Impacts of death perceptions, terminal care stress, and life satisfaction on attitudes toward end-of-life care among nurses at a tertiary hospital (죽음인식, 임종간호 스트레스, 삶의 만족도가 상급종합병원 간호사의 임종간호태도에 미치는 영향)

  • Purpose: This study investigated the factors influencing attitudes toward end-of-life care among nurses at a tertiary hospital in Korea. Specifically, it examined the roles of nurses' characteristics, death perceptions, terminal care stress, and life satisfaction. Methods: The participants included 150 nurses working at a tertiary hospital, who were recruited between August 15 and September 19, 2023. Data were collected through an online survey and analyzed using descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and multiple regression analysis with SPSS/WIN 28.0. Results: Nurses' attitudes toward end-of-life care were positively correlated with death perceptions (β = .28, p < .001), 3 years or more of clinical experience (β = .25, p = .001), the experience of an acquaintance death (β = .22, p = .002) and life satisfaction (β = .20, p = .004). These variables explained 34.0% of the total variance in attitudes toward end-of-life care. Conclusion: It is essential to develop and implement individualized end-of-life nursing education programs, particularly utilizing simulations, for nurses with limited clinical experience and low levels of death perceptions. Further research should explore attitudes toward end-of-life care among various healthcare providers with a broader regional scope to improve the overall quality of end-of-life care.

Relationship Between Type of Medical Institutions According to the Equipment List and Inspection Fee Computed Tomography (의료기관 종별에 따른 전산화단층촬영장비 보유현황과 검사료간의 관계)

  • This study demonstrates holding condition of CT by medical institution classification and by season, and examination fee in Korea currently to quantitatively understand frequency of examination region by change of CT equipment, domestic growing trend and change of distribution and using rate. Recent 10 years of CT holding condition by medical institution classification (Tertiary hospital, General hospital, Hospital, Clinic, Dental hospital, Dental clinic, Hospitalized health center) and by year (2003-2012), and CT examination fee of distribution of medical institution by year is surveyed. The holding ratio of Tertiary hospital level and General hospital level is 32.7% in 2003 and 33.0% in 2012. Whereas, Hospital and Clinic level is 74.2% in 2003 and 66.8% in 2012, which takes approximately 70%. Based on data in 2012, it is 82.2% of total examination fee in Tertiary hospital and General hospital, while 17.5% in hospital and clinic. CT holding rate of Hospital level is increasing, while Clinic level is decreasing. Approximately 80% of CT examination fee is claimed by Tertiary hospital and General hospital. Therefore, there is a significant correlation between CT holding condition of medical institution classification and examination fee. Particularly, correlation between CT holding number of Tertiary hospital and examination fee is significant (p<.001). The more CT holding number, the higher the amount claimed examination fee.

Panicle characteristics of Japonica × Indica type rice according to planting density

  • Rice yields are determined by the number of spikes per unit area, the number of rice per grain, the rate of ripening, and the weight of brown rice gravel. Among these yield components, the most important factor that reflects the characteristics of rice is the number of rice per grain and the number of rice per grain. The characteristics of rice panicle are influenced by genetic factors rather than environmental factors and revealed that it had different genetic characteristics in each of Japonica, Indica and Japonica ${\times}$ Indica type. In general, the number of rice per grain is affected by number of secondary rachis branches and generated spikelets number of secondary rachis branches. This study conducted to investigate the characteristics of the grain according to the planting density of two varieties of Jap. ${\times}$ Ind. type. Number of primary rachis branches and number of primary rachis branches of Palbangmi variety were 9.9 ~ 10.6 and 53.4 ~ 58.5, respectively. Number of secondary rachis branches and number of secondary rachis branches were 25.8 ~ 29.6, 85.8 ~ 101.4, Number of tertiary rachis branch and number of tertiary rachis branches were 1.0 ~ 2.4 and 2.0 ~ 6.1, respectively. Number of primary rachis branches and number of primary rachis branches of Semimyeon variety were 8.6 ~ 9.5 and 43.1 ~ 47.8, respectively. Number of secondary rachis branches and number of secondary rachis branches were 21.0 ~ 24.9, 66.2 ~ 77.9, Number of tertiary rachis branch and number of tertiary rachis branches were 1.6 ~ 2.6 and 3.8 ~ 6.3, respectively. The ratio of the spikelets of primary, secondary and tertiary rachis branches of Palbangmi variety were 37.7 ~ 39.4, 58.0 ~ 60.5 and 1.2 ~ 3.1%, respectively, and those of Semimyeon were 40.1 ~ 42.6 55.0 ~ 56.4 and 2.5 ~ 3.4% respectively. Number of primary and secondary rachis branches of Palbangmi variety showed no difference among the planting density. However, generated spikelets number of secondary rachis branches and ripening rate were higher with lower planting density. The number of secondary rachis branches and generated spikelets number of secondary rachis branches per acre decreased as the planting density increased in the Semimyeon variety

HISTOLOGY OF DENTAL PULP HEALING AFTER TOOTH REPLANTATION IN RATS (흰 쥐 치아 재식 후 치수 치유 양상의 조직학적 관찰)

  • The objective of this study was to observe the histology of dental pulp healing after tooth replantation in rats. The maxillary right first molars of 4-week-old rat were extracted, and then the teeth were repositioned in the original socket. At 3 days after replantation, there was localized inflammatory reaction. But, pulp revasculization and healing had already begun in the root area. At 5 days after replantation, odontoblast-like cells were observed. Tertiary dentin deposition was observed beneath the pulp-dentin border from 1 week after replantation. And tertiary dentin was increased at 2 weeks after replantation. The presence of odontoblast-like cells and the formation of tertiary dentin were continued to 4 weeks after replantation. At 4 weeks after replantation, the deposition of bone-like tissues and cementum-like tissues was observed. This results show that there is a possibility of pulp healing after tooth replantation in rats and the mineralization of tooth can progress. The mineralization of tooth after replantation was initially occurred by the deposition of tertiary dentin, but as time passed, the deposition of bone-like tissues and cementum-like tissues was begun and increased.

A Study on the Physician's Behavior of Notifiable Communicable Diseases Reporting and its Characteristics Related (법정전염병 신고행태 및 관련특성 연구)

  • The major concern for this research is to discuss and to offer some solutions to bring the effectiveness of existing notifiable diseases reporting system over the physicians' attitudes of reporting, the actual condition of performance and the reasons of inertia in notifiable diseases reporting through examining the physicians of medical institutions in nationwide such as pediatrics, internal medicine and family medicine. The actual conditions of notifiable communicable diseases(NCD) reporting was surveyed by mail objectifying an internal medicine, pediatrics and family medicine in nationwide on the basis of stratified random sampling method divided into the classification of medical institutions and areas. As a result of survey. the rate of respondents showed 145 persons from physicians, 105 persons from hospitals. 120 persons from general hospitals, and 51 persons from tertiary hospitals. The total number of respondents were 421 and was rated 59.0 %. The analysis of collected survey went through a descriptive analysis primarily to grasp physicians' attitudes on the notifiable communicable diseases reporting, and then upon the dependent variables. Following are major findings obtained form the data analysis. 1. The results of a descriptive analysis on physicians' attitudes towards reporting NCD were as follows: First, the respondents who didn't know that yellow fever is reporting NCD were 11.0% of clinic, 10.5% of hospital. 5.0% of general hospital. 11.8% of tertiary hospital. and in case of hepatitis B, were 26.9% of clinic, 35.2% of hospital. 35.0% of general hospital. 23.5% of tertiary hospital. Second, The rate of physicians' knowledge on penalties of not reporting the NCD by their medical institution were 35.2% of clinic, 45.7% of hospital. 36.7% of general hospital. 62.7% of tertiary hospital. Third, among the no-reporting physicians in whole, the major reason of not reporting NCD were uncertainty of diagnosis(78.9%), no need to report(46.4%), no adequate actions from PHC(29.1%), no knowledge of the cases being notifiable ones in the order of their frequencies(30.4%), meddling from PHC(29.1%), concerning of patient's privacy(26.3%). 2. To analyze the characteristics related to the physicians' behaviors to report NCD, univariate and multiple logistic regression analyses were applied to the variables related to physician, 4 medical facility, PHC, and reporting system. The result were as follows: First, the result of the univariate analysis on physicians' attitude to report NCD and characteristics related to reporting in odds ratio was in the case of hospital. 3.4 times higher positive responses on physicians' attitude to report NCD came up as compared to the clinic. Second, the result of the univariate analysis on physicians' action of reporting NCD and characteristics related to reporting by the classification of medical institutions showed that the odds ratio of hospital was 2.3 times, the odds ratio of general hospital was 2.0 times, the odds ratio of tertiary was 6.8 times significantly higher than clinic. And the medical institution with significantly higher positive attitudes rate by multiple logistic regression analysis was hospital that rated 2.5 times significantly higher than clinic. Also in the PHC related characteristics of reporting, the rate of action in reporting NCD was significantly higher in medical institution that were endowed with the good condition of reporting. In multiple logistic regression analysis, the medical institution that has a good conditions of reporting showed a significantly higher positive rate on the action of reporting than the others.

Evaluation of Long-Term Deformation Prediction Model on Frozen Sand Considering Fine Contents (세립분 함량을 고려한 동결 사질토의 장기변형 예측 모델 평가)

  • Many countries are interested in the development of the Antarctic area because of the abundant resources and living things of high research value. Korea completed the second Antarctic research station in 2014 and spurring the development of the Antarctic area by constructing runway for the airplanes and a third Antarctic research station. However, frozen soils, unlike typical soils, are sensitive to creep behavior due to the influence of ice and unfrozen water. The creep tests for evaluating creep behaviors on the frozen soils require expensive laboratory equipments and large amount of time. Thus, various empirical models had been developed to describe the unconfined compressive creep behavior of frozen soils. In this study, new analytical creep model on frozen sands was proposed by modifying Ting's Tertiary creep model with a new parameter considering fine contents. Thus, the unconfined compressive creep tests were conducted with the frozen specimens of dense Jumoonjin sand with fine contents of 0, 5, 10 and 15% under various loads at -$5^{\circ}C$, -$10^{\circ}C$ and -$15^{\circ}C$. Consequently, the modified Tertiary creep model with a new parameter for fine contents are not enough for the description of the acutal creep behavior of the frozen sand and new framework should be developed.

A Pilot Test for the Utilization of Road Subsoil of the Tertiary Mudstone in Pohang Basin (포항분지 제3기 이암의 도로 노체 활용을 위한 현장시험)

  • The purpose of this study is to present the possibility a utilization of the tertiary mudstone in Pohang as road subsoil material through pilot experiments on the road embankment structure. This mudstone is an unconsolidated rock that is distributed in the soft rock sedimentary layer, the tertiary layer of the Cenozoic, and causes physical problems such as slaking, swelling, and reduced shear strength and chemical problem like acid drainage. In order to solve various complex problems, an laboratory mixing test was conducted, and the optimal mixing conditions of the tertiary mudstone (90%), composite slag (steel making 70%, blast furnace 30%), and neutralization and coating agent treatment were derived. In order to prove its utilization, a real-scale road embankment structure was constructed and tests were conducted for each section. The pre-processing section is stable due to the design of optimal mixing conditions, while in post-processing section, natural weathering proceeded rapidly, and structural problems were concerned. Since the effect of neutralizing and coating agents was confirmed in temporary-staking section, the neutralizing and coating agents can be applied during the temporary storage period.

A Study on Appropriate Nurse Staffing Levels in Intensive Care Units and Improvement of the Critical Care Nursing Fee Schedules (중환자실 적정 간호사 배치수준과 간호관리료 차등제 개선 연구)

  • Purpose: This study attempted to analyze the staffing level and critical care nursing fees of intensive care units at tertiary and general hospitals and to provide a professional judgment-based recommendation on staffing level and critical care nursing fee schedules. Methods: Staffing grades and critical care nursing fee schedules for the first quarter of 2017~2020 and the fourth quarter of 2020~2022 were analyzed. A survey was conducted on nursing managers and nurses about the current and appropriate staffing levels. A total of 77 nurse managers and 708 nurses working in Intensive Care Unit(ICU)s at tertiary and general hospitals participated in the study. Results: Grade 1 staffing increased from 25.6% in 2017 to 92.1% in 2022 at tertiary hospitals and from 0.8% in 2017 to 28.4% in 2022 at general hospitals. The current staffing ratios of tertiary and general hospitals were 1:2.21 and 1:2.77, respectively. The appropriate staffing ratio according to nurse managers and nurses was 1:1.00 in patients with more than a ventilator application and 1:2.00 in patients without any ventilator application in tertiary hospitals, and it was 1:1.25 in patients with more than a ventilator application and 1:2.00 in patients without any ventilator application in general hospitals, respectively. Conclusion: The appropriate staffing level was suggested from 1:1.0 to 1:2.0. The new nursing fee schedules were suggested from 1:1.0 (Grade 1) to 1:3.0 (Grade 5) and recommended to be paid based on the staffing grade, minimum number of nurses, and standard annual working days. It is expected to increase staffing levels and provide a better nursing work environment.
