired people confusion and could not be perceived during the appreciation. On the basis of these outcomes, implications in this study are as follows. This study has contributed to providing specific development guidelines and indicators of non-visual multisensory interactions as a technical alternative to improve accessibility to cultural and artistic activities for the visually impaired. Furthermore, this study is expected to contribute to building a technical background, which can provide comprehensive sensory experiences with not only blind people but also non-blind people such as children and the elderly through universal interaction techniques beyond existing visual-oriented fragmentary experience.

Unplanned Readmission to Intensive Care Unit during the same Hospitalization at a Teaching Hospital (계획에 없던 중환자실 재입실 실태 및 원인)

  • Background : Because unplanned readmissions to intensive care unit(ICU)might be related with undesirable patient outcomes, we investigated the pattern of and reason for unplanned ICU readmission to provide baseline data for reducing unplanned returns to ICU. Methods : The subjects included all patients who readmitted to ICU during the same hospitalization at a tertiary referral hospital between January 1st and June 30th 2002. Quality improvement(QI) nurse collected the data through medical records and a medical director reviewed the data collected. Results : 1) The average unplanned ICU readmission rate was 5.6%(gastroenterology 14.6%, pediatrics 12.7%, pulmonology 11.9%, neurosurgery 6.3%, general surgery 5.3%, chest surgery 3.9%, and cardiology 3.3%). 2) Among the unplanned readmissions, more than 50% of cases were from patients older than 60 years, and the main categories of diagnose at hospital admission were neurologic disease(29.9%) and cardiovascular disease(27.6%). 3) Of unplanned ICU readmissions, 41.8% had recurrence of the initial problems, 44.8% had occurrence of new problems. And 9.7% required post-operative care after unplanned operations. 4) The most common cause responsible for unplanned ICU readmission were respiratory problem(38.3%) and cardiovascular problem(14.3%). 5) About 40% of unplanned ICU readmission occurred within 3 days after ICU discharge. 6) Average length of stay of the readmitted patients to ICUs were much longer than that of non-readmitted patients. 7) Hospital mortality rate was much higher for unplanned ICU readmitted patients(23.6%) than for non-readmitted patients(1.5%) (P<0.001). Conclusions : This study showed that the unplanned ICU readmitted patients had poor outcomes(high morality and increased length of stay). In addition study results suggest that more attention should be paid to patients in ICU with poor respiratory function or elderly patients, and careful clinical decisions are required at discharged from ICU to general ward.

  • This paper is to construct a healthcare database using information obtained from healthcare home environments, and use this one for healthcare home services, Especially, our researching focus in this paper is how to design healthcare database scheme and how to use this constructed database on the Framework for Supporting Healthcare Integrated Service(FSHIS) we developed previously. Healthcare information is designed to database schemes in accordance to the specific save types of the data collected from various typed-sensors. The healthcare database constructed by using this information for the purpose of healthcare home services is divided into the base information with real schemes and the context based information with view schemes. Firstly, the base information includes low data obtained from physical sensors relevant to locations, healths, environments, and the personnel healthy profiles. The other is the context based information that is produced and fused by using the based information. This context based information might be got via various view schemes according to healthcare application services. Finally, for verifying the practical use of healthcare database constructed in this paper, Via interconnecting this database to our FSHIS, we show an example of healthcare home monitoring service using information (basic and context based information), emergency call, home appliance control, and so on needed from living activity area for elderly living alone.

Characteristics of accurate token and all token diadochokinesis in patients with normal pressure hydrocephalus (정상압 수두증 환자와 정상 노인의 조음교대운동 수행력 비교)

  • Normal pressure hydrocephalus (NPH) is a condition wherein the cerebrospinal pressure in the brain is within the normal range, but the cerebrospinal fluid increases above the normal level, causing ventriculomegaly. In patients with NPH, the articulatory system exhibits reduced mobility and range, which may affect diadochokinesis (DDK) and speech intelligibility. In this study, we investigated the characteristics of DDK, including accurate-token DDK and all-token DDK including inaccurate tokens, in patients with NPH and healthy elderly adults (HE). We also examined the classification accuracy of DDK between the two groups. Finally, we investigated whether there was a correlation between speech intelligibility and DDKs in the NPH group. The results showed that NPH and HE groups differed significantly in both accurate-token DDK and all-token DDK, and their classification accuracy was relatively high. However, there was no correlation between speech intelligibility and DDK. The findings suggest that the DDK is a useful method for sensitively assessing speech motor performance in patients with NPH.
