흉관 삽입술 없이 시행한 전폐절제술 후 발생한 합병증에 대한 분석 (Analysis of Postpneumonectomy Complication without Balanced Chest Bottle)

  • 배경: 전폐절제술은 수술 그 자체가 환자의 심폐기능에 큰 영향을 줄 수 있으며 또한 합병증이 치명적 결과를 초래하는 경우가 많아, 합병증과 그 연관 인자 간의 많은 연구가 있어 왔다. 수술적 수기로 대부분 흉관을 삽입 후 수술을 종료하는 방법을 많이 사용하고 있으나 합병증 중 농흉의 높은 비율로 인해 그 위험 인자 중 융관 삽입술 없이 전폐절제술을 시행하여 발생한 술후 합병증에 대해 비교 분석하여 보고하고자 한다. 대상 및 방법: 1996년 1월부터 2000년 12월 까지 약 5년간 한양대학병원 흉부외과에서 수술 치험한 100례를 대상으로 환자기록지를 참조하여 나이와 성, 수술적응, 관련질환, 수술 환측으로 분류하고 술후 발생한 합병증과 사망률 등을 $\chi$$^2$-test 사용하여 통계학적으로 처리하였다. 수술은 전폐절제술 후 흉막강 내 압력을 유치도뇨관(nelaton catheter)을 사용하여 -15~-20 cm$H_2O$로 한 후 개흉창을 닫고 4~5일 이상 단순 흥부 촬영으로 액체 저류 및 출혈, 종격동의 위치를 감시 하였다. 결과: 총 100례 중 폐결핵 16례(16%), 폐종양 81례(81%), 기관지 확장증 2례(2%), 폐국균종을 동반한 기관지 확장증 1례(1%)였다. 수술 후 사망은 총 100례 중 8례로 8%열으며 합병증은 34명(34%)에서 44례로 조사되었다. 합병증은 나이, 성별, 수술 측과는 통계적 상관 관계가 없었으며, 결핵환자 16명 중 7명(44%), 폐종양 81명 중 27명(33%)로 결핵 환자에서 폐종양에 비해 3.86배로 합병률이 높았으나 통계적인 유의성은 확인할 수 없었다. 또한 술후 발생한 출혈 6례에서 결핵 16례 중 3례, 폐종양 및 기타 84례 중 3례로 결핵에서 술후 출혈률이 통계적으로 의미있게 높았다(p=0.019) 사망률은 총 100례 중 8례(8%)로 폐종양 81례 중 5례(6.1%), 폐결핵 16례 중 3례(18.7%)로 결핵 환자에서 폐종양에 비해 3.93배로 사망률이 높았으나 통계학적으로 유의성은 확인 할 수 없었다.

중고등학생의 에이즈 예방교육(豫防敎育)을 통한 에이즈 지식(知識)과 성의식(性意識) 변화(變化)에 관(關)한 연구(硏究) (A Study on the Change of AIDS Knowledge and Sexual Behavior among Middle and High School Students through AIDS Prevention Education)

  • AIDS is worldwide problem. It has threatens societies and is potentially a big problem among youth. UNAIDS has warned that collective global responses are necessary; with half of the newly infected being children and young people in 1998, educating youth below age 24 is essential. Because of both their unexpected, strong sexual activities and an easjer tendency to change their behavior than adults, UNAIDS emphasizes the importance of prevention education for youth. In Korea, 4 cases of HIV infection have officially been reported among high school students. Considering the potential seriousness of HIV infection among youth, the Korean AntiAIDS Federation (KAAF) began an education programme for Korean youth (middle & high school pupils) as one intervention method in 1993. The purpose of this study is to evaluate the impact of the education programs performed and to develop a better programme. The intervention method used in this study was mainly a lecture in a big or small auditorium or through broadcasting systems using audio-visual teaching aids. The period of this survey was from Nov. 10 to Dec. 26. in 1998. The subjects surveyed were 792 pupils who were sampled from 12 different middle and high schools in Seoul by a two-stage cluster sampling. Self-recording the structured questionnaire was used for data collection. Data were analyzed by using a paired t-test to compare the knowledge difference before and after the interrention method. A chi-square test to compare the consciousness difference between pre-post-education results was used, and an ANOVA was ased to compare the knowledge difference among the selected groups. Major results of this study are summarized as follows: (1) AIDS knowledge significantly changed after AIDS prevention education. (2) The direct lecture in a big or small auditorium is more effective than an indirect lecture through a broadcasting system or through audio-visual teaching aids. (3) Those who had not seen obscene materials (e.g. pornography films, magazines etc.) acguired much more knowledge than others after AIDS prevention education. (4) Those who had no girlfriend/or boyfriend acquired much more knowledge than the others after AIDS prevention education. (5) As to the attitudes toward chastity, more girls than boys and students who had not seen obscene materials rather than those who already had were liable to support the state ment: "all should remain chase" and their consciousness on "being chaste" changed much more than that of others after intervention. (6) As to the individual evaluation of lectures, 23.1% shows "very good", 44.1% "good", 26.9% "normar" '-' thus, the positive evaluation was to 94.2%. The negative response was 5.8%. (7) As to the desire for AIDS education sponsored by KAAF in the future, more than 90.5% showed a positive reaction. (8) As to the educators in future education methods, pupils wanted lecturers from outside their schood. (9) Students from vocational high schools have seen obscene materials much more than other groups and the percentage of those who had a girlfriend/or boyfriend was higher than that of the others as wello. (10) 90% of those who watched obscene materials responded that they experienced these materials while at middle school and 100% before reaching the second grade of high school. (11) The number of boys who have experienced obscene materials is twice as many as that of girls. (12) The percentage of boys who have a girlfriend is much higher than the percentage of girls who have a boyfriend. (13) Among those who have friends of the opposite sex, 11.1% say that they hare had sexual contact and 20.8% hare experienced kissing and caressing.

신체기능장애와 발달장애아동 어머니의 장애수용, 스트레스, 대처방식 및 웰니스 (Comparison of Acceptance of Disability, Stress, Coping Methods, and Wellness between Mothers of Physical Dysfunctional or Developmental Disabilities)

  • 본 연구는 신체기능장애와 발달장애아동 어머니의 장애수용, 스트레스, 대처방식, 웰니스 정도를 비교하여, 유형별 장애아동 어머니를 위한 효율적인 관리 방안을 마련하는데 필요한 기초자료를 제공하기 위한 조사연구이다. 연구대상은 장애아동을 양육하는 어머니이다. 자료수집은 2018년 10월부터 2019년 4월까지 실시하였고, 두군 간의 차이는 t-test, ANOVA, Pearson 상관관계로 분석하였다. 연구결과, 신체기능장애와 발달장애아동 어머니의 장애수용, 스트레스, 대처방식, 웰니스 정도에 따라 두군 간의 차이가 있는 것으로 나타났다. 신체기능장애아동 어머니의 장애수용과 대처방식(r=0.23, p=.033), 장애수용과 웰니스(r=0.51, p<.001)는 정적 상관관계를 나타냈고, 장애수용과 스트레스(r=0.72, p<.001)는 부적 상관관계를 나타냈다. 발달장애아동 어머니의 장애수용과 웰니스(r=0.40, p<.001)는 유의한 정적 상관관계를 나타냈고, 장애수용과 스트레스(r=-0.71, p<.001)는 유의한 부적상관관계를 나타냈다. 신체기능장애아동 어머니는 장애수용 정도가 높을수록 스트레스는 낮게, 대처방식과 웰니스는 높게 나타났다. 발달장애아동 어머니는 장애수용 정도가 높을수록 스트레스가 낮게, 웰니스 정도가 높게 나타났다. 신체기능장애아동 어머니의 장애수용 증진을 위해서는 스트레스를 낮춰주는 전략 및 대처방식과 웰니스를 모두 증진시켜주는 전략이 필요하고 반면 발달장애아동 어머니에게는 스트레스를 낮춰주는 전략과 웰니스를 증진시켜 주는 전략이 필요함을 알 수 있었다.

국군간호사관학교 교육과정 개선을 위한 기초 연구 (A Study on Curriculum Improvement of the Korea Army Nursing Academy)

  • 1. Need for and Purpose of the Study. There is an increasing demand for curriculum improvement of the Korean Army Nursing Academy (KANA), since it was upgraded into 4-year institution of higher learning from 3-year one. In particular, it is strongly advocated that the KANA needs the outside expertise for its curriculum improvement-namely not only from the internal military view of points but also from the viewpoints of professional educational society, In line with such a necessity for the study, this study was aimed at 1) analyzing the current actual practices of KANA'S curriculum, 2) investigating the desired practices of KANA'S curriculum, and 3) identifying the discrepancy between the actual and desired practices of curriculum. 2. Problems for the Study This study had 4 problems to be answeared as follows: 1) What are the actual curriculum practices of KANA? 2) What are the desired curriculum practices of KANA? 3) How are the extents of perception of actual and desired curriculum different in four groups (student, faculty & administrator, nurse, and medical doctor in militay hospital) ? 4) What are the restraining forces that impede the change from actual to desired curriculum practices? 5) What are the relationships of curriculum component,』 in actual and desired curriculum practices? 3. Methods and Procedures This study was conducted by means of document analysis in addition to literature review and by means of needs assessment questionnaire which was developed by the researcher. The questionnaire included 62 statments with 7 questions for demographic data collection. The needs assessment questionnaire was managed to a total of 243 subjects (100 students, 46 faculty & administrators, 55 nurses, and 42 medical doctors), The collected data were treated using SPSS computer system so as to calculate mean scores, standard deviations, and correlation coefficients. The significance test was made through t-test and one-way ANOVA. The statistical significance level was set at both .05 and .01 level. 4. Major findings The major findings in this study are as follows: 1) The score of desired practices was significantly greater than that of actual practices, representing a strong need for curriculum betterment. 2) There were significant differences in the perceptions of actual practices as well as desired practices among four groups (student, faculty & administrater, nurse, and medical doctor). 3) The most frequently selected restraining forces were army's inherent character, economical limitation, and educational expertise limitations. 4) Such variables as sex, position attachment to the KANA and grade made a statistically significant effect on the perception of desired curriculum practice, while the variables like marrige, position, and military class made it on the perception of actual curriculum practice. 5) The coefficients among the curriculum components were lower in perception of the actual curriculum practices than those in the desired practices. 5. Conclusions The conclusions based on the major findings of this study are as follows: 1) The current curriculum development procedure of the KANA is not consistent with the theoretical frame of systematic development sarategy of curriculum. 2) There are wide conflicts among the groups who are supposed to participate in curriculnm development, concerning the actual and desired practices of KANN'S curriculum. 3) A great deal of need for curriculum improvement for the KANA is clearly felt, and in particular, in the process of teaching and learning. 4) Each component of curriculum is not intergrated into a whole development procedure, being segregated each other. 5) For better curriculum improvement, such restraining forces as financial and professional limitations should be eliminated. 6. Recommendations 1) For Further Research a. There is a need to replicate this study after in-depth statistical analysis of each item of need assessment questionnaire, and with more representative subjects. b. A study should be conducted which. has its focus on the analysis of restraining forces for the change from actual to desired curriculum practices of the KANA. 2) For KANA'S Curriculum Improvement a. There is a need to promote the professional expertise of the participants in curriculum development and the communication among them. b. It is desirable to establish an institution or section of administration, which is soley in charge of curriculum development. c. To better develop KANA's curriculum not only faculty and administrators but also students should be encouraged to participate in development process, while the military medical doctors' participation should be carefully considered.

한국인에 있어서의 뚜렛 장애와 COMT유전자간의 상관 관계에 대한 연구 (ASSOCIATION BETWEEN TOURETTE DISORDER AND CATECHOL-O-METHYL TRANSFERASE(COMT) GENE IN KOREAN SUBJECTS)

  • 연구목적:본 연구는 한국인에 있어서의 뚜렛 장애 환아 가족에서 COMT유전자의 유전적 다형성과 뚜렛 장애와의 연관성에 대해 고찰하기 위해 수행되었다. 위험 대립 유전자와 특정 임상 양상(틱 증상의 중증도, 공존 질환, 약물 반응)과의 관련성에 대한 것도 함께 연구하였다. 방 법:환아군은 서울대학교 병원 소아 청소년 분과의 틱 장애 클리닉에서 모집하였고, 두 단계의 평가를 거쳐 선별되었다. 우선 모든 환아와 부모는 Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version(이하 K-SADS-PL)한국어판을 이용한 반구조적 면담을 받았고, 다음으로 모든 부모에게 임상적 면담을 시행하고 YGTSS한국어 판을 이용하여 틱 증상의 중증도에 대한 평가를 하였다. 대조군은 본 병원의 건강증진센터로부터 모집하였고 Symptom CheckList-90(이하 SCL-90)과 Structured Clinical Interview for DSM-IV(이하 SCID-IV)를 이용하여 평가하였다. 위 과정을 모두 거쳐 뚜렛 장애가 있는 총 42명의 소아, 청소년과 그들의 84명의 부모, 86명의 대조군이 최종적으로 선별되었다. 표준화된 방법을 이용하여 catecholamine-O-methyltransferase(이하 COMT)유전자의 Val158Met 다형성을 알아보기 위한 유전자형 채취를 시행하였다. 모든 환아와 부모, 대조군의 유전적 정보를 수집한 후 환아-대조군 비교와 TDT를 시행하였다. 결 과 : 환아-대조군 연구로부터 L대립 유전자와 LL유전형의 빈도가 뚜렛 장애 환아군에서 유의하게 높음이 관찰되었다. 그러나 TDT에서는 유의한 차이가 관찰되지 않았다. 또 뚜렛 장애 환아군의 세 가지 서로 다른 유전형 사이에 틱 장애의 가족력, 주의력결핍 과잉행동장애, 강박증, 약물에 대한 반응, 공존 질환 여부 등에 있어서 유의한 차이는 없었다. 결 론:본 연구에 있어 사례 수가 적고 TDT에서 유의한 결과가 발견되지 않았기 때문에 해석에 조심을 기할 필요는 있겠으나, 본 연구는 COMT유전자의 기능적 다형성과 뚜렛 장애 간에 연관 관계가 있음을 밝혀 낸 최초의 보고라 하겠다.

중학생의 성에 대한 지식, 태도, 행위에 관한 조사연구 (A Study on Knowledge, Attitude and Behavior to Sexuality in Middle School Students)

  • The purpose of this study is to find out methods to control, regulate and prevent sexual violence and provide guidelines for sex education in middle school. This study was conducted to examine factors in relation to sexuality, attitudes toward women, experience of sexual contact, frequencies and kinds of pornography with which they had contact, and sex education. For the data collection, a survey was conducted from September 25 to October 1, 1997 by using prepared questionnaires. Questionnaires were given to 503 students from three different kinds of middle schools in Seoul, Boys, Girls, and Coeducation schools. The data analyses used a PC-SAS. Each item was examined by frequencies and percentage. To compare Boys with Girls, I applied a T-test. In addition, with the kinds of school type, I applied anANOVA, ${\chi}^2$. Moreover, For investigating and analyzing each category, I categorized knowledge and attitude of sexuality, with precautionary measures of sexual violence. After that I applied T-test, ANOVA, and a Pearson's correlation for each category. The instruments of this study consisted of 9 domains. (1) General characteristics of the respondents (2) 9 questions about the interaction between he/she and his/her parent. (3) 12 questions, concerning A Scale for Attitudes of Adolescents Toward women(AWSA). (4) 26 questions related to sexual knowledge. (5) 25 questions about sexual attitude (6) strategies for prevention of sexual violence-33 questions (7) 5 questions related to sexual behavior (8) the experience of contact with pronography. (9) the experience of sex-education. The major results were summarized as follows; 1. Like many researches, This study shows that the girls are more eqalitarian than boys. The girls mean ($40.90{\pm}3.67$) is significantly higher than boys ($34.72{\pm}3.77$). Most Koreans have believed that there are many differences between men's roles and women's ones, because of confucianism which had been implied in most thought and ideas since the Lee dynasty. Therefore, the result is not surprising at all. 2. Most students answered correctly the questions of sexual knowledge. The girl's score was higher ($Mean{\pm}SD$ : $48.88{\pm}2.39$) than the boys' ($Mean{\pm}SD$ : $46.95{\pm}3.90$)(p 0.001). Nevertheless, the hypothesis that sexual knowledge influences sexual attitude and its behavior was rejected. 3. When twenty-five questions were asked about sexual attitude, the girl students investigated were found to have a more propre sexual attitude than that of the boys. A significant difference was found between boys and girls(p=0.001). The result shows that a person who has more experience in viewing pornography and who smokes will have a more distorted view of sexuality. 4. There are many students who have experienced of sexual contact. They need to take a sex education program about contraception and prevention of sexually transmitted diseases. Systematic and concrete sex education is one way to lead them to proper sexual behavior. 5. Most respondents rejected stereotyped attitudes towards women. Boys, however, were more likely to accept stereotypes of Women. The girl's mean($Mean{\pm}SD$ : $40.90{\pm}3.67$) is higher than the boys($Mean{\pm}SD$ : $34.72{\pm}3.77$)(p=0.0001). 6. Many boys ignored the rules for prevention of sexual violence. A boy or a girl who has more experience of sexual contact over kiss and sex-typed neglected the rules, too(p=0.001). Today, there is increasing juvenile delinquency related to sexuality. This study provided the basic material for a preventive education system. I believe that a systematic and concrete sex education system can be helpful for adolescents and promote their responsibility as well as cultivate morality about sexuality. As a result, juvenile delinquency can be decreased. This study is basically aimed to provide information for a prevenative education system of sexual violence. Further research is recommended to evaluate programs in schools.

경기 일부지역의 유질 및 낙농위생 관리에 관한 조사연구 (A Study on Milk Quality and Dairy Hygiene Management in Gyunggi Area)

  • 본 연구는 유질향상을 위한 기초자료를 제공하고자 1986년 5월부터 1987년 4월 까지 경기도 일부지역의 234개 목장을 대상으로 목장별 집합원유에 대해 유지방검사, 세균수시험 (MBRT), 체세포수시험을 실시하여 유질을 평가하였고, 목장의 낙농위생관리실태를 조사하여 유질과의 관련성을 분석한 결과 다음과 같은 결론을 얻었다. 1. 유지방율검사, 평균 유지방율은 3.56%였고, 월별로는 1월 3.67%, 4월 3.64%, 7월 3.43%, 10월 3.48%였으며, 월별 유지방율의 차이는 총계학적으로 유의성이 인정되었다(p<0.005). 2. 세균수시험 (MBRT) 결과, 집합원유의 세균수가 200만/ ml 이하인 I급 목장이 평균 132개(56.4%)였고, 월별로는 1월 153개(65.4%), 4월 150개(64.1%), 7월 76개(32.5%), 10월 141개(60.2%)였으며, 월별 MBRT등급의 차이는 총계학적으로 유의성이 인정되었다(p<0.005). 또한 납유량이 많은 목장구가 적은 목장구보다, 냉각기를 보유하고 있는 목장구가 보유하지 않은 목장구보다, 탱크집유목장구가 통집유목장구보다, 집합원유 MBRT I 등급의 비율이 더 높았으며, 납유량이 다소 (p<0.05), 냉각기 보유여부(p<0.01), 집유수단(p<0.05) 등과 MBRT 등급간에는 유의한 관련성이 인정되었다. 3. 체세포수시험 결과, 집합원유 체세포수가 50만 cells/ ml 이하인 I급 평균 223개(95.3%)였고, 월별로는 1월이 227개(97.0%), 4월 222개(94.9%), 7월 218개(93.2%), 10월 223개(95.3%)였으며, 월별 체세포수 등급의 차이는 유의성이 인정 되지 않았다. 한편, 착유후 유두침지소독 및 건유기유방염 치료를 병행실시한 목장구가 이의 단일실시 목장구보다, 유방세척용수건을 개체별로 사용한 목장구가 여러개체에 공동으로 사용한 목장구보다 체세포수가 더 낮았으며, 착유후 유두침지소독 및 건유기유방염 치료여부(p<0.01), 유방세척용 수건사용방법(p<0.01) 등과 체세포수간에는 유의한 관련성이 인정되었다.

고등학생의 건강 및 삶의 질에 대한 진단적 연구 - PRECEDE 모형을 근간으로 - (A Diagnostic Study on High School Students' Health and Quality of Life - Based on the PRECEDE model -)

  • Health education, as the most fundamental concept for national health promotion, alms for developing the self-care ability of the general public. High school days are regarded as the period when most important physical, mental and social developments occur, and most health-related behaviors are formed. School health education is one of the major learning resources influencing health potential in the home and community as well as for the individual student. High school health education in Korea has a fundamental systemic flaw in that health-related subjects are divided and taught under various subjects areas at school. In order to achieve the goal of school health education, it is essential to make a systematic assessment of the learner's concerns connected with his health and life, and the factors affecting them. So far, most of the research projects that had been carried out for improving high school health education were limited in their concerns to a particular aspect of health. Even though some had been done in view of comprehensive school health education, they failed to Include a health assessment of the learner. Therefore, in this study the high school students' concerns related to health and life were investigated in the first place on the basis of the PRECEDE model, developed by Green and others for the purpose of a comprehensive diagnostic research on high school health education. This study was done in two steps : one was the basic study for developing research instrument and the other was the main one. The former was conducted at five high schools in Seoul and Cheongju for 2 months-beginning in March, 1996. The students were asked to respond to questions related to their health and lives in unstructured open-ended question forms. On the basis of analysis of the basic study, the diagnostic instruments for the quality of life, health problems, health behavior and educational factors were constructed to be used for the collection of data for main study. An expert panel and the pilot study were used to improve content validity and reliability of the instruments. The reliability of the instruments was measured at between .7697 and .9611 by the Cronbach $\alpha$. The data for this study were collected from the sample consisted of the junior and senior classes of twenty general and vocational high schools in Seoul and Cheongju for two months period beginning in July, 1996. In analyzing the data, both t-test and $X^2$-test were done by using SAS-$PC^+$ Program to compare data between the sexes of the high school students and the types of high school. A canonical correlation analysis was carried out to determine the relationships among the diagnostic variables, and a multivariate multiple regression analysis was conducted by using LISREL 8.03 to ascertain the influences of variables on the high school students' health and quality of life. The results were as follows : 1) The findings of the hypothesis tests (1) The canonical correlation between the educational diagnosis variables and behavioral, epidemiological, social diagnosis variables was .7221, which was significant at the level of p<.001. (2) The canonical correlation between the educational diagnosis variables and the behavior variables was .6851, which also was significant (p<.001). (3) The canonical correlation between the behavioral diagnosis variables and the epidemiological variables was 4295, which was significant (p<.001). (4) The canonical correlation between the epidemiological diagnosis variables and the social variables was .6005, which was also significant (p<.001). Therefore, the relationship between each diagnosis variable suggested by the PRECEDE model had been experimentally proven to be valid, supporting the conceptual framework of the study as appropriate for assessing the multi-dimensional factors affecting high school students' health and quality of life. Health behavior self-efficacy, the level of parents' interest and knowledge of health, and the level of the perception of school health education, all of which are the educational diagnostic variables, are the most influential variables in students' health and quality of life. In particular, health behavior self-efficacy, a causative factor, was one of the main influential variables in their health and quality of life. Other diagnostic variables suggested in the steps of the PRECEDE model were found to have reciprocal relations rather than a unidirectional causative relationship. The significance of this research is that it has diagnosed the needs of high school health education by the learner-centered assessment of variety of factors related to the health and the life of the students. This research findings suggest an integrated system of school health education to be contrived to enhance the effectiveness of the education by strengthening the influential factors such as self-efficacy to improve the health and quality of the lives of high school students.

Training Program to Raise Consciousness Among Adolescents for Protection Against Skin Cancer through Performance of Skin Self Examination

  • Background: Overexposure to sunlight in childhood and the adolescent period and associsated sunburns significantly increase the risk of skin cancer in adulthood. In Turkey, the incidence of skin cancer in the general population is 0.8%. The incidence is 0.6% and the mortality rate is 0.4% for men, while these rates are 1.0% and 0.7%, respectively, for women. If skin cancer is found early, its treatment is facilitated. Therefore, personal skin examination is important for early diagnosis. Objectives: Our aim was to determine the effects of training for raising consciousness among adolescents to protect against skin cancer by influencing skin self examination behavior. Method: This quasi experimental intervention study was conducted between February and April 2012 in Izmir. The study population consisted of students attending $6^{th}$, $7^{th}$ and $8^{th}$ classes of a primary school (n:302). No sampling was performed. Data were collected with a form developed by the researchers based on the literature. The first part of form is aimed to determine demographic characteristics of adolescents (3 questions) and their risk status of skin cancer (6 questions). The second part was prepared for skin cancer risks of adolescents (8 questions) and indications of skin cancer (12 questions). The last part was intended to determine their knowledge about skin self examination (4 questions) and behavioral stages of skin self examination (1 question). Data collection was achieved with a questionnaire form in three phases. In the 1st phase, data about demographic characteristics of students, risk status of skin cancer, knowledge level of skin cancer and behavior stages were collected. In the $2^{nd}$ phase, skin self examination training based on the transtheoretical model was performed within the same day just after obtaining preliminary data. In the $3^{rd}$ phase, adolescents were followed up three times to establish the efficacy of the training (on the $15^{th}$ day after training program and at end of the $1^{st}$ and $2^{nd}$ months). Follow-up data were evaluated by questioning skin self examination performing behavior stages through electronic mail. Results: Half of the adolescents (50.5%) are male, and 58.4% of them are 13 years old with a mean age of $12{\pm}1.15$ years. About 29.4% of adolescents had brown hair color, 37.9% had brown/hazel eye color, 29.4% had white skin, and 47.2% had fewer than 10 moles in their body. The pretest mean score on knowledge level about risks of skin cancer was found to be $4.19{\pm}1.96$, while the post-test mean score was $6.79{\pm}1.67$ (min:0, max:8).The pretest mean score about indications of skin cancer was $7.45{\pm}3.76$, while the post-test mean score was $10.7{\pm}2.60$ (min:0, max:12). The increases were statistically significant (p<0.05). The behavior "I do not perform skin self examination regularly in every month and I do not think to perform it in the next 6 months" was reduced from 52.8 to 35.5% after training. Conclusion: The training program organized to raise consciousness among adolescents for protection against skin cancer increased the knowledge level about risks and indications of skin cancer and it also improved the behavior of performing skin self examination.

전통시장 상인의 건강상태와 건강행태 (Health Conditions and Health Behaviors of Merchants at Traditional Markets)

  • 본 연구는 전통시장 상인의 건강상태와 건강관련 행태를 남녀별, 전통시장 종사기간별로 규명하고 일반인들과 비교하기 위한 횡단적 서술적 조사연구이다. 자료수집은 C시 D보건소에서 2015년 6월 1일부터 6월 20일까지 C시 N전통시장에서 종사하는 상인 307명에게 신체계측과 채혈을 한 후 설문조사를 실시하였고 수집된 자료는 SPSS WIN 21.0을 이용하여 분석하였다. 연구결과 전통시장 종사자들의 현재 흡연율은 34.2%, 3.5%, 11.1%(남, 여, 전체), 연간 음주율은 68.4%, 38.5%, 45.9%(남, 여, 전체), 스트레스를 많이 느낀 대상자와 우울감 경험률은 각각 23.7%, 28.5%, 27.4%(남, 여, 전체), 5.3%, 6.1%, 5.9%(남, 여, 전체)로 일반인에 비해서 낮았으나 중등도 신체활동 시행률과 걷기 실천율은 각각 32.9%, 19.9%, 23.1%(남, 여, 전체), 85.5%, 79.2%, 80.8%(남, 여, 전체)로 매우 높아 좋은 건강행위를 실천하고 있으며 정신건강도 상대적으로 양호한 상태를 유지하고 있는 것으로 파악되었다. 그러나 일반인에 비해서 고혈압, 당뇨병, 고콜레스테롤혈증, 대사증후군 유병률이 매우 높았고 전통시장 종사기간이 길수록 복부둘레, 저 HDL-콜레스테롤혈증(low HDL-cholesterol)과 고혈압, 대사증후군, 관절염의 유병률이 높은 것으로 나타났다. 따라서 이에 대한 구체적인 원인 규명과 함께 대상자들의 특성에 맞는 장기적인 생활개선 프로그램을 마련하고 지속적으로 적용해야 할 것으로 판단된다.
