Effects of the Acceptance and Commitment Therapy Group Program (ACT) on the Psychological Flexibility, Self-Compassion, and Recovery in Alcoholics (수용-전념치료(ACT)가 알코올중독자의 심리적 유연성과 자기-자비 및 회복에 미치는 효과 연구)

  • The purpose of this study is to evaluate the effects of the acceptance commitment therapy program (ACT) on the psychological flexibility, self-compassion, recovery in alcohol dependents. The ACT program contains skills that establish acceptance and willingness, defusion, self-as-context, contact with the present moment, clarifying values, and committed action. This study was intended to enhance the effects on the psychological flexibility, self-compassion, recovery in alcohol dependents with ACT program. Fifteen inpatients with alcohol dependence were participated in the ACT group and the same number of participants were in the both CBT control group and TAU control group. The ACT program was held once a week, eight times, and each session was 90-minute long. Repeated measure of MANOVA and post-hoc were used to compare pre-test mean with post-test and with follow-up test mean. As a result of ACT program, positive changes were assured in the psychological flexibility including acceptance, cognitive defusion, being present, committed action and the increase of self-compassion and of recovery in alcoholics.

Comparison of Time-Management Ability and ADL between Elderly People Living Alone and Living with Family (독거노인과 가족동거 노인의 시간관리능력과 일상생활활동의 비교)

  • Objective : The objective of this study is to present basic data to find health care plans for the elderly by comparing time-management ability and ADL and identifying the relationships between groups with subjects of elderly people living alone and living with family in Daejeon Metropolitan City. Method : A total of 80 elders who lived alone or with family that were aged 65 or older were selected with MMSE-K, 40 people were selected as subjects for each group. For time-management ability, a questionnaire was used. ADL were assessed by using FIM. The study period was May to June 2013. Result : Comparison of scores for time-management ability and FIM of the elderly who live alone or living with family did not show any statistically significant difference. In comparison of detailed scores between groups, there were statistically significant differences between the two groups being social interaction, problem solving and memory in social cognition items among detailed items. Conclusion : Through this study, we understood that social cognitive function of the aged living alone who had less opportunity of interaction compared to that of the aged living with family was lowered. Based on this, development and study on various programs should be made with consideration of sociodemographic characteristics of the elderly within community-based occupational therapy in the future.

  • The social attention about disaster psychiatry was increased after 911 terror in New York. The role of child psychiatrist and specific consideration for the treatment of child victim in disaster were reviewed. The following were main points. 1) The most single determining factor of prognosis is supporting system and parental attitude to their child victim. So family therapy and parental eucation are needed. 2) Cognitive Behavior Therapy is known to the most effective treatment in many literature. 3) Brief group therapy with fellow victim is cost effective preventive methods and screening tool for more serious victim, 4) Eye Movement Desensitization and Reprocessing(EMDR) could be a very amazing method in reducing repetative horrible traumatic image. 5) Many kinds of drug using in adult are considered with caution.

The Effects of Floral Arrangement on the Stress Index of the Elderly with Chronic Diseases and Its Correlation with Cognition

  • This study aims to see what changes flower arrangement programs bring to the stress index of elderly people with chronic diseases and its correlation with cognition. Furthermore, seniors from a day care center and a nursing home were compared for the purpose of identifying the effectiveness of flower arrangement activities as a supplementary remedy designed to relieve the symptoms and improve the quality of life of patients with chronic diseases. In this study, 24 seniors with chronic diseases were divided into two groups: Group A consists of 15 seniors from a day care center and Group B consists of nine seniors from a nursing home. Both groups participated in simple flower arrangement activities in 10 sessions. In each session, red and green cut flowers that were preferred by the elderly were provided to elicit and develop their thoughts and behaviors. The results showed that the overall average stress index was significantly reduced from 58.0±11.6 before the program to 50.6±17.1 after the program (p = .037). This study examined that the program was more effective for Group A (p = .021) than for Group B (p = .678). Results of the correlation analysis showed that there was no correlation between cognitive function and stress index (p = .569), but that the activity was more effective for Group A. This study shows that arranging flowers effectively lowers the stress index of elderly people with chronic diseases. Also, sharing the outcomes of such activities with others or continuously caring for the flowers even after the activity can maximize the effectiveness of the therapy and rehabilitation. Thus, the study concluded that the program needs to be applied continuously, not in the short-term, in order to relieve or treat the symptoms of elderly patients with chronic diseases.

Effects of CO-OP Interventions on Affected Upper Extremity Use, Execution Function and Occupational Performance in Patients With Stroke (CO-OP 중재 적용이 뇌졸중 환자의 상지 기능과 실행기능 및 작업 수행 증진에 미치는 영향)

  • Objective : In this study, the upper extremity use, executive function, and occupational performance effects of cognitive orientation to daily occupational performance (CO-OP) interventions for patients with stroke were assessed. Methods : The study was designed as a single-group pre-post test with 20 sessions. The participants were five hospitalized patients with stroke who were present in a rehabilitation setting, and their onsets were more than 3 months previously. Outcomes were measured using the Canadian Occupation Performance Measure (COPM), Performance Quality Rating Scale (PQRS), Executive Function performance Test - Korean version (EFPT-K), and Motor Activity Log (MAL). The Wilcoxon signed-rank test was conducted to determine the difference between the pre-and-post of CO-OP interventions. The statistical significance level was p<.05. Results : The upper extremity function showed significant changes and the execution function showed significant changes in preparation, sequencing, judgment and safety, and closing, except for items to be started. The performance of the task also showed significant changes. Conclusion : Through 20 sessions of CO-OP interventions, especially in patients with chronic stroke, the upper extremity function, execution function, and task performance were improved. We found that CO-OP intervention had a positive effect on the improvement of detailed task elements as well as the performance of tasks overall, in patients with stroke.

  • Frequencies of polymorphisms of genes BRCA1 and ТР53 in breast cancer (BC) patients with a BC family history and radiation history were assessed and compared in the Semey region of Kazakhstan. The study included 60 women directly irradiated by the activities of the Semipalatinsk test site with a calculated effective equivalent dose of 500 mSv and their first generation descendants (group BC+Her+Exp); 65 women with family BC and absence of radiological history - the effective equivalent dose due to anthropogenic sources not exceeding 50 mSv (group BC+Her-Exp). The comparison group consisted of 65 women patients with breast cancer without family and radiological history (BC-Her-Exp). The control group comprised 60 women without breast cancer and without family and radiological history (nonBC). We carried out the genotyping of the polymorphisms c.2311T>C, c.4308T>C and 5382insC of the BRCA1 gene and rs1042522 of the ТР53 gene. The frequency of the polymorphism c.2311T>C was significantly higher in patients of the group BC+Her+Exp than in healthy women, and of the polymorphism 5382insC in BC+Her+Exp compared to all other groups. The frequency of the rs1042522 polymorphism of ТР53 was significantly higher in all groups of patients with breast cancer compared with the control group. Differences between groups of women with breast cancer were significant only in BC+Her+Exp vs. BC+Her-Exp. Combinations of polymorphisms of the genes BRCA1 and TP53 predominated in women with a family and radiological history.

Study on Development of Perception Arts Action Group Counseling Program Focused on Popcorn Brain Phenomenon (팝콘브레인 현상에 대한 지각예술작용 집단상담 프로그램 개발에 관한 연구)

  • This thesis revolves around dual sensory therapy techniques using perception arts group counseling program focused on Popcorn Brain phenomenon. The program emphasizes intrinsic motivations for changes by activating the brain's dual sensory such as a sense of smell, tactile sensation and vision to deal with brain issues caused by digital devices addiction. Perception arts group counseling program is based on voluntary satisfaction of relationship desires, which can lead to a sense of accomplishment and belonging. Expression methods using dual sensory were presented as an alternative to resolving digital devices addiction by blending cognitive-behavioral counseling technique and group art therapy technique. The blending method offers an environment that can maintain a change to a senseless and lackluster brian due to excessive uses of digital devices, the main cause of Popcorn Brain, by emphasizing "thoughts${\rightarrow}$emotions${\rightarrow}$behavior" with a structural expression approach. If perception arts group counseling program is established in more systematic fashion and used strategically to offer such environment, it may be used as a treatment for brain issues described above. In addition, the program can be used a basis for encouraging voluntary treatment to satisfy relationship desires.

Meta-Analysis of Research Papers on Horticultural Therapy Program Effect (원예치료 효과 검증을 위한 연구논문의 메타분석)

  • To analyze the effectiveness of horticultural therapy reported in research papers, a meta-analysis was carried out with 108 of 377 Korean and overseas research papers on horticultural therapy from 1985 to February 2009 (196 Korean dissertations, 113 Korean journal articles, 5 overseas dissertations, and 63 overseas journal articles). The results revealed a large effect size of 0.71. The effectiveness according to the type, age and number of participants were also analyzed. The therapy was most effective in children, and it was more beneficial for children with special needs and elderly people affected by dementia than for the typical population. The greatest effect was found with group sizes of five people or less, and when carried out by researchers from departments related to horticultural therapy than by researchers from unrelated fields of study. The most effective horticultural therapy programs involved floral decorations; and participants who took part in 21 to 30 sessions or at least 31 sessions benefited the most. Among the main effects of short-term horticultural therapy are cognitive, social, physical and emotional; the greatest effect was observed in the physical domain.

Job Characteristics and Status of Community Occupational Therapist : Focus on OTs in Public Health Centers (지역사회 작업치료사의 업무 특성 및 실태 조사 : 보건소 근무 작업치료사를 중심으로)

  • Objective : This study was conducted to identify occupational therapists working in public health centers, the characteristics and actual conditions of occupational therapists in the community, and use them as basic data on occupational therapists in the community as of 2020. Methods : 77 questionnaires were replied by e-mail from OTs work at nationwide health public centers. Job characteristics and status were analysed by descriptive statistics and check correlation between job satisfaction and other factors. Results : Most survey respondents were female(77.9%) and 20-30(96.1%).. Some occupational therapists worked for dementia related team(72.7%) and others worked for like visiting care, health care, and rehabilitation center etc. Rate of experiences of public health center was 1-2 years(67.5%), the most common type of contract was flexible part-time worker(61%) and work intensity(94.8%) and satisfaction of work was very high(85.7%). The highest difficulty of their job was budget administrative work(26.7%) and of non-work difficulty was inequality under contracts(27.2%). They usually participated at dementia shelter, visiting OT, group OT. Difficulty of their job was high in budget administration, dementia shelters, and visiting work treatments. Goals of treatment were high in improvement of cognitive ability and, family support. Frequency of treatment was high in improvement of cognitive therapy, family support, and evaluation. Occupational therapy targets for health centers were dementia, the general elderly, and adult brain lesions, including those for ordinary people, psychiatric disorders and children. It was found that the primary occupations for evaluation were nurses (35.7%) and occupational therapists (33.7%), and that MMSE-DS, SGDS, and SMCQ were used a lot. Conclusion : This study could identify the job characteristics and status of community OTs. We hope that this result could be basic data for building expertise and role for community OTs in changing situations like community cares.

Analysis of the Current Status of Dementia Rehabilitation in Busan, South Korea (부산지역 치매 재활의 현황 분석)

  • Purpose: The purpose of this study is to understand and analyze the current status of dementia management and rehabilitation services in Busan, South Korea, in response to the rapid increase of people with dementia due to the aging of the population. Methods: To investigate the current status of dementia rehabilitation in Busan, a survey was disseminated to dementia safety centers and day- and night-care centers in 16 districts/counties in Busan. Of the 209 day- and night-care centers, 23 institutes were registered in the National Health Insurance Service and received the highest grade (Grade A) in the institute evaluation that was implemented in each district. A telephone interview was conducted, and survey questions were related to the existence of an ongoing dementia rehabilitation program, program presenter, number of participants, progress method, program time, program duration, program contents, and participation path. Results: Dementia safety centers were implementing dementia prevention program, cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient. The majority of the presenters of all four dementia-related programs were occupational therapists. The highest number of participants in the dementia prevention program was 15, and the highest number of participants in the cognitive enhancing program, dementia program, self-help meeting and education program for family of dementia patient was 10. All institutes' programs delivered group therapy. As for the time and frequency of the program, most dementia program included three-hour sessions five times a week. Most dementia prevention program, cognitive enhancing program, self-help meeting and education program for family of dementia patient included 60-minute sessions once a week. The most frequently observed program duration for the dementia prevention program and cognitive enhancing program was six months, and the most frequently observed duration for the dementia program was three months. Lastly, study participants most often reported that self-help meeting and education program for family of dementia patient lasted for two months. Among day- and night-care centers in Busan, programs related to cognition were implemented in 18 institutes, and the majority of the program presenters were social workers. Conclusion: In response to the rapidly growing number of dementia patients due to the aging of the population, this study examined the current status of dementia rehabilitation in Busan. The study results underscore the need to develop systems that consider the circumstances in Busan and continuously and systematically support dementia programs.
